OBJECTIVE: To analysis the efficacy of 0.2% GTN in patients undergoing fistulectomy for fistula in ano low variety in terms of early wound healing. METHODOLOGY: A randomized controlled trail was conducted at general surgery department of Liaquat University Hospital Hyderabad from February 2013 to August 2013 among patients with low anal fistula. All participants underwent fistulectomy by a consultant general surgeon. Patients were divided into two groups using lottery methods. Group A treat fistula in ano with 0.2% glyceryl trinitrate ointment and group B treat fistula in ano without 0.2% glyceryl trinitrate ointment and wounds covered with pyodine soaked dressings. All patients were discharged on second postoperative day. Wound healing was assessed weekly for six weeks. Parameters of incomplete wound healing such as discharge, perineal irritation and pain were noted. Presence of granulation tissue at the end of 6 weeks was considered as sign of healing. Data was collected via study proforma. RESULTS: This study was conducted on 94 patients in Liaquat University Hospital Hyderabad. In GTN group 35(74.46%) were male while in Control group 38(80.85%) were male. In GTN group mostly observed single opening 37(78.72%) cases while in control group 35(74.46%) cases. The healing time in GTN Group was found to be between 21-36 days (mean 26.38 days), while the healing time control group was 32-46 days with a mean of 38.64 days (P. value <0.001). The complications were also found significantly higher in the control group CONCLUSION: In conclusion our study revealed that postoperative 0.2% GTN after fistulectomy can be used as a primary treatment of low fistula in ano as a shorter period of time for wound to early heal and the less incidence of complications is comparable to that of control group. In the last few decades, the incidence of caesarean section is increasing in the world, especially in Indonesia. One of the way to treat tissue scar is through biologic and synthetic dressing where nowadays, amnion has been used as biologic dressing frequently. This study was conducted to determine the effect of the use of fresh amniotic membrane on wound incision Caesarean section compared with Caesarean section incision wound covered using regular gauze bandages and fixated with plaster in RS. Dr. Reksodiwiryo Padang. The design of this study is an experimen-tal study with Post test design with control group design. Sampling was done using a formula consecutive sampling two different test samples obtained an average of 72 people for each group. The analysis used include univariate and bivariate analyzes. The average wound healing time the difference was statistically significant (p value <0.05) in the treatment and control groups. There was highly significant difference in the proportion of local infection on day 3 between the treatment and control groups (p value <0.05). There were very significant differences in the proportion of local allergic reactions at day 3, and 5 between the treatment and control group (p <0.05). There are significant differences in terms of the cost of care per day between treatment and control groups (p <0.05). From this study, the average wound healing time has a very significant difference.Keywords: Fresh Amniotic Membrane, Wound Cesarean Section, Wound Healing

Childbirth is a natural process, but sometimes labor can also cause birth canaltrauma, especially in the aluminum region, this trauma can be injuries to the periniumarea, the cause can be intentional biases such as episiotomy or unintentional actions such as spontaneous tears in the process of removing the baby. Injury to the perineum if not treated properly can cause postpartum infection because the wound area will be a medium for developing germs. The purpose of this study was to analyze the effect of the use of breast milk in the treatment of periniem wounds with the duration of perinium wound healing at the Ramlah Parjib Clinic in Samarinda. The method used in this study was Posttest Only Control Group Desigen research subjects were postpartum motherswith a sample of 60 respondents taken by random sampling and grouped based on treatment using ASI. Data analysis using ANOVA test. The results showed that the use of ASI had a significant effect on the treatment of periniem wounds with a healing time seen from the P value of 0,000. Conclusion: Periniuem wound care techniques are factors that influence the duration of perinieum wound healing

The aim of this paper was to evaluate the effect of using artificial dermis matrix plus autologous split-thickness skin (ADM and ASTS) in the treatment of deep-burns in hands of severely burned patients?We recruited a total of 58 patients with large area burns greater than 80% that were eschar-excised. Twenty-eight of them were treated with ADM and ASTS (test group); 30 were treated with autologous medium-thickness skin (AMTS) (control group). The healing time of the hand wound was noted, clinical and photographic evaluations were performed, and a Jebsen-Taylor hand function test was compared and analyzed in the two groups. The wound healing time in the test group (24.22?3.34 days) were longer than that of the control group (13.42?3.36 days) and statistically significant. The healing time of skin graft donor sites was shorter than that of the control group (7.14?1.63 vs. 14.28?2.37 days) and statistically significant (P<0.05). The 3rd and 6th month follow-up with clinical and functional evaluations revealed no differences between the two groups. In addition, there was no obvious scar formation and less pigmentation in either group. The repair of deeply burned hands with artificial dermis matrix was beneficial to both wound healing and the donor site, and was beneficial to the whole body rehabilitation of severely burned patients.

Background: Fistula in ano is a silent menace in human beings, the treatment is a challenging one even for experienced surgeons, fistula in ano forms a good majority of treatable benign lesions of rectum and anal canal. Aim was to study the efficacy of fistulotomy and fistulectomy in the treatment of low anal fistulae.Methods: A randomized control study was conducted to compare fistulotomy with fistulectomy in patients with low anal fistulae.Results: The operating time in fistulotomy group was 12.13minutes±2.11minutes and in fistulectomy group was 22.23±3.36minutes. The post-surgery hospital stays in Group I was 1.80±0.66days and in Group II was 2.60±0.563 days. The wound healing time in Group I was 24.20±2.95 days which was considerably less when compared to patients in Group II where it was 31.50±4.34 days.Conclusions: Fistulotomy has a slight edge over fistulectomy in the treatment of low anal fistulas since it has shorter operating time, less post-operative pain, less complications, quicker wound healing time, less incontinence and a comparable recurrence rate.

Objectives: To compare the efficacy of 0.2% glyceryl trinitrate ointment versus internal sphincterotomy after hemorrhoidectomy. Study Design: Randomized Controlled Trial. Setting: Department of Surgery Unit III, at Lahore General Hospital, Lahore. Period: 6 months (August 2017 to February 2018). Material & Methods: The OPD of the Department of Surgery included 124 patients who satisfied the inclusion criteria. There were two groups of patients, Group A and Group B. Group-A was told to use gloved finger three times daily for 0.2 percent GTN ointment topically. For Group B, lateral internal sphincterotomy was performed as day care procedure. On visit, patients were assessed for pain relief by using VAS. All data was collected using a pre-designed (attached) proforma. In SPSS v23.0 the data were input and analyzed. It was stratified for age, sex, disease duration and hemorrhoid stage. The effectiveness of both groups was compared by using a p-value of 0.05 as significant for a chi-square test. Results: Male predominance; in group-A, 41(66.1%) patients were male and 21(33.9%) were female, while in group-B, 52(83.9%) patients were male and 10(16.1%) were females. Efficacy was found in 11(17.7%) patients in group-A (0.2% GTN) while in 25(40.3%) patients in group-B (Internal sphincterotomy) with p-value of 0.006 which is statistically significant. Conclusion: Open hemorrhoidectomy internal sphincterotomy is an effective method to reduce pain after hemorrhoidectomy in contrast to Glyceryl Trinitrate (GTN) topical cream of 0.2 percent.

Objective: to determine the prevalence of explained and unexplained recurrent miscarriages (RM) and to find out if there is a significant relationship between recurrent miscarriages and consanguinity. Methods: A cross sectional in which the cases group included all women with RM attending the outpatient clinic at Sultan Qaboos University Hospital from July 2006 to April 2012 and the controls group included women with no history of RM after matching them with cases for age (case to control ratio was 1:1). The main outcome measures were the prevalence of consanguinity in women with or without recurrent miscarriages. Results: During study period a total of 290 women with RM were seen. Of which, 150 (51.7%) women had unexplained RM. Control group with no history of RM were 300 women. Consanguinity rate among cases (49.5%) %) was less than the controls (52.7 %%). Both first cousin and second cousin marriages were more common in the controls than the cases and it was not statistically significant (p value 0.476, chi squared test). Conclusion: In this study we found that more than half of RM cases were unexplained and there was no significant association between RM and consanguinity.

Infeksi nifas menjadi salah satu penyebab kematian ibu di negara berkembang termasuk Indonesia. Beberapa penyebab pemicu terjadinya infeksi nifas seperti pelayanan kebidanan, daya tahan tubuh, perawatan nifas, gizi, anemia dan kebersihan genetalia. Pada masa nifas penting untuk merawat luka perineum karena kuman bisa masuk melalui luka bekas jahitan pada perineum agar tidak menyebabkan infeksi. Salah satu alternatif terapi nonfarmakologis yang dapat digunakan untuk penyembuhan luka perineum masa nifas adalah daun binahong. Tujuan penelitian ini menguji pengaruh daun binahong dalam mempercepat penyembuhan luka perineum pada masa nifas. Penelitian ini menggunakan rancangan quasi experimental dengan pendekatan posttest only control group design. Posttest only control group design merupakan pengumpulan data dilakukan sesudah perlakuan. Total sampel yang digunakan dalam penelitian ini adalah 32 orang ibu nifas sesuai dengan kriteria inklusi. Data diobservasi berupa derajat kesembuhan luka perineum dengan menggunakan instrumen lembar observasi penyembuhan luka perineum pada skala REEDA (Redness, Edema, Ecchymosis, Discharge dan Approximation). Hasil penelitian menunjukan terdapat pengaruh pemberian rebusan Daun binahong terhadap penyembuhan luka perineum pada ibu nifas. Rerata hasil uji Uji statistik Mann Whitney, pada kelompok perlakuan adalah 11,83 lebih rendah dibandingkan pada kelompok kontrol, yaitu 21,8 (p-value = 0,001). Penelitian ini menunjukan bahwa ada pengaruh pemberian rebusan daun binahong mampu mempercepat penyembuhan luka perineum dibandingkan iodine povidone 10%. Daun binahong dapat menjadi alternatif terapi komplementer pada ibu nifas yang mengalami luka robekan perineum.   One of the main causes of maternal death in developing countries is puerperal infection. Midwifery care, body immunity, postnatal care, nutrition, anemia and genetalia hygiene are factors that trigger puerperal infections. The perineal rupture if not treated properly will cause an infection in the suture wound in the perineum. Wound care is important because germs can enter through the suture marks on the perineum and cause infection. Non-pharmacological therapies that can be used to accelerate wound healing are Anredera cordifolia. The research design was quasi experimental with posttest approach Only Control Group Design, namely data collection was done after treatment. The total sample was 32 postpartum mothers according to inclusion criteria. The data observed in this study were the degree of healing of perineal wounds using the REEDA scale perineal wound healing observation sheet instrument (Redness, Edema, Ecchymosis, Discharge and Approximation). Based on the results of the Mann Whitney test, the mean rank of the results in the treatment group was 11.83 lower than the control group, namely 21.8 (p-value = 0.001). Effect of giving Anredera cordifolia leaves on the healing of perineal wounds in postpartum mothers significantly. Care with giving Anredera cordifolia leaves can accelerate the healing of perineal wounds compared to 10% iodine povidone. Anredera cordifolia leaves can be used as a complementary therapy for postpartum mothers who experience perineal torn wounds.

Tooth extraction is a process of removing teeth from the alveolar bone. In wound healing, fibroblast are very important cells. The main purpose of this study was to determine the effect of mobe leaf 1% and 3% extract gel (Artocarpus lakoocha) on fibroblast proliferation in post extraction tooth socket wound healing. This research used 16 samples of wistar rats, divided into 4 groups, a positive control group, a negative control group and a 1% and 3% mobe leaf extract gel group. The left mandibular incisors were extracted, then 1% and 3% gels of mobe leaf extract were applied on day 1 to day 7. Data analysis was calculated using the Kruskal-Wallis test on clinical data and one way ANOVA test for microscopic. The result of the socket wound healing activity test for a good concentration of mobe leaf extract gel was 3%. This research shows significant resultith p-value of 0.018 (< 0.05) on the closure of the socket wound clinically which means the closure of the wound accelerates because of the mobe leaf 3% extract gel treatment. The distance of fibroblast on microscopically shows significant resultith a p-value of 0.002 (< 0.05), which means that there was an enlargement of the distance fibroblast at the socket wound closure with application of mobe leaf 3% extract gel. From the results of the study it can be concluded that mobe leaf 3% extract gel has the best ability to show acceleration the closure of the socket wound either clinically or microscopically.

The growth hormone is involved in skin homeostasis and wound healing. We hypothesize whether it is possible to improve pressure ulcer (PU) healing by locally applying the recombinant human growth hormone (rhGH) in a human skin mouse model. Non-obese diabetic/severe combined immunodeficient mice (n = 10) were engrafted with a full-thickness human skin graft. After 60 days with stable grafts, human skin underwent three cycles of ischemia-reperfusion with a compression device to create a PU. Mice were classified into two groups: rhGH treatment group (n = 5) and control group (n = 5). In the rhGH group for local intradermal injections, each had 0.15 mg (0.5IU) applied to the PU edges, once per week for four weeks. Evaluation of the wound healing was conducted with photographic and visual assessments, and histological analysis was performed after complete wound healing. The results showed a healing rate twice as fast in the rhGH group compared to the control group (1.25 ± 0.33 mm2/day versus 0.61 ± 0.27 mm2/day; p-value < 0.05), with a faster healing rate during the first 30 days. The rhGH group showed thicker skin (1953 ± 457 µm versus 1060 ± 208 µm; p-value < 0.05) in the repaired area, with a significant decrease in collagen type I/III ratio at wound closure (62 days, range 60–70). Local administration of the rhGH accelerates PU healing in our model. The rhGH may have a clinical use in pressure ulcer treatment.
