Osteoporosis is a worldwide disease characterized by reduction of bone mass and alteration of bone architecture resulting in increased bone fragility and increased fracture risk. Although it is seen in all age groups, gender, and races, it is more common in Caucasians (white race), older people, and women. With an aging population and longer life span, osteoporosis is increasingly becoming a global epidemic. Currently, it has been estimated that more than 200 million people are suffering from osteoporosis. Moreover, osteoporosis results in a decreased quality of life, increased disability-adjusted life span, and big financial burden to health insurance systems of countries that are responsible for the care of such patients. Therefore, increasing awareness in medical field, which, in turn, facilitates increase awareness of the normal populace, will be effective in preventing this epidemic.

Tietze syndrome is a self-limiting, rare and benign condition that might be mistaken with potentially fatal chest diseases and misdiagnosed mostly as costochondritis.  This disease is most often diagnosed in young individuals under the age of 40, with painful, localized inflammation.Tinea incognito is a ringworm infection that has been altered by corticosteroids and other immunosuppressive medications. Corticosteroids are administered for pre-existing illness or are used incorrectly for the treatment of tinea. We report a case of 20years old male patient admitted in emergency department with complaints of chest pain and SOB with normal ECG while neutrophils, ESR, CRP have found to be abnormal. Patient has been using steroids and itraconazole for maculopapular rashes in lower limb since 1year. Other diagnostic methods such as CT, MRI should be performed to avoid misdiagnosis.  He was prescribed with NSAIDS, antifungals, antihistamines and other supportive measures which helped him to relieve from pain. Proper diagnostic criteria and early diagnosis remain challenging tasks, resulting in undue treatment costs for patients. Before confirming a diagnosis, other underlying diseases should be ruled out.

Kleine-Levin Syndrome is a rare sleep disease characterized by relapsing episodes of hypersomnia interconnected with behavioral and cognitive disorders along with hyperphagia and hypersexuality. This disease occurs more often in young male adolescents. The diagnosis was clinical, primarily based on the nosologic distinction of non-identical forms of recurrent hypersomnia. The brief duration of KLS varies from 7 days to 2 months and the people affected are completely asymptomatic between episodes but during episodes, electroencephalography shows diffuse or local slow activity. Investigations for KLS comprise of medical history, magnetic resonance imaging, polysomnography, 24- hours time profile of growth hormone, melatonin, TSH, and cortisol. The Pathophysiology of this sleeping beauty syndrome is based on Neuropathological examinations, cerebrospinal fluid hypocretin-1 monitoring, clinical patterns, and neuroimaging. Imaging of brain function reveals the hypoactivity in thalamic and hypothalamic regions and imaging along with the cognitive capacity evaluation gives insights into neurobiological mechanisms of KLS. No effective treatment, as well as indefinite pathology, has been identified even though the illness is having clearly defined clinical features but some medications like stimulants and mood stabilizers can be beneficial in the treatment of severe cases.

Background: Pneumonia remains the prime infectious disease that increases the mortality rate among children under five claimed the lives of nearly 1.5 million children in 2015. Mortality due to childhood pneumonia is linked to the prevalence and relapse increases and usage of antibiotics is more. In such circumstances, the main. Aim: The Aim of the study was to assess the utilization of drugs and to spot the factors that contribute to the pediatric pneumonia patients at tertiary care teaching hospitals. Material and methods: A prospective, observational study was conducted from May 2021 to October 2021Overall 310 prescriptions were collected, 204 patients were included in the study based on inclusion criteria and data was collected from a proforma and by using WHO prescribing indicators study is analyzed. Results: Age group with pneumonia of<5years was 171 patients, followed by 33 patients between 6-8years.The male children constituted the major portion i.e. 120(58.82%) followed by female children were 84(41.18%). Distribution of drugs per prescription, majority of prescriptions with<5 drugs i.e., 110(56%) followed by 82 prescriptions with 5-10 drugs constituted (43%), this indicates the polypharmacy and 1 prescription with >10 drugs i.e., (1%). The distribution of antibiotics where majorly preferred drug in penicillin’s were amoxiclav- 164(59.20%), least preferred was piperacillin/tazobactam-9(3.28%), followed by cefotaxime-38(13.71%), ceftriaxone -17(6.13%), amikacin- 31(11.19%),  and azithromycin- 18(6.49%). Conclusion: In our study, we observed an irrational use of antibiotics and polypharmacy of drugs in the treatment of pediatric pneumonia.

Pemphigus Foliaceus (PF) is second most common autoimmune vesicular disease in the pemphigus family characterized by superficial blisters of the skin and rarely mucous membranes which rupture to produce scaly, crusted lesions. A 54 year old female patient admitted in female derma ward in tertiary care hospital, with the chief complaints of painful ruptured blisters all over the body with crusty, scaly patches associated with serous discharge and itching since 15 days. A positive Nikolsky’s sign is when the skin shears off easily when the surface is wiped sideways with a cotton swab or a finger. Based on both subjective and objective evidence, this case is assessed as Pemphigus Foliaceus. The plan incudes Corticosteroids, Immunosuppressants, Antibiotics, Vitamin A & D supplements, IV fluids, calcium and aminoacids supplements, antivirals and antifungals (if needed), proper wound management and finally plasmapheresis.

Fluvoxamine is an antidepressant that functions pharmacologically as a selective serotonin reuptake inhibitor. Though it is in the same class as other SSRI drugs, it is most often used to treat obsessive-compulsive disorder. As the biological half-life of the drug is 15.6 hrs and belongs to BCS class II. To overcome these problems, Nanoparticles of Fluvoxamine were formulated by using Ethyl Cellulose, Eudragit RS 100 & Eudragit Rl 100 as a polymer by emulsification method. Among all the 9 formulations F6 formulation is optimized, as it shows maximum drug release at the end of 12hrs which suits the controlled release drug delivery system criteria as per our studies, having acceptable particle size, SEM, and Zeta potential value. From the drug release kinetics of the F8 formulation of Losartan Nanoparticles dispersion, it was concluded that the F8 formulation follows Zero-order drug release with super case II transport mechanism.

A simple, Accurate, precise method was developed for the simultaneous estimation of the Levodropropizine and Chlorpheniramine in Tablet dosage form. The chromatogram was run through Ascentis C18 150 x 4.6 mm, 5m. Mobile phase containing Buffer Kh2po4: Acetonitrile was taken in the ratio 40:60was pumped through the column at a flow rate of 1.0ml/min. The buffer used in this method was Kh2po4. The temperature was maintained at 30°C. The optimized wavelength selected was260nm. The retention time of Levodropropizine and Chlorpheniramine was found to be 2.276min and 2.848. %RSD of the Levodropropizine and Chlorpheniramine was and found to be 0.7 and 0.7 respectively. %Recovery was obtained as 100.73% and 99.03% for Levodropropizine and Chlorpheniramine respectively. LOD, LOQ values obtained from regression equations of Levodropropizine and Chlorpheniramine were 0.14, 0.02, and 0.43, 0.06 respectively. Regression equation of Levodropropizine is y = 67089x + 5956.8 and y = 226526x + 13941 of Chlorpheniramine. Retention times were decreased and that run time was decreased, so the method developed was simple and economical that can be adopted in regular Quality control tests in Industries.
