The lagoon of Boughrara is the largest lagoon in Tunisia. For several decades it has been subject to the impact of increasing anthropogenic activities, and also to environmental stressors due to climate change and the low renewal of its waters. The present work is a contribution to the study of the functional organization of the benthic macrofauna of the lagoon of Boughrara 3 years after the extension of the channel ‘El Kantra’ which connects this lagoon to the open sea. In total, 13 stations facing the main prospective sources of disturbance and in areas likely to be more polluted were seasonally sampled during 2009–2010. Seasonal monitoring of the abiotic parameters shows a spatial heterogeneity linked to environmental and anthropogenic factors, including hydrodynamics, sedimentary texture and anthropogenic activities. The community of benthic macrofauna is generally poorly diversified at lagoonal-marine stations undergoing influences of both the marine and lagoon environment. The extreme seasons in terms of temperature and salinity seem to have an important role in the reduction of biodiversity in the lagoon of Boughrara. The trophic structure of the macrobenthic community is generally dominated by selective deposit feeders, and seems to be linked more to the availability of trophic resources than to disturbance.

The coronavirus disease (COVID-19) pandemic altered environmental factors. We studied the impact of these changes on asthma exacerbation (AE) by comparing the AE-related environmental factors between COVID-19 (2020) and pre-COVID-19 (2011–2019) eras. Between 2011 and 2020, 278,465 children (<16 years old) visited our emergency department, and 7476 were diagnosed with AE. The number of patients showed spring and fall peaks in 2011–2019. Multivariate analyses showed significant positive relationships of the number of AE patients with the average temperature among all patients and 0–5-year-olds and with sulfur dioxide (SO2) levels in 2011–2019 among 0–5-year-olds. Although the spring peak in the number of patients was not observed in 2020 after declaration of a state of emergency, the fall peak was again observed after the state of emergency was lifted. No changes in average temperature were detected, but SO2 was significantly reduced following declaration of the state of emergency in 2020. Therefore, SO2 reduction might have contributed to the disappearance of the peak of AE. However, a fall peak was observed again in 2020, although SO2 levels continued to be low. These data suggest that person to person interaction seems to be associated with AE, presumably due to unknown viral infections.

This article analyzes the problem of the state of the water bodies in Ishim by anthropogenic pressures. The study shows that the water reservoirs are negatively influenced by the economic activities of the population. The author defines their main directions: industrial wastewater discharge to the city water bodies, construction and operation of engineering constructions, activity in the private sector. To assess the state of the water bodies the author uses the results of the visual assessment of the state of the riverbeds and their coastal zone, the organoleptic water indicators (turbidity, color and smell), the analysis of the hydrobiological studies (determination of the degree of saprobity of the water bodies as indicators ciliated infusoria). The study showed the satisfactory condition of all the water reservoirs and the non-critical impact. As the enterprises for light manufacturing and food industries in Ishim are not sources of chemical, organic and other pollution the visual assessment didn't reveal significant deformations of the riverbeds and any changes in the coastal zone. The impact of the private sector is local and not the significant cause of organic pollution.

Background: We aimed to analyze the impact of cefotaxime non-susceptibility on the 30-day mortality rate in patients receiving a third-generation cephalosporin for pneumococcal bacteremic pneumonia. Methods: We conducted a retrospective observational study of prospectively collected data from the Hospital Clinic of Barcelona. All adult patients with monomicrobial bacteremic pneumonia due to Streptococcus pneumoniae and treated with a third-generation cephalosporin from January 1991 to December 2016 were included. Risk factors associated with 30-day mortality were evaluated by univariate and multivariate analyses. Results: During the study period, 721 eligible episodes were identified, and data on the susceptibility to cefotaxime was obtainable for 690 episodes. Sixty six (10%) cases were due to a cefotaxime non-susceptible strain with a 30-day mortality rate of 8%. Variables associated with 30-day mortality were age, chronic liver disease, septic shock, and the McCabe score. Infection by a cefotaxime non-susceptible S. pneumoniae did not increase the mortality rate. Conclusion: Despite the prevalence of cefotaxime, non-susceptible S. pneumoniae has increased in recent years. We found no evidence to suggest that patients hospitalized with bacteremic pneumonia due to these strains had worse clinical outcomes than patients with susceptible strains.

Abstract Introduction Oestrogen receptor beta (ER-β) is abundantly expressed in breast cancer (BC), but its impact on neoadjuvant chemotherapy outcome is unknown. Patients and Methods Patients treated in the neoadjuvant GeparTrio trial with available tissue for immunohistochemical analyses were included. Nuclear ER-β expression was correlated with clinico-pathologic characteristics. The impact of its expression on pathological complete response (pCR [ypT0/ypN0]) and survival was determined. Results Samples of 570 patients were available. Low nuclear ER-β expression (IRS < 9) was observed in 48.4% of hormone receptor positive and 58.6% of hormone receptor negative tumours. Low nuclear ER-β expression was associated with higher pCR rates compared to high nuclear ER-β expression (16.1% vs. 4.7%, p = 0.026). Low ER-β expression was no independent predictor of pCR in multivariate analyses. Disease-free and overall survival were not statistically different between patients with high and low nuclear ER-β expression. Triple-negative BCs showed low nuclear ER-β expression in 57.7%, and pCR rates were 27.1% and 0% (p = 0.23) in low and high ER-β expressing tumours, respectively. Conclusion Low ER-β expression is associated with improved pCR rates in univariate analyses. However multivariate analyses and survival analyses do not indicate an impact of ER-β on survival in patients undergoing neoadjuvant chemotherapy. Further examination of ER-β as predictor for endocrine therapy might be of value.

Drawing on postdeportation surveys ( N = 1,109) with Mexican migrants, we examine the impact of immigration enforcement programs and various social factors on repeat migration intentions. Our multivariate analyses suggest immigrants with strong personal ties to the United States have higher relative odds of intending to cross the border again, even when controlling modes of removal from the United States. Our findings highlight the inevitable failure of immigration policy and enforcement programs when placed against the powerful pull of family and home. These findings shed greater insight on the complex nature of unauthorized migration in an era of increased securitization and deportation.

Although Sense of Belonging has long been an important construct in understanding student success in higher education, it has not been examined in the alumni context. In this article, we explore the association between graduate students’ Sense of Belonging and alumni engagement. We draw on an original data set ( n = 1,601) that combines administrative records on alumni giving and data from a 2017 survey. Using multivariate analyses, we show that alumni with a stronger Sense of Belonging are more likely to give to their alma mater and to hold pro-philanthropic attitudes. Furthermore, Sense of Belonging is positively associated with other forms of alumni engagement and participation, including volunteering. Our findings highlight the need to examine the link between unintentional social interactions and alumni engagement and giving.

Abstract INTRODUCTION Cognitive concerns and objective cognitive impairment are both common in cancer patients, although the relationship between these variables is uncertain. Brain metastases have the potential to affect both outcomes. We examined the relationships between cognitive concerns, cognitive performance, and psychological disturbance in mNSCLC patients with and without brain metastases. METHODS mNSCLC patients completed standardized neuropsychological tests (HVLT-R, TMTA/B, COWA) and questionnaires measuring cognitive and neurobehavioral concerns, depression, demoralization, self-esteem, illness intrusiveness, and physical symptoms. Perceived cognitive impairment was defined as FACT-Cog scores ≤1.5 SD below the normative mean. Impaired cognitive performance was defined as ≥2 test scores ≤1.5 SD or one test score ≤2.0 SD below normative means. Multivariate analyses used the mean composite of neuropsychological test z-scores (COG-comp). Progressive disease (extra- and/or intracranial) was defined as radiological progression or clinical deterioration within one month before the study. RESULTS 74 patients participated (53% female; median age 62 (37–82); 30% EGFR mutated). Time since mNSCLC diagnosis was 11 months (median; 0–89). 50% had brain metastases, 21 of whom received cranial radiation (11 SRS, 10 WBRT±SRS). 27% reported cognitive concerns and 42% were impaired on objective testing (HVLT-R:34%, TMT:27%, COWA:16%). Neither was more common in patients with brain metastases. COG-comp (mean±SD: –0.39±0.91) was unrelated to cognitive concerns. In multivariate analyses, less depression (p&lt; 0.01) and illness intrusiveness (p=0.03), and absence of disease progression (p=0.03) were related to better perceived cognitive function. No significant associations were found between cognitive performance and demographic, disease, or psychological variables. CONCLUSION Cognitive impairment is common in mNSCLC patients with or without brain metastases, but is independent of cognitive concerns. Cognitive concerns which are linked to depression, illness intrusiveness and disease progression, may be more likely to reflect the impact of cognitive changes in daily life. Both merit further clinical and research attention.

426 Background: Most upper urinary tract urothelial carcinomas (UTUC) are invasive and advanced with a poor prognosis. Patients often relapse after treatment with radical nephroureterectomy (RNU). However, little is known about an association between sites of recurrence after RNU and oncological outcomes. The present study retrospectively evaluated the impact of a first recurrence site on survival. Methods: A total of 650 patients with UTUC treated by RNU at 12 institutions that participated in the Tohoku Urological Evidence-Based Medicine Study Group between 2000 and 2011 were initially enrolled. Patients who experienced relapse other than intravesical recurrence after RNU were included in this study. Those who had metastasis at the time of UTUC diagnosis or missing data regarding the time and location of relapse were excluded. Finally, 173 patients were eligible to participate in this study. Survival rates were analyzed using Kaplan-Meier curves and the log-rank test, and factors predicting survival were assessed using multivariate analyses. Results: The median follow-up after relapse was nine months. The cancer-specific survival rates at one and two years after relapse were 49% and 22%, respectively. Cancer in 59 (34%), 32 (18%), 23 (13%), and 94 (53%) patients recurred in the lung, liver, bone and lymph nodes, and 23 (13%) had local recurrence. The one-year cancer-specific survival rates were 44%, 27%, 22% and 52% among patients with metastasis of the lungs, liver, bone and lymph nodes respectively, and 41% among those with local recurrence. The survival rate was worse among 48 patients with metastasis at multiple sites, than in those with metastasis at a single site. Fourteen (8%) patients with a single lymph, lung or local recurrence survived for over three years after a first relapse. Multivariate analyses selected the liver, bone and local recurrence as critical factors for cancer-specific survival. Conclusions: Liver or bone metastasis was a negative prognostic factor for UTUC with relapse after RNU. Information about recurrence sites might be helpful for patient counseling.

Knowledge of concentrations or loads in wastewater flows is a basic prerequisite for the design of wastewater treatment units and the assessment of the environmental impacts. With respect to new sanitation concepts that are based on the source-separation of domestic wastewater flows not much general data is available yet on characteristics or design values of the different flows. A desk study of more than 130 references was carried out in order to arrive at design values for different source-separated wastewater flows including the fractions urine, faeces and greywater. The evaluation was carried out focussing on European data. The different values were analysed by the use of statistical parameters. In order to arrive at design values for different parameters, the median as well as minimum-maximum ranges of the available data were calculated. The collected data include volumes and characteristics like organic pollution (COD and BOD), nutrients (N, P, K & S) and heavy metals for the different source-separated flows. Loads and concentrations are listed respectively. A comparison is drawn between data from different regions in order to assess the impact of differences in nutrition and habits.
