Abstract Background Air pollutants such as particulate matter (PM2.5) and nitrogen dioxide (NO2) in outdoor air have long been suspected of causing the development of asthma and allergic rhinitis. However, a variety of systematic reviews have reached different conclusions in the last 15 years on whether these air pollutants do in actual fact play a causal role in the onset of asthma, allergic rhinitis, and eczema. Methods Based on published systematic reviews and the most recent publications, the current state of knowledge on epidemiological evidence is presented and the potential for primary prevention of these allergic diseases by reducing or avoiding exposure to these air pollutants evaluated. Results Despite conducting an extensive literature search, analyzing the most recent results, and focusing on the birth cohort studies most relevant to the question in hand, epidemiological results do not adequately support the concept of a causal relationship between the two air pollutants in question, PM2.5 and NO2, and asthma. Epidemiological studies predominantly show no effect of these air pollutants on allergic sensitization and the onset of allergic rhinitis. The small number of studies that have investigated the link between air pollutants and eczema largely revealed there to be no link. Conclusion If the evidence for the causal role of air pollutants in the onset of allergies is inconclusive, one must assume that it is probably not possible to achieve primary prevention of allergies by improving air quality. However, there is sufficient evidence to show that air pollutants can trigger exacerbations of allergic diseases. This alone justifies ensuring that the existing threshold values for air pollutants are adhered to, in order to protect particularly allergy sufferers from health impairments.

Background The aim of this study was to investigate the prevalence of allergic rhinitis and atopy in adolescents and whether air pollution in their schools contributed to allergic sensitization. Methods Analyses were performed in 1992–1994 on 511 18-year-old students at four schools and on the indoor air and floor dust of their classrooms. The students underwent skin-prick tests (SPTs) and a nasal lavage and answered a computer-based questionnaire containing questions on allergy and nasal symptoms. Results Atopy, defined as at least one positive SPT response, was found among 37% of the students, with no difference between students of the four schools, regardless of whether the data were adjusted for gender, hereditary disposition to allergy, and smoking habits. The number of students who had positive SPT and reported nasal symptoms when exposed to possible allergens, which were found among 35%, did not differ between the schools. No correlation was found between the prevalence of atopic individuals and the levels of different indoor air pollutants in the schools. Conclusion The prevalence of allergic rhinitis among adolescents is very high and suggests that it is, at least at the time of our study and in comparison with other studies, still increasing. Our results indicate that the indoor air and floor dust at the schools of the students do not contribute to allergic sensitization.

Atopic march is a variant flowing of atopia that begins in early childhood as atopic dermatitis, then developinto other allergic diseases (bronchial asthma, allergic rhinitis) at an older age. The state of the epidermal barrier and sensitization to inhaled allergens are considered as predictors for the development of atopic march. Data on the importance of these factors in the development of atopic march and information about possible approaches to prevention are presented in this article.

The aim of our study was to evaluate the diagnostic parameters of immunoblot and Immunocap ISAC methods to determine allergic sensitization to wormwood and hazel in patients with respiratory allergic diseases – allergic rhinitis and bronchial asthma. Materials and methods. In this study, 40 patients with bronchial asthma and / or allergic rhinitis were examined with two different methods of specific allergic diagnosis (in vitro). The study was open-ended, comparative. Quantitative determination of specific IgE in the serum was performed using the RIDA® AllergyScreen immunoblot method (R-Biopharm AG, Germany) on the basis of the private laboratory of LLC “Allergy-Immunological Center KPP”. Immunocap ISAC testing was performed at “Forpost Allergy and Immunology Clinic”. Results and Discussion. Among patients, allergic sensitization to wormwood was 27.5% (11 people) in the presence of specific IgE by the Rida AllergyScreen method, 25.0% (10 people) in the presence of specific IgE using ImmunoCAP ISAC; sensitization to hazel allergen was 27.5% (11 people) in the presence of specific IgE on Rida AllergyScreen, 30.0% (12 people) in the presence of specific IgE from ImmunoCAP ISAC. For the determination of specific IgE to wormwood allergen the immunoblot has a high sensitivity and negative predictive value (100.00% in both cases) compared to ISAC, a high specificity of 96.67% (95% CI: 82.78; 99.92), but a positive predicitive value was 90.91% (95% CI: 59.28; 98.57) and accuracy was 97.50% (95% CI: 86.84; 99.94). Conclusions. Immunoblot compared to ISAC for the determination of specific IgE to hazel allergen has a relatively high specificity and negative predictive value (92.86% and 89,66%), but the sensitivity and positive predicitive value were 75.00% (95 % CI: 42.81; 94.51) and 81.82% (95% CI: 53.23; 94.68), respectively, and the accuracy of the method was 87.50% (95% CI: 73.20; 95.81).

Расширение спектра изучаемых клонов Тхелперов определило более сложные иммунные механизмы реализации аллергического воспаления. Цель. Характеристика показателей и взаимосвязей цитокинового профиля сыворотки и субпопуляционного состава Тлимфоцитов периферической крови у детей с бронхиальной астмой и аллергическим ринитом. Материалы и методы. Проведено комплексное обследование 150 детей в возрасте 311 лет с верифицированным диагнозом бронхиальной астмы, аллергического ринита и 30 здоровых сверстников. Иммунологические параметры крови оценивали методом проточной цитометрии, концентрации интерлейкинов и IgE в сыворотке крови определяли методом твердофазного иммуноферментного анализа. При статистической обработке использовали программы Statistica 10 с критическим уровнем значимости р0,05. Результаты. У детей с аллергическими заболеваниями в сыворотке крови определены высокие уровни содержания интерлейкинов4, 8, 13, 17А, сопоставимый с показателями группы контроля уровень IL17F и низкое содержание IFNy. При бронхиальной астме и аллергическом рините у детей выявлено увеличение количества CD3CD8CD45RO, CD3CD8CD45RACD45RO Тлимфоцитов и CD3CD4 Тхелперов и повышение количество Th17 при снижении CD3CD4CD45RO клеток памяти. В группе здоровых детей популяция Th17 составляла 9,491,6, у детей с аллергическими заболеваниями количество данных клеток было значимо выше 14,50,77 (р0,001). Анализ сывороточного содержания цитокинов у детей с изолированным течением БА и в сочетании с аллергическим ринитом выявил разнонаправленные корреляции, отличающиеся по силе и направленности от таковых в группе здоровых детей. Заключение. У детей при изолированном течении бронхиальной астмы и в сочетании с аллергическим ринитом выявлены: сопоставимое с показателями здоровых детей количество CD3CD4 Тклеток, дисбаланс в субпопуляционном составе Тхелперов за счет преобладания Th2 и Th17, активация синтеза IL17A, IL4, IL8, IL13, низкий уровень сывороточного IFNy, изменения силы и направленности взаимосвязей цитокинового профиля и спектра субпопуляций Тлимфоцитов.Expansion of the range of examined Thelper clones has determined more complex immune mechanisms for the implementation of allergic inflammation. Objective. Immunological parameters were assessed by flow cytometry, the concentration of serum interleukins and IgE were determined by means of enzymelinked immunosorbent assay. Statistical analysis was performed with Statistica 10 program with a critical level of significance p0.05. Results. High levels of interleukins 4, 8, 13, 17A were determined, IL7F level was not significantly different from that in control group and low level of IFNy was found in the serum of children with allergic diseases. The number of CD3CD8CD45RO, CD3CD8CD45RACD45RO Tlymphocytes, CD3CD4 Thelper cells and Th17 were increased and at the same time CD3CD4CD45RO memory cells were decreased In bronchial asthma and allergic rhinitis children. Number of Th17 cells in healthy children was 9.491.6, in allergic children it was significantly higher 14.50.77 (p0.001). Analyses of serum cytokine count in children with isolated BA and in association with allergic rhinitis revealed multidirectional correlations differing in strength and direction from those in the group of healthy children. Conclusion. In children with isolated bronchial asthma and associated with allergic rhinitis the following parameters were found: CD3CD4 Tcells count was comparable to that in healthy children, the imbalance of Thelper subpopulation: prevalence of Th2 and Th17, activation of IL17A, IL4, IL8, IL13 synthesis and low level of serum IFNy.

ABSTRACT Objective: To investigate whether there is an association between suicidal ideation (SI) and allergic diseases in adults. Methods: This was a comparative cross-sectional study involving individuals ranging from 20 to 50 years of age recruited from a university hospital in the city of Guadalajara, Mexico. We included patients with a confirmed diagnosis of allergic asthma, those with a confirmed diagnosis of allergic rhinitis, and healthy controls. All subjects completed the Beck Depression Inventory-II (BDI-II), which includes an item that evaluates the presence of suicidal thoughts or desires within the last two weeks, in order to identify SI. Results: The sample comprised 115 patients with allergic asthma, 111 patients with allergic rhinitis, and 96 healthy controls. The number of individuals identified with SI in the three groups were, respectively, 17 (14.8%), 13 (11.7%), and 8 (8.3%). Regarding the presence of SI, no statistically significant association was found in the allergic asthma group (OR = 1.98; 95% CI: 0.78-4.64; p = 0.154) or in the allergic rhinitis group (OR = 1.46; 95% CI: 0.58-3.68; p = 0.424) when they were compared with the control group. However, the presence of depression was associated with SI in the three groups: allergic asthma (OR = 12.36; 95% CI: 2.67-57.15; p = 0.001); allergic rhinitis (OR = 6.20; 95% CI: 1.66-23.14; p = 0.006); and control (OR = 21.0; 95% CI: 3.75-117.36; p < 0,001). Conclusions: In comparison with the control group, no association was found between SI and the groups with allergic diseases. In contrast, there was association between SI and depression in the three groups.

Asthma, eczema, food allergy, and allergic rhinitis are some of the most common pediatric, chronic conditions in the world. Breastfeeding is the optimal way to feed all infants. For those infants who are exposed to infant formula, some studies suggest that certain partially hydrolyzed or extensively hydrolyzed formulas may decrease the risk of allergic disease compared to nonhydrolyzed formulas for children with a family history of atopic disease. Overall, there is some evidence to suggest that partially hydrolyzed whey formulas and extensively hydrolyzed casein formulas may decrease the risk of developing eczema for infants at high risk of allergic disease. The evidence for a preventive effect of hydrolyzed formulas on allergic rhinitis, food allergy, and asthma is inconsistent and insufficient. Finally, the qualitative changes to the peptides by the method of hydrolysis, not just the degree of protein hydrolysis, may have a large influence on the preventive effect of a particular infant formula for the potential risk of allergic disease. As a result, it may be difficult to generalize findings from clinical studies using a specific infant formula to other infant formulas from different manufacturers using different methods of hydrolysis. Further clinical studies are needed to help clinicians identify which infants may benefit from early intervention, as well as which specific hydrolyzed formulas are best suited to decrease the risk of future allergic disease.

Allergic rhinitis and bronchial asthma are widespread respiratory allergic diseases. In some territories of the Russian Federation, the dominant cause of pollinosis is ragweed. The aim of the study was to evaluate the clinical and immunological efficacy of ASIT with the allergen Ambrosia artemisiifolia in patients sensitized to Ambrosia trifida in the Samara region. Patients with proven sensitization to Ambrosia trifida was held immunotherapy with Ambrosia artemisiifolia allergoid preseason. After treatment, patients had a decrease in the severity of symptoms of allergic rhinitis according to VAS (p = 0.00001), a decrease in the need for medications (p = 0.0003), as well as the need for corticosteroids against the background of therapy from 34.6% to 0% (p = 0.00001). In 8% of cases, the result of treatment was good, in 69% satisfactory, in 23% unsatisfactory. In the control group, there were no changes in the severity of symptoms (p = 0.858). Also, in the control group, the need for medications remained unchanged and 14.3% of patients continued to use corticosteroids.After ASIT, there was a decrease in the level of IL-4 (p = 0.002), and a decrease in the ratio of IL-4/ IL-10 (p = 0.0063); at the same time, changes in the level of other cytokines (IL-10; IFNγ) were statistically insignificant (p > 0.05). Before treatment, the levels of IL-4/ IL-10 in both groups were comparable, and after treatment, the differences became statistically significant (p = 0.031). We did not get a statistically significant change in the level of IgG4 Amb a 1 or IgG4 Amb trifida. There was no correlation between the level of individual cytokines and the results of treatment. As a result of the conducted ASIT, positive clinical and immunological results were obtained. In most patients, the disease has acquired a controlled course. At the same time, the lack of excellent and low number of good results of ASIT is probably due to the intraspecific allergenic properties of ragweed.

Objectives to reveal cause-and-effect relationships in the system "air pollution morbidity of the population" in a large industrial city. Material and methods.The dynamics of the pollutants concentration, such as formaldehyde and benzopyrene, through the period of 20152017 was analyzed in relation to the indicators of the morbidity of the respiratory system in the population of Samara. In particular, the study focused on the ratio of allergic rhinitis and asthma cases and the concentration of the formaldehyde and benzopyrene. Results.The incidence of respiratory diseases and the general morbidity increased both among children (by 46.7%) and adults (by 11.4%). There was the causal correlation between the effect of benzopyrene on the development of allergic rhinitis and bronchial asthma in children(r = 0.57 and r = 0.76) and in adults (r = 0.45 and r = 0.51) in Samara. The significant differences between the level of the pollutants and the frequency of the respiratory system diseases were registered in children and adults in different areas of Samara. Discussion.As a result of the increase of the maximum allowable concentration (MAC) of formaldehyde in atmospheric air, this pollutant has now ceased to be a priority. The revealed negative correlations point to the absence of an adverse effect of formaldehyde on the development of allergic rhinitis and bronchial asthma in both children and adults in Samara. Conclusion.According to the results of the study, the preventive measures aimed at reducing the impact of main air pollutants on the health of children and adults in Samara were suggested. Their implementation is possible by the immediate informing of the executive authorities to ensure they can make timely management decisions to improve the quality of the environment.

The aim: To study the dynamics of the level of 25(ОН)D, ІL-4, ІL-10, and IgG in the blood serum of children with allergic diseases and to study the clinical effect of vitamin D3 administration n different dosage in this category of patients. Materials and methods: 153 children aged 3-16 with such allergic diseases as bronchial asthma, atopic dermatitis and allergic rhinitis have been examined. The level of 25(ОН) D was determined using the electrochemiluminescence method, while the levels of ІL-4, ІL-10 and IgG were assessed using enzyme-linked immunoassay. Results: In the contrasting of the initial level of 25(ОН)D in the blood serum of patients after administration of 2,000 IU of vitamin D3 over 2 months, after summer and after treatment with cholecalciferol in higher doses (4,000–5,000 IU) over 2 months, significant difference was established between the indicators by the Friedman criterion (λ2 = 41.211; P < 0.05). In the similar contrasting of ІL-4 indicators, a significant difference between them was traced (P < 0.05) in the period of acute disease as well as the downward tendency in the period of remission. In the similar contrasting of ІL-10 indicators, a significant difference between them was traced (P < 0.05) in the acute period and in the period of disease remission. In the similar contrasting of IgG indicators, a downward tendency was traced in the period of acute disease and significant decrease (P < 0.05) – in the period of disease remission. In the contrasting of 25(ОН)D and ІL-4, ІL-10 figures a strong reverse correlation relationship was traced. The therapeutic effect of the administration of vitamin D3 medication in different doses in children with allergic diseases was traced. Conclusions: The data obtained shows that in the treatment of children with bronchial asthma, allergic rhinitis and atopic dermatitis the complex therapy should include vitamin D3 medications in different doses within a long-term course of treatment.
