Background: Although aflatoxin exposure has been associated with micronutrient deficiency in animals, there are few investigations on the effects of aflatoxin exposure on micronutrient metabolism in humans. Objective: To examine the relationship between aflatoxin B1 (AFB1) albumin adducts (AF-ALB) in plasma and the aflatoxin M1 (AFM1) metabolite in urine and plasma concentrations of retinol (vitamin A) and alpha-tocopherol (vitamin E) in Ghanaians. Methods: A cross-sectional study of 147 adult participants was conducted. Blood and urine samples were tested for aflatoxin and vitamins A and E levels. Results: Multivariable analysis showed that participants with high AF-ALB (>= 0.80 pmol/mg albumin) had increased odds of having vitamin A deficiency compared to those with lower AF-ALB [Odds Ratio (OR) = 2.61; CI = 1.03 - 6.58; p = 0.04]. Participants with high AF-ALB also showed increased odds of having vitamin E deficiency but this was not statistically significant (OR = 2.4; CI = 0.96 - 6.05; p = 0.06). Conversely, those with higher AFM1 values had a statistically nonsignificant reduced odds of having vitamin A deficiency (OR = 0.31; CI = 0.09 - 1.02; p = 0.05) and a statistically significant reduced odds of having vitamin E deficiency (OR = 0.31; CI = 0.10 - 0.97; p = 0.04). Participants with high AF-ALB or high AFM1 (>= 437.95 pg/dL creatinine) were almost 6 times more likely to be hepatitis B virus surface antigen (HBsAg)-positive (OR = 5.88; CI = 1.71 - 20.14; p = 0.005) and (OR = 5.84; CI = 1.15 - 29.54; p = 0.03) respectively. Conclusions: These data indicate that aflatoxin may modify plasma micronutrient status. Thus, preventing aflatoxin exposure may reduce vitamin A and E deficiencies.

Background: College students may have risk of fat-soluble vitamins deficiencies due to unhealthy dietary habits, especially for vitamin A and E. They are important members in the human antioxidant network, deficiencies of these vitamins may increase risk of many critical diseases. Objective: The current study was undertaken to determine the status of vitamin A and E in college students. Methods: Healthy college students were recruited, and fasting blood samples of them were collected and used for determining serum levels of retinol and α-tocopherol by the HPLC method. Results: We found that there was no vitamin A deficiency in college students. However, vitamin E deficiency existed in 34.5% of college students, especially in males. All the students had no vitamin E adequacy. In addition, our findings showed that BMI was inversely associated with serum α-tocopherol, but not serum retinol. Conclusion: These results suggest that vitamin E deficiency in college students should be given more attention, and it is necessary to consider using vitamin E supplements.

Abstract Objective To test the association between serum vitamin A and vitamin E and the prevalence of recurrent respiratory tract infections (RRTIs) of children and adolescents. Method A total amount of 5780 children and adolescents aged 2 to 17 were involved, who were classified into:A) Control group ;B) Diagnosed as RRTI with RTI symptoms; C) With RRTI but currently no RTI symptoms being noticed, and D) Not RRTI but showing respiratory disease symptoms. The correlation between serum vitamin A/E level and the presence of RRTIs were analyzed. Results Comparing with the prevalence of RRTI in children with normal vitamin A level, the Odds Ratio(OR) of vitamin A deficiency for RRTIs with symptoms was 8.32 (95%CI: 6.15 ~ 11.27), while the OR of marginal vitamin A deficiency was 1.30 (95%CI: 1.10 ~ 1.55). The OR of vitamin A deficiency or marginal vitamin A deficiency for asymptomatic RRTIs were 1.52 (95%CI: 1.00 ~ 2.32) and 1.30 (95%CI: 1.10 ~ 1.55) respectively, while the OR of vitamin A deficiency or marginal vitamin A deficiency for other respiratory diseases were 7.09 (95%CI: 5.37 ~ 9.37) and 1.60 (95%CI: 1.38 ~ 1.86). Whereas, the OR of vitamin E deficiency or marginal vitamin E deficiency for RRTIs or normal respiratory infections were showed without statistical significance. Conclusion The low level of vitamin A in serum is a risk factor for children and adolescents getting RRTIs or RTIs, indicating a possibly regulatory role of vitamin A in the respiratory diseases. In comparison, the vitamin E deficiency seems to have a weaker but still positive effect on the RRTIs or RTI

Effect of vitamin E supplements on liveweight gain and wool production, and their effectiveness in increasing plasma alpha -tocopherol concentrations and preventing nutritional myopathy was investigated. Commercial preparations were compared in pen and grazing experiments in the first part of the study. In a pen experiment, 3 different treatments, intramuscular oily injection, oral drench and dried supplement added to feed, were compared; in grazing sheep, intramuscular oily injection and oral drench were compared. The intramuscular oily injection was slow to increase the plasma concentrations of alpha -tocopherol in pen experiments and did not prevent vitamin E deficiency and development of subclinical myopathy in grazing experiments. Oral treatments increased plasma alpha -tocopherol concentrations in the pen experiment; though repeated oral drench did not increase plasma alpha -tocopherol in the grazing experiment, the development of subclinical nutritional myopathy was prevented. Aqueous preparations of vitamin E and the effect of selenium supplements in grazing sheep were studied in the second part of the study. Aqueous injections of 2000 mg of vitamin E acetate by intramuscular and subcutaneous routes in December and February were successful in rapidly increasing and maintaining plasma alpha -tocopherol concentrations in grazing weaner sheep. Injections of aqueous emulsions of vitamin E acetate could be the most useful preventative treatment for nutritional myopathy if problems of tissue damage can be overcome. None of the vitamin E supplements increased liveweight gain in any of the experiments, and there was no increase in wool quantity or quality in any of the grazing experiments. When selenium and vitamin E supplementation were compared, selenium-supplemented sheep had greater wool length and fibre diameter over summer-autumn than vitamin E-treated or control sheep. Vitamin E supplements are expensive; unless flocks are susceptible to vitamin E-responsive myopathy, it is suggested that there is little economic justification for using vitamin E as a supplement over the summer.

Rationale: Cystic fibrosis (CF), caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene, leads to impaired pancreatic function and therefore reduced intestinal absorption of lipids and fat-soluble vitamins especially in patients with CF developing pancreatic insufficiency (PI). Previous studies showed that CFTR modulator therapy with lumacaftor-ivacaftor (LUM/IVA) in Phe508del-homozygous patients with CF results in improvement of pulmonary disease and thriving. However, the effects of LUM/IVA on plasma concentration of the lipid soluble vitamins A and E remain unknown. Objectives: To investigate the course of plasma vitamin A and E in patients with CF under LUM/IVA therapy. Methods: Data from annual follow-up examinations of patients with CF were obtained to assess clinical outcomes including pulmonary function status, body mass index (BMI), and clinical chemistry as well as fat-soluble vitamins in Phe508del-homozygous CF patients before initiation and during LUM/IVA therapy. Results: Patients with CF receiving LUM/IVA improved substantially, including improvement in pulmonary inflammation, associated with a decrease in blood immunoglobulin G (IgG) from 9.4 to 8.2 g/L after two years (p < 0.001). During the same time, plasma vitamin A increased significantly from 1.2 to 1.6 µmol/L (p < 0.05), however, levels above the upper limit of normal were not detected in any of the patients. In contrast, plasma vitamin E as vitamin E/cholesterol ratio decreased moderately over the same time from 6.2 to 5.5 µmol/L (p < 0.01). Conclusions: CFTR modulator therapy with LUM/IVA alters concentrations of vitamins A and vitamin E in plasma. The increase of vitamin A must be monitored critically to avoid hypervitaminosis A in patients with CF.

Background: Like many developing countries, Senegal does not have data on the extent of vitamin A deficiency (VAD) that is representative of its population. The present survey was conducted to fill this gap and to identify factors associated with VAD, prior to the introduction of a large-scale vitamin A oil fortification program. Procedures: A nationwide representative cross-sectional survey involving 1887 children 12 to 59 months old and 1316 women of reproductive age (WRA) was conducted. Blood samples were collected and plasma concentrations of retinol (PR), C-reactive protein (CRP), and alpha-1-acidglycoprotein were measured. PR was adjusted for subclinical inflammation using the BRINDA regression methodology. Multivariate logistic regression was used to identify factors associated with VAD. Findings: The adjusted prevalence of VAD (PR ≤ 0.7 μmol/L) in children was 15.3% and differed by age group, area of residence, and socioeconomic status and half of them had subclinical inflammation. Among WRA, VAD was low (2.3%) and 18.1% had vitamin A insufficiency (VAI). Pregnant women were more affected by VAI (28.4%) and Dakar had lower figures compared with other cities and rural strata. Prevalence of VAI decreased with increasing wealth quintile. In logistic regression, abnormal CRP, poverty, scarce consumption of poultry, oysters, melon, red palm oil, palm kernel oil, Saba senegalensis fruit pulp (Maad) and cowpea, frequent consumption of leeks and consumption of Leptadenia hastata leaves (Mbuum tiakhat), were associated with VAD in children. For women, lower socioeconomic status, fair or poor health status and anemia were negatively associated with VAI. Conclusions: In Senegal, VAD is a moderate public health problem in children and slight among women. Particular attention should be paid to children older than 23 months, pregnant women, rural populations, and poorest households. Nutritional interventions should be implemented alongside morbidity prevention and control. Keywords: vitamin A deficiency, children 12-59 months, women of reproductive age, Senegal.

 Background: Family conscious nutrition (Kadarzi) is an Indonesia government program that aims to address the nutrition problem. The family expected to be able to independently giving the best nutrition to improve health. Each Kadarzi indicator has an important role to resolve and prevent the occurrence of utrition problem as indicator of vitamin A supplementation is used to resolve vitamin A deficiency.  Nutrition problems can be seen by using nutritional status. Children under five years are age-prone to experiencenutritional problem such as underweight, stunting, and wasting. Objective: The purpose of this study was to analyze the relationship between the application of Kadarzi behavior with the nutritional status of children under five (WAZ, HAZ, and WHZ). Method: The research was an analytic obsevation research, using cross sectional design. The sample consist of 72 toddlers in Salakkembang Village, Kalidawir Subdistrict, Tulungagung Regency. The data were collected by using weight and height measurement, also Kadarzi bahavior questionnaire. The data were analyzed using Fisher Exact test. Result: The results of the study showed that there was a correlation between Kadarzi behavior with nutitional status of toddlers based on WAZ (p=0.010), and HAZ (p=0.000) but not with WHZ (p=0.368). Conclusion: The better apllication of Kadarzi behavior, the better the nutritional status of toddlers WAZ and HAZ. Mothers should apply Kadarzi behavior to prevent toddlers from nutritional problems. ABSTRAK Latar belakang: Keluarga Sadar Gizi (Kadarzi) merupakan program pemerintah Indonesia yang bertujuan mengatasi masalah gizi. Keluarga diharapkan dapat secara mandiri mewujudkan keadaan gizi yang sebaik-baiknya untuk meningkatkan kesehatan. Setiap indikator Kadarzi memiliki peran penting untuk mengatasi dan mencegah terjadinya masalah gizi seperti indikator pemberian suplemen vitamin A digunakan untuk mengatasi kekurangan vitamin A. Masalah gizi dapat dilihat salah satunya dengan menggunakan status gizi. Balita merupakan usia yang rawan mengalami masalah gizi seperti underweight, stunting, dan wasting.Tujuan: Tujuan dari penelitian ini adalah menganalisis hubungan antara penerapan perilaku Kadarzi dengan status gizi balita (BB/U, TB/U, dan BB/TB).Metode: Penelitian ini merupakan penelitian observasinol analitik, dengan desain penelitian cross sectional. Sampel terdiri dari 72 balita di Desa Salakkembang, Kecamatan Kalidawir, Kabupaten Tulungagung. Pengumpulan data menggunakan penimbangan berat badan, tinggi badan, dan kuesioner perilaku Kadarzi. Data dianalisis menggunakan Fisher Exact test. Hasil: Hasil dari penelitian menunjukkan terhadap hubungan antara perilaku Kadarzi dengan status gizi balita BB/U (p=0,010) dan TB/U (p=0,000) namun tidak dengan BB/TB (p=0,368).Kesimpulan: Semakin baik penerapan perilaku Kadarzi maka semakin baik status gizi balita BB/U dan TB/U. Ibu dan seluruh anggota keluarga seharusnya menerapkan perilaku Kadarzi agar balita terhindar dari masalah gizi.

Background: To assess the awareness about breast feeding practices and to assess the knowledge, attitude and practice about the signs and symptoms of vitamin A deficiency (VAD) and vitamin A rich foods intake among mothers of children below 5 years in the Srikakulam district of Andhra Pradesh (AP) in South India.Methods: A population based cross-sectional study was conducted on 284 mothers (age range 14 to 36 years) during June 2011 and September 2011. A combined simple and systematic random sampling strategy was used to select mothers from one tribal, rural and semi urban area with a representative sample of the Srikakulam district. A standardized structured questionnaire that was developed and validated by the National institute of nutrition, Hyderabad was utilized for this study.Results: The awareness of vitamin A and night blindness was 68.7% (95% confidence intervals (CI): 62.9–74.0) (n=195) and 93.7% (95% CI: 90.2–96.2) (n=266) respectively. The knowledge of signs and symptoms of vitamin A deficiency (VAD) was nil among illiterate mothers and low among literates; with primary education, adjusted odds ratio (OR): 0.01 (95% CI: 0.00, 0.06) and with secondary education, OR: 0.19 (95% CI: 0.06, 0.62) compared to mothers with graduation and above.Conclusions: There is a need to increase the awareness and knowledge about VAD among mothers with children below 5 years in the southern state of AP.

1. Changes in total retinol-binding protein (RBP), the holoprotein (holoRBP) and prealbumin (PA) concentrations have been monitored in plasma of thirty protein- and vitamin A-deficient preschool children from within a few hours up to 7 weeks after treatment with retinol and a good-quality protein diet.2. The children were classified into groups according to nutritional status as having either kwashiorkor, marasmus-kwashiorkor or marasmus, and given formula diets whose protein and energy contents increased stepwise from 1 g and 105 kJ/kg body-weight respectively up to 4 g and 733 kJ/kg body-weight after 4 weeks. Retinol was administered in the forms of retinyl palmitate either orally or intramuscularly.3. PA and total RBP were determined by electroimmunoassay procedures and the holoRBP by its fluorescence after separation from other plasma proteins.4. RBP in plasma of the vitamin A-deficient child is largely denatured and incapable of binding administered retinol, which must first be taken up by the liver before native holoRBP is released. An increased pool of native apoprotein accumulates in the liver during vitamin A deficiency which is released into plasma quickly after retinol uptake to form peak concentrations of total and holoRBP approximately 3 h after dosing intramuscularly and 6 h orally.5. The accumulated pool of RBP was highest in livers from the marasmus group and lowest in those from the kwashiorkor group, reflecting their relative capacities to synthesize plasma proteins.6. The mean plasma concentrations of total and holoRBP for the various groups were minimal 24–48 h after dosing with retinol and then improved almost linearly over the following week.7. Mean plasma PA concentrations of the various groups on admission were also in order of the severity of their malnutrition. There was little or no change in this protein concentration over the first 24 h after dosing with retinol, but thereafter the mean values rose almost linearly over 2 weeks. Albumin on the other hand changed little during the first week. The results show that PA is the more sensitive measurement of protein nutritional status.

Abstract Background: Vitamin A is a fat-soluble vitamin essential for the proper functioning of the immune system. It fondes both animal and plant sources in the form of retinol from animal sources or beta-carotene from plant source foods. Due to increased nutrient need and the severity of the potential health consequences are associated with vitamin A deficiency (VAD), preschool children and pregnant women are considered as the most at risk segments of the community. Hence, this study aimed to assess the prevalence of VAD and associated factors among children aged 6-59 months in Dera woreda, Northwest Ethiopia. Methods: Community based quantitative cross-sectional study was conducted among children aged 6-59 months in Dera woreda from December 1/2018 to January 1/2019. Multistage random sampling was used to select the required samples (267). Data were coded and entered into Epi-data version 3.1 and exported to SPSS (Statistical Package for Social Sciences) version 20 for analysis. Binary logistic regression was fitted. Both bivariable and multivariable analyses were used to identify factors associated with VAD. Those variables that were showed significant association in multivariable association at P value less than 0.05 declared as statistically significant factors of VAD among children age 6-59 months. Results: Two hundred sixty seven mother-children Pairs were included in the study making a response rate of 94.7%. The overall prevalence of Vitamin A deficiency among children age 6-59 months was 7.8% (95% CI (4.7, 11.3). Antenatal care follow up (AOR: 0.446,95 % CI (0.155, 0.980)); birth interval within 2 year (AOR: 0.392, 95 % CI (0.107, 0.839)), and age group of24-35 months (AOR; 1.206, 95 % CI (1.037, 6.129)) were significantly associated with vitamin-A deficiency. Conclusion: The overall prevalence of vitamin A deficiency in this study was found to be lower than the national and regional prevalence. According to this study, VAD is a mild public health problem. Age of children, birth interval, ANC follow up, remain the most important associated factors for vitamin A deficiency. Therefore to decrease the prevalence of VAD, ANC service should be strengthened and we should focus on older children birth interval and age- based food needs.
