Knee osteoarthritis (OA) is a highly prevalent chronic degenerative joint disease that mainly affects the elderly population. The aim of this study was to investigate serum signature metabolites as potential biomarkers for early diagnosis of knee OA. Global serum metabolic profiles of 40 patients with knee OA and 20 healthy controls (HC) were analyzed by ultra-performance liquid chromatography coupled to mass spectrometry. An OA-specific metabolic profile was established that can clearly discriminate patients with OA from HCs. Fourteen metabolites that are involved in the metabolism of amino acids, purine, energy, glycolysis, fatty acids, and lipids were significantly altered in patients with OA compared to HCs. These metabolites could be potentially used as biomarkers for the diagnosis of knee OA.

AbstractPurposeKnee osteoarthritis (OA) is a common type of degenerative joint disease which decreases the quality of life. Sex-determining region Y box 9 (SOX9) and hypoxia-inducible factor-1 (HIF1) are considered as the key regulators of OA. We investigated the effect of combined therapies with mesenchymal stem cells (MSCs), ozone (O3) and exercise training on SOX9 and HIF1 expression in the cartilage of rats with knee OA.MethodsKnee OA was induced by surgical method. OA rats were divided into model, MSCs, ozone, exercise, MSCs + ozone, MSCs + exercise, ozone + exercise and MSCs + ozone + exercise groups. Rats in the MSCs group received intraarticular injection of 1 × 106 cells/kg. Rats in the ozone group received O3 at the concentration of 20 μg/mL, once weekly for 3 weeks. Rats in the exercise group were trained on rodent treadmill three times per week. 48 hours after the programs, cartilage tissues were isolated and the expression of SOX9 and HIF1 was determined using Real-Time PCR.ResultsSignificant differences were found in the expression of SOX9 and HIF1 between groups (P < 0.0001). Although combined therapies with exercise, MSCs and O3 significantly increased the expression of SOX9 and HIF1 in the cartilage tissue of rats with knee OA, combination of exercise with O3 was significantly more effective compared to the other combined therapies (P < 0.001).ConclusionsCombined therapy with exercise, MSCs and O3 significantly increased the expression of SOX9 and HIF1 genes in the cartilage of rats with knee OA; however, exercise + O3 was significantly more effective.

It is well established that obesity is related to osteoarthritis of the knees. The aim of this study was to investigate the risk of knee osteoarthritis in relation to body mass index (BMI) and specifically, to assess the risk of knee osteoarthritis (OA) attributable to obesity. We also aimed to determine if there is a higher risk with an increase in BMI. This is an observational case-referent study of patients with knee pain. Patients were recruited from the orthopaedics clinic. The cases were patients that presented with radiologically diagnosed knee osteoarthritis, while the referents were other patients that also attended the orthopedic clinic at the same time. The referents were randomly selected. Body Mass Index was calculated and classified based on the WHO international classification. Data were analyzed using SPSS version 21. A total of 152 patients were included in the study and 67.1% were females while 32.9% were males giving a ratio of 2:1. The mean age was 57.72+/-12.5 years, the mean weight was 79.47+/- 13.41kg (95% CI 77.32 – 81.62) and the mean BMI was 27.80+/-6.2kg/m2. 56.6% had knee osteoarthritis. 8.6% (n = 13) were bilateral, 18.4% (n = 28) in the left knee and 29.6% (n = 45) in the right knee. The relationship between obesity and the presence of knee osteoarthritis was statistically significant (p 0.047). A Chi-squared test for trend also did not show an increased risk for knee osteoarthritis with increasing BMI (p 0.105). In conclusion, knee osteoarthritis is a degenerative joint disease that mostly affects the elderly. Our findings give strong support to the already existing knowledge of the positive relationship between obesity and knee osteoarthritis; however, increasing BMI did not increase the risk of knee OA.

Knee osteoarthritis (OA) is a degenerative knee joint disease that results from the breakdown of joint cartilage and underlying bone, affecting about 3.3% of the world's population. As OA is a multifactorial disease, the underlying pathological process is closely associated with genetic changes in articular cartilage and bone. Many studies have focused on the role of small noncoding RNAs in OA and identified numbers of microRNAs that play important roles in regulating bone and cartilage homeostasis. The connection between other types of small noncoding RNAs, especially tRNA-derived fragments and knee osteoarthritis is still elusive. The observation that there is limited information about small RNAs different than miRNAs in knee OA was very surprising to us, especially given the fact that tRNA fragments are known to participate in a plethora of human diseases and a portion of them are even more abundant than miRNAs. Inspired by these findings, in this review we have summarized the possible involvement of microRNAs and tRNA-derived fragments in the pathology of knee osteoarthritis.

 Objective: To determine how frequent NSAIDs are used for osteoarthritic patients because OA is common old age patients and NSAIDs can cause divesting complications on their health. In our study we tried to identify the main reasons and suggest a best possible solution. Study Design: Community Based Cross-sectional study. Setting: Mohammad Medical College and Hospital Mirpurkhas Pakistan. Period:  June 2018 to August 2018. Material & Methods: In this study, we included more than 300 prescriptions written for Osteoarthritis, a degenerative joint disease. Prescriptions collected from hospital pharmacy (80%) and from other pharmacies (20%). Data was collected on preformed proforma and was analyzed in SPSS version 25. Results: Osteoarthritis mainly affects elderly population. NSAIDs are used for pain relief but can cause acute renal failure and GIT bleeding ulcers. In our study 69% patients were prescribed double NSAIDs. Traditional NSAIDs were used 95% of the time and only 5% percent of the time selective cox 2 were used. Along with these NSAIDS only 20% patient were given prophylactically gastroprotective agents like PPI and other medications. Conclusion: Prescription osteoarthritis include multiple NSAID without any protocol and over dosage was observed commonly.

Abstract Background Knee osteoarthritis is a joint disease which is characterized by degeneration of articular cartilage and subsequent subchondral bone changes. Polymorphisms of IL-17A/F gene were the recognized candidate genes associated with knee osteoarthritis risk although the results were conflicting. The aim of this study was to determine whether IL-17A(rs2275913) and IL-17F(rs763780) polymorphisms confer susceptibility to knee osteoarthritis. Method Literature search was performed in PubMed, Medline, Cochrane Library, Web of science, Embase, and Google Scholar (last search was updated on June 20, 2019), and assessing this association was performed by calculating odds ratios with 95% confidence intervals. Statistical heterogeneity was quantitatively evaluated by using the Q statistic with its p value and I2 statistic. Result Six case-control based studies were included involving IL-17A(rs2275913) (2134 cases and 2306 controls) and IL-17F(rs763780) (2134 cases and 2426 controls). The overall analysis suggested that the A allele of the rs2275913 polymorphism, and the C allele of the rs763780 polymorphism in the IL-17 gene may increase the risk of OA. However, subgroup analysis revealed that no association between IL-17A(rs2275913) gene and knee OA risk was found in Caucasian population. Conclusions This meta-analysis revealed that the IL-17A(rs2275913) gene polymorphisms may increase the risk of knee OA in Asians, and the IL-17F(rs763780) gene polymorphisms may increase the risk of knee OA both in Asians and Caucasians. However, because of the limitations of the present study, additional larger studies are needed to confirm our findings in the future.

Osteoarthritis (OA) is a progressive joint disease driven by a blend of inflammatory and biomechanical processes. Studies using human samples to understand inflammatory mechanisms in OA frequently recruit OA patients with different affected joints, even though recent evidence indicates that OA is a heterogeneous disease which only culminates in a common end point. Differences in age of onset and the dynamics of disease progression suggest that different joints may represent different disease entities, thereby diluting the discovery potential in a combined analysis. We hypothesized that different OA joints may also differ in immunopathology within the synovium. To investigate this hypothesis, we profiled the immune cell contribution (flow cytometry) and cytokine release profiles (ELISA) in purified synovial membrane mononuclear cells from 50 patients undergoing either hip (n = 34) or knee (n = 16) replacement surgery. Unsupervised computational approaches were used for disease deconstruction. We found that hip and knee osteoarthritis are not identical in respect to the inflammatory processes that take place in the synovial membrane. Instead, we report that principally CD14+ macrophages are expanded fourfold in the synovial membrane of patients with knee OA compared to hip OA, with a trend to higher expression in CD8+ T cells, while CD4+ T cells, B cells, and NK cells were found at comparable quantities. Upon isolation and culture of cells from synovial membrane, isolates from hip OA released higher concentrations of Eotaxin (CCL11), G-CSF, GM-CSF, INF-γ, IP-10 (CXCL10), TNF-α, MIP-1α (CCL3), MIP-1β (CCL4), IL-4, IL-10, IL-17, and lower concentrations of stem cell factor (SCF), thereby highlighting the difference in the nature of hip and knee osteoarthritis. Taken together, this study establishes hip and knee OA as immunologically distinct types of OA, and creates a resource of the cytokine expression landscape and mononuclear cell infiltration pattern of patients with hip and knee osteoarthritis.

Abstract Background Chronic knee osteoarthritis (OA) is a frequent disease among the elderly. Intra-articular corticosteroid injection (IACSI) was commonly adopted to alleviate knee OA-related pains. Recently, Genicular nerve block (GNB) has emerged as a new alternative technique. The current study aimed to investigate the efficacy of those two approaches when guided with ultrasound and to determine which one offers better results. This study included 40 patients with painful chronic knee OA (Nine males and 31 females, age ranged from 44 to 65 years) and were randomly assigned to two equal groups. Groups 1 was managed with ultrasound-guided IACSI and group 2 with ultrasound-guided GNB using a mixture of lidocaine and Triamcinolone Acetonide. The baseline mean visual analogue scale (VAS) and Oxford knee score (OKS) for the group 1 were 87.10 and 51.3, while for group 2 were 87.75 and 53.25 respectively. Follow up values were obtained at 2, 4 and 8 weeks using VAS and OKS. Results The VAS score and OKS score were significantly lower in the GNB group and IACSI group at 2, and 4 weeks after the procedure (p < 0.001 for all), then returned near baseline values at 8 weeks. When the two groups were compared according to changes in VAS and OKS from baseline at 2, 4, and 8 weeks, GNB group showed significant alleviation of pain (mean reduction of 58.5, 53.3, and 9.25 points at 2, 4, and 8 weeks versus 44.9, 39.4, and 5.6 points at the IACSI groups, p < 0.001 at 2 and 4 weeks, p < 0.006 at 8 weeks). Similarly at the OKS, the GNB group showed significantly better results (33.50, 28.60, and 8.5 at GNB and 26.45, 20.10, 5.25 at IACSI. p < 0.001 at all periods). Conclusions Both GNB and IACSI are effective methods to relieve chronic knee osteoarthritis-related pains. When compared to each other, GNB showed more significant pain relief and functional improvement than IACSI.

Background: Osteoarthritis (OA) of the knee is among the most common disabling diseases which may cause pain and decrease in functional status. It is the commonest form of arthritis and is most prevalent in the elderly, with 50% of adults aged 65-75 years and almost 70% of those 75+ years suffer from this disease. The aim of this study was to investigate consistency of radiologic findings with histomorphologic structure of bone in patients with severe gonarthrosis.Methods: 62 knees of 57 patients over 60 years old who had stage 3-4 gonarthrosis according to Kellgren-Lawrence classification were included in the study. Patients were separated into two groups as having stage 3 or stage 4 gonarthrosis. All the patients underwent total knee replacement procedure. During the operation distal femoral medial/lateral condyle and proximal tibial medial/lateral plateau were removed and sent to histologic examination for the measurement of thickness of cartilage layer and subchondral bone, number and thickness of trabeculae, space between two trabeculae.Results: Average thickness of subchondral bone was measured at stage 3 gonarthrosis and at stage 4 gonarthrosis. Only the difference between medial tibial condyle values of two groups was statistically significant. Average trabecula thickness was measured both at stage 3 and at stage 4 gonarthrosis. Only the difference between lateral tibial condyle values of two groups was statistically significant. Furthermore, as for the number of trabeculas and cavity between trabeculae, a significant difference couldn’t be found.Conclusions: Despite having radiological differences two groups can be said to show similar histopathological characteristics.

Osteoarthritis is a degenerative joint disease which is related with the damage of cartilage. Xray is one of the osteoarthritis radiological examinations but only to see the damage of bone not cartilage. One of an effective examination to visualize the damage of cartilage is Ultrasound. The objective of this study is to determine the relation between osteoarthritis grading scale with cartilage ultrasonographic in knee osteoarthritis patients. This was a Cross-sectional study on 32 knee osteoarthritis patients. Statistical analysis was performed using Kolmogorov-Smirnov Test showed there was a correlation between osteoarthritis grading scale with cartilage ultrasonographic. The conclusion in this study, there is a significant correlation between osteoarthritis grading scale with cartilage ultrasonographic in knee osteoarthritis patients.


Osteoarthritis (OA) is a chronic degenerative joint disease that doubled in prevalence since the mid of 20th century most commonly due to obesity and aging. Osteoarthritis can affect any joint in the body. The pathogenesis of OA is multifactorial influenced by range of biochemical and mechanical factors. Oxidative stress is described to play an important role in many diseases including OA. Accumulating evidences suggested the beneficial effect of anti-oxidants for reducing OA severity. Curcumin is a well-known antioxidant agent that acts by different mechanisms in modulating oxidative stress status. This study was designed to evaluate the antioxidant effect of curcumin as adjuvent therapy to a non-steroidal anti-inflammatory drug, meloxicam, in the management of knee osteoarthritis. This prospective open-labelled randomized controlled study was carried out on forty-two eligible patients who were allocated in two groups, serum superoxide dismutase 3 (SOD3) and glutathione reductase (GR) were measured at baseline and after 3 months of the study. Pain and physical function assessment were evaluated by oxford knee score (OKS). Results illustrated highly significant improvement in pain and physical function scores when curcumin used as adjuvant to meloxicam, also curcumin supplementation resulted in significant increase in SOD3 serum level and only a modest decrease in GR serum level when compared to meloxicam alone. In conclusion, this study demonstrated benefit of curcumin when used in combination with meloxicam over using meloxicam alone in modulating antioxidant parameters in blood, in addition to significantly improving pain and physical function after 3 months of treatment.
