Abstract 101: Is Intracerebral Hemorrhage-Associated Ischemia a Consequence of Blood Pressure Lowering?

Introduction: The cause of (presumed) ischemic lesions associated with intracerebral hemorrhage (ICH) is poorly understood. We investigated the relationship between BP lowering and the incidence of ipsilateral diffusion weighted imaging (DWI) lesions in a prospective ICH cohort. Methods: We prospectively enrolled consecutive ICH patients in the NIH-funded DiAgnostic Utility of MRI in Spontaneous Intracerebral Hemorrhage (DASH) study. Two neuroradiologists reviewed the MRIs for evidence of ischemia, defined as: reduced diffusivity ipsilateral to the ICH without evidence of blood products on FLAIR or GRE. Only DWI lesions attributed to tissue compression; vessel compression; or hypoperfusion were included. Patients with post-operative MRIs or insufficient BP data were excluded. Mean arterial blood pressures (MAP) were recorded on admission, and at 6, 12, 18, and 24 hours. Chi-square and t-tests were used as appropriate. Receiver operator characteristic (ROC) curves were created to assess accuracy of predicting DWI lesions. Results: Of 160 patients, 136 met inclusion criteria (median age: 63 (IQR 50-77); median ICH volume: 10 (IQR 4-33cc); median NIHSS: 6 (IQR 2-16); median GCS: 15 (IQR 10-15); median onset to MRI 40 hrs (IQR 25-75). DWI lesions were observed in 78 (57%) patients. Patients with DWI lesions had higher ICH volumes (32 vs 12cc, p < 0.001); higher admission MAP (125 vs 113mmHg, p=0.006); higher maximal MAP reduction (46 vs 33mmHg, p=0.008); and higher mean %MAP reduction (25 vs 17% p=0.006). DWI lesions were not associated with lowest MAP (80 vs 79mmHg, p=0.97) or mean MAP (90 vs 91, p=0.62). ICH volume and maximum MAP reduction predicted DWI lesions with an area under curve (AUC) of 0.70 (95% CI: 0.61-0.78) and 0.63 (95% CI: 0.53-0.72) respectively. Controlling for ICH volume using logistic regression: for every 10% reduction in MAP the risk of DWI lesions increased substantially (OR 1.28, 95% CI: 1.01-1.62). Similarly, each 10% reduction in mean MAP over the first 24 hours had an increased risk of detecting DWI lesions (OR 1.3, 95% CI: 1.01-1.69). The likelihood of having a DWI lesion was highest in patients with > 30mmHg drop in MAP (OR 2.3, 95% CI: 1.09-4.6). In ICH < 10cc (N=70), DWI lesions were not associated with ICH volume (4.1 vs 4.8cc, p=0.40) but with higher admission MAP (125 vs 112mmHg, p=0.045); maximum MAP reduction (45 vs 31 mmHg, p=0.03); and maximum % MAP reduction (34 vs 25%, p=0.03). Conclusions: ICH volume and large BP reductions are both associated with the presence of DWI lesions. The likelihood of having a DWI lesion went up by 30% for each 10% drop in MAP from admission, and was 230% higher in patients with > 30 mmHg reduction in MAP. These data suggest that aggressive BP reduction may contribute to ICH associated ischemia, and that percentage-based BP goals may be more appropriate than “one-size fits all” for clinical trial design. Future studies are needed to clarify causation.

Aims: The objective of this review was to examine the relationship between osteoarthritis (OA) and mobility-related comorbidities, specifically diabetes mellitus (DM) and cardiovascular disease (CVD). It also investigated the relationship between OA and mortality. Methods: An overview of meta-analyses was conducted by performing two targeted searches from inception to June 2020. The association between OA and (i) DM or CVD ( via PubMed and Embase); and (ii) mortality ( via PubMed) was investigated. Meta-analyses were selected if they included studies that examined adults with OA at any site and reported associations between OA and DM, CVD, or mortality. Evidence was synthesized qualitatively. Results: Six meta-analyses met inclusion criteria. One meta-analysis of 20 studies demonstrated a statistically significant association between OA and DM, with pooled odds ratio of 1.41 (95% confidence interval: 1.21, 1.65; n = 1,040,175 patients). One meta-analysis of 15 studies demonstrated significantly increased risk of CVD among OA patients, with a pooled risk ratio of 1.24 (1.12, 1.37, n = 358,944 patients). Stratified by type of CVD, OA was shown to be associated with increased heart failure (HF) and ischemic heart disease (IHD) and reduced transient ischemic attack (TIA). There was no association reported for stroke or myocardial infarction (MI). Three meta-analyses did not find a significant association between OA (any site) and all-cause mortality. However, OA was found to be significantly associated with cardiovascular-related death across two meta-analyses. Conclusion: The identified meta-analyses reported significantly increased risk of both DM and CVD (particularly, HF and IHD) among OA patients. It was not possible to confirm consistent directional or causal relationships. OA was found to be associated with increased mortality, but mostly in relation to CVD-related mortality, suggesting that further study is warranted in this area.

Objectives. High levels of non high density lipoprotein cholesterol (non-HDL-C) are known to contribute to an increased risk of ischemic stroke. This systematic review was conducted to identify the relationship between high non-HDL-C levels and the risk of ischemic stroke. Material and methods. This systematic review using PRISMA (preferred reporting items for systematic reviews and meta-analyses) statement guideline, with a pre-determined search strategy. The search was conducted in Pubmed Central (PMC) and Pubmed from 2009 until 2020 with inclusion criteria, patients with a diagnosis of ischemic stroke, age >18 years, with non-HDL-C levels. The title and abstract of these articles were reviewed for relevance, based on inclusion criteria. This systematic review using STROBE to evaluate the quality in individual study which consists of 22 domains. Results. In the final stage, this systematic review identifies six cohort study consisting 166.720 participants. Overall, the quality in individual studies based on STROBE is good. We reported that there are two studies report that high non-HDL-C levels can significantly increase the risk of ischemic stroke. Meanwhile, there are four studies that report there was no significant relationship between high non-HDL-C levels and the risk of ischemic stroke. Conclusions. Overall, this systematic review provides the result about relationship between high levels of nonHDL-C and the risk of ischemic stroke, there are two studies with a larger population support that high levels of non-HDL-C can increase the risk of ischemic stroke. However, further studies is needed with a large population that specifically identify the relationship between non-HDL-C levels and risk of ischemic stroke and makes a better adjustments for confounding variables.

Bullying merupakan perilaku negatif yang dilakukan secara berulang-ulang oleh seseorang atau sekelompok orang yang bersifat menyerang karena adanya ketidakseimbangan kekuatan antara pihak yang terlibat.Tindakan bullying dapat berdampak buruk bagi korban maupun pelakunya di masa depannya. Dampak tersebut meliputi kesepian, pencapaian akademik yang buruk, kesulitan penyesuaian (adaptasi), meningkatnya risiko penggunaan zat, keterlibatan dalam tindakan kriminal dan kerentanan gangguan mental emosional seperti cemas, insomnia, penyalahgunaan zat, depresi, mempunyai self-esteem rendah, kesulitan interpersonal, gangguan konsep diri, dan depresi (ketidakberdayaan). Tujuan penelitian ini adalah untuk mengetahui hubungan antara konsep diri dan ketidakberdayaan dengan risiko bunuh diri pada remaja yang mengalami bullying di SMA Negeri 7 Kota Bogor. Penelitian ini merupakan penelitian kuantitatif dengan menggunakan metode cross-sectional study dengan bentuk pendekatan rancangan correlation study. Sampel dalam penelitian ini berjumlah 183 orang didapat dari teknik randomsampling. Hasil penelitian menunjukkan bahwa berdasarkan hasil uji hubungan dengan menggunakan Chi-Square antara konsep diri dengan ketidakberdayaan didapatkan nilai ρ (0,000) lebih kecil daripada nilai α (0,05), sedangkan uji hubungan antara konsep diri dengan risiko bunuh diri didapatkan nilai ρ (0,013) lebih kecil daripada nilai α (0,05). Dengan demikian bahwa ada hubungan antara konsep diri dan ketidakberdayaan dengan risiko bunuh diri pada remaja yang mengalami bullying di SMA Negeri 7 Kota Bogor. Kata kunci: bullying, konsep diri, ketidakberdayaan, risisko bunuh diri SELF-CONCEPT AND INEQUALITY CONNECTED WITH RISK OF SELF-SUFFICIENT IN ADOLESCENTS WHO HAVE BULLYING ABSTRACTBullying is a negative behavior that is carried out repeatedly by a person or group of people who are attacking because of an imbalance of power between the parties involved. Bullying actions can have a negative impact on victims and perpetrators in the future. These impacts include loneliness, poor academic achievement, adaptation difficulties, increased risk of substance use, involvement in criminal acts and susceptibility to mental emotional disorders such as anxiety, insomnia, substance abuse, depression, low self-esteem, interpersonal difficulties, disturbances self concept, and depression (helplessness). The purpose of this study was to determine the relationship between self-concept and powerlessness with the risk of suicide in adolescents who experienced bullying in SMA Negeri 7 Kota Bogor. This research is a quantitative study using a cross-sectional study method with a correlation study design approach. The sample in this study amounted to 183 people obtained from random sampling techniques. The results showed that based on the results of the relationship test using Chi-Square between self-concept and powerlessness, the value of ρ (0,000) was smaller than the value of α (0.05), while the test of the relationship between self-concept and risk of suicide obtained a value of ρ (0.013 ) is smaller than the value of α (0.05). Thus that there is a relationship between self-concept and powerlessness with the risk of suicide in adolescents who experience bullying in SMA Negeri 7 Kota Bogor. Keywords: bullying, self-concept, helplessness, suicide risk

ABSTRACT: THE CORRELATIONS OF AGE AND HYPERTENSION WITH THE OCCURRENCE OF BPH IN THE SURGICAL WARD AT RSUD DR.H.ABDUL MOELOEK IN 2020Introduction: Lower Urinary Tractus Symptoms (LUTS) is a problem that is experienced by men around the world and one that often occurs is Benign prostate hyperplasia (BPH). BPH is a histological disorder characterized by the proliferation of prostate cells. It is estimated that 50% of men show BPH histopathology at the age of 60 years old and an increase of 90% at the age of 80 years old. Hypertension is also known to have a role in increasing prostate volume, in a cohort study it was found that hypertension resulted in an increased risk of 1.5 times to cause LUTS/BPH.Objective: To determine the relationship between age and hypertension on the incidence of BPH in Dr. H. Abdul Moeloek in 2020.Methods: This study is quantitative research, an observative analytic study design with a cross-sectional approach was carried out at RSUD Dr. H. Abdul Moeloek which was taken on October 16, 2020. The population was all patients in the Surgical Ward with total sampling. Data collection was obtained from secondary data from medical records. Data analysis was performed Univariate analysis (frequency distribution) and bivariate analysis with chi-square.Results: Respondents with BPH aged >50 years old were 32 respondents (97%) and respondents with BPH and hypertension were 20 respondents (60.6%). The results of the bivariate analysis using chi-square showed a significant relationship between BPH and age p value=0.000 (P<0.05) and the relationship between BPH and hypertension with p value=0.000 (p<0.05).Conclusion: There is a significant relationship between BPH with age and hypertension with the occurrence of BPH in the Surgical polyclinic at RSUD Dr.H.Abdul Moeloek in 2020. Keywords: BPH, Age, Hypertension  INTISARI: HUBUNGAN USIA DAN HIPERTENSI TERHADAP KEJADIAN BPH DI RSUD Dr.H.ABDUL MOELOEK Pendahuluan: Lower Urinary Tractus Symptoms (LUTS) adalah masalah yang banyak dialami oleh laki-laki di seluruh dunia dan salah satu yang sering terjadi adalah Benigna Prostat Hyperplasia (BPH). BPH adalah kelainan histologis yang khas di tandai dengan proliferasi sel-sel prostat. Diperkirakan 50% laki-laki menunjukan histopatologi BPH pada umur 60 tahun dan meningkat 90% pada umur 80 tahun  Hipertensi juga diketahui memiliki peranan dalam peningkatan volume prostat yakni pada suatu penelitian cohort diketahui adanya hipertensi mengakibatkan peningkatan resiko sebanyak 1,5 kali untuk menimbulkan gejala LUTS/BPH.Tujuan: Untuk mengetahui adanya hubungan usia dan hipertensi terhadap kejadian BPH di RSUD Dr. H. Abdul Moeloek Tahun 2020Metode: Jenis penelitian kuantitatif, rancangan penelitian analitik observatif dengan pendekatan cross sectional telah dilakukan di RSUD Dr.H.Abdul Moeloek Provinsi Lampung yang berlangsung pada 16 Oktober 2020. Populasi adalah seluruh pasien di Poli Bedah dengan pengambilan sampel secara total sampling. Pengumpulan data diperoleh dari data sekunder yang diperoleh dari rekam medis. Analisis data secara univariat (distribusi frekuensi) dan uji bivariat mengunakan chi squareHasil: Responden dengan BPH yang berusia >50 tahun sebanyak 32 responden (97%) dan responden dengan BPH dengan hipertensi sebanyak 20 responden (60,6%). Hasil Uji bivariat menggunakan chi square menunjukkan adanya hubungan signifikan antara BPH dengan usia diperoleh nilai p=0,000 (P<0,05) dan hubungan BPH dengan hipertensi nilai p=0,000 (p<0,05).Kesimpulan: Terdapat hubungan signifikan antara BPH dengan usia dan terdapat hubungan sgnifikan antara BPH dengan hipertensi di poli klinik bedah RSUD Dr.H.Abdul Moeloek tahun 2020.Kata kunci: BPH, Usia, Hipertensi

Background: The knowledge of pregnant women about cosmetics' dangers is still shallow; this is because pregnant women mostly have never consulted a dermatologist regarding the cosmetic products they use. Aim: This study aimed to determine the relationship between pregnant women's knowledge about cosmetics and the use of facial cream. Method: This research applied an analytical survey with a cross-sectional approach. The study samples were 40 pregnant women taken from a population of 44 pregnant women who used facial cream with Slovin formula and a purposive sampling technique that met the inclusion criteria. The instrument used questionnaires. Results: Data analyzed by the Chi-Square test showed bivariate results (ρ = 0.001) <0.05 indicating that there were relationships between 2 variables. Conclusion: In conclusion, there was a relationship between maternal knowledge about cosmetics and the use of facial cream in the Bre Village of Palibelo Primary Health Center in 2017. Objective :To find out the relationship between the duration of MgS04 administration in severe preeclampsia patients and eclampsia with the incidence of hypermagnesium.Method :This study is a comparative observational study using a cross sectional study design. The study was conducted in the medical record of Dr.M.Djamil Padang Hospital during the period January 1, 2016 to August 31, 2017 of pregnant women with severe preeclampsia and eclampsia, found 30 study subjects who met the inclusion criteria. Furthermore, data is recorded in a research form that has been provided, then carried out research data processing. Statistical analysis to assess significance using the chi-square test.Results : Magnesium levels increased in pregnant women with severe preeclampsia and eclampsia by 21 correspondents. Increased Magnesium levels in patients with severe preeclampsia and eclampsia who received MgS04 treatment for 48 hours by 16 correspondents (84.2%) and 5 correspondents with MgS04 administration for 24 hours.Conclusion : There is a significant relationship between the duration of Magnesium Sulfate with the incidence of hyperpermagnesium in patients with severe preeclampsia and eclampsia. Keywords: severe preeclampsia, eclampsia, magnesium sulfate, magnesium levels

Introduction: Education and behavior (knowledge, attitudes and actions) about healthy living are important things, especially applied in life, so as not to fall sick so that they can carry out activities properly. Worldwide, there are more than 2500 species of mosquitoes, although most of these mosquito species are not associated with disease. Larva eradication practice can be done with 3M plus. The role of the community in eradicating mosquito nests is prioritized by housewives because they are generally responsible for taking care of the household including household hygiene problems. The purpose of this study was to determine the relationship between education and knowledge with the behavior of housewives in eradicating mosquito nests. The method used was cross-sectional analytic, the sampling technique used simple random sampling and inclusion criteria in order to obtain 50 respondents. Data analysis used Chi-Square with a significant level of 95% with a significance value of 5%. Results: Respondents with higher education and good behavior were 73.1%. Respondents with low education and bad behavior were 70.8%. Respondents with good knowledge and good behavior were 75%, respondents with bad knowledge and bad behavior were 77.3%. Conclusion: There is a relationship between education and the behavior of housewives in eradicating mosquito nests, and there is a relationship between knowledge and behavior of housewives in eradicating mosquito nests. Abstract Background How best to characterize intraoperative hypotension remains unclear. Thus, the authors assessed the relationship between myocardial and kidney injury and intraoperative absolute (mean arterial pressure [MAP]) and relative (reduction from preoperative pressure) MAP thresholds. Methods The authors characterized hypotension by the lowest MAP below various absolute and relative thresholds for cumulative 1, 3, 5, or 10 min and also time-weighted average below various absolute or relative MAP thresholds. The authors modeled each relationship using logistic regression. The authors further evaluated whether the relationships between intraoperative hypotension and either myocardial or kidney injury depended on baseline MAP. Finally, the authors compared the strength of associations between absolute and relative thresholds on myocardial and kidney injury using C statistics. Results MAP below absolute thresholds of 65 mmHg or relative thresholds of 20% were progressively related to both myocardial and kidney injury. At any given threshold, prolonged exposure was associated with increased odds. There were no clinically important interactions between preoperative blood pressures and the relationship between hypotension and myocardial or kidney injury at intraoperative mean arterial blood pressures less than 65 mmHg. Absolute and relative thresholds had comparable ability to discriminate patients with myocardial or kidney injury from those without. Conclusions The associations based on relative thresholds were no stronger than those based on absolute thresholds. Furthermore, there was no clinically important interaction with preoperative pressure. Anesthetic management can thus be based on intraoperative pressures without regard to preoperative pressure.

Background The incidence of atopic disease tends to increaseover the past few decades and its morbidity interferes with thequality of life and health. Prediction of the disease is importantfor early prevention.Objective To evaluate the relationship between atopicmanifestations, family history (FH) of atopic disease and cordblood IgE (CB-IgE) levels.Methods We conducted an analytic observational study withcohort retrospective design on children with an average age of 3years whose CB-IgE had been measured at delivery inKiaracondong Primary Health Care during October–December2004. Manifestations of atopic disease were recorded using ISAACquestionaire for allergy. Chi-square, Mann-Whitney test, andlogistic regression analysis were used for analysis.Results Cord blood IgE was measured on 124 children after birth.Only 94 children (76%) fulfilled the inclusion criteria. Atopicdisease was found in 17 children (18%), consisting of 8 childrenwith atopic dermatitis, 4 with allergic rhinitis, and 5 suffered fromboth. There were significant differences in the mean value of CB-IgE (Z M-W =4.60; P<0.001) and FH (x 2 =19.059; P<0.001)between atopic and non atopic children. Cut off point of the CB-IgE concentration was 1.4 IU/mL (77.7%). The highest probabilityfor atopic manifestations was found in children who had highCB-IgE and positive FH (P=45%). Relative risk of children withhigh CB-IgE level in positive FH group was 3.636 (95% CI0.943;14.016).Conclusion CB-IgE level and family history of atopic disease arerisk factors for the development of atopic manifestation.

AbstractPre-diabetes increases the risk of diabetes and CVD. Several studies have investigated the relationship between food intake and pre-diabetes morbidity, but the dietary patterns of pre-diabetes subjects were not taken into consideration. The aim of this study was to determine the relationship between dietary patterns and pre-diabetes. In this regard, 150 pre-diabetic subjects and 150 healthy controls, who attended the diabetes screening centre in Shahreza, Iran, were matched for age group and sex. The weight, height, waist circumference, physical activity, systolic and diastolic blood pressures, and blood glucose levels of all participants were measured. Dietary information was collected using a semi-quantitative FFQ. Using factor analysis, two dietary patterns were identified: the vegetables, fruits and legumes (VFL) dietary pattern and the sweet, solid fat, meat and mayonnaise (SSMM) dietary pattern. Multivariate logistic regression was used to assess the relationship between pre-diabetes and dietary patterns. After adjusting for age, education, physical activity, BMI and energy intake, the VFL dietary pattern was found to be negatively associated with lower pre-diabetes (OR 0·16; 95 % CI 0·10, 0·26). Furthermore, the SSMM dietary pattern was positively associated with pre-diabetes (OR 5·45; 95 % CI 3·22, 9·23). In conclusion, the VFL dietary pattern is inversely related to pre-diabetes, whereas the SSMM dietary pattern is associated with increased risk of pre-diabetes.
