Cimetidine as adjunctive treatment for acetaminophen overdose

The aim of this study was to determine if cimetidine in addition to N-acetylcysteine and standard supportive care provide additional hepatoprotection following acute acetaminophen poisoning. It was designed as a prospective study with alternate month treatment protocol, and the work was carried out at a regional certified poison information centre. For a 2-year period, consultations received by the Rocky Mountain Poison Center involving acute aceta minophen overdose patients with a serum level above the nomogram line, but who would not receive N-acetylcys tine therapy until at least 8 h postingestion, were prospec tively evaluated for adjunctive treatment with cimetidine. All patients received standard supportive therapy and N- acetylcysteine treatment. During odd numbered months, cimetidine 300 mg was administered intravenously every 6 h for the duration of N-acetylcysteine therapy. Forty-one cimetidine treated patients were compared to 66 patients in the control group. The peak measured AST levels (+/- s.e.) were 1259+/-330 and 1301+/-451 for the control and cimetidine treatment groups, respectively (P = 0.94). Fourteen of 64 patients (21%) in the control group and 8/41 patients (20%) in the cimetidine group developed an AST > 1000 IUL-1. There were no statistical differences between the cimetidine-treated and control groups when classified by AST<100IUL -1, 100- 1000IUL-1, or >1000IUL-1. The addition of cimetidine therapy to standard N- acetylcysteine treatment did not provide additional hepatoprotection in acutely acetaminophen poisoned patients when treatment was started later than 8 h post overdose. Larger doses of cimetidine along with more fre quent or continuous intravenous administration may war rant further study.

Background. Several studies have shown the association between periodontitis and rheumatoid arthritis (RA) and some similarities in the pathogenesis of these two diseases but clinical trials which have evaluated the effect of periodontal therapy on clinical signs of active RA are scarce, particularly in Iran. The aim of this study was to evaluate the effect of non-surgical periodontal treatment on the improvement of clinical signs of patients with RA. Methods. Fifty-six patients with active RA and generalized moderate-to-severe chronic periodontitis were selected and divided into case (periodontal treatment) and control (without periodontal treatment) groups. Periodontal parameters and RA disease activity scores (DAS28 index) were compared at 6- and 12-week intervals. Results. In the case group, a significant reduction in all the periodontal indices was observed 6 weeks after treatment. At the 12-week evaluation, periodontal indices showed a significant reduction in comparison with baseline and a little increase in comparison with 6-week interval. Six weeks after treatment, DAS28 decreased significantly in the case group (P<0.05). DAS28 also decreased at 12-week interval but its reduction was not significant in comparison with 6-week interval. DAS28 in the case group at 12-week interval was significantly less than that in the control group (P<0.05). Conclusion. Since periodontal care as a part of treatment protocol in RA patients can be effective in improving clinical signs, the recall intervals are better to be 3 months or shorter.

The present study was conducted to investigate the efficacy of Ovsynch and Ovsynch plus protocol in postpartum (day 60) Sahiwal cows (n=18). Animals were randomly divided into three equal groups, viz., Ovsynch group Ovsynch plus group and Control group. Animals of group I (n = 6) were treated with traditional Ovsynch protocol. The animals (n = 6) of group II were treated with Ovsynch plus protocol which consisted of an initial intramuscular injection of eCG (Folligon) @ 250 IU on day 60 postpartum followed 3 days later by GPG (Ovsynch) protocol. In group-III Control, no treatment was given to animals (n = 6). Treated animals were inseminated at a fixed time between 14 and 20 hrs after second GnRH injection, irrespective of estrus detection. Blood samples were collected from each animal on days 50 and 60 postpartum to determine the status of cyclicity in animals based on serum concentrations of progesterone (P4). A third blood sample was collected on the day of prostaglandin treatment to determine the response of first GnRH injection. Four animals each were cyclic, and two were acyclic in both treatment groups. Four animals each responded to first GnRH treatment in both treatment groups. Similarly, two animals each got conceived giving conception rate of 50% (2/4) in each treatment. In the control group, one out of 6 animals got conceived yielding 16.66 % conception rate (1/6) during the study period. It may be thus concluded that Ovsynch and Ovsynch plus protocol may be used during the early postpartum period to improve the reproductive efficiency in postpartum Sahiwal cows.

There has been much debate over a direct role for progesterone (P4) in early bovine embryo development. While previous attempts to supplement bovine embryos in vitro with P4 produced results that vary and are often contradictory, this may be a response of administering P4 at inappropriate times. Therefore, the objective of these experiments was to determine if P4 could exert a direct effect on developing IVF-derived bovine embryos when administered at an appropriate time of embryo development. In Exp. I, IVF-derived bovine 8-cell embryos were randomly allotted to treatments: (1) control, CR1aa medium (n = 168); (2) vehicle, CR1aa + ETOH (0.01%) (n = 170); and (3) P4, CR1aa + ETOH + P4 (20 ng/mL in 50-μL droplet) (n = 173). In Exp. II, IVF-derived bovine 8-cell embryos were randomly allotted to treatments: (1) control, CR1aa medium (n = 160); (2) vehicle, CR1aa + DMSO (0.01%) (n = 180); and (3) P4, CR1aa + DMSO (0.01%) + P4 (20 ng/mL in 50-μL droplet) (n = 170). All embryos were evaluated on Days 6 to 9 post-insemination and rates calculated from 8-cell embryos. In Exp. I, ETOH tended to have a detrimental effect with significantly fewer (P < 0.05) embryos (53%) developing to the blastocyst stage on Day 7 compared with the control (62%) and P4 (71%) groups. At Day 7, significantly more embryos cultured in P4 (71%) developed to the blastocyst stage compared with the control group (62%). P4 treatment significantly increased the number of Grade 1 blastocysts (25%) on Day 7 compared with vehicle (15%) and control (17%) groups. At the end of culture, there were also significantly more Day 9 hatched blastocysts in the P4 group (33%) compared with vehicle (22%) and control (21%) groups. Supplementing P4 in the culture medium increased the rate of development, resulting in significantly more blastocysts (8%) on Day 6 and hatched blastocysts (21%) on Day 8 compared with vehicle (3% and 12%) and control (0% and 8%) groups, respectively. In Exp. II, there were no significant differences between treatment groups for Day 7 blastocysts (control 54%, DMSO 61%, P4 57%) and Day 9 hatched blastocysts (control 46%, DMSO 51%, P4 46%). However, there were significantly more Grade 1 blastocysts in the P4 group (22% and 36%) on Days 6 and 8 compared with vehicle (11% and 23%) and control (13% and 23%) groups, respectively. The lack of improvement in Day 7 blastocysts and Day 9 hatched blastocysts rates leads to further uncertainty in understanding the P4 vehicle interactions. In conclusion, the results of these two experiments indicate that P4 can exert a direct effect on the developing IVF-derived bovine embryo; however, due to P4 vehicle interactions; other inert vehicles need to be explored to further evaluate the direct effects of P4 on the developing bovine embryo.

The purpose of this study was to determine the effect of AIUEO therapy on the speech ability of stroke patients who have motor aphasia in Raja Ahmad Thabib Hospital Tanjungpinang. The research design used was quasi experiment with the Nonequivalent Control Group Design approach to 9 respondents who were divided into 9 treatment groups and 9 control groups. The results showed that there were significant differences in the functional ability of communication between the control and treatment groups with a value of p <0.05 (p = 0.007 at a = 0.05) using the Wilcoxon Test statistical test. Conclusion, the influence of AIUEO therapy on the speech ability of stroke patients with motor aphasia in the treatment and control groups at Ahmad Thabib Hospital Tanjungpinang.   Keywords: Speech Ability, Motor Aphasia Stroke, AIUEO Therapy

Mastery of keyboarding being part urgent to communicate through the computer, and very important, especially students who study in the field of business. On the other side, individual performance also influenced by the work environment, and music is one form the environment that could effect keyboarding performance. A model that can describe this is the stimulus-responsible model of mehrabian-russell which explains that music can affect the performance of individual writing. Almost all of Indonesia has traditional music. Therefore, research is needed to identify the effects of traditional music to enrich the area of innovation in strategies and techniques of learning keyboarding skills. In this study conducted an experiment on two groups, the treatment groups(traditional music) and control group (without traditional) and questionnaire to strengthen the experimental results. In preliminary tests of typing, both groups had a mean AWPM identical or similar. Furthermore, tests carried one, where the treatment group listened to five kinds of music Java, Sunda, and Bali. The test result and the result of the questionnaire showed that (typing skills shown by the average value of AWPM) will increase significantly used of traditional music as the background. So, the genre of traditional music can be stimulant to improve average performance of AWPM. There are three traditional kinds of music that can improve student typing speed and accuracy significantly, a regional music cublek-cublek suweng, kebiar bali, and oleg tambuliningan, but cublek-cublek suweng is a genre of traditional music that has the best effect. Therefore, the process of learning courses keyboarding skills needs to use instrumental traditional music, especially instrumental music that has a fast and cheerfully tempo.

Abstract Background: We conducted a prospective study to investigate the efficacy of pleural blood patching to reduce the need for chest tube placement in pneumothorax of CT-guided percutaneous lung biopsy. Methods: We enrolled each 77 patients in study and control groups. If the patient of study group developed pneumothorax ≥1 cm on post-biopsy CT, we drew 15 mL blood, then performed simple aspiration followed by pleural blood patching. In control group, we performed only simple aspiration or no interventions. Results: Of the 77 patients of study group, 41 developed pneumothorax, 9 of which were ≥ 1 cm, and 8 patients underwent pleural blood patching. None of these 8 patients (0%) required chest tube placement. In comparison between study group and control group, pleural blood patching reduced the chest tube insertion rate from 23.1% to 11.1% in patients pneumothorax ≥ 1 cm, but not statistically significant (p=0.26) Conclusion: Selective pleural blood patching reduced chest tube insertion rate in patients pneumothorax ≥ 1 cm, large-scale studies are warranted to confirm the result. Abstract A total of 67 leukaphereses were performed with the continuous-flow centrifuge (CFC) on 27 healthy donors for the purpose of obtaining increased yields of granulocytes for infusion into septic patients with acute leukemia accompanied by severe granulocytopenia. The addition of hydroxyethyl starch (HES) to the input line of the CFC significantly (p < 0.005) increased the total number of leukocytes and/or granulocytes collected per donation. A mean yield of 9.72 x 109 and 4.65 x 109 total granulocytes were collected by the HES-treated and control-group donors, respectively. The efficiency of cell collection as evidenced by the total number of leukocytes and/or granulocytes harvested per liter of processed blood was also significantly (p < 0.005) improved by the addition of HES to the continuous-flow centrifuge. Significant alterations in hematologic parameters were not experienced by HES-treated donors undergoing initial and multiple procedures. Pre- and postdonation leukocyte and platelet counts, hemoglobin, prothrombin time, partial thromboplastin time, and leukocyte differential counts were no different whether or not HES was employed for granulocyte collection. The results of the present study demonstrate that increased yields of granulocytes can be harvested by the addition of HES to the continuous-flow centrifuge and made available for supportive therapy to patients experiencing granulocytopenia induced by malignant disease or its treatment.

Background : Perinatal asphyxia is an insult to the fetus or newborn infant due to lack of oxygen (hypoxia) and/or a lack of perfusion (ischemia) to various organs, which will manifest as difficulty in establishing spontaneous respiration evident by delayed cry after birth, at least after one minute. World-wide, perinatal asphyxia accounts for about 900,000 deaths each year. In Bangladesh it is a major cause of neonatal death. A substantial proportion of the children that survive suffer late effects such as cerebral palsy and epilepsy. Objective : To determine the efficacy of erythropoietin in improving the neurological outcome of term neonates with perinatal asphyxia (HIE stage II and III). Materials and methods : A Randomized Controll Trial was carried out in the Neonatal ward and NICU of Dhaka Shishu Hospital from 1st April 2014 to 30th Sep 2015. A total 68 neonates with perinatal asphyxia (both HIE stage II and III) who fulfill the inclusion criteria were enrolled and randomly assigned to intervention group (n=35) and control group (n=33). Intervention group received rHuEPO 300- 500 U/kg/dose daily subcutaneously for 5 days within first 48 hours of birth along with the standard treatment protocol and control group received standard treatment protocol only. Results : Baseline clinical characteristics, USG of brain during hospital stay were almost similar in both groups. Statistically significant effect was noted in seizure control, tolerance of oral feeding, hospital stay and neurological outcome at 3 months of age (p=008). USG of brain at 3 months of age also improved significantly (p=0.027). Conclusion : This study demonstrates the effectiveness of early administration of rHuEPO to term neonates with moderate to severe asphyxia, beneficial effect on short term outcomes like seizure control, tolerance of oral feeding and neurological outcome at 3 months of age. Objective: The objective of this study was to assess the antiproliferative pleiotropic effects of statins on thyroid function, volume, and nodularity. Subjects and Methods: One hundred and six hyperlipidemic patients were included in this prospective study. The 69 patients in the statin groups received atorvastatin (16 received 10 mg and 18 received 20 mg) or rosuvastatin (20 received 10 mg and 15 received 20 mg). The 37 patients in the control group, assessed as not requiring drugs, made only lifestyle changes. Upon admission and after 6 months, all patients were evaluated by ultrasonography as well as for lipid variables (total cholesterol, high- and low-density lipoprotein cholesterol, and triglycerides) and thyroid function and structure. Results: After 6 months, no differences in thyroid function, thyroid volume, the number of thyroid nodules, or nodule size were observed in the statin and control groups. In a subgroup analysis, total thyroid volume had decreased more in patients receiving 20 mg of rosuvastatin than that in the control group (p < 0.05). Maximum nodule size had decreased more in those receiving 10 mg of rosuvastatin (p < 0.05). Conclusions: Our results suggest an association between rosuvastatin treatment and smaller thyroid volume and maximum nodule diameter; this could be attributable to the antiproliferative effects of statin therapy on the thyroid.

Pregnant multiparous Brahman cross cows grazing native pasture at Samford in south-east Queensland were allotted to three treatment groups in late winter; one group received a daily supplement of 180 g crude protein (as peanut meal) plus 10 g phosphorus (P), the second group 10 g P and the third (control) group neither protein nor P. The P and control groups received approximately 320 g DOM per day, mainly composed of sugar and grain, to balance the DOM provided to the protein + P group as peanut meal. Supplementation continued for a period approximately spanning the third to eighth months of gestation. The protein plus phosphorus supplement significantly reduced the interval from calving to first postpartum oestrus by 46 per cent; all animals in this group had exhibited oestrus within two months of calving. By three months after calving, only 50 per cent of controls and 70 per cent of those given phosphorus had exhibited oestrus. Calf growth was unaffected by the supplements. Samples of grazed pasture were collected using non-pregnant cows with oesophageal fistulae. The provision of P or P plus protein tended to render the animals less selective in their grazing. The evidence suggested that the higher feed requirements of the pregnant cows removed any possible effect of the supplements on their selectivity. Native pasture provided a slightly sub maintenance ration during the period of supplementation, and a major limiting nutrient appeared to be protein.
