Introduction Disease recurrence after surgery is a crucial predictor of poor prognosis in colorectal cancer, where disseminated disease at the time of intervention can also be observed in localized early-stage cases. We evaluated the ability to predict disease recurrence of miRNAs from two signatures that we have found linked to the presence of colorectal cancer (CL signature) or adenoma (HgA signature) in higher-risk subjects. Methods miRNAs from the signatures were studied longitudinally by quantitative real-time polymerase chain reaction in plasma from 24 patients with resectable colorectal cancer collected at the time of surgery and during scheduled follow-up across 36 months. Patients either showed relapse within 36 months (alive with disease (AWD)), or remained disease-free (no evidence of disease (NED)) for the same period. Results Although the signatures did not predict recurrence, expression of the miRNAs from the CL signature decreased 1 year after surgery, and one miRNA of the signature, miR-378a-3p, almost reached significance in the NED subgroup (Wilcoxon signed-rank test: p-value = 0.078). Also, miR-335-5p from the HgA signature was higher in AWD patients before surgery (Kruskal–Wallis test: p-value = 0.019). Conclusions These data, although from a small cohort of patients, support the possible use of miRNAs as non-invasive biomarkers in liquid biopsy-based tests to identify patients at risk of relapse and to monitor them during follow-up.

Immune checkpoint inhibitors (ICI) have reshaped therapeutic strategies for cancer patients. The development of ICI for early stage colorectal cancer is accompanied by specific challenges: (i) the selection of patients who are likely to benefit from these treatments, i.e., patients with tumors harboring predictive factors of efficacy of ICI, such as microsatellite instability and/or mismatch repair deficiency (MSI/dMMR), or other potential parameters (increased T cell infiltration using Immunoscore® or others, high tumor mutational burden, POLE mutation), (ii) the selection of patients at risk of disease recurrence (poor prognostic features), and (iii) the choice of an accurate clinical trial methodological framework. In this review, we will discuss the ins and outs of clinical research of ICI for early stage MSI/dMMR CC patients in adjuvant and neoadjuvant settings. We will then summarize data that might support the development of ICI in localized colorectal cancer beyond MSI/dMMR.

Background: Plants are a good source of drugs in traditional and modern medicine. One among these plants is “tulasi” The queen of herbs” which is used in ayurveda for its diverse properties like antioxidant, analgesic, anti-inflammatory, antidiabetic, and others. It belongs to labiate family and is the most sacred and valued in traditional culture of India. AIMS of the study was to evaluate the efficacy of Tulasi in the treatment of OSMF.Methods: The study included 20 individuals, females and males of age group 20-50yrs who visited the outpatient department who were diagnosed clinically as having OSMF, categorized according to pindborg’s classification. Patients were asked to apply 500mg of Tulasi medicine paste (Tulasi powder mixed with glycerine) twice daily for 1month and were recalled for follow up every week for 1month. All the clinical parameters - mouth opening, burning sensation, pain with the lesion and others were evaluated and recorded in specially designed proforma.Results: Friedman test and wilcoxon signed rank test was used for statistical evaluation. Significant improvement with P value <0.05 was noticed in burning sensation, mouth opening and pain associated with the lesion.Conclusions: The study concludes that Tulasi can be used as a main treatment modality in the management of patients with OSMF.

Background Calcitriol has long been used as the main therapy in renal osteodystrophy, but the efficacy of the oral route is not always as high as expected. Objective To asses the safety and efficacy of intraperitoneal calcitriol in infants undergoing peritoneal dialysis (PD). Patients and Methods PD patients on oral calcitriol therapy, with serum parathyroid hormone (PTH) >1000 pg/mL during the previous 3 months of treatment, were switched to intraperitoneal calcitriol therapy, 1 μg twice per week. Dose was increased to 1 μg three times per week if PTH remained >1000 pg/mL, and was later readjusted. Target PTH was 200 – 300 pg/mL according DOQI guidelines. Statistics: All results are expressed as mean ± SE. The Wilcoxon signed rank test was used to evaluate differences in measurements for each pair of values. The confidence interval for differences between population medians was 96.9%. A p value less than 0.05 was considered significant. Results Six male children, mean age 17 ± 3.86 months, completed a 12-month follow-up. Mean pretreatment PTH was 1654 ± 209 pg/mL. Mean PTH at months 0, 3, 6, 9, and 12 was 1448 ± 439*, 1277 ± 723, 910 ± 704, 582 ± 282*, and 465 ± 224* pg/mL, respectively (*p < 0.05). Twelve hypercalcemic and 10 hyperphosphatemic episodes were successfully treated. Conclusion Infants on PD who fail to respond to oral calcitriol therapy can be safely treated with intraperitoneal administration of active vitamin D.

<p class="IABSTRAK"><strong>Abstract: </strong>This research aims to determine the effectiveness of emotional regulation training in increasing resilience in women victims of sexual harassment. Participants in this study were four female victims of sexual harassment. Participants age are 18-30 years and had a moderate to low level resilience score. The research design was one group pre-test post-test. Data collected using resilience scale with Likert model based on resilience aspect from Kumpfer (1999) prepared by the researcher. Emotion regulation training modules were prepared by researcher based on Greenberg theory (2002). Wilcoxon Signed Rank Test technique is used by researcher to test for differences of pre-test, post-test, and post-test2 (follow up) data score. Results of pre-test - post-test analysis showed p value of 0.034 and post-test2 (follow up) with p value of 0.033 (p &lt;0.05). From the result can be concluded that emotional regulation training is effective in increasing resilience in women victims of sexual harassment.</p><strong>Abstrak: </strong>Penelitian ini berujuan untuk mengetahui pengaruh pelatihan regulasi emosi terhadap peningkatan resiliensi pada perempuan korban pelecehan seksual. Partisipan pada penelitian ini merupakan empat perempuan korban pelecehan seksual. Usia partisipan adalah 18-30 tahun dan memiliki skor tingkat resiliensi sedang sampai dengan sedang. Desain penelitian yang digunakan adalah <em>one group pre-test post-test. </em>Pengumpulan data penelitian dilakukan dengan menggunakan skala resiliensi model <em>Likert </em>yang disusun oleh peneliti berdasarkan aspek resiliensi dari Kumpfer (1999). Modul pelatihan regulasi emosi disusun berdasarkan teori Greenberg (2002). Teknik analisis <em>Wilcoxon Signed Rank Test </em>digunakan peneliti untuk menguji perbedaan skor data <em>pre-test, post-test, </em>dan <em>post-test2 </em>(<em>follow up</em>). Hasil analisis <em>pre-test – post-test </em>menunjukkan nilai p sebesar 0.034 dan <em>post-test2 (follow up) </em>dengan nilai p sebesar 0.033 (p&lt;0.05). Hasil penelitian menunjukkan bahwa pelatihan regulasi emosi efektif untuk meningkatkan resiliensi pada perempuan korban pelecehan seksual.

Adjuvant treatment for patients with early stage colorectal cancer (eCRC) is currently based on suboptimal risk stratification, especially for elderly patients. Metabolomics may improve the identification of patients with residual micrometastases after surgery. In this retrospective study, we hypothesized that metabolomic fingerprinting could improve risk stratification in patients with eCRC. Serum samples obtained after surgery from 94 elderly patients with eCRC (65 relapse free and 29 relapsed, after 5-years median follow up), and from 75 elderly patients with metastatic colorectal cancer (mCRC) obtained before a new line of chemotherapy, were retrospectively analyzed via proton nuclear magnetic resonance spectroscopy. The prognostic role of metabolomics in patients with eCRC was assessed using Kaplan–Meier curves. PCA-CA-kNN could discriminate the metabolomic fingerprint of patients with relapse-free eCRC and mCRC (70.0% accuracy using NOESY spectra). This model was used to classify the samples of patients with relapsed eCRC: 69% of eCRC patients with relapse were predicted as metastatic. The metabolomic classification was strongly associated with prognosis (p-value 0.0005, HR 3.64), independently of tumor stage. In conclusion, metabolomics could be an innovative tool to refine risk stratification in elderly patients with eCRC. Based on these results, a prospective trial aimed at improving risk stratification by metabolomic fingerprinting (LIBIMET) is ongoing.

Extravasation of blood in the central nervous system (CNS) represents a very strong damaged associated molecular patterns (DAMP) which is followed by rapid inflammation and can participate in worse outcome of patients. We analyzed cerebrospinal fluid (CSF) from 139 patients after the CNS hemorrhage. We compared 109 survivors (Glasgow Outcome Score (GOS) 5-3) and 30 patients with poor outcomes (GOS 2-1). Statistical evaluations were performed using the Wilcoxon signed-rank test and the Mann–Whitney U test. Almost the same numbers of erythrocytes in both subgroups appeared in days 0–3 (p = 0.927) and a significant increase in patients with GOS 2-1 in days 7–10 after the hemorrhage (p = 0.004) revealed persistence of extravascular blood in the CNS as an adverse factor. We assess 43.3% of patients with GOS 2-1 and only 27.5% of patients with GOS 5-3 with low values of the coefficient of energy balance (KEB < 15.0) in days 0–3 after the hemorrhage as a trend to immediate intensive inflammation in the CNS of patients with poor outcomes. We consider significantly higher concentration of total protein of patients with GOS 2-1 in days 0–3 after hemorrhage (p = 0.008) as the evidence of immediate simultaneously manifested intensive inflammation, swelling of the brain and elevation of intracranial pressure.

A new paradigm in cancer chemotherapy derives from the interaction between chemotherapeutics, including irinotecan and 5-fluorouracil (5-FU), and the immune system. The patient’s immune response can modulate chemotherapy effectiveness, and, on the other hand, chemotherapeutic agents can foster tumor cell immunogenicity. On these grounds, the analysis of the cancer patients’ immunogenetic characteristics and their effect on survival after chemotherapy represent a new frontier. This study aims to identify genetic determinants in the immuno-related pathways predictive of overall survival (OS) after FOLFIRI (irinotecan, 5-FU, leucovorin) therapy. Two independent cohorts comprising a total of 335 patients with metastatic colorectal cancer (mCRC) homogeneously treated with first-line FOLFIRI were included in the study. The prognostic effect of 192 tagging genetic polymorphisms in 34 immune-related genes was evaluated using the bead array technology. The IL15RA rs7910212-C allele was associated with worse OS in both discovery (HR: 1.57, p = 0.0327, Bootstrap p-value = 0.0280) and replication (HR:1.71, p = 0.0411) cohorts. Conversely, SMAD3 rs7179840-C allele was associated with better OS in both discovery (HR:0.65, p = 0.0202, Bootstrap p-value = 0.0203) and replication (HR:0.61, p = 0.0216) cohorts. A genetic prognostic score was generated integrating IL15RA-rs7910212 and SMAD3-rs7179840 markers with inflammation-related prognostic polymorphisms we previously identified in the same study population (i.e., PXR [NR1I2]-rs1054190, VDR-rs7299460). The calculated genetic score successfully discriminated patients with different survival probabilities (p < 0.0001 log-rank test). These findings provide new insight on the prognostic value of genetic determinants, such as IL15RA and SMAD3 markers, and could offer a new decision tool to improve the clinical management of patients with mCRC receiving FOLFIRI.

One of the nutritional problems that often occurs among adolescents is iron deficiency anemia. Especially for adolescent girls who have a higher risk of anemia than adolescent boys. Based on data from Riskesdas 2018, the prevalence of anemia in adolescents aged 15-24 years is 32%, meaning that there are still anemia problems in Indonesia that have not been resolved. The purpose of this community dedication activity is to provide education about iron deficiency anemia for adolescent girls using leaflet media. The target of this community dedication activity is adolescent girls aged 12-19 years in the JABODETABEK area. The community dedication activity is carried out online through the WhatsApp group due to the COVID-19 pandemic. There are 31 adolescent girls who participated in this community dedication activity. This community dedication activity consists of three stages including pretest, education with leaflets, and posttest. The amount and types of pretest and posttest questions are the same. The amount of questions given is 15 questions. The correct answer is given a score of 10 and the wrong one is given a score of 0. The level of knowledge of adolescent girls is categorized as 3 groups, which is less if the correct answer is <60%, sufficient if the correct answer is 60-80%, and good if the correct answer is >80%. Most of the adolescent girls are in the age range of 17-19 years (54.8%). The results showed that there was an increase in knowledge about iron deficiency anemia in adolescent girls. The results of the Wilcoxon signed rank test showed significantly different pretest and posttest results (p-value = 0.000), so it can be concluded that providing education using leaflets can help increase knowledge of iron deficiency anemia for adolescent girls.Salah satu masalah gizi yang sering terjadi pada kalangan remaja yaitu anemia defisiensi besi. Khususnya bagi remaja putri yang memiliki risiko lebih tinggi mengalami anemia dibandingkan remaja putra. Berdasarkan data Riskesdas 2018, prevalensi anemia pada remaja berusia 15-24 tahun sebesar 32%, artinya masih terdapat permasalahan anemia di Indonesia yang belum teratasi. Tujuan dari pengabdian masyarakat ini adalah untuk memberikan edukasi mengenai anemia defisiensi besi bagi remaja putri dengan media leaflet. Sasaran dari kegiatan pengabdian masyarakat ini adalah remaja putri berusia 12-19 tahun di wilayah JABODETABEK. Kegiatan pengabdian masyarakat dilakukan secara online melalui grup whatsappkarena kondisi pandemi covid-19. Total remaja putri yang mengikuti kegiatan pengabdian masyarakat berjumlah 31 orang. Kegiatan pengabdian masyarakat ini terdiri dari tiga tahapan diantaranya pretest, edukasi dengan leaflet, dan posttest.  Jumlah dan jenis pertanyaan pretest dan posttest sama. Jumlah pertanyaan yang diberikan adalah 15 soal. Jawaban yang benar diberikan nilai 10 dan yang salah diberikan nilai 0. Tingkat pengetahuan remaja putri dikelompokkan menjadi 3 diantaranya kurang jika jawaban yang benar < 60%, cukup jika jawaban yang benar 60-80%, dan baik jika jawaban yang benar > 80%. Sebagian besar remaja putri berada pada rentang usia 17-19 tahun (54,8%). Hasil menunjukkan bahwa terdapat peningkatan pengetahuan mengenai anemia defisiensi besi pada remaja putri. Hasil uji Wilcoxon signed rank test menunjukkan hasil pretest dan posttest yang berbeda nyata (p-value = 0,000), sehingga dapat disimpulkan bahwa pemberian edukasi menggunakan leaflet dapat membantu meningkatkan pengetahuan anemia defisiensi besi bagi remaja putri.

Background: The first labor anxiety is an unpleasant psychological condition due to physiological changes that cause psychological instability. The condition of excessive anxiety, worry, fear without cause, and stress, causes the muscles of the body to tense up, especially the muscles in the birth path to become stiff and hard, making it difficult to expand. This study analyzed the effect of pregnancy exercise on third trimester primigravida anxiety in dealing with childbirth. Methods: Quasi experimental design with nonrandomized pretest-postest control group. The sampling technique used was purposive sampling with 34 research subjects. 17 people for the treatment group were given pregnancy exercise 8 times, and 17 people for the control group were given health education about preparation for childbirth. Results: The results of data analysis using the Wilcoxon signed rank test showed that the average reduction in the treatment group was 8.00 and the control group was 3.00. The effect of pregnancy exercise on third trimester primigravida anxiety in the treatment group with a p-value (0.000) <(0.05), while the control group with a p-value (0.180)> (0.05). Conclusion: There is an effect of pregnancy exercise on third trimester primigravida anxiety in dealing with childbirth in the Wuryantoro Health Center work area.

Hemoglobin is the red pigment-protein contained in red blood cells. The function of hemoglobin is to transport oxygen from the lungs and in the bloodstream to be carried to the tissues. Hemoglobin can increase by increasing the intake of iron in the body. One way to increase hemoglobin can be done by giving method Infused Water Dates. Infused water is a drink consisting of plain water with fresh fruit added and soaking or settling together for a certain time. The making of infused water is carried out by soaking for a period of 0 to 12 hours, so that the infused water becomes a liquid medium that carries more nutrients than ordinary water and will be more easily absorbed by cells and distributed throughout the body (Sidauruk, 2018 ). In several studies, it is stated that dates can increase hemoglobin levels in the blood. Where dates contain iron, vitamin A, vitamin C, protein to form red blood cells. With the method, Infused Water the date palm water becomes alkaline so that it can accelerate the absorption process in the body. The purpose of this study was to determine the effectiveness of dates infused water provision to increase hemoglobin levels of midwifery adolescent girls at Medan Health Polytechnic in 2020. This research was quasi-experimental study using the One Group Pretest-Posttest Without Control Design approach. The sample in this study were 20 respondents of midwifery adolescent girls from Medan Health Polytechnic, using the technique of purposive sampling. The statistical test used wa the Wilcoxon Signed Rank Test because the data was not normal. The results of the Wilcoxon Signed Rank Test obtained the value of P-Value = 0.001. So it can be concluded that the date infused water  is effective in increasing hemoglobin levels of midwifery adolescent girls at Medan Health Polytechnic in 2020.   Keywords: Dates Infused Water, Increased Hemoglobin Levels, Girls Adolescent
