Background: Few studies exist in Rwanda to establish the reasons behind delays in seeking treatment in the health institutions which further affected timely diagnosis and case detection. The study investigated factors associated with delays in seeking treatment among tuberculosis (TB) patients at Muhima District Hospital, Rwanda.Methods: The study adopted descriptive cross- sectional study of 49 smear-positive TB patients derived using proportion estimation of case registry (November 2016 to January 2017) newly- diagnosed patients, first and second months treatment. Systematic sampling technique used to select 60 TB patients from target population. Data collected by researcher and trained research assistant, using a close- ended questionnaire through a translation in local dialect.Results: About 49% (24) of the smear- positive TB patients experienced delayed in seeking treatment for more than 180 days (6 months). Also 53% (26) were within the 36-45 age-cohort, while majority 75.5% (37) of the respondents was male. About 57% (28) of the respondents have poor knowledge of TB transmission. Similarly, about 51% (25) were the results on the perception of taking anti- TB medication, while median time interval from onset of symptoms and initiation of treatments was 45 days, average 71 days between 15 -180 days.Conclusions: Statistically significant relationship exist between initial source of treatment using home remedies (p=0.0) and private insurance cover (p=0.0) in relation to delay, which further contributed to delay among majority. Recommendations of the study are strengthening medical services utilization, improving case detection, medical infrastructures and collaboration with non- formal healthcare practices.

Partographs are guidelines for childbirth observations that will facilitate labor assistants in first identifying emergency cases and complications for mothers and fetuses. Preliminary survey at the Sigi Community Health Sub-Center (Pustu) of the 8 Pustu midwives found two midwives (25%) to complete a complete partograph, six midwives (75%) incomplete. The purpose of this study was to determine the determinant factors associated with the use of partographs in labor. The design of this study used observational analytic methods with a cross-sectional approach. 24 BPM survey results were obtained with 30 samples of midwives who met the research criteria and data completeness. The sampling technique was by the total population. Data analysis used logistic regression. The multivariate analysis results showed that APN training was the most influential factor in the use of partographs in labor by midwives. Statistical test results obtained a POR value of 37.7 (95% CI 12.1 - 60.2). This study suggests that midwives must have APN certificates to be valid in providing services.

Background: Several factors are reported to be associated with palliative care utilization among patients with various cancers, but literature is lacking on multiple myeloma (MM) specific factors. MM patients have a high symptom burden and early involvement of palliative could increase their quality of life. We examined factors associated with palliative care utilization among MM patients and explored prevalence trends in palliative care utilization among patients with MM. Methods: Cross-sectional analyses were conducted using the National Inpatient Sample data collected between 2016 and 2018. Descriptive analyses were used to explore prevalence trends in palliative care utilization over time. Multivariable logistic regression models were used to examine sociodemographic and hospital-level factors associated with palliative care utilization in MM patients. Results: Overall prevalence of palliative care utilization in our population was 7.7% with a trend of increasing use of palliative care from 7.3% in 2016 to 8.2% in 2018. MM patients aged 70 years and above had 1.30 times higher odds (95% CI: 1.20-1.42) of receiving palliative care relative to those younger than 70 years. Compared to non-Hispanic whites, non-Hispanic blacks (Adjusted odds ratio (AOR): 0.86; 95% CI: 0.79-0.94) were less likely to utilize palliative care. Patients on Medicaid (AOR: 1.27; 95% CI: 1.08-1.49), private insurance (AOR: 1.27; 95% CI: 1.16-1.39) and other insurance types (AOR: 2.10; 95% CI: 1.79-2.47) had significantly higher odds of receiving palliative care when compared to those on Medicare. Other factors identified were hospital region, location, patient disposition, admission type, length of stay, and number of comorbidities. Conclusion: Our findings highlight the urgent need for education of hospital physicians on the need for early palliative care involvement in the care of hospitalized MM patients. Messaging interventions such as the delivery of pop-up messages in electronic medical records to serve as reminders for physicians can be explored as a potential way to increase palliative care consultations for patients who need them.

The percentage of families in Lapulu Village that had healthy latrines was 64,84% who met the health requirements and 35,16% who did not meet the health requirements in 2019. This shows that the ownership of healthy latrines is still lower than the national achievement. This study aims to determine the factors associated with low ownership of healthy latrines in the coastal areas of Lapulu Village, Kendari City. The research design was observational, with a cross-sectional approach. The population in this study was 437 respondents, while the study sample was 209 respondents. The sampling technique used was proportional random sampling technique. The analysis were performed using the Chi-Square test. The results of statistical tests with chi-square obtained the value of land availability (p-value=0,000), knowledge (p-value=0,031), and income (p-value=0,000). It can be concluded that there is a relationship between land availability, knowledge, and income levels associated with low ownership of healthy latrines in the tidal area of ​​Lapulu Village, Kendari City. Therefore, it is hoped that the community and local government will establish this inter sector collaboration with related agencies to increase community ownership of healthy latrines.

Dehidrasi merupakan kekurangan cairan tubuh karena jumlah cairan yang keluar lebih banyak dari pada jumlah cairan yang masuk. Salah satu pekerjaan yang beresiko mengalami dehidrasi adalah pada perebusan pembuatan tempe dimana menggunakan lingkungan yang cukup panas sehingga banyak mengeluarkan keringat dan mebutuhkan asupan cairan yang lebih. Tujuan penelitian adalah untuk mengetahui faktor-faktor yang berhubungan dengan kejadian dehidrasi pada pekerja pengrajin tempe. Penelitian ini merupakan penelitian deskriptif analitik dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah seluruh jumlah pengrajin tempe. Sampel berjumlah 30 responden. Teknik pengambilan sampel dengan cara total sampling. Analisis data dalam penelitian ini secara univariat dan bivariat dengan uji chi square. Hasil penelitian ada hubungan konsumsi cairan (0,004) dan beban kerja (0,007) dengan kejadian dehidrasi pada pekerja pengrajin tempe. Tidak ada hubungan berat badan dengan kejadian dehidrasi pada pekerja pengrajin tempe (p=0,875). Diharapkan pemilik usaha pengrajin tempe untuk dapat selalu menyediakan air minum yang cukup bagi pekerjanya saat pekerja membutuhkan sehingga kejadian dehidrasi dan dampaknya dapat diminimalisir. Dehydration is a lack of body fluids because the amount of fluid that comes out is more than the amount of fluid that is entered. One of the jobs that are at risk of dehydration is in boiling the manufacture of tempeh which uses a hot enough environment so that you sweat a lot and require more fluid intake. The research objective was to determine the factors associated with the incidence of dehydration in tempe craftsmen. This research is a descriptive analytic study with a cross sectional approach. The population in this study were all of the tempe craftsmen. The sample is 30 respondents. The sampling technique was total sampling. Data analysis in this study was univariate and bivariate with the chi square test. The results showed a relationship between fluid consumption (0.004) and workload (0.007) with the incidence of dehydration among workers of tempe craftsmen. There is no correlation between body weight and dehydration in tempe craftsmen workers (p = 0.875). It is expected that the tempe craftsman business owners can always provide sufficient drinking water for their workers when they need them so that the incidence of dehydration and its impact can be minimized.

Nutrition is a major factor that has an important role in the growth anddevelopment in the toddlers. In the nutritional fulfillment of the toddler,the mother plays very important role. This study aimed to determinethe factors associated with the behavior of parents with the nutritionalstatus of toddlers. The research design was used analytical surveywith cross sectional study approach. The population in this researchwas 213 Balita, the sample in this research was 136 children in BulaloVillage Kwandang District, North Gorontalo District, the samplingmethod was used purposive sampling technique. Data collection ofrespondents in general using questionnaires, data analysis usingSPSS program version 21.0. Bivariate analysis to know the existenceof relation between dependent variable and independet by using chisquared test α = 0,05. The results showed that there was a correlationbetween parents' knowledge on the Nutritional Status of Toddlers (p =0,000), there was a correlation between parental attitudes with thenutritional status in the toddler (p = 0,000), there was a correlationbetween parental action with the nutritional status in the toddler (p =0,000 ). This study recommends to parents to always pay attention tothe type of food consumed and the mothers should often follow thecounseling done by midwives and nutrition officials so that thenutritional status of toddlers has increased.

Background: Self-referral to higher-level hospitals by women seeking skilled birth attendance services reflects in part their non-adherence to established referral pathways. This choice results in an inappropriate utilization of resources within health system. The Kenya Health Sector Referral Strategy aims at optimising the utilization and access of facilities. The aim of this study was to determine the prevalence and factors associated with self-referral among women seeking skilled birth attendance services in Marsabit County between 1st and 31st Oct 2019.Methods: A cross-sectional study was adopted at the maternity department in the selected public hospitals in Marsabit County, by use of interviewer-administered questionnaires to collect information from 161 women, through systematic sampling between 1st and 31st Oct 2019. Chi-square and multiple logistic regression analysis were used to test for factors associated with self-referral at 95% confidence interval.Results: Of the 161 women interviewed, 47.2% (n=76) were self-referrals. The odds of self-referral to the higher level health facilities were more likely among women: - aged 25-29 (AOR 5.174, CI 1.015-26.365, p-value 0.048); those referred for other ANC services (AOR 4.057, CI 1.405-11.720, p-value 0.010); and those, - who visited the referral facility before for delivery (AOR 5.395, CI 1.411 – 20.628, p-value 0.014). However, self-referral were less likely among women who perceived privacy and confidentiality of services at the referral hospitals (AOR 0.370, CI 0.138-0.990, p-value 0.048).Conclusions: Almost half of women seeking skilled birth attendance were self-referrals, relates to a possible implication on an unprecedented increased workload at referral hospitals and underutilization of primary health facilities.

Background: Rohingyas living in refugee camps of Bangladesh have been identified as a high-risk group for HIV transmission. This study aimed to assess gender differences in risk perception towards HIV/AIDS among Rohingyas in Cox’s Bazar. Methods: A cross-sectional design was adopted to assess the level of risk perception towards HIV/AIDS. This analysis utilizes data from a survey of 130 Rohingya people through face-to-face interviews utilizing a 28-item HIV-Knowledge Questionnaire. A systematic sampling technique was used to accomplish data collection survey. Analyzing was done by SPSS 25. Results: Only 41.8% of males and 58.2% of females had a good knowledge score (8 out of 11 items) regarding the transmission of HIV. Knowledge is significantly associated with female gender (χ2 = 8.927, p < 0.005). Gender differences were also significantly associated with the perceived risk of contracting HIV/AIDS in the future (p < 0.001). When rank averages were investigated, perceived risk of contracting HIV/AIDS score of the female participants (mean rank = 76.71) appeared to be higher compared to the score of male participants (mean rank = 54.29). Conclusion: Female Rohingyas perceive a higher risk of contracting HIV/AIDS than male Rohingyas. Programs should address harmful gender norms, HIV testing and education emphasizing the risks involved in transactional sex within the camps.

Abstract Uganda officially introduced the inactivated polio vaccine (IPV) in May 2016 as part of the polio eradication strategy and integrated it into its routine immunization programme in addition to the oral polio vaccine. The current coverage stands at 60% as of July 2017. We therefore aimed to determine factors associated with the uptake of IPV among children in Kalungu District so as to inform the implementation of the vaccine policy. A community-based cross-sectional study was conducted among caregivers of 406 eligible children aged 12–23 months through multi-stage systematic sampling and a standardized semi-structured questionnaire. Nine key informant interviews were conducted through purposive selection of health care providers and members of Village Health Teams (VHTs) based on their expertize. Modified Poisson regression and thematic content analysis were used to determine factors significant to IPV uptake among children. 71% of sampled children aged 12–23 months had received IPV in Kalungu District. The survey found that being encouraged by health workers and VHTs was significant to children’s uptake of IPV (Adjusted PR 1.24, 95% CI; 1.22–3.47). Distance to the immunization point (Adjusted PR 0.32,95% CI; 0.16–0.62) and caregiver’s education level (Adjusted PR 1.16,95% CI; 1.05–2.22) were also associated with IPV uptake. Qualitative findings from health workers and VHT members further confirmed the perception that distance to the immunization post was important, and VHTs also stated that being encouraged by health workers was critical to IPV uptake. The current prevalence of IPV uptake among children aged 12–23 months in Kalungu is 71%, higher than the last reported national coverage (60%), though still below the recommended national coverage of 95%. Efforts should be focused on sensitization of caregivers through health workers and VHTs. Immunization outreach should be strengthened so as to bring services closer to patients.

Background. Hypertension imposes stresses on many organs like heart and kidney. However, studies that show the effect of hypertension on the lungs are limited. Objective. To assess pulmonary function status of hypertensive patients aged 30-64 years at Zewditu Memorial Hospital, 2017. Methods. Hospital based comparative cross-sectional study was conducted on 61 hypertensive patients (cases) and 61 nonhypertensive clients (controls) aged 30-64 years. Computerized spirometry was done in all cases and controls which were selected by systematic sampling technique. The study was conducted from January 20, 2017, to May 25, 2017. Result. The values of FVC, FEV1, and FEF25-75% were 3.52±1.02 liters, 2.97±0.89 liters, and 3.34±1.3 liters/second in hypertensive patients and 4.31±0.82 liters, 3.54±0.7 liters, and 3.94±1.09 liters/second in controls, respectively. These values were significantly lower (p<0.05) in hypertensive patients compared to controls. Restrictive pulmonary defect was dominant in hypertensive patients. FEV1% which was 85%±7% in hypertensive patients and 82%±5% in controls was significantly higher (p<0.05) in hypertensive patients compared to controls. Conclusion. Hypertensive patients exhibit lower pulmonary function values. Routine check-up of the pulmonary function status of such patients should be done to prevent undesired outcomes.

Introduction HIV/AIDS is one of the most dreaded diseases of this century. Condom use is probably the most effective way to combat the disease. Objective: To study the factors associated with condom use among high school students in Natitingou in 2017. Methods A descriptive cross-sectional analytical study involving 202 individuals, based on non-random sampling technique with exhaustive choice. Collected data were processed and analyzed with Epi-Data 3.1 and Epi-Info 7. Results 128 male and 74 female students were included in the study. Mean age was 19.51 years. Prevalence of condom use was 40.63%. Factors associated with condom use were: level of education, injecting drug use, knowing one’s HIV status, ease access to condom, friends' point of view on voluntary testing. Conclusions There is still room for improving the prevalence of condom use among high school students, and this suggests the social gradient perspective that other more in-depth studies will be able to specify.
