ABSTRACTThe government had held sub-national diphtheria immunization days in 2012 to combat an outbreak of diphtheria in East Java province. However, there werestill regencies or cities which had high incidence and Bangkalanregencywas the highest. The research aimed to analyze the determinants of diphtheria in Bangkalan after SNIDs. This research was case control study with 31 subjects as case and 124 as control spread in 25 case villages and 25 control villages. Bivariate analysis used chi-square and simple logistic regression. Multivariate analysis used logistic regression. Bivariate analysisresult showed that individual and household variables that had associate with incidence of diphtheria in the Bangkalan after SNIDs of diphtheria vaccine in 2012 were DPT immunization status, the status of SNIDs, age, and mother's education level. In this study, there were not associate between village variables with diphtheria in the Bangkalan after SNIDs. Multivariate analysis result showed that had associate with incidence of diphtheria in Bangkalan after SNIDs were no DPT immunization (p=0,012; OR=4,765), incomplete DPT immunization (p=0,001; OR=6,276), 3-7 years of age (p=0,014; OR=15,137), 7-15 years of age (p=0,001;OR=41,984), and no immunization in SNIDs (p=0,020; OR=3,553). Conclusion, the dominantfactors that affected diphtheriainBangkalanwere status of DPTimmunization, ageand the SNIDs of diphtheria. Recommendation, DPTimmunizationshould becomplete, boosterimmunization should be needed, monitoring theimplementation ofsubPIN should be strict, especially inschool, and thecoverage ofDPT3andsubPIN should be increased and its report should be improved.Keywords: diphtheria, sub-nationaldiphtheriaimmunization days, DPT                              immunization

Latar belakang: Stunting merupakan retardasi pertumbuhan linier kurang dari standar menurut usianya. Masalah stunting masih merupakan salah satu masalah terbesar di Kabupaten Gorontalo. Tujuan penelitian: Diketahuinya pengaruh faktor ibu, pola asuh, dan variasi MPASI terhadap kejadian stunting di Kabupaten Gorontalo. Metode: Desain penelitian menggunakan Case Control Study. Seluruh balita di Kabupaten Gorontalo menjadi Populasi studi. Sampel kasus merupakan balita yang menderita stunting, dan sampel kontrol adalah balita normal. Jumlah sampel sebanyak 118 balita. Sampel kasus dipilih dengan menggunakan purposive sampling dan sampel kontrol dipilih menggunakan random sampling. Data dianalisis dengan menggunakan uji Chi Square dan Logistic Regression. Hasil: pola asuh (OR = 3,901, 95% CI 1,692 – 8,994), variasi MPASI (OR = 3,260, 95% CI 1,371 – 7,750), riwayat KEK (OR = 2,482, 95% CI 1,013 – 6,081) dan pendidikan ibu (OR = 2,345, 95% CI 1,007 – 5,456). Umur ibu, pemberianASI Ekslusif, makanan pendamping ASI (MPASI), konsumsi snak hampir tiap hari, dan konsumsi mie instan 3 kali dalam seminggu bukan merupakan faktor risiko kejadian stunting. Simpulan: Faktor ibu yaitu pendidikan ibu, riwayat KEK, pola pemberian MPASI, dan pola asuh merupakan faktor risiko kejadian stunting.

Demam Berdarah Dengue (DBD) merupakan salah satu masalah kesehatan di negara berkembang dan global. Penyakit DBD dikenal juga dengan istilah Dengue Haemoragic Fever (DHF), merupakan penyakit infeksi akut menular kepada manusia melalui perantara gigitan nyamuk Aedes yang mengandung virus dengue. Tujuan penelitian untuk mengetahui faktor risiko kejadian demam berdarah dengue (DBD) di Kabupaten Indramayu. Jenis penelitian ini adalah penelitian observasional analitik dengan menggunakan rancang bangun case control study. Populasi pada penelitian ini seluruh penduduk yang tinggal di Kabupaten Indramayu. Sampel penelitian terdiri dari sampel kasus dan kontrol yang terdiri dari 34 kasus dan 68 kontrol sehingga total keseluruhan sampel sebanyak 102 sampel. Teknik pengambilan sampel dipilih secara simple random sampling. Analisis data menggunakan uji statistik simple logistic regression. Hasil penelitian yaitu praktek 3M di rumah (p=0,020, OR=2,778; 95% CI:1,174-6,574) dan kebiasaan mengantung pakaian (p=0,015, OR=3,470; 95% CI:1,271-9,472) merupakan faktor risiko kejadian demam berdarah dengue (DBD) di Kabupaten Indramayu. Perlunya masyarakat melakukan upaya pencegahan penyakit DBD seperti mempraktekkan 3M di rumah, serta berperilaku hidup bersih dan sehat.

ABSTRACT               The objective of the research was to analyze the risk factors of  women with the history of normal childbirth had delivery by section caesarea.             The research used observational analytic method with case-control study design. The samples consisted of 42 cases and 42 controls, taken by using consecutive sampling technique. Dependent variable was childbirth with CS and independent variables were parity, accompanied illness, and reference. The gathered data were analyzed by using univatriate analysis, bivariate analysis with chi square test, and multivatriate analysis at α = 0.05.             The result of the research from multivariate analysis showed that the variables which had significant influence on childbirth with SC were parity (p = 0.006; OR = 5.801 95% CI = 1.642-19.110),accompanied illness (p = 0.03; OR = 6.382 95% CI = 1.198-33.992), and reference (p = 0.003; OR = 6.350 95% CI = 1.874-21.522). The variable which had the most dominant influence childbirth with CS was accompanied illness. p-value = 97% which indicated that childbirth women with the parity > 2 had accompanied illness and the reference to the hospital had the possibility to give birth with CS of 97%.             It is recommended to do family planning programme, and prevent the illness during pregnant, apply ANC according to the standard, and make sure that the childbirth is safe from Period I to Period III.   Keywords: Risk Factor, Caesarea, Case-Control

Prolap organ panggul merupakan kondisi yang mempengaruhi kualitas hidup wanita. Prolaps organ panggul ini dapat disebabkan oleh perlukaan sewaktu proses persalinan, proses penuaan, komposisi jaringan pada seorang wanita, batuk- batuk kronis, atau sering melakukan pekerjaan berat. Pengenalan dini prolaps terkait dengan prognosis pemulihan anatomik dan fungsional organ panggul. Hingga kini, penerapannya dalam dunia klinis belum banyak sehingga pelatihan dan pembelajaran lebih lanjut tentang pelvic organ prolapse quantification (POPQ) jelas diperlukan. Penelitian ini dilakukan dengan metode case control study di polikilinik Obgin RSUP. Dr. M. Djamil Padang mulai bulan September 2013 sampai jumlah sampel terpenuhi sebanyak 98 orang. Dengan 49 orang kelompok kontrol dan 49 orang kelompok kasus .Analisis dilakukan untuk menilai hubungan usia, paritas, pekerjaan dan indek massa tubuh dengan kejadian prolap organ panggul berdasarkan skor POPQ. Data disajikan dalam bentuk tabel. Data diuji dengan t test dan chi square test. Jika p<0,05 menunjukan hasil yang bermakna. Terdapat hubungan yang bermakna antara usia dengan kejadian prolap organ panggul dengan (p<0,05) dan OR 27,871.terdapat hubungan yang bermakna antara paritas dengan kejadian prolap organ panggul dengan (p<0,05) dan OR 52,970.Dari analisa statistik pekerjaan tidak bisa di uji secara statistik.indek massa tubuh tidak terdapat hubungan yang bermakna terhadap kejadian prolap organ panggul.(p>0,05)

Masalah kesehatan masyarakat yang cukup seriuspada kelompok usia balita sampai saat ini adalah kejadian malnutrisi, hal iniberdampak pada gangguan pertumbuhan dan perkembangan fisik balita. Malnutrisi juga dapat menyebabkan balita menjadi rentan terhadap penyakit infeksi. Beberapa faktor penyebab malnutrisi diantaranya faktor makanan dan penyakit infeksi yang mungkin diderita anak, faktor ketahanan pangan dikeluarga, pola pengasuhan anak, pelayanan kesehatan dan kesehatan lingkungan. Tujuan penelitian ini adalah untuk mengetahui apakah sindrom penyakit tropis merupakan prediktor terjadinya gizi kurang pada balita. Penelitian ini termasuk penelitian observasional analitik dengan rancangan Case Control Study. Populasi dalam penelitian ini adalah seluruh pasangan ibu dan balita yang ada di daerah pesisir kecamatan Nambo Kota Kendari denganteknik simple randam sampling di dapatkan jumlah sampel  sebanyak 164 orang yang terdiri dari dua kelompok yaitu kelompok kasus dan kelompok kontrol dengan perbandingan 1:1. Data dianalisis secara statistik dengan uji Chi square pada tingkat kepercayaan 95% (α=0,05) dengan mempertimbangkan nilai Odd Ratio dan nilai Confidence Interval. Hasil penelitian ini menunjukan bahwa sindrom penyakit tropis (diare dengan р-value = 0,001 dan OR = 4,162, batuk – batuk dengan р-value = 0,001 dan OR = 3,552,ISPAnon pneumoni dengan р-value = 0,004 dan OR = 3,003) merupakan faktor prediktor terjadinya malnutrisi pada balita sedangkan infeksi cacing dengan р-value = 0,056 dan OR= 1,773 bukan merupakan faktor prediktor terjadinya malnutrisi pada balita di daerah pesisir Kecamatan Nambo Kota Kendari

BACKGROUND: Subacromial pain (SAP) is a common complaint of young athletes, independently of the sport engaged. The prevalence of SAP in some sports is up to 50%. OBJECTIVE: The study was aimed to investigate some new factors possibly associated to subacromial pain in young athletes. The factors considered were the grade of tightness of the clavicular portion of the pectoralis major, dysfunction of the sternoclavicular joint, and serratus anterior and lower trapezius strength. METHODS: This case-control study included 82 young athletes 9–15 years, 41 with the symptoms of SAP and 41 controls. All participants self-reported whether they had subacromial pain. In addition, Hawkins–Kennedy Test was performed to all the participants to evaluate the subacromial pressure. Main outcome measures were the grade of tightness of the clavicular portion of the pectoralis major, dysfunction of the sternoclavicular joint, and serratus anterior and lower trapezius strength. The grade of tightness of the clavicular portion of the pectoralis major and the dysfunction of the sternoclavicular joint were measured with an inclinometer. Serratus anterior and lower trapezius strength were measured by a handheld dynamometer with external belt-fixation. The data were analyzed using t-test for independent samples, Mann-Whitney U test, contingency coefficients and a stepwise binary logistic regression. RESULTS: Significant statistical difference was observed in the grade of tightness of the clavicular portion of the pectoralis major and in the variable representing the physiological functioning of the sternoclavicular joint, between the cases and the controls. There was no significant difference in serratus anterior and lower trapezius strength between the cases and the controls. Logistic regression analysis showed that the variable representing the physiological functioning of the sternoclavicular joint and the grade of shortening of the clavicular portion of the pectoralis major were good predictors for presence of SAP. CONCLUSIONS: A strong association was determined between subacromial pain in young athletes, clavicular portion of pectoralis major tightness and the dysfunction of the sternoclavicular joint.

Background: Infusion reactions (IRs) are the most common adverse events (AEs) of infliximab (IFX) treatment in patients with inflammatory bowel disease (IBD). Prophylactic premedication (PM) with corticosteroids or antihistamines prior to IFX infusions has been used in clinical practice, but its efficacy is not known. The aim of this study was to assess the influence of steroid PM on IR incidence in pediatric patients with IBD receiving IFX. Methods: We performed a case–control study that included pediatric patients with IBD receiving IFX. Patients were divided into four subgroups according to the agent and PM they received: Remicade (original drug) + PM, and two biosimilars—Reshma +/− PM, and Flixabi—PM. At our site, until 2018, PM with steroids was used as a part of standard IFX infusion (PM+); however, since then, this method has no longer been administered (PM−). IRs were divided into mild/severe reactions. Differences between subgroups were assessed with the appropriate chi-square test. Multivariate logistic regression was used to assess associations between PM and IR incidence, correcting for co-medication usage. Results: There were 105 children (55 PM+, 44 male, mean age 15 years) included in the study who received 1276 infusions. There was no difference between the PM+ and PM− subgroups, either in incidence of IR (18.2% vs. 16.0% of patients, p > 0.05) or in percentage of infusions followed by IR (2.02% vs. 1.02% of infusions, p > 0.5). The OR of developing IR when using PM was 0.34, and the difference in IRs ratio in PM+ and PM− patients was not statistically significant (95% CI, 0.034–1.9). There were 11/18 (61.1%) severe IRs (anaphylactic shock) reported in all patients (both PM+ and PM−). Conclusion: At our site, the incidence of IR was low, and PM did not decrease the incidence of IR in pediatric patients with IBD receiving IFX. These results indicate that PM with steroids should not be a standard part of IFX infusion to prevent IR.

Abstract Background Conflicting information exists regarding the association between insulin resistance (IR) and left ventricular hypertrophy (LVH). We described the associations between obesity, fasting insulinemia, homeostasis model assessment of insulin resistance (HOMA-IR), and LVH in Black patients with essential hypertension. Methods A case–control study was conducted at the Centre Médical de Kinshasa (CMK), the Democratic Republic of the Congo, between January and December 2019. Cases and controls were hypertensive patients with and without LVH, respectively. The relationships between obesity indices, physical inactivity, glucose metabolism and lipid disorder parameters, and LVH were assessed using linear and logistic regression analyses in simple and univariate exploratory analyses, respectively. When differences were observed between LVH and independent variables, the effects of potential confounders were studied through the use of multiple linear regression and in conditional logistic regression in multivariate analyses. The coefficients of determination (R2), adjusted odds ratios (aORs), and their 95% confidence intervals (95% CIs) were calculated to determine associations between LVH and the independent variables. Results Eighty-eight LVH cases (52 men) were compared against 132 controls (81 men). Variation in left ventricular mass (LVM) could be predicted by the following variables: age (19%), duration of hypertension (31.3%), body mass index (BMI, 44.4%), waist circumference (WC, 42.5%), glycemia (20%), insulinemia (44.8%), and HOMA-IR (43.7%). Hypertension duration, BMI, insulinemia, and HOMA-IR explained 68.3% of LVM variability in the multiple linear regression analysis. In the logistic regression model, obesity increased the risk of LVH by threefold [aOR 2.8; 95% CI (1.06–7.4); p = 0.038], and IR increased the risk of LVH by eightfold [aOR 8.4; 95 (3.7–15.7); p < 0.001]. Conclusion Obesity and IR appear to be the primary predictors of LVH in Black sub-Saharan African hypertensive patients. The comprehensive management of cardiovascular risk factors should be emphasized, with particular attention paid to obesity and IR. A prospective population-based study of Black sub-Saharan individuals that includes the use of serial imaging remains essential to better understand subclinical LV deterioration over time and to confirm the role played by IR in Black sub-Saharan individuals with hypertension.

OBJECTIVE: To determine patient and perioperative characteristics associated with unexpected postoperative clinical deterioration as determined for the need of a postoperative emergency response team (ERT) activation. DESIGN: Retrospective case–control study. SETTING: Tertiary academic hospital. PARTICIPANTS: Patients who underwent general anaesthesia discharged to regular wards between 1 January 2013 and 31 December 2015 and required ERT activation within 48 postoperative hours. Controls were matched based on age, sex and procedure. MAIN OUTCOME MEASURES: Baseline patient and perioperative characteristics were abstracted to develop a multiple logistic regression model to assess for potential associations for increased risk for postoperative ERT. RESULTS: Among 105 345 patients, 797 had ERT calls, with a rate of 7.6 (95% CI, 7.1–8.1) calls per 1000 anaesthetics (0.76%). Multiple logistic regression analysis showed the following risk factors for postoperative ERT: cardiovascular disease (odds ratio [OR], 1.61; 95% CI, 1.18–2.18), neurological disease (OR, 1.57; 95% CI, 1.11–2.22), preoperative gabapentin (OR, 1.60; 95% CI, 1.17–2.20), longer surgical duration (OR, 1.06; 95% CI, 1.02–1.11, per 30 min), emergency procedure (OR, 1.54; 95% CI, 1.09–2.18), and intraoperative use of colloids (OR, 1.50; 95% CI, 1.17–1.92). Compared with control participants, ERT patients had a longer hospital stay, a higher rate of admissions to critical care (55.5%), increased postoperative complications, and a higher 30-day mortality rate (OR, 3.36; 95% CI, 1.73–6.54). CONCLUSION: We identified several patient and procedural characteristics associated with increased likelihood of postoperative ERT activation. ERT intervention is a marker for increased rates of postoperative complications and death.
