Purpose Hyperkalemia more commonly affects patients with a glomerular filtration rate of less than 60 mL/min. Using intravenous (IV) insulin to shift potassium intracellularly may cause hypoglycemia, requiring additional treatment or longer hospitalization. Literature on insulin dosing in this context is limited, with one previous study indicating that 5 units of IV insulin might be as effective and result in less hypoglycemia than the standard dose of 10 units of IV insulin. The hyperkalemia treatment pathway at our institution was revised in May 2018 to include a reduced-dose option (5 units of insulin) for patients with end-stage renal disease. This study aimed to compare the prevalence of hypoglycemia between patients who received standard-dose vs reduced-dose IV insulin. Methods This single-center, retrospective, quasi-experimental study evaluated the impact of revision of the hyperkalemia treatment pathway by assessing rates of hypoglycemia during the 6 months before and after implementation of the revised pathway. The primary endpoint was prevalence of hypoglycemia, defined as a blood glucose level of less than or equal to 70 mg/dL. Results There was no statistically significant difference in the occurrence of hypoglycemia when comparing the pre- and postimplementation groups (36 [17.7%] patients vs 34 [18.7%] patients; P = 0.7924). The postimplementation group had a statistically significant lower reduction in potassium levels after treatment than the preimplementation group (mean [interquartile range], –0.9 [–1.3, –0.5] mEq/L vs –0.6 [–1.2, –0.2] mEq/L; P = 0.0095). Baseline potassium levels were similar between the groups. Conclusion Administration of reduced-dose IV insulin for treatment of hyperkalemia was significantly less effective in lowering serum potassium levels and did not decrease prevalence of hypoglycemia. When accounting for potential confounders, the only variable that was associated with hypoglycemia was pretreatment glucose level.

Abstract: The Effect Of Lime (C10H16O) And Camphor (C10H16O) Against The Density Of Flies On Organic Waste. Fly is a disease-carrying vector so it is important to measure the density for control purposes. One of the controls that is done is on organic waste which is a breading source place of flies. Control is done by affixing the mixture of lime and camphor. This research aimed to determine the effect of mixture of lime and camphor against the density of flies in organic waste that is carried out in the garbage construction site while Southern Loktabat. This type of research is quasi-experimental that experimenting the provision of nine variations of dosage mixture of lime and camphor against the density of flies in organic waste with the organic waste sample was about 0,16m3. This research design used Pretest-posttest design. The analysis using statistical paired sample t-test, Kruskal Wallis test, and Least Significant Differences Test. The measurements showed a decrease in the density of the flies after administration of the lime and camphor mixture. Paired T-test results showed that in each treatment the p-value (2-tailed) <α (0.05) means that there is significant difference of flies density before and after treatment. Kruskal Wallis test result showed the p value (0,000) <α (0.05) which means there are significant differences between dosages. LSD test result showed that the most effective dosage is sixth dosage (35gr camphor+ 55gr lime/0,16m3 waste).

Completeness of nursing documentation is one of the quality indicators of nursing care. Based on the nursing practice law no. 38 article 37, nurses in carrying out Nursing Practices are obliged to document Nursing Care in accordance with the standards. Documentation in Inpatient PKM consisting of Siko, Jambula, and Sulamadaha PKM itself, based on the observations of researchers found several obstacles, namely, the lack of nurses given the large number of PKM programs that must be run, varying levels of education so documentation has a different understanding, this also supported by the lack of education and training related to temporary nursing care documentation relating to the Nursing Process and Documentation experiencing some recent changes. The purpose of this study was to determine the effect of applying the nursing documentation format for the model Problem-Oriented Record (POR) on the performance of nurses in the Inpatient Health Center in Ternate City. The design of this study is quasi-experimental, namely, pre and post-test without control group. The population is nurses with 41 people in PKM Inpatient who are in Ternate City. Samples were taken by total sampling technique and analyzed using statistical tests t-test, Anova, and correlation. The results of the study found a significant difference between the performance of nurses before and after the application of the documentation format for nursing care in the POR model (p-value: 0,0005 α: 0.05) while the characteristics of respondents included age, sex, marital status, length of work and education had no effect on performance.

Introduction: Benign hematological disorders (BHDs) such as anemias and sickle cell disease have a major burden of disease in Saudi Arabia (SA). Their prevalence is estimated to exceed 50% of the total population. and they are responsible for one fifth of total hospital admissions. Although treatable, most of these hematological conditions are of chronic nature and remain without a cure. The chronicity of these conditions poses a great risk of developing depression among patients, especially among those who experience symptoms. The development of depression, besides its own social and psychological burden on patients, it further worsens the impact of these BHDs as it negatively affects patients' compliance and adherence to treatment leading to worse prognosis. One of the biggest challenges in managing depression, among those with BHDs, is its detection by the treating physician. This study aims to examine physicians' perception and attitude towards depression among their patients. Methods: A cross-sectional study, in which an anonymized survey was distributed to 29 physicians who are working in SA who treat BHDs. Physicians who cannot read English, are no longer practising or not dealing with patients with hematological conditions were excluded from the study. The survey had 2 sections to assess physicians' knowledge of depression and their attitudes towards it e.g. willingness to attend mental health training, discussion in current practice and updating working guidelines. A knowledge score (K score) and an attitude score (A score) were calculated based on their survey responses to a maximum of 16 and 4, respectively. The distribution of K and A scores were determined using Shapiro Wilk Test. Multivariate analysis was used to compare K and A scores according to the independent physicians' variants. Correlations were determined using two-tailed Pearson's correlation coefficient. Statistical significance was denoted at a P value less than 0.05. This study was approved by Taibah University College of Medicine Research Ethics Committee. Results: Twenty-nine physicians were enrolled in this study, of which 27% were female (n=8). The median physicians' age was 27 years (range 24 - 50 years old). Female physicians were significantly older than male physicians, and subsequently their years of experience was significantly longer (P value < 0.0001). The physicians' specialties included Family Medicine (n=10), Internal Medicine (n=10), Pediatrics (n=2) and others (n=7). All but one physician completed their medical education in SA, and only 2 physicians completed their training outside SA. Sixteen physicians have referred patients to a mental health specialist in the last 3 years, of which only 2 female physicians (25% of the total female physicians) as compared to 14 male physicians (~67%).The median K and A scores for the cohort was 9 (out of 16) and 2.5 (out of 4), respectively. There was no significant difference between the K score of the male and female physicians, however, female physicians had significantly lower A score. Using 2-way ANOVA to assess if physicians' gender and their referral of patients to mental health specialists affect any of the scores, it was found that gender significantly influence A score (P value < 0.05). There was no statistical difference in K and A scores between physicians based on their place of work, the number of patients seen daily or whether they referred a patient to a mental health specialist in the last 3 years. Both K and A scores correlated negatively with physicians' years of experience, however, only A scores correlated significantly with years of experience (r=-0.4, P value < 0.01). Upon further grouping of physicians based on their years of experience (<1 year; 1-5 years and > 5 years), physicians with less than 1 year of experience had the highest K and A scores compared to the rest of the cohort. Conclusion: This study demonstrates that early career physicians are more knowledgeable and have a better attitude towards depression, predominantly among male physicians. These findings indicate that undiagnosed depression among patients with BHDs can be attributed to physicians' knowledge and attitude towards depression. Training programs and practice guidelines should prepare physicians to adopt a holistic approach in dealing with patients with BHDs and actively screen for depression among their patients. Disclosures No relevant conflicts of interest to declare.

Abstract Background The COVID-19 pandemic has led to massive restrictions in public and private lives, including a shut-down of face-to-face teaching at universities in Germany. We aimed to examine the impact of these changes on perceived stress, mental health and (study-)related health behavior of students in a longitudinal study. Methods For two timepoints – the year before the COVID-19 pandemic (2019, n = 1377) and the year during the COVID-19 pandemic (2020, n = 1867) – we surveyed students of all faculties at one German university for perceptions and preventive behavior regarding the COVID-19 pandemic using standard instruments for stress, anxiety, depression, and behavior and experience patterns. Results About 90% of students (n = 1633) in 2020 did not have a known contact infected with SARS-CoV-2, while 180 (9.8%) did have one. Only 10 respondents (0.5%) reported an infection with SARS-CoV-2. Wearing masks and washing hands more often were practiced by ≥80% of students. Taking more care about cleanliness (51.8%) and using disinfectants (39.2%) were practiced much less. A higher percentage of female compared with male students and medical/health science students compared with science, technology, engineering, and mathematics students engaged more frequently in specific or nonspecific preventive measures. More than three quarters (77.1%) of all students rated their general health as (very) good. There were no significant differences in general health, stress, and depression between 2019 and 2020 in the students who responded at both timepoints. The distribution of behavior and experience patterns for this group showed a slight but significant difference from 2019 to 2020, namely decreasing proportions of students with a healthy pattern and a risk pattern for overexertion. Students with different behavior and experience patterns showed marked differences in perceptions and reaction to the COVID-19 pandemic as well as psychosocial stress and symptoms, with higher scores for mental health symptoms and lower scores in preventive behavior regarding risk patterns. Conclusion Despite massive alterations to students’ lives in 2020, there were only moderate consequences for mental health compared with 2019 in the total student group of this German university. However, identifying students at risk would offer opportunities to foster mental health in relevant subgroups.
