Osteoarthritis (OA): A Modern Disease of the Anthropocene Era

Abstract In this paper we discuss recent studies that have unravelled important roles for modern-day environmental factors in the pathogenesis of osteoarthritis (OA). OA is a relatively modern disease in terms of human evolution. While our ancestors have been around for about six million years, modern humans (Homo sapiens) only evolved about 200,000 years ago. Early humans did not live long enough to suffer from age-related musculoskeletal conditions such as OA and arthritic diseases are thought to have been extremely rare in early Natufians and Egyptians, but this is probably because of an underrepresentation of older adults in the skeletal records. Examination of Egyptian skeletal records from burial sites provides some evidence of arthritic diseases such as OA, but mainly in the mummified remains of pharaohs and high-ranking officials, viziers, priests and nobles. Examination of the skeletal records in Roman burial ground does reveal more evidence of arthritic diseases but the evidence is sketchy and we can only assume that arthritic diseases were more prevalent in the slave population in ancient Egypt and the Roman Empire. Recent studies indicate that independent risk factors for knee OA either arose or have become amplified in the post-industrial era, suggesting that the prevalence of OA has gradually increased since the industrial revolution. Alarmingly, the incidence of knee OA has doubled since the middle of the 20thcentury, suggesting that OA is a modern disease of the Anthropocene era and there may be unknown environmental factors that have led to its increased incidence since industrialization.

About 10,000 years ago, at the beginning of the agriculturalrevolution, on the whole earth lived between 5 and 8 million hunter-gatherers, all belonging to the Homo sapiens species. Five thousand years later, freed from the primary needs for survival, some belonging to that species enjoyed the privilege of devoting themselves to philosophical speculation and the search for transcendental truths. It was only in the past two hundred years, however, with the advent of the Industrial Revolution, that reaping nature’s secrets and answering fundamental questions posed by the Universe have become for many full-time activities, on the way to becoming a real profession. Today the number of scientists across the globe has reached and exceeded 10 million, that is, more than the whole human race 10,000 years ago. If growth continues at the current rate, in 2050 we will have 35 million people committed full-time to scientific research. With what consequences, it remains to be understood. For almost forty years I myself have been concerned with science in a continuing, direct, and passionate way. Today I perceive, along with many colleagues, especially of my generation, that things are evolving and have changed deeply, in ways unimaginable until a few years ago and, in some respects, not without danger. In the post-industrial revolution world, social change is often studied and understood in the context of change in means of production, mobility, urbanization and change in the constitution of workforce. Role of ethical values is generally confined to personal conduct and manners. Industrial society is supposed to have its own work ethics which may or may not agree with personal ethics and morality. Ethics and morality are generally considered, in the Western thought, as a social construct. Therefore, with the change in means of production or political system, values and morality are also expected to be re-adjusted in order to cope with the changed environment. Sometimes a totally new set of values emerges as a consequence of the change in economic, political, or legal set up. The present research tries to understand the meaning and place of these values in a global socio-cultural framework. Relying essentially on the divine principles of the Qur'ān it makes an effort to understand relevance of these universal and ultimate principles with human conduct and behavior in society.  It indicates that essentially it is the core values, principles, or norms which guide human beings in their interpersonal, social, economic and political matters. Islam being a major civilizing force, culture, and the way of life, provides values which guide both in individual and social matters. The values given by the Qur’ān and the Sunnah are not monopoly of the Muslim. These values are universal and are relevant in a technological society.

Housing and its evolution constitutes an important study for all councils. This paper limns the encyclopaedic timeline of housing from the times of pre-urban dwellings of nomadic, semi-nomadic, and sedentary agricultural societies to the present day, while focusing on the chunks of a comprehensive architecture, history and anthropology. A detailed literature review made it evident that early urban dwellings were insular and extended around an internal patio. Lately, these housing forms lasted in the original metropolitan house arrangements in the Islamic world, China, India, Latin America, the Iberian Peninsula and the Indian subcontinent like Indus valley civilization. After the fall of the Roman Empire, there was a drift towards peripheral house forms which engaged the early forms of urban settlement in the world today. The study also revealed that the Middle Age dwellings functioned as both residences and work places, yet with the passage of time the buildings became more functionalized, thus dividing dwellings and work places from each other. With the advent of the industrial revolution, there were remarkable variations in the suburban expansion of housing in the western world that became isolated along socioeconomic outlines and the housing types diverged with less populated, single-family communities at one extreme and densely populated, high rise, multi-family apartments at the other extreme. It is concluded that the side effects of the American transportation system have resulted into rigorous peripheral dwellings which includes ineffective use of land, air contamination and the city degeneration suggesting solutions based on a rich variety of historical examples.

The relative contribution of genetic factors and more modifiable environmental factors to a clinically<br />relevant osteoarthritis (OA) diagnosis is unkown. In this paper we present an ongoing study on the genetic<br />contribution to total joint replacement (TJR) due to hip and knee OA and effects of lifestyle and lifestyle<br />related conditions on TJR due to OA. We have linked data on incident OA from the Norwegian Arthroplasty<br />Registry were l with the Norwegian Twin Registry on the National ID number in 2014, thus obtaining a<br />population based cohort of same-sex twins born 1915-60. Data on height, weight and lifestyle were selfreporteded<br />in questionnaires conducted between 1978 and 1992. The monozygotic (MZ) and dizygotic<br />(DZ) concordances as well as the genetic contribution vs. contribution of more modifiable, environmental<br />factors to arthroplasty will be examined in separate analyses for the hip and the knee joint. The sample<br />comprised N=18058 twins (N=3803 MZ and N=5226 DZ pairs) including N=9650 (53.4%) females and a<br />mean (SD) age of 38 (12.3) years at questionnaire response. Some preliminary analyses have been performed<br />showing a higher concordance for TJR due to hip OA among MZ (0.36) than DZ twins (0.16), which may<br />be consistent with a genetic contribution to hip OA. TJR due to hip OA may be determined by genetic<br />factors. Results for the knee joint as well as final results from hereditary analyses and co-twin control<br />analyses of will be published consecutively from 2016.

Objective: To study the differences in clinical characteristics, risk factors, and complications across age-groups among the inpatients with the coronavirus disease 2019 (COVID-19).Methods: In this population-based retrospective study, we included all the positive hospitalized patients with COVID-19 at Wuhan City from December 29, 2019 to April 15, 2020, during the first pandemic wave. Multivariate logistic regression analyses were used to explore the risk factors for death from COVID-19. Canonical correlation analysis (CCA) was performed to study the associations between comorbidities and complications.Results: There are 36,358 patients in the final cohort, of whom 2,492 (6.85%) died. Greater age (odds ration [OR] = 1.061 [95% CI 1.057–1.065], p &lt; 0.001), male gender (OR = 1.726 [95% CI 1.582–1.885], p &lt; 0.001), alcohol consumption (OR = 1.558 [95% CI 1.355–1.786], p &lt; 0.001), smoking (OR = 1.326 [95% CI 1.055–1.652], p = 0.014), hypertension (OR = 1.175 [95% CI 1.067–1.293], p = 0.001), diabetes (OR = 1.258 [95% CI 1.118–1.413], p &lt; 0.001), cancer (OR = 1.86 [95% CI 1.507–2.279], p &lt; 0.001), chronic kidney disease (CKD) (OR = 1.745 [95% CI 1.427–2.12], p &lt; 0.001), and intracerebral hemorrhage (ICH) (OR = 1.96 [95% CI 1.323–2.846], p = 0.001) were independent risk factors for death from COVID-19. Patients aged 40–80 years make up the majority of the whole patients, and them had similar risk factors with the whole patients. For patients aged &lt;40 years, only cancer (OR = 17.112 [95% CI 6.264–39.73], p &lt; 0.001) and ICH (OR = 31.538 [95% CI 5.213–158.787], p &lt; 0.001) were significantly associated with higher odds of death. For patients aged &gt;80 years, only age (OR = 1.033 [95% CI 1.008–1.059], p = 0.01) and male gender (OR = 1.585 [95% CI 1.301–1.933], p &lt; 0.001) were associated with higher odds of death. The incidence of most complications increases with age, but arrhythmias, gastrointestinal bleeding, and sepsis were more common in younger deceased patients with COVID-19, with only arrhythmia reaching statistical difference (p = 0.039). We found a relatively poor correlation between preexisting risk factors and complications.Conclusions: Coronavirus disease 2019 are disproportionally affected by age for its clinical manifestations, risk factors, complications, and outcomes. Prior complications have little effect on the incidence of extrapulmonary complications.

Since the Industrial Revolution, a new era has arisen called the Anthropocene, in which human actions have become the main driver of global environmental change outside the stable environmental state of the Holocene. During the Holocene, environmental change occurred naturally, and the Earth’s regulatory capacity maintained the conditions that enabled human development. Resource overexploitation of the industrial “Anthropocene”, under the principle of profit maximization, has led to planetary ecological crises, such as overloaded carbon sinks and climate changes, vanishing species, degraded ecosystems, and insufficient natural resources. Agro-based society, in which almost all demands of humans can be supported by agriculture, is characterized by life production. The substitution of Agro-based society for a post-industrial society is an evolutionary result of social movement, it is an internal requirement of a sustainable society for breaking through the resource constraint of economic growth. The core feature of agriculture is to use organisms as production objects and rely on life processes to achieve production goals. The substitution of Agro-based society for a post-industrial society is the precondition for a sustainable carbon cycle, breaking through the resource limits of the industrial “Anthropocene”, alleviating the environmental pressure of economic development, and promoting society from increasing disorderly entropy to orderly decreasing entropy. Meanwhile, technological advancements and growing environmental awareness of society make it feasible for the substitution of an agro-based society for a post-industrial society.

In this chapter photographer John Darwell, reminds us that big environmental issues and Homo sapiens’s problematic response to them are also evident in the mundane experience of our day-to-day environment. Darwell, who until fairly recently had based his photographic practice on post-industrial landscapes of Sheffield and Manchester, and the area around Chernobyl, has now turned his attention to the ‘edgeland’ of his twice-daily dog walks. This immersion in the landscape throws up new subjects for his work. One aspect that stands out is the phenomenon of discarded dog-shit bags. Dog owners have taken the trouble to clean up after their pets, but then discard the bags by hanging them in trees or just throwing them away. Bringing photographic aesthetics to what is a disturbing subject has allowed him to develop a typology of this practice. While we may prefer to look away, these images do pose big questions for environmentalists. How can we solve the big issues of climate change and loss of biodiversity, if we can’t even carry through to its conclusion this modest attempt not to despoil the environment?

Fear is a human emotion that allows a person to survive. It has a function to ensure the continuity of life. The definition of fear has changed over time. Along with human development, transition to sedentary life, the industrial revolution, and modern life, fear and the things feared have changed. Fear has started to be marketed, especially in post-industrial societies. The governments have seen that fear and violence work to regulate, control, and passivate people. Political governments have had the unique opportunity to use fear as a mechanism for control and surveillance. The governments have aestheticized the fear and presented it indirectly with the support of media. The masses have been shaped as weak, scared, anxious, and helpless in the shadow of fear and violence. This study tries to shed light on the attempt to persuade the society about the legitimacy of the military government by presenting fear and violence to people in aestheticized forms in the 1980s in Turkey.
