Abstract Lyfas is a smartphone-based biomedical application that captures the cardiovascular autonomic biomarkers (CVb), surrogating for mental health attributes. SD1/SD2 biomarker assesses the sympathovagal balance and is considered to be a potential indicator of Lyfas anxiety score (LAS). A total of 1837 males and 973 females took Lyfas (hypersensitivity-checked) and Hamilton Anxiety Rating Scale (HAM-A) self-scoring tests. LAS has been statistically validated by Linear regressions, one-way ANOVA, t-stat, correlations (r), and Bland Altman agreement assessment with respect to HAM-A. Sensitivity, specificity, precision, accuracy, Fscore, and Youden’s index (j-stat) are computed. Results show that (i) Lyfas is not a very hypersensitive instrument (mean-variance is 0.6). (ii) It can predict HAM-A with 94.7% accuracy (R2) and is a statistically significant model (p <0.05). (iii) LAS and HAM-A are positively correlated by 97%, the t-stat value of 5.38 for the population indicates that the two instruments have a significant mean difference. (iv) Bland Altman test showcases the overall agreement of 12.95% due to different modes and scales of measurements. (v) on average, LAS is 87.78% accurate, 86.82% precise, and its’ 65.2% j-stat value proves that Lyfas is a novel industry-standard smartphone biomarker application that can be used to accurately screen anxiety disorders.

Background: Anxiety is a condition of psychological and physiological disorders characterized by cognitive, somatic, emotional disturbances and components of behavioral sequences. Purpose: This study aims to describe the anxiety of chronic kidney disease patients undergoing hemodialysis. Methods: This study uses univariate analysis which will describe the anxiety level of chronic kidney disease patients undergoing hemodialysis. The anxiety variable was measured using the HARS (Hamilton Anxiety Rating Scale) anxiety instrument with a validity value of 0.68 dd 0.93 and a reliability value of 0.93. The population in this study were all chronic kidney disease patients who underwent hemodialysis and experienced anxiety. The sampling technique used in this study was purposive sampling technique. The sample in this study were patients with chronic kidney disease who met the requirements of the study patients, namely: Patients who underwent hemodialysis for less than one year, patients undergoing hemodialysis with femoral vein puncture, patients undergoing hemodialysis twice a week. Results: The results showed that the study respondents had a mild anxiety rate of 9 patients (56.25%), moderate anxiety as many as 8 patients (21.875%) and severe anxiety as many as 8 patients (21.875%). Conclusion: conclusions and implications for nursing practice. The results showed a high incidence of anxiety in patients undergoing hemodialysis and distributed evenly on mild, moderate and severe anxiety.

Abstrak: Kecemasan dialami oleh hampir semua orang di dunia, termasuk mahasiswa baru kedokteran. Mahasiswa baru kedokteran memiliki banyak stressor termasuk ujian yang menimbulkan kecemasan dan dapat mempengaruhi hasil ujian mereka. Sampai saat ini, hanya sedikit penyelidikan yang ditemukan menyelidiki fenomena ini. Di Fakultas Kedokteran Universitas Sam Ratulangi, tidak ada yang meneliti fenomena tersebut. Penelitian ini bertujuan untuk menilai hubungan antara kecemasan dan hasil ujian semester 1 (UAS-1) mahasiswa baru program studi kedokteran umum tahun akademik 2012/2013 di Universitas Sam Ratulangi. Ini merupakan penelitian analitik potong lintang dengan metode survei dan sensus sebagai cara dalam pengambilan sampel. Sampel penelitian yang telah memenuhi kriteria inklusi dan ekslusi didapatkan berjumlah 298 orang dari 319 mahasiswa baru program studi kedokteran umum tahun akademik 2012/2013 di Universitas Sam Ratulangi. Populasi tersebut kemudian diberi informed consent, kuesioner data sosiodemografi, dan dinilai kecemasannya menggunakan Hamilton Anxiety Rating Scale (HARS). Tidak ada hubungan yang signifikan antara kecemasan dan hasil UAS-1 (p=0,602>0,05). Ada hubungan yang signifikan antara jenis kelamin dan kecemasan (p=0,005<0,05) with odds ratio 2,91. 267 dari 298 mahasiswa (89,6%) mengalami kecemasan dengan kecemasan ringan sebagai tingkat kecemasan yang paling banyak ditemukan, berjumlah 177 orang (59,4%). Semua mahasiswa baru program studi kedokteran umum tahun akademik 2012/2013 di Universitas Sam Ratulangi mengalami kecemasan, terutama kecemasan ringan. Tidak terdapat hubungan antara kecemasan dengan hasil UAS-1 mereka, tetapi terdapat hubungan antara jenis kelamin dan kecemasan dengan kecenderuang hampir 3 kali bagi mahasiswa baru perempuan untuk mengalami kecemasan dibandingkan dengan mahasiswa baru laki-laki. Dibutuhkan penelitian lebih lanjut terkait coping strategies dan defense mechanism yang dilakukan mahasiswa untuk menganggulangi kecemasan mereka. Kata Kunci: Kecemasan, Ujian, Mahasiswa kedokteran.   Abstract: Anxiety is experienced by almost all people around the world including new medical students. New medical student experiences a lot of stressor include examination which cause anxiety to occur and may affect their exam results. Somehow, there are a few study found to date that has investigate this phenomenon. In Sam Ratulangi University, which is the nearest medical faculty, there is no research for such phenomenon. This study aims to investigate the relationship between anxiety and new medical student’s achievement on their last exam in Medical Faculty of Sam Ratulangi University. This is an analytic research using a survey method with census as the option of sampling. However, the research samples are who meet inclusion category and not in the exclusion category. The nearest samples are in Sam Ratulangi University who are given questioners to measure their anxiety by using Hamilton Anxiety Rating Scale (HARS) before their exam and to collect their sosiodemograpic data as well. There is no significant relationship between anxiety and their last exam results (p=0,602>0,05), but there is a significant relationship between gender and anxiety (p=0,005<0,05) with odds ratio 2,91. According to anxiety test results, 267 of 298 grade-1 medical students (89,6%) present an anxiety and the most prevalence anxiety level is mild anxiety with total 177 people (59,4%). In conclusion, almost all grade-1 new medical students in Medical Faculty of Sam Ratulangi University experienced anxiety with mild anxiety as the most prevalence anxiety level. There is no significant relationship between anxiety and their last exam results, but there is a significant relationship between gender and anxiety with almost 3 times for new female medical students experiencing anxiety than new male medical students. Further research is needed to investigate coping strategies and defense mechanisms which may be used to cope with their anxiety. Keywords: Anxiety, Exam, Medical Student

Lanjut usia (lansia) merupakan tahap akhir perkembangan pada daur kehidupan manusia yang telah mencapai usia lebih dari 60 tahun. Pada lansia terjadi proses degeneratif yang akan berdampak terjadinya perubahan-perubahan diri manusia. Perubahan yang terjadi pada lansia mengakibatkan berbagai masalah yang kompleks baik secara fisik maupun psikologis. Kecemasan merupakan salah satu respon yang paling dini dan sering muncul pada lanjut usia. Selain dengan farmakologis, kecemasan juga dapat ditangani secara non farmakologis salah satunya dengan membaca Al-Qur’an. Penelitian ini bertujuan mengetahui pengaruh membaca Al-Qur’an terhadap kecemasan pada lansia di Panti Sosial Tresna Werdha (PSTW) Khusnul Khotimah Pekanbaru. Desain penelitian adalah quasy eksperiment dengan metode penelitian one group pretest & posttest. Sampel penelitian adalah lansia berjumlah 42 orang menggunakan metode probability sampling dengan teknik simple random sampling. Pengumpulan data dilakukan dengan lembar observasi untuk membaca Al-Qur’an dan penilaian kecemasan dengan Hamilton Anxiety Rating  Scale. Data dianalisis secara statistik deskriptif dan Paired Sample T Test. Hasil penelitian menunjukkan bahwa ada pengaruh membaca Al-Qur’an dengan kecemasan pada lanjut usia dengan p value 0.002 < 0.05. Penelitian ini diharapkan dapat meningkatkan asuhan keperawatan gerontik sehingga membaca Al-Qur’an dapat dijadikan salah satu intervensi dalam meningkatkan spiritualitas serta menurunkan kecemasan pada lansia.

Clinical psychological factors may predict medical diseases. Anxiety level has been associated with osteoporosis, but its role on bone mineral density (BMD) change is still unknown. This study aimed to investigate the association between anxiety levels and both adherence and treatment response to oral bisphosphonates (BPs) in postmenopausal osteoporosis. BMD and anxiety levels were evaluated trough dual-energy X-ray absorptiometry and the Hamilton Anxiety Rating Scale (HAM-A), respectively. Participants received weekly medication with alendronate or risedronate and were grouped according to the HAM-A scores into tertiles (HAM-A 3 > HAM-A 2 > HAM-A 1). After 24 months, BMD changes were different among the HAM-A tertiles. The median lumbar BMD change was significantly greater in both the HAM-A 2 and HAM-A 3 in comparison with the HAM-A 1. The same trend was observed for femoral BMD change. Adherence to BPs was >75% in 68% of patients in the HAM-A 1, 79% of patients in the HAM-A 2, and 89% of patients in the HAM-A 3 (p = 0.0014). After correcting for age, body mass index, depressive symptoms, and the 10-yr. probability of osteoporotic fractures, anxiety levels independently predicted lumbar BMD change (β = 0.3417, SE 0.145, p = 0.02). In conclusion, women with higher anxiety levels reported greater BMD improvement, highlighting that anxiety was associated with adherence and response to osteoporosis medical treatment, although further research on this topic is needed.

Background The value of clot imaging in patients with emergent large vessel occlusion (ELVO) treated with thrombectomy is unknown. Methods We performed retrospective analysis of clot imaging (clot density, perviousness, length, diameter, distance to the internal carotid artery (ICA) terminus and angle of interaction (AOI) between clot and the aspiration catheter) of consecutive cases of middle cerebral artery (MCA) occlusion and its association with first pass effect (FPE, TICI 2c-3 after a first attempt). Results Patients ( n = 90 total) with FPE had shorter clot length (9.9 ± 4.5 mm vs. 11.7 ± 4.6 mm, P = 0.07), shorter distance from ICA terminus (11.0 ± 7.1 mm vs. 14.7 ± 9.8 mm, P = 0.048), higher perviousness (39.39 ± 29.5 vs 25.43 ± 17.6, P = 0.006) and larger AOI (153.6 ± 17.6 vs 140.3 ± 23.5, P = 0.004) compared to no-FPE patients. In multivariate analysis, distance from ICA terminus to clot ≤13.5 mm (odds ratio (OR) 11.05, 95% confidence interval (CI) 2.65–46.15, P = 0.001), clot length ≤9.9 mm (OR 7.34; 95% CI 1.8–29.96, P = 0.005), perviousness ≥ 19.9 (OR 2.54, 95% CI 0.84–7.6, P = 0.09) and AOI ≥ 137°^ (OR 6.8, 95% CI 1.55–29.8, P = 0.011) were independent predictors of FPE. The optimal cut off derived using Youden’s index was 6.5. The area under the curve of a score predictive of FPE success was 0.816 (0.728–0.904, P < 0.001). In a validation cohort ( n = 30), sensitivity, specificity, positive and negative predictive value of a score of 6–10 were 72.7%, 73.6%, 61.5% and 82.3%. Conclusions Clot imaging predicts the likelihood of achieving FPE in patients with MCA ELVO treated with the aspiration-first approach.

Background: In this study, we want to search the effectiveness of Duloxetine on the severity of pain and quality of life in patients with chronic low back pain who had posterior spinal fixation. Methods: In this randomized, placebo-controlled trial done in 6 months 50 patients who had CLBP and were candidates for PSF surgery selected and divided into two groups (drug and placebo). They filled the VAS, SF-36, and Hamilton questionnaires before surgery and after 6 weeks from using 30 mg of duloxetine or placebo. Results: Significant differences were evidenced among groups for the Visual Analogue Scale (P = 0.005) and Verbal Analogue Scale (p = 0.003). Patients in the Duloxetine group have more visual and verbal pain scores than the placebo group. In the quality of life, there was a significant difference between the two groups before the intervention. Also, significant differences were evidenced among groups for the Hamilton Anxiety Rating Scale (p = 0.17). After the intervention, only the Hamilton Anxiety Rating Scale (p = 0.001) and ‘bodily pain’ and ‘general health’ subscales of quality of life (p = 0.008, 0.004, respectively) have a significant difference between the two groups. There was a significant difference between pre and post-intervention in the Hamilton Anxiety Rating Scale only in the duloxetine group. Also, in terms of quality of life, the subscales of ‘physical role’, ‘emotional role’, ‘physical pain’ and ‘total score of quality of life’ in the duloxetine and placebo groups were significantly different between pre and post-intervention. However, the subscales of ‘physical function’ and ‘general health’ were significantly different only in the duloxetine group between pre and post-intervention. Conclusion: The results suggest that the use of duloxetine in patients who had spinal surgery can help to better control back pain, on the other hand, it can cause a better psychological condition that affects the quality of life.

Background. Sleep problems in patients with advanced Parkinson’s disease (PD) have a deleterious impact on quality of life. Objective. To assess the effect of levodopa-carbidopa intestinal gel (LCIG) infusion on sleep quality in advanced PD patients. Methods. Seven patients participated in a prospective pilot study. Before and after 6 months of LCIG infusion, an overnight polysomnography was performed and the Epworth Sleepiness Scale, fatigue scale, Pittsburgh Sleep Quality Index, Beck Depression Inventory, and the Hamilton Anxiety Rating Scale were administered. Results. PSG showed low sleep efficiency. REM sleep without atony was found in 5 patients. After 6 months of LCIG infusion, the percentage of REM sleep decreased as well as the number of arousals especially due to reduction of spontaneous arousals and periodic leg movements during REM sleep, but differences were not statistically significant. Also, scores of all study questionnaires showed a tendency to improve. Conclusion. The results show a trend toward an improvement of sleep quality after 6 months of LCIG infusion, although differences as compared to pretreatment values were not statistically significant. The sleep architecture was not modified by LCIG. Further studies with larger study samples are needed to confirm these preliminary findings.

Kekerasan dalam rumah tangga adalah bentuk kekerasan yang paling banyak dialami oleh perempuan berusia 25 – 40 tahun. Kecemasan merupakan salah satu bentuk reaksi emosional yang menyertai perempuan ketika mengalami kekerasan dalam rumah tangga. Kecemasan pada perempuan yang menjadi korban kekerasan dalam rumah tangga diukur dengan Hamilton Anxiety Rating Scale (HARS) dan melihat gejala kecemasan dari segi kognitif, somatis, motorik dan afektif. Penelitian ini menggunakan metode kualitatif untuk menggambarkan kecemasan dan metode kuantitatif untuk melihat penurunan tingkat kecemasan dengan terapi seni pada perempuan korban kekerasan dalam rumah tangga. Subyek penelitian ini adalah dua perempuan yang telah bercerai dan mengalami kekerasan dalam rumah tangga dalam bentuk fisik, seksual, psikis atau verbal dan penelantaran rumah tangga. Teknik pengambilan sampel dengan menggunakan metode purposive sampling. Pemberian terapi seni pada masing-masing subyek dilakukan sebanyak tujuh sesi. Dalam penelitian ini, terapi seni telah terbukti dapat mengurangi kecemasan pada perempuan korban kekerasan dalam rumah tangga dengan menunjukan perubahan tingkat kecemasan sebelum dan sesudah intervensi. Keberhasilan terapi seni ini juga dipengaruhi oleh adanya kesadaran pada masing-masing subyek untuk konsisten menjalani terapi.

Tekanan darah adalah faktor penting dalam sistem sirkulasi tubuh manusia. Tekanan darah dapat dengan mudah berubah meski dalam hitungan detik (Sasmalinda, Syafriandi, & Helma, 2013). Pada 2 Maret 2020, pemerintah Indonesia pertama kali mengumumkan dua kasus pasien postif Covid-19. (Pranita, 2020). Pasien tidak berani melakukan pemeriksaan ke rumah sakit, sehingga jika ada keluhan yang tidak begitu berat mereka akan membeli obat di apotik tanpa mengetahui tekanan darahnya. Hal ini sangat mengkhawatirkan karena tekanan darah yang tidak terkontrol dapat menyebabkan komplikasi lain seperti stroke. Tujuan penelitian menganalisis faktor yang berhubungan dengan tekanan darah. Desain penelitian analitik observasional, dengan pendekatan Cross Sectional (Notoatmodjo, 2012). Pelaksanaan bulan  Maret – Mei 2020. Populasi dari Seluruh warga  babatan RT 8 RW 2 Kelurahan Babatan Kecamatan Wiyung sebanyak 110 orang. Teknik Sampel total sampling. Variabel independen; jenis kelamin, beban kerja, pendapatan, tingkat kecemasan dan riwayat keluarga. Variabel dependen; tekanan darah. Instrument penelitian; timbangan injak digital, tensi digital, dan kuesioner. Variabel Tingkat kecemasan  menggunakan HARS (Hamilton Anxiety Rating Scale). Dianalisis uji statistik Chi Square dengan nilai p value <0.05. Hasil penelitian chi square  beban kerja nilai p-value 0,004<0,005 ada hubungan beban kerja dengan  tekanan darah. Hasil  p – value 0,002<0,05 ada hubungan antara jenis kelamin dengan tekanan darah.  Hasil p value 0,463<0,05 tidak ada hubungan antara tingkat kecemasan, hasilnya p – value 0,000<0,05 ada hubungan riwayat keluarga dengan tekanan darah. Kesimpulan faktor yang berhubungan dengan tekanan darah yaitu jenis kelamin, beban kerja, pendapatan, riwayat keluarga sedangkan faktor yang tidak berhubungan dengan tekanan darah yaitu kecemasan
