Towards an Affordable Human Waste Management Programme for the Poor – A Report on Bangladesh

Nearly 80% of all illness in Bangladesh is related to water and sanitation. The combined rate of enteric diseases in Bangladesh is the highest in the world. 30% of all the deaths of children under 5 are due to diarrhoea, claiming more than 200,000 lives annually. Environmentally sound human waste management, along with personal hygiene awareness, are of critical importance to the prevention of faecal-oral disease transmission. present rural sanitation coverage is about 3%. A national target has been set to ensure that 13% of the rural poor will be covered under the human waste management programme by 1990, the end of the U.N. Water Decade. This however, contrasts with the similar target of 77% for the rural water supply programme, which received higher resource allocation and priority for a much longer period. This lack of correlation between water and sanitation has highly reduced the positive health impact of these developments. An affordable human waste management programme for the rural people, where 87% of the total population lives under conditions of the greatest poverty and underdevelopment, urgently needs serious and realistic consideration in the context of the limited resource prospects for future years. This report addresses the above issues and describes the commendable momentum that has been gained through the continuous efforts of national planners and policy makers. It is hoped that this might in the near future, through the rural human waste management programme, produce the long desired continual reduction of faecal-oral disease transmission.

AbstractCholera is a waterborne disease that continues to pose serious public health problems in many developing countries. Increasing water and sanitation coverage is a goal for local authorities in these countries, as it can eliminate one of the root causes of cholera transmission. The SIWDR (susceptible–infected–water–dumpsite–recovered) model is proposed here to evaluate the effects of the improved coverage of water and sanitation services in a community at risk of a cholera outbreak. This paper provides a mathematical study of the dynamics of the water and sanitation (WatSan) deficits and their public health impact in a community. The theoretical analysis of the SIWDR model gave a certain threshold value (known as the basic reproductive number and denoted $\mathcal{R}_{0}$ R 0 ) to stop the transmission of cholera. It was found that the disease-free equilibrium was globally asymptotically stable whenever $\mathcal{R}_{0} \leq 1$ R 0 ≤ 1 . The unique endemic equilibrium was globally asymptotically stable whenever $\mathcal{R}_{0} >1$ R 0 > 1 . Sensitivity analysis was performed to determine the relative importance of model parameters to disease transmission and prevention. The numerical simulation results, using realistic parameter values in describing cholera transmission in Haiti, showed that improving the drinking water supply, wastewater and sewage treatment, and solid waste disposal services would be effective strategies for controlling the transmission pathways of this waterborne disease.

The limitations of waste management in the Cipayung Landfill (TPA) causing a buildup of garbage up to more than 30 meters. This condition has a health impact on people in Cipayung Village. This study aims to analyze the impact of waste management at Cipayung Landfill on public health in Cipayung Village, Depok City. The research is descriptive qualitative. Data obtained by purposive sampling. Data was collected by interviews, observation and documentation. Based on interviews with 30 respondents, it was found that the most common diseases were diarrhea, then other types of stomach ailments, subsequent itching on the skin and coughing. This is presumably because the environmental conditions in the form of unhealthy air and water and clean and healthy living behaviors (PHBS) have not become the habit of the people. The results indicated that there were no respondents who had implemented all of these criteria. In general respondents have implemented  3 criteria, namely maintaining hair hygiene, maintaining skin cleanliness, and maintaining hand hygiene. While maintaining clean water storage is the most often overlooked behavior. To minimize this health impact, improvements in waste management in Cipayung landfill are needed along with continuous socialization and education to develop PHBS habits and the importance of maintaining a clean environment.

Background: Whole milk is a good source of all the nutrients, and it also contains a sufficient number of vitamins to permit regular the growth of the neonate. Dairy cow milk can create allergy in infants less than 12 months old because of the high caseins and β-lactoglobulin content. In these circumstances, donkey milk can represent a good replacement for dairy cows’ milk in children affected by Cow Milk Protein Allergy (CMPA) because of its close chemical composition with human milk, mainly due to its low protein and low mineral content. Milk vitamin content is highly variable among mammalian species and it is strictly correlated with the vitamin status and the diet administered to the mother. Fat-soluble vitamins content in donkey milk is, on average, lower compared to ruminants’ milk, while vitamin C content determined in donkey milk is higher compared to dairy cows’ milk, showing a great similarity with human milk. In donkey milk, the content of vitamins of the B-complex such as thiamine, riboflavin, niacin, pyridoxine, and folic acid is higher compared to human milk. The use of donkey milk as a new functional food must be further evaluated in interdisciplinary clinical trials in which pediatricians, dietitians, and food scientists must be involved to deepen the knowledge about the positive health impact of donkey milk in different sensitive people, especially children and the elderly.

Flavonoids are a major group of dietary plant polyphenols and have a positive health impact, but their modification and degradation in the human gut is still widely unknown. Due to the rise of metagenome data of the human gut microbiome and the assembly of hundreds of thousands of bacterial metagenome-assembled genomes (MAGs), large-scale screening for potential flavonoid-modifying enzymes of human gut bacteria is now feasible. With sequences of characterized flavonoid-transforming enzymes as queries, the Unified Human Gastrointestinal Protein catalog was analyzed and genes encoding putative flavonoid-modifying enzymes were quantified. The results revealed that flavonoid-modifying enzymes are often encoded in gut bacteria hitherto not considered to modify flavonoids. The enzymes for the physiologically important daidzein-to-equol conversion, well studied in Slackiaisoflavoniconvertens, were encoded only to a minor extent in Slackia MAGs, but were more abundant in Adlercreutzia equolifaciens and an uncharacterized Eggerthellaceae species. In addition, enzymes with a sequence identity of about 35% were encoded in highly abundant MAGs of uncultivated Collinsella species, which suggests a hitherto uncharacterized daidzein-to-equol potential in these bacteria. Of all potential flavonoid modification steps, O-deglycosylation (including derhamnosylation) was by far the most abundant in this analysis. In contrast, enzymes putatively involved in C-deglycosylation were detected less often in human gut bacteria and mainly found in Agathobacter faecis (formerly Roseburia faecis). Homologs to phloretin hydrolase, flavanonol/flavanone-cleaving reductase and flavone reductase were of intermediate abundance (several hundred MAGs) and mainly prevalent in Flavonifractor plautii. This first comprehensive insight into the black box of flavonoid modification in the human gut highlights many hitherto overlooked and uncultured bacterial genera and species as potential key organisms in flavonoid modification. This could lead to a significant contribution to future biochemical-microbiological investigations on gut bacterial flavonoid transformation. In addition, our results are important for individual nutritional recommendations and for biotechnological applications that rely on novel enzymes catalyzing potentially useful flavonoid modification reactions.

ABSTRACTThe Swedish nuclear waste management programme has entered the site investigation phase. Early 2002 SKB received permission from the municipalities of Östhammar and Oskarshamn to perform site investigations for a potential deep geologic repository for spent nuclear fuel. The goal of the site investigation phase is to obtain a permit to build the deep repository for spent nuclear fuel. In parallel with the investigations, consultations will be held with county administrative boards, regulatory authorities and municipalities, as well as with members of the public.

Since 1984, ENRESA is responsible of the radioactive waste management and the decommissioning of nuclear installations in Spain. The major recent challenge has been the approval of the Sixth General Radioactive Waste Plan (GRWP) as “master plan” of the activities to be performed by ENRESA. Regarding the LILW programme, the El Cabril LILW disposal facility will be described highlighting the most relevant events especially focused on optimizing the existing capacity and the start-up of a purpose–built disposal area for VLLW. Concerning the HLW programme, two aspects may be distinguished in the direct management of spent fuel: temporary storage and long-term management. In this regards, a major challenge has been the decision adopted by the Spanish Government to set up a Interministerial Committee for the establishment of the criteria that must be met by the site of the Centralized Intermediate Storage (CTS) facility as the first and necessary step for the process. Also the developments of the long-term management programme will be presented in the frame of the ENRESA’s R&D programme. Finally, in the field of decommissioning they will be presented the PIMIC project at the CIEMAT centre and the activities in course for the decommissioning of Jose´ Cabrera NPP.

The negative impact from industrial pollution of the environment is still a global occurrence, and as a consequence legislation and subsequent regulation is becoming increasingly stringent in response, in particular, to minimising potential impact on human health. These changes have generated growing pressures for the steel industry to innovate to meet new regulations driving a change to the approach to waste management across the industrial landscape, with increasing focus on the principles of a circular economy. With a knowledge of the compositional profiles of process by-products, we have assessed chemical cleaning to improve environmental performance and minimise disruption to manufacturing processes, demonstrating re-use and recycling capacity. We show that with a knowledge of phase composition, we are able to apply stabilisation methods that can either utilise waste streams directly or allow manipulation, making them suitable for re-use and/or inert disposal. We studied blast furnace slags and Portland cement mixes (50%/50% and 30%/70%) with a variety of other plant wastes (electrostatic precipitator dusts (ESP), blast furnace (BF) sludge and basic oxygen furnace (BOF) sludge) which resulted in up to 90% immobilisation of hazardous constituents. The addition of organic additives i.e., citric acid can liberate or immobilise problematic constituents; in the case of K, both outcomes occurred depending on the waste type; ESP dust BF sludge and BOF fine sludge. Pb and Zn however were liberated with a 50–80% and 50–60% residue reduction respectively, which generates possibilities for alternative uses of materials to reduce environmental and human health impact.

This study determines the tipping fee of municipal solid waste in the city of Depok. Two methods used to determine the fee were the income and outcome approach, and the limited resource approach. Two conditions were assumed (i.e., waste management and landfill gas bioreactor availability). From the results, the ideal tipping fee was 97,704 IDR/tonne and the application of a landfill gas reactor could boost income, thus reduced the amount of the fee collected, especially in the early years of the landfill bioreactor in operation. The fees were 40,032 and 63,337 IDR/tonne for scenario 1 and 2, respectively.
