<p>Patient safety incidence make adverse patient and cause loss of life. Drug administration errors in Indonesia is the highest incidence. Incidents can occur due to human error one of health workers. This study aims to describe the characteristics of the respondent in the application of the principles of correct drug administration in PKU Muhammadiyah Yogyakarta Unit II Hospital. This research type is descriptive quantitative. Respondents were taken by purposive sampling, were 32 nurses who fulfill the inclusion criteria. The instrument uses an observation sheet. Characteristics of respondents by age at most &lt;25 years of 56.2%, based on sex is 90.7% female, long work is &lt;1 year 68,8%, education D3 84.4%. The percentage of correct application of the principle of drug administration most is in enough category is 69.4%. Characteristics of respondents by age at most &lt;25 years, gender is female, long work is &lt;1 year, namely education D3. Application of principles of correct drug administration is in fair category.</p>

<p><em>Low weight for infant is one of problem that often occurs in treatment infant when getting birth. The number of low weight for infant becomes the problems for health workers. Around a quarter of low weight for infants die before stable or in 12 hours first of living infant. Result of SDKI 2002-2003 percentages of low weight for infant shows 7.6%. Based on Riskesdas (basic health research) 2007 around 11.5% infant getting birth with low weight lack of 2500 gram (minister health 2008). So, purpose of this research is to know description mothers’ anxiety with treatment low weight for infants. This research is descriptive research, type of the research qualitative method to know description mothers’ anxiety with treatment low weight for infants in Ahmad Muchtar hospital Bukittinggi 2018. This research is started on june 20-august 15 2018. The total respondent is 10 respondents with purposive sampling. In this research is done input aspect, process</em><em>, o</em><em>utput is done by four steps, they are collecting data through interviewing collection, display and drawing.    </em><em>Based on the result of the research shows that mother who has low weight infant causes anxiety is mothers’ anxiety to see the infant. It is suggested that workers health when giving the information is more skillful and service improvement. Mothers’ anxiety with treatment low weight for infant can be concluded that informant says that the causes of anxiety is the mother feels anxiety and worried toward the infant.   </em></p>

Medication errors are incidents that are detrimental to patients due to handling by health workers (human error) which is preventable. This study aims to analyze the application of drug administration standards by the pharmacy staff at Dr. Tadjuddin Chalid Makassar. This type of research is a combination research method (Mixed Methods) using the Sequential Explanatory model. The informants of this research were the pharmacy staff of the Central General Hospital (RSUP) dr. Tadjuddin Chalid Makassar. The results showed that the pharmacist's lack of accuracy was like a misreading of the doctor's writing. It is recommended that the hospital management conduct special training on the implementation of good drug administration standards and according to the SOP for drug administration, involving all health workers (doctors, nurses, pharmacists). Then conduct socialization of standard drug delivery SOPs and conduct regular evaluations (once a month) by including all officers related to drug administration.

Introduction: In doing their work, nurses often deal with other nurses, other health workers, patients, and patients' families. In addition, the job as a nurse in a hospital is related to the ability to be able to work together in teams. Therefore, a nurse is expected to have soft-skill attributes which, in this Millennial era, have different challenges from the previous era. Objective: The purpose of this study is to include soft-skills inherent in self-care nurses who have just connected the workforce from the point of view of senior nurses associated with them in the hospital. Method: Qualitative research involving 7 informants, namely senior nurses in the hospital, the informants were selected by purposive sampling based on inclusion criteria. Results: The results raised the theme of millennial nurses' soft-skills which are grouped into 3 groups of soft-skills, namely: (1) personal skills (personal skills), (2) social skills (social skills), (3) methodological skills (methodological skills), and the theme of soft-skill internalization. Discussion: Can reduce soft skills still need to be internalized for students since the time of education.

<p>Health services provided in the hospital are likely to cause patient safety incidents which are adverse patient. The most incidents occurred in Indonesia are errors drug administration. Incidents can be prevented, one of them by training nurses on patient safety with S-BAR approach communications. The study aimed to determine the effectiveness of patient safety training; S-BAR communication in reducing injection drug administration errors in PKU Muhammadiyah Hospital in Yogyakarta Unit II. The type of research pre-experiment with the design of one group pretest-posttest design. Respondents were taken by purposive sampling technique, which total of 32 nurses who fulfilled the inclusion criteria. This study used the observation sheet instruments of 10 true principle of injection drug delivery. Analyzed by Wilcoxon test. After the training of patient safety: the SBAR communication given to nurses, there were signifi cant differences in injection drug administration errors based on the principle really patient, route, medicine, time, assessment, information and evaluation (p&lt;0.05), but no signifi cant differences in principle correct dose, expired and documentation (p&gt; 0.05). Conclusions and suggestions: patient safety training; SBAR communication on nurses wasc reduced injection drug administration errors effectively in PKU Muhammadiyah Hospital Unit II Yogyakarta based on the principle of right of patients, service, medicine, time, assessment, information and evaluation. Suggested Hospital to provide periodic training to all health workers, considering the creation of a patient safety culture is the responsibility of all parties in the Hospital. Thus increase the productivity of health workers as well as the creation of a patient safety culture.</p>

AbstrakKeselamatan pasien merupakan dasar dari pelayanan kesehatan yang baik. Pengetahuan tenaga kesehatan dalam sasaran keselamatan pasien terdiri dari ketepatan identifikasi pasien, peningkatan komunikasi yang efektif, peningkatan keamanan obat yang perlu diwaspadai, kepastian tepat lokasi, prosedur, dan tepat pasien operasi, pengurangan risiko infeksi, pengurangan risiko pasien jatuh. Tujuan penelitian untuk mengetahui hubungan antara pengetahuan dengan penerapan keselamatan pasien pada petugas kesehatan di Puskesmas Kedaung Wetan Kota Tangerang. Metode Penelitian menggunakan deskriptif korelasi menggunakan pendekatan cross sectional. Populasi sebanyak 50 responden. Teknik pengambilan sampel menggunakan total sampling. Instrumen yang digunakan berupa lembar kuesioner. Teknik analisa diatas menggunakan analisa Univariat dan Bivariat. Hasil Penelitian ada Hubungan Pengetahuan dengan Penerapan Keselamatan Pasien pada Petugas Kesehatan, dengan hasil, p value sebesar 0,013 &lt; 0,05 maka dapat disimpulkan bahwa ada Hubungan Pengetahuan dengan Penerapa Keselamatan Pasien pada Petugas Kesehatan. Kesimpulan penelitian ada Hubungan Pengetahuan dengan Penerapan Keselamatan Pasien.. AbstrackPatient safety is the basis of good health services. Knowledge of health personnel in patient safety targets consists of accurate patient identification, increased effective communication, increased safety of the drug that needs to be watched, certainty in the right location, procedure, and precise patient surgery, reduction in risk of infection, reduction in risk of falling patients. The purpose of this study was to determine the relationship between knowledge and the application of patient safety to health workers in the Kedaung Wetan Health Center, Tangerang City. The research method uses descriptive correlation using cross sectional approach. The population is 50 respondents. The sampling technique uses total sampling. The instrument used was a questionnaire sheet. The analysis technique above uses Univariate and Bivariate analysis. The results of the study there is a Relationship of Knowledge with the Implementation of Patient Safety in Health Officers, with the result, p value of 0.013 &lt;0.05, it can be concluded that there is a Relationship between Knowledge and Patient Safety Implementation in Health Officers. The conclusion of the study is the Relationship between Knowledge and the Implementation of Patient Safety.Keywords Knowledge, Patient safety, Health workers

The research of this research to analysis dietary compliance of low purine diet in uric acid patients in Wara Health Clinic Year 2016. Type of this research that was used cross sectional approach and use a sampling technique was purposive sampling. The sample of the research was a portion of gout patients enrolled in Wara Health Clinic that fulfilled the inclusion criteria were 42 people.Based on the analysis by using statistical test Fisher's exact test, the value p = 0468 (p> 0.05%), it can be concluded that the hypothesis "there is no correlation between dietary compliance low purine with uric acid in Puskesmas Wara Palopo".Based on the result of research purpose recommendations were provide clearly information about the low purine diet and provide support in the form of motivation for pay attention of type food consumed so as to prevent the increasing of uric acid. <p><strong><em>Abstract: Knowledge of Pregnant Women About Sexual Relations Position During Pregnancy</em></strong><em>. The purpose of this study was to determine the knowledge of pregnant women about the position of sexual relations during pregnancy in BPM Wahidah, S.ST Year 2017. This research type is descriptive research with cross sectional approach by using accidental sampling technique. The population of this study were pregnant women who did the examination in BPM Wahidah, S.ST. The number of samples is 50 respondents who meet the inclusion criteria. This research started from 6-21 february 2017. The result showed that pregnant woman's knowledge about the position of sexual relationship during pregnancy was mostly (54%) knowledgeable enough. A small percentage of respondents aged 20-35 years ie 11 people (22%) have enough knowledge, from the work showed that almost half of respondents ie 23 people (46%) who do not work have enough knowledge, from education showed that almost half of respondents or 16 people (32%) who have medium education have enough knowledge, and from parity shows that almost half of respondents are 14 people (28%) who are multiparous have enough knowledge.</em></p><p><strong> </strong></p><p><strong>Abstrak : Pengetahuan Ibu Hamil Tentang Posisi Hubungan Seksual Pada Masa Kehamilan. </strong>Tujuan penelitian ini adalah untuk mengetahui pengetahuan ibu hamil tentang posisi hubungan seksual pada masa kehamilan di BPM Wahidah, S.ST Tahun 2017. Jenis penelitian ini adalah penelitian deskriptif dengan pendekatan <em>cross sectional </em>dengan menggunakan teknik <em>accidental sampling. </em>Populasi penelitian ini adalah ibu hamil yang melakukan pemeriksaan di BPM Wahidah, S.ST.Jumlah sampel sebanyak 50 responden yang memenuhi criteria inklusi.Penelitian ini dimulai dari tanggal 6-21 februari 2017. Hasil penelitian menunjukkan bahwa pengetahuan ibu hamil tentang posisi hubungan seksual pada masa kehamilan sebagian besar (54%) berpengetahuan cukup. sebagian kecil responden umur 20-35 tahun yaitu 11 orang (22%) memiliki pengetahuan cukup, dari pekerjaan menunjukkan bahwa hampir setengahnya responden yaitu 23 orang (46%) yang tidak bekerja memiliki pengetahuan cukup, dari pendidikan menunjukkan bahwa hampir setengahnya responden atau 16 orang (32%) yang berpendidikan menengah mempunyai pengetahuan cukup, dan dari paritas menunjukkan bahwa hampir setengahnya responden yaitu 14 orang (28%) yang multipara memiliki pengetahuan cukup.</p>

Background: Lymphatic filariasis (LF) is a parasitic disease that has been targeted for elimination through the Mass Drug Administration (MDA.) Although the MDA started in the Ankobra community in Ghana in 2000, LF prevalence as reported in 2014 was relatively high (4.5%). Non-compliance to the MDA has been associated with the persistent LF prevalence in endemic regions. Objective: This study determined the factors associated with the non-compliance to the MDA among patients living in the Ankobra community, Ghana. Methods: A cross-sectional study using a one-stage cluster sampling method was used to collect data between June and July, 2017 in Ankobra. Questionnaires were used to collect data from health workers, the MDA drug distributors and study participants in Ankobra. Data analysis was performed using STATA 14. Logistic regression was used to measure the degree of association between the dependent (non-compliance) and independent variables. Non-compliance rate was defined as the percentage of individuals who self-reported that they did not actually swallow the drugs provided during the MDA. Results: The MDA coverage and non-compliance rates were 73.5% (147/200) and 33.33% (49/147) respectively. The main reason for non-compliance was fear of drug adverse events (75.51%, 37/49). Thought of “not being susceptible to LF” was significantly associated with the non-compliance (aOR= 2.83, [CI= 1.15, 6.98]). Conclusion: Health education about the susceptibility of residents getting LF disease in endemic community must be intensified to improve compliance to MDA medication ingestion and thus meet the Global Elimination of Lymphatic Filariasis by 2020.

The paper summarises previous theories of accident causation, human error, foresight, resilience and system migration. Five lessons from these theories are used as the foundation for a new model which describes how patient safety emerges in complex systems like healthcare: the System Evolution Erosion and Enhancement model. It is concluded that to improve patient safety, healthcare organisations need to understand how system evolution both enhances and erodes patient safety.
