Herbal medicines are becoming popular worldwide, despite their mechanisms of action being generally unknown, the lack of evidence of efficacy, and inadequate toxicological data. An estimated one third of adults in developed nations and more than 80% of the population in many developing countries use herbal medicines in the hope of promoting health and to manage common maladies such as colds, inflammation, heart disease, diabetes and central nervous system diseases. To date, there are more than 11 000 species of herbal plants that are in use medicinally and, of these, about 500 species are commonly used in Asian and other countries. These herbs are often co-administered with therapeutic drugs raising the potential of drug–herb interactions, which may have important clinical significance based on an increasing number of clinical reports of such interactions.The interaction of drugs with herbal medicines is a significant safety concern, especially for drugs with narrow therapeutic indices (e.g. warfarin and digoxin). Because the pharmacokinetics and/or pharmacodynamics of the drug may be altered by combination with herbal remedies, potentially severe and perhaps even life-threatening adverse reactions may occur. Because of the clinical significance of drug interactions with herbs, it is important to identify drugs and compounds in development that may interact with herbal medicines. Timely identification of such drugs using proper in vitro and in vivo approaches may have important implications for drug development.

Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease which mainly targets synovial membrane during its disease pathogenesis. Available therapeutic drugs for the treatment of RA provide only symptomatic relief and are associated with severe side effects. Herbal plants comprise many active biological compounds that cure the disease with minimal adverse effects. Pyrenacantha volubilis is a climber and member of Icacinaceae family. Gas chromatography- mass spectrometry (GC-MS) analysis of ethanolic extracts of leaves of Pyrenacantha volubilis (EEPV) reveals the presence of 2-isopropyl-5-methylcyclohexyl 3-(1-(4- chlorophenyl)-3-oxobutyl)-coumarin-4-yl carbonate and 1-naphthalenepropanol, alpha-ethyldecahydro-5- (hydroxymethyl)-alpha,5,8A-trimethyl-2-methyl phytoconstitutents. Hence these compounds were docked with various pathological mediators of RA using Autodock 4.2. The docking results unveils that these compounds had better binding energy against inflammatory, oxidative stress and receptor for advanced glycation end products (RAGE) mediators that plays a pivotal role in the progression of RA. However, this study warrants further in- vitro and in-vivo studies to be carried out to establish the anti-inflammatory and anti-arthritic activity of selected phytoconstitutents.

Herbal remedies have been used for thousands of years in worldwide traditional medicines for their potential health benefits. Although they are generally presumed safe unless a significant risk has been identified in humans, increasing number of case reports notify acute or chronic intoxications resulting from their use. This study aims to produce a scientific guide for the evaluation of traditional herbal medicines (THMs) in terms of their toxicity risks based on the published regulatory documents. For this purpose recommended in vitro and in vivo toxicity tests on medicinal products for human use issued by the international regulatory bodies are overviewed and they are then adopted to be used for the toxicity assessment of THMs. Accordingly, based on compilation of these issued regulations, the following tests are recommended for the toxicity assessment of THMs; in vitro cytotoxicity, genotoxicity, acute and repeated dose toxicity, carcinogenicity, reproductive and developmental toxicity, local tolerance tests, toxicokinetic studies, and additional toxicity tests including safety pharmacology, immunotoxicity and antigenicity, endocrine system toxicity, gastro-intestinal toxicity, renal and hepatotoxicity, and drug interaction studies. This study describes and discusses the applicability of these tests for the risk assessment in THMs.

Herbal remedies are widely used for the treatment and prevention of various diseases and often contain highly active pharmacological compounds. Many medicinal herbs and pharmaceutical drugs are therapeutic at one dose and toxic at another. Toxicity related to traditional medicines is becoming more widely recognized as these remedies become popular in the Mediterranean region as well as worldwide. Most reports concerning the toxic effects of herbal medicines are associated with hepatotoxicity although reports of other toxic effects including kidney, nervous system, blood, cardiovascular and dermatologic effects, mutagenicity and carcinogenicity have also been published in the medical literature. This article presents a systematic review on safety of traditional Arab medicine and the contribution of Arab scholars to toxicology. Use of modern cell biological, biochemical,in vitroandin vivotechniques for the evaluation of medicinal plants safety is also discussed.

Introduction: Malaria remains a life-threatening disease, mainly in tropical and sub-tropical countries of the world. The problem caused by the disease is further compounded by the emergence and spread of multidrug resistant Plasmodium falciparum. Coupled with the poor distribution of modern health facilities, there is resurgence in the use of herbal remedies to treat malaria. In this study, we evaluated the antiplasmodial activities of six commercially available herbal formulations using in vivo and in vitro methods to assess their claimed antimalarial properties.Methods: The antiplasmodial activities of the six herbal formulations were assessed using Chloroquine sensitive P. falciparum parasite strain 3D7 using the SYBR Green in vitro method and the in vivo curative test (established infection) in Plasmodium berghei infected Mus musculus.Results: The six herbal formulations had values of IC50 > 100 μg/mL on 3D7 strain of P. falciparum compared to controls which had IC50 values of 6.92nM (Chloroquine) and 0.75nM (Artesunate). In the curative evaluation (in vivo) the herbal formulations significantly reduced parasitaemia on day 4 (26.3%-77.3 %) and day 7 (45.54%-94.81%) post-treatments (P < 0.05) when compared to the untreated group, which recorded high mortality rate. Conclusion: Findings made in this study lend support to the claim that these herbal formulations have antiplasmodial activities. Percentage inhibitions of parasitaemia of the formulations were all above 50% except M??T capsule which had lower percentage inhibition of parasitaemia.

Atopic Dermatitis (AD) is a prolonged reverting skin ailment with characteristically distributed skin lesions. In the previous decades, researchers had shown a marked interest in AD due to its increased prevalence in developed countries. Although different strategies including biological and immune modulators are available for the treatment of AD, each has certain limitations. The researchers had shown considerable interest in the management of AD with herbal medicines. The establishment of herbal drugs for AD might eliminate local as well as systemic adverse effects associated with long term use of corticosteroids and also higher cost of therapy with biological drugs. The present review discusses the traditional East Asian herbal medicines and scientific data related to newer herbal extracts or compositions for the treatment of AD. In vivo animal models and in vitro cell cultures, investigated with herbal medicines to establish a possible role in AD treatment, have also been discussed in the paper. The paper also highlights the role of certain new approaches, i.e. pharmacopuncture, a combination of allopathic and herbal medicines; and novel carriers (liposomes, cubosomes) for herbal drugs on atopic skin. In conclusion, herbal medicines can be a better and safe, complementary and alternative treatment option for AD.

The genus Lagochilus (Lamiaceae) is native to Central, South-Central, and Eastern Asia. It comprises 44 species, which have been commonly used as herbal medicines for the treatments of various ailments for thousands of years, especially in Asian countries. This review aims to summarize the chemical constituents and pharmacological activities of species from the genus Lagochilus to unveil opportunities for future research. In addition, we provide some information about their traditional uses, botany, and diversity. More than 150 secondary metabolites have been reported from Lagochilus, including diterpenes, flavonoids, phenolic compounds, triterpenoids, iridoid glycosides, lignans, steroids, alkaloids, polysaccharides, volatile, non-volatile and aromatic compounds, lipids, carbohydrates, minerals, vitamins, and other secondary metabolites. In vitro and in vivo pharmacological studies on the crude extracts, fractions, and isolated compounds from Lagochilus species showed hemostatic, antibacterial, anti-inflammatory, anti-allergic, cytotoxic, enzyme inhibitory, antispasmodic, hypotensive, sedative, psychoactive, and other activities.

Introduction. Since drug interactions may result in serious adverse effects or failure of therapy, it is of huge importance that health professionals base their decisions about drug prescription, dispensing and administration on reliable research evidence, taking into account the hierarchy of data sources for evaluation. Clinical Significance of Potential Interactions - Information Sources. The sources of data regarding drug interactions are numerous, beginning with various drug reference books. However, they are far from uniformity in the way of choosing and presenting putative clinically relevant interactions. Clinical Significance of Potential Interactions - Interpretation of Information. The difficulties in interpretation of drug interactions are illustrated through the analysis of a published example involving assessment made by two different groups of health professionals. Systematic Evaluation of Drug-Drug Interaction. The potential for interactions is mainly investigated before marketing a drug. Generally, the in vitro, followed by in vivo studies are to be performed. The major metabolic pathways involved in the metabolism of a new molecular entity, as well as the potential of induction of human enzymes involved in drug metabolism are to be examined. In the field of interaction research it is possible to make use of the population pharmacokinetic studies as well as of the pharmacodynamic assessment, and also the postregistration monitoring of the reported adverse reactions and other literature data. Conclusion. In vitro and in vivo drug metabolism and transport studies should be conducted to elucidate the mechanisms and potential for drug-drug interactions. The assessment of their clinical significance should be based on well-defined and validated exposure-response data.

Abstract Dodonaea viscosa L. Jacq. is an evergreen shrub and native to Asia, Africa and Australia. It has been used as traditional medicine in different countries. The foremost objective of the current study was to discover protective potential of D. Viscosa flowers Methanol (DVM) and Chloroform (DVC) extracts against CCL4 induced toxicity in mice. This study was intended to identify phytochemicals through HPLC, GCMS and FT-IR as well as in vitro antioxidant and in vitro antituberculosis activity. Our comprehensive findings indicate that Dodonaea viscosa is valuable and widespread herbal medicines through therapeutic potentials for curing various ailments. Dodonaea viscosa flowers are found to have protective effect against oxidative stress produced by CCL4 in liver, kidney and spleen. The level of hepatic enzymes (ALP, AST ALT and Direct bilirubin), hematological parameters (RBCs, WBCs and Platelets), total protein and liver antioxidant enzymes (SOD, GPx and CAT) were restored by the intake of DV extracts after decline in levels by CCL4. Histopathological results discovered the defensive effect of 300mg/kg of DVM extract against CCL4 induced damage, thus having improved protective effect as compared to DVC and control. As a result of analysis total flavonoids and total phenolics were also revealed. Phytochemical investigation by HPLC identified gallic acid, epicatechin, cumeric acid, flavonoids while Oleic acid (Octadecenoic acid) (C18H34O2), Stearic acid (C18H36O2), Ricinoleic acid (C18H34O3) and Cedrol (C15H26O) was estimated by GCMS. DVM extract exhibited resistance against in vitro Mycobacterium tuberculosis strains. This study proposed that protective effect of DV against oxidative damage induced in Liver, Kidney and Spleen can possibly be correlated to their antioxidant as well as free radical scavenging property.

The inhibition of alpha-glucosidase and alpha-amylase is one of clinic strategies for remedy the type II diabetes. Herbal medicines are reported to alleviate hyperglycemia. However, the constituents from those sources whether are targeted to the alpha-glucosidase and alpha-amylase still unexplored. This study attempted to select the compounds for efficacy of hypoglycemia via cellular and mouse levels. The results illustrated that the cytotoxicity in all tested compounds at various concentrations except the concentration of 16-hydroxy-cleroda-3,13-dine-16,15-olide (HCD) at 30 &micro;M were not significant difference (p &gt; 0.05) when compared with the untreated control. Acarbose (reference drug), Antroquinonol, Catechin, Quercetin, Actinodaphnine, Curcumin, HCD, Docosanol, Tetracosanol, Berberine, and Rutin could effectively inhibit the alpha-glucosidase activity of Caco-2 cells when compared with the control (maltose). The compounds (Curcumin, HCD, Tetracosanol, Antroquinonol, Berberine, Catechin, Actinodaphnine, and Rutin) could reduce blood sugar level at 30 min in tested mice. The effects of tested compounds on area under curve (AUC) were significant (p &lt; 0.05) among Acarbose, Tetracosanol, Antroquinonol, Catechin, Actinodaphnine, and Rutin along with Berberine and Quercetin. In in vitro (alpha-glucosidase) with in vivo (alpha-amylase) experiments suggest that bioactive compounds can be a potential inhibitor candidate of alpha-glucosidase and alpha-amylase for the alleviation of type II diabetes.

The oral cavity contains a large number of bacteria, some of which are involved in the development of caries and periodontitis (S. mutans, S. sobrinus, Lactobacilli spp, P. intermedia, P. gingivalis, and T. forythus). The disadvantages of existing antiseptics used in dentistry necessitate the study of antibacterial properties of herbal medicines, and, in particular, of essential oils. The aim of this review is to provide the analysis of literature sources from PubMed and Google Scholar databases related to the effects of essential oils of cloves, mint, thyme, eucalyptus, tea tree and their components on cariogenic and periodontopathic microflora. It was found out that the most in vitro studies evaluated the effects of essential oils or isolated compounds (eugenol, menthol, thymol, carvacrol, eucalyptol, and terpinene-4-ol) on S. mutans, which is considered to be the most cariogenic of oral streptococci, and the researchers limited to defining the susceptibility of the microorganism and effects on biofilm formation. Only in a few studies, the effects of essential oils on the virulence factors of oral pathogens, in particular glycosyl transferase, are represented. Clinical trials of essential oils, their components and combinations confirm the therapeutic potential of these agents in vivo, but raise the question of their effectiveness, taking into account the short-term action, which does not exceed the potency of chlorhexidine. Essential oils of cloves, mint, thyme, eucalyptus, tea tree and their components should be used for treating caries and periodontitis. They are also promising when used as agents of the oral care products, preservatives of the dental medicinal forms, and as remedies for halitosis. With a rational prescription, essential oils can be useful in improving the quality of dental treatment and preventive procedures.
