Combat pathology, particularly mine-blast injury is the main cause of military casualties. In our country, as a factor of destabilization, are widely used terrorist attacks using explosive devices of different capacities. Blast injury over 60% is cause of military casualties during armed conflicts. It is known that the condition for the formation of air-shock wave is creating waves of pressure, which is distributed at supersonic speed as possible with pulsed gas explosion and expansion of compression ambient air. The brain, chest, abdomen, and bladder are the most sensitive parts of the human body to blast. But the pathogenesis, diagnosis, treatment and rehabilitation of post-traumatic explosion-induced disorders, namely, neurodegenerative complications psychosomatic, cognitive impairment, currently not fully understood and are not clear enough for an adequate therapy. The purpose of the study was to analyze the advantages and disadvantages of experimental models of blast-induced injury and to improve method and compressed air-driven shock tube. Material and methods. We used the following methods: analysis and evaluation of experimental models of explosion-induced injury by scientific publications, monographs and invention obtained in stages patent information search in the library collection of the State institution "Dnipropetrovsk Medical Academy of the Ministry of Health of Ukraine" (October 2019), a retrospective search of the literature database PubMed (February 2020). Results and discussion. A retrospective analysis of the number of literary sources on the experimental reproduction of explosive trauma has shown a high interest of a large circle of scientists in the last decade. A qualitative study of scientific publications has shown a wide range of physical characteristics of an experimental shock wave, methods and devices for simulating an explosive injury. The absence of a standardized model of explosive injury with characteristics as close as possible to real circumstances creates conditions for the implementation of our own proposals. Conclusion. This work presents a tested modified experimental model for reproducing an air shock wave under laboratory conditions, which makes it possible to study the features of the course of an explosive injury of various organs and organ systems at various periods after injury

Relevance. Pneumonia is an actual problem of practical medicine. Diagnosis of pneumonia is quite complex and requires a comprehensive study, taking into account both clinical symptoms and the results of paraclinical research methods. Objective. The aim of the work is to generalize the data of the literature and our own experience of modern radiological and acoustic methods of pneumonia diagnosis. Methods. Analysis of scientific publications in the international electronic scientometric databases Scopus, PubMed by keywords. Search depth – 15 years (2007-2021). Results. The article summarizes its own data and presents a scientific review of the medical literature, which is devoted to the analysis of the problem of diagnosing pneumonia at different stages of treatment. Indications and contraindications to use, advantages and disadvantages of X-ray method, CT method and ultrasound diagnostics are described. The characteristic of a new method of hardware diagnostics which without external irradiation gives the chance to carry out primary acoustic remote diagnostics of pneumonia is resulted. Conclusion. Today there is a wide range of instrumental methods of studying the respiratory system that allow accurate diagnosis, but have some limitations in use in clinical practice, in particular in pediatrics. Promising is the development of new acoustic methods that have high diagnostic value, lack of additional radiation and contraindications.

The subject of the research – is a set of organizational-economic relations arising in the process of structural transformation of financial and credit relations in the agricultural business. The purpose of the article is a retrospective analysis of structural transformations of financial and credit relations in the agricultural business, evaluation of the effectiveness and feasibility of the introduction of agricultural receipts as a new instrument of lending to the agricultural business of the regions. Methodology of work – system-structural and comparative analyzes (to determine the effectiveness of the crediting mechanism according to the agricultural receipts of the farmers of the region); monographic (when studying the problems of the functioning of the mechanism of lending to agrarians by agrarian receipts) economic analysis (when carrying out a comparative analysis of the mechanism of classical bank lending to the agrarian business and the mechanism of lending to agrarians according to agrarian receipts); modeling and forecasting (when determining ways to overcome the existing deficiencies in the mechanism of lending to agrarian business entities of the region according to agrarian receipts). The results of the work – a retrospective analysis of the structural transformation of financial and credit relations in the agricultural business was carried out. The mechanism of crediting agrarians according to agrarian receipts and the scale of its distribution in the agrarian business of the region are considered. A comparative analysis of the mechanism of classical bank lending to the agrarian business and the mechanism of lending to agrarians according to agrarian receipts was carried out. In the framework of the pilot project “Agrarian receipts in Ukraine” of the international financial corporation (IFC) in partnership with the Swiss Confederation in Ukraine, the example of the Poltava region defined the effectiveness of the crediting mechanism according to the agrarian receipts of the agrarians of the region. The advantages and disadvantages of the mechanism of crediting the subjects of the agrarian business on agrarian receipts are noted. The ways to overcome the existing shortcomings of the mechanism of crediting the subjects of the agrarian business of the region according to agricultural receipts are determined. Conclusions – according to the results of the conducted research, the effectiveness of the mechanism of lending to the agricultural business of the regions according to agricultural receipts was proved, its advantages and disadvantages were noted, and attention was also focused. Proposed in Art. 7 of Law No. 5479-VI clearly delineate cases and restrictions on the debtor’s reimbursement of expenses incurred by the lender with the acquisition of the right to grow and harvest the pledged crop of agricultural products, which will ensure the principle of equality of parties on economic benefits and distribution of credit risks according to agricultural receipts.

The presented study reviews practically relevant research papers in the field of network structures, modern network business models and platforms.Aim. The study aims to elaborate and explain the concept of network structure and platform and to show the reasons for the progressiveness and potential of network organizational structure at the current stage of socio-economic and scientific development.Tasks. The authors highlight the main scientific ideas about network structures in business, including significant studies in this area; provide and explain the main terms and definitions and examine the key characteristics of network business structures; characterize “platforms” as an important concept for modern business and show the relationship between platforms and network structures.Methods. This study uses analysis of information and subsequent synthesis of new knowledge in the form of the authors’ conclusions and a wide range of relevant scientific publications of Russian and foreign authors, including original publications in English and French.Results. The history of network structures is briefly provided. Definitions and characteristics of such concepts as “network structure” and “platform” in relation to business are provided and explained by the authors.

The Tonsillectomy in children or adults is an intervention commonly encountered in the ENT (Ear Nose and Throat) and Head and Neck surgeon practice. The current tendency is to perform this type of surgery in major ambulatory surgery centers. Two objectives are thus pursued: first of all, the increase of the patient quality of life through the reintegration into the family as quickly as possible and secondly, the expenses associated with continuous hospitalization are reduced. Any tertiary (multidisciplinary) sleep center must ensure the complete diagnosis and treatment (including surgery) of sleep respiratory disorders. Under these conditions the selection of patients and especially the implementation of the specific protocols in order to control the postoperative complications it becomes essential. The present paper describes our experience of tonsillectomy as treatment for selected patients with chronic rhonchopathy (snoring) and mild to moderate obstructive sleep apnoea. It was presented the impact of antibiotics protocols in reducing the main morbid outcomes following tonsillectomy, in our day surgery center. The obtained results can also be a prerequisite for the integrative approach of the patients with sleep apnoea who were recommended surgical treatment. Considering the wide range of therapeutic modalities used in sleep apnoea, each with its specific advantages and disadvantages, more extensive and multicenter studies are needed. The utilisation of bioartificial organs is of significant interest to many due to their versatility in treating a wide range of disorders. Microencapsulation has a potentially significant role in such organs. In order to utilise microcapsules, accurate characterisation and analysis is required to assess their properties and suitability. Bioartificial organs or transplantable microdevices must also account for immunogenic considerations, which will be discussed in detail. One of the most characterized cases is the investigation into a bioartificial pancreas, including using microencapsulation of islets or other cells, and will be the focus subject of this review. Overall, this review will discuss the traditional and modern technologies which are necessary for the characterisation of properties for transplantable microdevices or organs, summarizing analysis of the microcapsule itself, cells and finally a working organ. Furthermore, immunogenic considerations of such organs are another important aspect which is addressed within this review. The various techniques, methodologies, advantages, and disadvantages will all be discussed. Hence, the purpose of this review is providing an updated examination of all processes for the analysis of a working, biocompatible artificial organ.

Amyloidosis constitutes a heterogeneous group of disorders of protein misfolding that can involve different organ systems. The disease can occur either in a systemic or localised manner that is well known to involve the gastrointestinal (GI) tract. GI amyloidosis can present with a wide range of symptoms including diarrhoea, bleeding and obstruction. This case illustrates a patient with localised jejunal amyloid light chain disease that was diagnosed serendipitously during a workup for haematuria. Our patient was otherwise asymptomatic, but this case underscores the importance of considering amyloidosis as a possible cause of isolated masses of the small intestine.

Microglia, besides being able to react rapidly to a wide range of environmental changes, are also involved in shaping neuronal wiring. Indeed, they actively participate in the modulation of neuronal function by regulating the elimination (or “pruning”) of weaker synapses in both physiologic and pathologic processes. Mounting evidence supports their crucial role in early synaptic loss, which is emerging as a hallmark of several neurodegenerative diseases, including multiple sclerosis (MS) and its preclinical models. MS is an inflammatory, immune-mediated pathology of the white matter in which demyelinating lesions may cause secondary neuronal death. Nevertheless, primitive grey matter (GM) damage is emerging as an important contributor to patients’ long-term disability, since it has been associated with early and progressive cognitive decline (CD), which seriously worsens the quality of life of MS patients. Widespread synapse loss even in the absence of demyelination, axon degeneration and neuronal death has been demonstrated in different GM structures, thus raising the possibility that synaptic dysfunction could be an early and possibly independent event in the neurodegenerative process associated with MS. This review provides an overview of microglial-dependent synapse elimination in the neuroinflammatory process that underlies MS and its experimental models.

A canister-based sampling method along with preconcentrator-Gas chromatography-Mass Spectrometry (GC-MS) analysis was applied to ethylene oxide (EtO or EO) and 75 other volatile organic compounds (VOCs) in ambient air. Ambient air can contain a large variety of VOCs, and thorough analysis requires non-discriminatory sampling and a chromatographic method capable of resolving a complex mixture. Canister collection of whole air samples allows for the collection of a wide range of volatile compounds, while the simultaneous analysis of ethylene oxide and other VOCs allows for faster throughput than separate methods. The method presented is based on US EPA Method TO-15A and allows for the detection of EtO from 18 to 2500 pptv. The method has an average accuracy of 104% and precision of 13% relative standard deviation (RSD), with an instrument run time of 32 min. In addition, a link between canister cleanliness and ethylene oxide growth is observed, and potential mechanisms and cleaning strategies are addressed.

Ureido-functionalized compounds play an indispensable role in important biochemical processes, as well as chemical synthesis and production. Isocyanates, and KOCN in particular, are the preferred reagents for the ureido functionalization of amine-bearing compounds. In this study, we evaluate the potential of urea as a reagent to graft ureido groups onto amines at relatively low temperatures (<100 °C) in aqueous media. Urea is an inexpensive, non-toxic and biocompatible potential alternative to KOCN for ureido functionalization. From as early as 1864, urea was the go-to reagent for polyurea polycondensation, before falling into disuse after the advent of isocyanate chemistry. We systematically re-investigate the advantages and disadvantages of urea for amine transamidation. High ureido-functionalization conversion was obtained for a wide range of substrates, including primary and secondary amines and amino acids. Reaction times are nearly independent of substrate and pH, but excess urea is required for practically feasible reaction rates. Near full conversion of amines into ureido can be achieved within 10 h at 90 °C and within 24 h at 80 °C, and much slower reaction rates were determined at lower temperatures. The importance of the urea/amine ratio and the temperature dependence of the reaction rates indicate that urea decomposition into an isocyanic acid or a carbamate intermediate is the rate-limiting step. The presence of water leads to a modest increase in reaction rates, but the full conversion of amino groups into ureido groups is also possible in the absence of water in neat alcohol, consistent with a reaction mechanism mediated by an isocyanic acid intermediate (where the water assists in the proton transfer). Hence, the reaction with urea avoids the use of toxic isocyanate reagents by in situ generation of the reactive isocyanate intermediate, but the requirement to separate the excess urea from the reaction product remains a major disadvantage.
