Abstract. Introduction: The deciduous tooth period is an important period in child development. Premature loss of deciduous teeth is defined as the loss of deciduous teeth before they approach the eruption of permanent teeth. The prevalance of premature loss offered in several studies obtained was between 4.3% and 42.6%. Method: An 10 years-old-boy accompanied by her mother came to RSGMP Unjani, complaints of lower right nack teeth missing because they have to extracted since 3 mouth ago. Tooth have been extracted because of caries. Result: Result of clinical examination was found premature loss teeth 75, 74, and 85. The space in Moyers analysis was +0.2 mm in region 3 and +0.1 mm in region 4. Patient was treated with a fungsional removable-space-maintainer on teeth 75, 74, and 85. Conclusion: The results of premature loss is excess space in the arch, so to prevent further occlusion abnormalities in child’s growth and development process, we need a special appliance that is used to protect the space due to premature loss of deciduous teeth. Keywords: Premature loss, space maintainer, deciduous teeth

Introduction: The period of deciduous teeth is an important period in child development. Damage to deciduous teeth that occur and cannot be treated conservatively will cause premature decay of teeth which is often called premature loss. A space maintainer is a passive tool used to maintain dental arches due to premature loss of deciduous teeth. Method: An 11-year-old girl accompanied by her mother came to RSGMP Unjani in 2018 with complaints of lower right back teeth missing because they have removed. The patient's mother is worried that her replacement teeth will not grow properly. Intraoral examination revealed tooth loss 85. The study model was analyzed using the Moyers method. Results: Analysis of the lower jaw model showed excess space. Patients were treated with a semi-fixed band and loop space maintainer on teeth 85. Conclusion: Premature loss results in excess space in the arch, so as to prevent further occlusion abnormalities in the child's growth and development process, we need a special tool used to maintain space due to premature loss of deciduous teeth.

Premature loss of temporary teeth, both the lateral and the frontal area can have unfavorable consequences on the development of permanent dentition, the support serving to maintain space for permanent teeth eruption in the dental arches. The aim of the study was assessment of different types of fixed and removable space maintainers in terms of their longevity in the dental arches that were applied as interceptive orthodontic treatment for edentulous space management transiently due to premature loss of deciduous teeth. The study followed the evolution of space maintainers properly applied to clinical cases that required these therapeutic options, and appreciated the success rate and the failure of these devices, important being the time at which the assessments were made. Findings failure rate of applied space maintainers was reported to the main causes that could lead to remove the devices: loss of space maintainers, especially those removable, fracturing both fixed and removable devices, loosening orthodontic rings and of course, taking into account the absence of some subjects at periodic reassessment meetings. The space maintainer was considered a success when it fulfill the main role for which it was applied - transitional management of edentulous space, and then being removed when the permanent succession tooth eruption occurred.

Small Gestational Age (SGA) is the term used for a new born baby with birth-severe below the 10th percentile on the intrauterine Lubchenco curve, due to Intra Uterine Growth Restriction (IUGR) which can cause defects of several organs such as deciduous teeth enamel. Dental age is the indicator for growth and development of the teeth that determine biological age which is more accurate than chronological age and could be determined by calcification or teeth eruption. The purpose of this study was to obtain dental age in SGA children based on the enamel defect severity level of deciduous teeth as compared to the chronological age. This study was a comparative analytic study with given sample size, conducted towards 18 SGA children aged 4-6 years old, and 21 children with Appropriate Gestational Age (AGA) aged 4-6 years old as the control group. Deciduous teeth enamel defect severity level determined by the developmental defect of enamel (DDE) score FDI modified. In order to determine the permanent teeth growth and development, the indicator was based on dental age using the panoramic radiograph with Dermijian method. The t-test was used to compare the permanent teeth dental age difference between SGA and AGA children as well as the permanent teeth dental age difference of SGA children based on the enamel defect severity level of deciduous teeth as seen in their chronological age. The result showed that there was a difference in the permanent teeth dental age between the SGA and AGA children at chronological ages of 4, ,5, and 6 years old (t count = 3,36; 2,35; and 3,49). Based on the average of permanent teeth dental age, the value of AGA children was higher than SGA children. Furthermore, it also showed that the severe score of EDS in the SGA children at the age of 4, 5, and 6 years old, their permanent teeth dental age was lower than the mild EDS (t count = 1,45). The conclusion of this study was the permanent teeth dental age in SGA children was lower than the AGA children as well as difference between the permanent teeth dental age in SGA children based on their deciduous teeth enamel defect severity, and the severe defect refered to slower permanent teeth dental age than the mild defect.

Premature loss of a deciduous tooth is its loss 3 to 4 years before its physiological replacement, in other words – one year before the date of physiological resorption of its roots. The most common reasons for premature loss of deciduous teeth include complications of caries such as: pulp gangrene, periodontitis, inflammation of the periapical area, periapical lesions, abscesses, and osteitis. Mechanical traumas and cysts belong to other reasons. Direct consequences of premature loss of a deciduous tooth include inhibition of growth of the alveolar process bone of the maxilla or alveolar part of the mandible in the area where a tooth is missing. Presence of space in the dental arch leads to various dental displacements such as tooth displacement along the arch, rotations, inclinations. It leads to dental abnormalities, including secondary crowding, dental abnormalities associated with time and place of eruption of permanent teeth, arch midline shift, and Godon’s effect. Consequences of premature loss of deciduous teeth also include worsening of pre-existing malocclusions and dysfunctions of the masticatory organ associated with chewing, speech and swallowing. Aim. The paper aimed to present consequences of premature loss of deciduous teeth and to determine indications for prophylactic treatment. Material and methods. A review of literature in Polish and English from the period 2006–2017 was performed using PubMed, PBL, Embase, Scopus databases. “Premature loss of deciduous teeth” was a key word. Results. Consequences of premature loss of various groups of deciduous teeth were presented and indications for prophylactic treatment were discussed based on literature collected. Conclusions. The analysis performed concluded that due to various consequences of premature loss of deciduous teeth orthodontic management is recommended in many cases in order to minimise consequences of premature loss of deciduous teeth. Parents’ knowledge of risk factors of caries and the need to visit a dentist for routine check-ups affect the children’s dentition status in the future. Lack of activities in this field often results in premature loss of deciduous teeth and it adversely affects a position of permanent teeth undergoing eruption. Aim. To compare the dentition status of children under the care of an orthodontist and of children who are not covered by orthodontic care, and to assess the effects of parents’ education on the dentition status of their children. Material and methods. The study included 232 children from primary schools in Lublin (110 boys and 122 girls). The age of subjects was between 6 years to 8 years and 11 months. The study included a survey questionnaire for parents and a clinical examination of children. The number of DMFT and dmft was calculated in each child based on a clinical examination. In the study population the incidence of caries (Fp) and mean numbers of dmft (for deciduous teeth) and DMFT (for permanent teeth) were calculated. Results. In the study group of children 12.93% were receiving orthodontic treatment with removable appliances, 10.78% were supervised by an orthodontist or were orthodontic treatment scheduled, and 75.86% of studied children had never been at an orthodontist’s office. One person (0.43%) did not provide answers to questions asked. Children receiving orthodontic care had significantly lesser carious defects in their deciduous teeth compared to children who were not receiving such care. Conclusions. 1) Providing orthodontic care to children had a beneficial effect on their dentition. 2) It is worth to pay more attention to prophylaxis of caries and to supervise the process of dentition replacement. 3) It is necessary to promote health-promoting education regarding caries and malocclusions among parents. Objective: To assess primary molars early loss in mixed dentition prevalence. Method: A study was carried out in children aged 6 to 12 years. Deciduous molar loss was considered early when it occurred before Nolla stage 6 (full formed crown) of the permanent successor or one year before its physiological exfoliation. Periapical radiographs were obtained to verify the presence of the permanent successor and the level of root formation, as well as the need for placement of fixed space maintainer lingual arch or Nance button. For the study of frequency distribution of early dental loss in relation to gender and dentition, we used the Chi-square test of 5% probability level and the non-parametric Mann-Whitney test for adequate statistical analysis. Results: Of the 315 children examined, 159 (50.47%) were male and 156 (49.52%) were female. Only 25 (7.9%) presented multiple dental losses in the posterior region, 19 in the lower arch and 6 in the upper arch. In the inferior one there was predominance of first and second deciduous molars loss. There was no difference in the association between genders and the occurrence of multiple dental losses (χ² (1) = 0.03, p>0.05). Conclusion: The prevalence of deciduous teeth premature loss was of 7.9% and the most affected teeth were the first and second molars in the lower arch. Lingual arch space holders were installed in 6 children, who will remain with it until the permanent successors erupt into the oral cavity.

: Most conventional treatments for neurodegenerative diseases fail due to their focus on neuroprotection rather than neurorestoration. Stem cell‐based therapies are becoming a potential treatment option for neurodegenerative diseases as they can home in, engraft, differentiate and produce factors for CNS recovery. Stem cells derived from human dental pulp tissue differ from other sources of mesenchymal stem cells due to their embryonic neural crest origin and neurotrophic property. These include both dental pulp stem cells [DPSCs] from dental pulp tissues of human permanent teeth and stem cells from human exfoliated deciduous teeth [SHED]. SHED offer many advantages over other types of MSCs such as good proliferative potential, minimal invasive procurement, neuronal differentiation and neurotrophic capacity, and negligible ethical concerns. The therapeutic potential of SHED is attributed to the paracrine action of extracellularly released secreted factors, specifically the secretome, of which exosomes is a key component. SHED and its conditioned media can be effective in neurodegeneration through multiple mechanisms, including cell replacement, paracrine effects, angiogenesis, synaptogenesis, immunomodulation, and apoptosis inhibition, and SHED exosomes offer an ideal refined bed-to-bench formulation in neurodegenerative disorders. However, in spite of these advantages, there are still some limitations of SHED exosome therapy, such as the effectiveness of long-term storage of SHED and their exosomes, the development of a robust GMP-grade manufacturing protocol, optimization of the route of administration, and evaluation of the efficacy and safety in humans. In this review, we have addressed the isolation, collection and properties of SHED along with its therapeutic potential on in vitro and in vivo neuronal disorder models as evident from the published literature.
