Vitamin D is an essential micronutrient in children for bone mineralization and bone growth. There are two forms of natural vitamin D, vitamin D2 and vitamin D3. Natural vitamin D is obtained from 2 main sources, dietary vitamin D intake and cutaneous vitamin D synthesis from sunlight exposure. Natural vitamin D requires a 2-step enzymatic hydroxylation process to be turned into active form of 1,25-dihydroxyvitamin D, firstly, 25-hydroxylation in the liver, and secondly, 1α-hydroxylation in the kidneys. Although breast milk is highly nutritive containing adequate amounts of most important vitamins, it contains only a small amount of vitamin D, approximately 15-50 IU/L, which does not reach the recommended daily requirement of 400 IU for infants. Exclusively breastfed infants are at risk to develop vitamin D deficiency if sunlight exposure is limited. Despite the excess of sunshine for the whole year in Southeast Asian countries including Thailand, various studies have found that more than 30.0% of children and adolescents had vitamin D insufficiency. The factors associated with vitamin D insufficiency in children include maternal vitamin D deficiency, exclusively breastfed as infants, insufficient daytime outdoor activities, use of sunscreens, extensive skin coverage by clothing, and lack of vitamin D supplementation and dietary fortification. To prevent vitamin D deficiency in infants, a global consensus in 2016 recommended that vitamin D supplementation should be included in routine national health care programs along with other essential micronutrients and immunizations.

AbstractThe incidence of vitamin D deficiency is high globally, and vitamin D supplementation draws particular attention. The objective of this study was to investigate the effects of stratified vitamin D supplementation in middle-aged and elderly individuals with vitamin D insufficiency in Beijing. A total of 448 subjects aged over 40 years old were selected from a community in Beijing. Among them, 100 middle-aged and elderly people with vitamin D insufficiency were randomly selected on a voluntary basis. They were further divided into control group and intervention group. The control group received health education and lifestyle guidance, and the intervention group received lifestyle guidance and vitamin D supplementation for nine months. The doses were stratified as follows: for vitamin D insufficiency, oral vitamin D3 supplement was given at 5000 IU/w; for mild vitamin D deficiency, oral vitamin D3 supplement was given at 10 000 IU/w; for severe vitamin D deficiency, oral vitamin D3 supplement was given at 15 000 IU/w. Safety evaluation was conducted after three-month treatment. The intervention group consisted of 8%, 62%, and 30% of cases who had vitamin D insufficiency, mild vitamin D deficiency, and severe vitamin D deficiency, respectively, which were similar with the control group. It showed that the blood 25(OH)D level increased significantly in the intervention group, from 14.30±4.30 ng/ml to 33.62±6.99 ng/ml (p<0.001), in contrast to insignificant change in the control group. Stratified vitamin D supplementation effectively increased the blood 25(OH)D level, as well as the number of cases with corrected vitamin D insufficiency or deficiency.

Background: Vitamin D has several roles, namely physiological function of calcium and bone metabolism, cell growth and differentiation, immune and cardiovascular function. Vitamin D deficiency can cause the risk of cancer. Head and neck cancer is one of the cancers that occur due to vitamin D deficiency in the body. This literature review was to asses and evaluate the impact and benefits of vitamin D supplementation for head and neck cancer.Discussion: A study found an association between vitamin D supplementation and a low risk of recurrence in conditions of high total vitamin D levels. Another study found a significant increase in quality of life (QOL). The primary mechanism of vitamin D action is mediated through binding of either 1,25(OH)2D3 (active form) or 25(OH)D (less active form) to the VDR, which is a member of the nuclear receptor superfamily of steroid and thyroid hormones with gene-regulatory and consequent anti-proliferative properties.Conclusions: Vitamin D supplementation provides a role in improving the condition of patients with head and neck cancer. Both in terms of suppressing recurrence and in terms of increasing quality of life

AimsTo audit the investigation, identification and treatment of Vitamin D deficiency within Men's Secure Services.BackgroundVitamin D and/or vitamin D deficiency has been suggested to play a role in the pathogenesis of mental illness. There is evidence that Vitamin D inadequacy is pandemic among rehabilitation patients in inpatient settings. Patients within secure hospitals are similarly considered to be at high risk, due to their limited solar exposure during often lengthy admissions. It has been suggested that these patients should be considered an ‘at-risk’ cohort, for whom Vitamin D supplementation should be routine. Men's secure services in Birmingham comprise of two medium secure units and a low secure rehabilitation unit. Here we present an audit of Vitamin D monitoring and treatment completed in 2019.MethodA three year retrospective review of electronic patient records, for all inpatients admitted within men's secure services as of 1 September 2019 (n = 188). Standards were based on the Trust accepted guidelines for management of Vitamin D deficiency.ResultKey findings included:- The majority of inpatients were Caucasian (43%) and African-Caribbean (24%). Ages ranged from 18 to 70, with a mean age of 39.Approximately two-thirds (65%) had been in hospital for over a year, of which 44% had been admitted for more than 3 years. The average length of admission was 885 days.Only 47% of patients had their Vitamin D level checked within the study period.Of those checked, 24% were tested within 1 month of admission. The mean duration between admission and Vitamin D testing was 464 days.Results ranged from 10.3 to 118.5nmol/L. A high rate of Vitamin D deficiency was identified (54%), whilst a further 16% had ‘inadequate’ levels.23% of those identified as requiring treatment did not receive any supplementation, whilst 59% of those with sufficient Vitamin D were prescribed treatment.Only 48% had their levels rechecked following treatment; of these, only 59% now had an adequate Vitamin D status.ConclusionThis audit demonstrates limited Vitamin D monitoring within male forensic inpatients. There was a high prevalence of Vitamin D insufficiency in this population, yet a substantial proportion of patients with identified deficiency were not prescribed any treatment. Ongoing monitoring and review of treatment effectiveness was poor. We argue that more consideration should be given to this population, with robust guidelines introduced for the treatment of this specific ‘at-risk group’.

Introduction: Vitamin D deficiency has been linked to a whole spectrum of diseases including osteoporosis, cancer, diabetes, and cardiovascular and immune disorders. Though Bangladesh is low latitude country, vitamin D Deficiency is serious and wide spread problem in Bangladesh. Physicians of Bangladesh are more vulnerable to low vitamin D status due to long indoor work hours and lack of sunlight exposure in both government & private institute. So, the aim of this study to evaluate vitamin D status among the physicians working around the Bangladesh. Methods: This was a cross-sectional, multicenter study where we enrolled 1112 doctors from 9 different areas of Bangladesh from May 2018 to June 2019. A Blood sample was collected from all participants to measure Serum 25(OH)D and various patient data such as age, gender, BMI, physical activity level during leisure time, use of vitamins and medications, sunlight exposure time, tea/coffee drinking, smoking, H/O comorbidities etc. was collected. Results: Of the 1112 doctors assessed in the study,794 (71.4%) was male and 318 (28.6%) was female. Mean age of the participants was 45.5±11.1 Years. Vitamin D deficiency and insufficiency was found in 734 (65.8%) and 105 (9.4%) participants respectively. Mean vitamin D level was 18.9 ng/ml (±8.6 ng/ml). Less than 5% of participants of Rajshahi, Rangpur, Dhaka, Bogura and Sylhet had sufficient vitamin D level. Vitamin D deficiency was significantly associated with age, obesity, sunlight exposure, physical inactivity, hypertension and vitamin D supplementation. Conclusion: Vitamin D deficiency is very common among physicians of all over Bangladesh. The high prevalence of vitamin D deficiency in the present study points towards urgent need of an integrated approach to detect vitamin D deficiency among health care professionals and treat appropriately. Background: Preeclampsia (PE) is a disease specific to pregnancy affecting many bodily systems. This is characterized by high blood pressure and proteinuria after the 20th week of pregnancy. The objective of this study was to evaluate of association of vitamin D insufficiency with gestational hypertension in pregnant women.Methods: This was a cross-sectional study. A total of 104 pregnant women were included in the study. The diagnosis of gestational hypertension was confirmed using the “Report of the American College of Obstetricians and Gynaecologists’ Task Force on Hypertension in Pregnancy” criteria. Based on these criteria, patients with systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg (measured after a period of rest of four hours, twice daily) and proteinuria (≥300 mg protein/24 h) were diagnosed as gestational hypertension.Results: Vitamin D deficiency was found among 78.9% (90/114) women. There was no significant (p >0.05) difference in age of women between vitamin D deficient and sufficient. Vitamin D level was significantly (p = 0.0001) lower among the women between Vitamin D deficient (15.93±4.66) and sufficient (35.70±3.25). There was no significant (p >0.05) association of Vitamin D level with parity, family history of hypertension, blood pressure, gestational age and fasting blood glucose. There was significant (p = 0.0001) difference in BMI between Vitamin D deficient (30.10±4.95) and sufficient (24.04±2.75). Tobacco consumption was also associated with the level of Vitamin D. There was significant negative correlation between BMI and Vitamin D level (r = -0.56, p = 0.00011).Conclusions: The present study demonstrates that vitamin D plays a role in the ethology and pathophysiology of gestational hypertension. Among the population having the risk of vitamin D deficiency, the risk of pregnancy induced hypertension may be decreased through vitamin D supplementation.

Abstract Background : Vitamin D insufficiency and deficiency in childhood are common. However, the status and influential factors of vitamin D during different ages are not clear. This study aimed to survey vitamin D concentrations in children aged 0 to 6 years and explore its influential factors. Methods: A total of 6953 children were recruited in Wuxi City of East China from January to December in 2016. Enzyme-linked immunosorbent assay was used to determine the serum concentrations of 25-hydroxyvitamin D [25(OH)D]. Results: The median vitamin D concentrations in the infant group (0-1 years of age) was 69.40 nmol/L, which were higher than that in both the toddlerhood group (1-3 years of age; 62.30 nmol/L) and the preschool group (3-6 years of age; 50.85 nmol/L). In addition, the median vitamin D concentrations were 71.70 nmol/L in summer, which was higher than that in spring (64.25 nmol/L), autumn (62.95 nmol/L) and winter (64.10 nmol/L). However, no difference was observed between genders ( P = 0.974). Furthermore, the prevalence of vitamin D deficiency (<50 nmol/L) was 48.1% in the preschool group (3-6 years of age), which was higher than the 21.2% vitamin D deficiency in the toddlerhood group (1-3 years of age) and the 17.9% vitamin D deficiency in the infant group (0-1 years of age). Interestingly, a nonlinear association between 25(OH)D and air temperature was observed. Conclusions: A high prevalence of vitamin D deficiency was common in a Chinese population of children 0-6 years old, especially in the preschool-aged children. Therefore, we suggested that we should pay more attention to vitamin D supplementation in Chinese young children.

Vitamin D deficiency, if left untreated, is associated with bone disorders, cardiovascular damage, and an increased risk of ischemic stroke. While there are various nutritional options for the natural intake of vitamin D, we hope to elucidate the potential mechanisms dietary vitamin D may play in hemorrhagic stroke pathology. This scoping review outlines findings from studies relevant to the biochemical activity of vitamin D, the impact of vitamin D deficiency on hemorrhagic stroke outcomes, and the potential benefit of nutritional vitamin D on hemorrhagic stroke outcomes. Here, we analyze the relevant factors that can lead to vitamin D deficiency, and subsequently, a higher risk of hemorrhagic stroke incidence with worsened subsequent outcomes. The neuroprotective mechanisms through which vitamin D works to attenuate hemorrhagic stroke onset and post-stroke outcomes have not yet been thoroughly examined. However, researchers have proposed several potential protective mechanisms, including reduction of blood brain barrier disturbance by inhibiting the production of reactive oxygen species, mitigation of inflammation through a reduction of levels of proinflammatory cytokines, and prevention of cerebral vasospasm and delayed cerebral ischemia following subarachnoid hemorrhage and intracerebral hemorrhage. While more research is needed and there are limitations to vitamin D supplementation, vitamin D as a whole may play a significant role in the dynamics of hemorrhagic stroke. Further research should focus on expanding our understanding of the neuroprotective capacity and mechanisms of vitamin D, as well as how vitamin D supplementation could serve as an effective course of treatment of hemorrhagic strokes.

The risk of vitamin D insufficiency is increased in persons having limited sunlight exposure and dietary vitamin D. Supplementation compliance might be improved with larger doses taken less often, but this may increase the potential for side effects. The objective of the present study was to determine whether a weekly or weekly/monthly regimen of vitamin D supplementation is as effective as daily supplementation without increasing the risk of side effects. Participants were forty-eight healthy adults who were randomly assigned for 3 months to placebo or one of three supplementation regimens: 50 μg/d (2000 IU/d, analysed dose 70 μg/d), 250 μg/week (10 000 IU/week, analysed dose 331 μg/week) or 1250 μg/week (50 000 IU/week, analysed dose 1544 μg/week) for 4 weeks and then 1250 μg/month for 2 months. Daily and weekly doses were equally effective at increasing serum 25-hydroxyvitamin D, which was significantly greater than baseline in all the supplemented groups after 30 d of treatment. Subjects in the 1250 μg treatment group, who had a BMI >26 kg/m2, had a steady increase in urinary Ca in the first 3 weeks of supplementation, and, overall, the relative risk of hypercalciuria was higher in the 1250 μg group than in the placebo group (P= 0·01). Although vitamin D supplementation remains a controversial issue, these data document that supplementing with ≤ 250 μg/week ( ≤ 10 000 IU/week) can improve or maintain vitamin D status in healthy populations without the risk of hypercalciuria, but 24 h urinary Ca excretion should be evaluated in healthy persons receiving vitamin D3 supplementation in weekly single doses of 1250 μg (50 000 IU).

Vitamin D deficiency affects more that 1 billion people worldwide. Although thought to increase risk of bacterial infections, the importance of vitamin D on host defense against intestinal bacterial pathogens is currently unclear since injection of the active form of vitamin D, 1,25(OH)2D3, increased susceptibility to the enteric bacterial pathogen Citrobacter rodentium by suppressing key immune/inflammatory factors. To further characterize the role of vitamin D during bacteria-induced colitis, we fed weanling mice either vitamin D3-deficient or vitamin D3-sufficient diets for 5 wk and then challenged them with C. rodentium. Vitamin D3-deficient mice lost significantly more body weight, carried higher C. rodentium burdens, and developed worsened histological damage. Vitamin D3-deficient mice also suffered greater bacterial translocation to extra-intestinal tissues, including mesenteric lymph nodes, spleen, and liver. Intestinal tissues of infected vitamin D3-deficient mice displayed increased inflammatory cell infiltrates as well as significantly higher gene transcript levels of inflammatory mediators TNF-α, IL-1β, IL-6, TGF-β, IL-17A, and IL-17F as well as the antimicrobial peptide REG3γ. Notably, these exaggerated inflammatory responses accelerated the loss of commensal microbes and were associated with an impaired ability to detoxify bacterial lipopolysaccharide. Overall, these studies show that dietary-induced vitamin D deficiency exacerbates intestinal inflammatory responses to infection, also impairing host defense.
