Background. Access to proper medical attention and hygienic conditions during delivery can reduce the risk of complications and infections that may lead to serious illness or death or for the mother, baby, or both. In Ethiopia, the high maternal mortality rate with delivery by unskilled birth attendants shows low utilization of maternal health services. Objective. This study was aimed at assessing factors determining the choice of childbirth place among women of childbearing age in Jimma Arjo District. Method. A cross-sectional design was conducted in Jimma Arjo District, East Wollega Zone, Ethiopia, from March 20 to April 20, 2018. Multistage sampling technique was used to select a total sample of 506 participants. Data were collected using structured questionnaires to interview women of childbearing age with two trained data collectors. Data was entered into Epi Info and exported to SPSS software version 20 for analysis. Data was checked for its completeness, cleaned, entered, and analyzed accordingly. Bivariate and multivariable data analyses were used to examine factors affecting choice of childbirth place. Results. A total of 506 women participated in this study, giving a response rate of 97.8%. The study investigated that home delivery was found to be 200 (39.5%)in the study area. Factors found to be statistically associated with choice of institutional delivery at p<0.05 were history of obstetric difficulties (AOR=6, 95%CI=2.08,17.60), woman educational status (AOR=4.4, 95%CI=1.47,13.42), husband educational status (AOR=4, 95%CI=1.43,11.60), two or more ANC visits (AOR=4, 95%CI=1.95,8.52), and accessibility to vehicle transportation (AOR=2.8, 95%CI=1.23,6.46). Conclusion. Preferring health facility as the birthplace in this study seems relatively better compared to other studies. It is shown that both mothers and their husbands attending secondary and greater educational level, history of obstetric difficulties, two or more ANC visits, and physical accessibility to health care facility have influenced mothers to prefer a health institution as the childbirth place. Therefore, any programs aimed at increasing the choice of institutional delivery should work on increasing ANC attendance and transportation facilities in the study area.

BackgroundBreast cancer represents the most frequent neoplasm diagnosed in women of childbearing age. When the tumour is oestrogen receptor-positive, tamoxifen is among the recommended endocrine treatments. Lactating women are advised not to breastfeed while receiving tamoxifen. However, information about tamoxifen transfer into breast milk is lacking.MethodsWe measured the concentration of tamoxifen and its metabolites by liquid chromatography-tandem mass spectrometry in the milk of a nursing mother that was treated for pregnancy-associated breast cancer diagnosed a few months after delivery. She was advised not to breastfeed her child and she collected milk samples for 23 days while the baby was fed with formula.ResultsTamoxifen concentrations in milk increased reaching a maximum of 214 nM. The two active metabolitesZ-4-hydroxy-tamoxifen and Z-endoxifen, could not be quantified in milk the first days after tamoxifen intake, but increased over time and reached clinically significant levels after day 18.ConclusionThis study demonstrates for the first time in human that tamoxifen and its metabolites transfer into milk. Since tamoxifen has a complete oral bioavailability, a long half-life (>7 days) and may interfere with the normal development of the infant, mothers should not breastfeed during tamoxifen treatment.

In spite of huge global investments in the production and distribution of mosquito nets to protect people from malaria; it has continued to militate against the reduction of mortality in Nigeria. The study, therefore, examined the nexus between utilization of mosquito nets and malaria related under-five health outcome. Two local government areas were randomly selected from each of the three senatorial districts, and stratified along urban and rural communities. Purposive sampling technique was used to select 609 mothers within childbearing age and who had under-five children across the communities. Descriptive statistics, Chi-square tests and logistic regression at p<0.05, and thematic content analysis were used for data analyses. The respondents’ age was 32.6±5.8 years, 84.6% were married/living together, 55.0% had secondary education, while 42.7% were in paid/civil service jobs. Under-five mortality was 19.3% higher amongst rural than urban dwellers. Ownership of mosquito nets (treated or untreated) varied by place of residence with 60.1% and 39.9% for urban and rural areas respectively. About 62% of the dead under-five children never slept under mosquito nets. The likelihood of under-five mortality significantly increased by nonownership of mosquito nets (Odds-Ratio (OR): 2.476) and reduced by number of mosquito nets (OR: 0.278). Wrong methods adopted by mothers in using mosquito nets, was deduced as a contributory factor causing underfive mortality for home with fewer nets. Information on the proper use of the mosquito nets would help achieve the goal of universal access for the atrisk populations of children under age five and pregnant women.

Breast cancer is a serious threat for women all over the world including Indonesia. Self Breast-Examination (BSE) is one of important early detection efforts in reducing morbidity and mortality due to breast cancer. The implementation of BSE as a behavior is influenced by many factors, including knowledge. This study aims was to analyze the relation of knowledge about breast cancer with the behavior of BSE implementation on childbearing age women at Bandung, West Java. Cross-sectional design was used with 420 respondents. Women over the age of 20 were randomly selected to participate in this study. Questionnaire was used as data collection instrument. Data was analyzed by using Chi-Square with statistical significance level at p <0.05. The results showed that 56.2% of respondents had good knowledge about breast cancer and 43.8% were still at low level, while the implementation of BSE was also still low at 39% (IK 95%: 34, 5% -43.8%). There was significant relation between knowledge about breast cancer and the behavior of BSE implementation on childbearing age women in Bandung (p <0.001).
