Introduction: Triage is prioritizing patients by disease severity in the shortest possible time. Proper triage increases the quality of patient care services, increases patient’s satisfaction, decreases patient waiting time, decreases mortality, and enhances emergency department efficiency. Objectives: The aim of this study was to evaluate the concordance of triage performed by nurses with ESI standard, at Rasoul Akram and Haft Tir hospitals, which are the most important educational centers of Iran University of Medical Sciences. Patients and Methods: This is a cross-sectional design study done in 2019. The study population is patients referred to Rasoul Akram and Haft Tir hospitals. A sample of 800 patients was selected. In this study, we determined how triage level were determined and compared with triaging by emergency medicine assistant. Data were analyzed by SPSS 23. Results: Out of 800 samples, the highest triage level was related to level three (79.1%) and levels two, four and one were in the next levels. 13.5% of the cases were not properly triaged according to the ESI standard and the adaptation coefficient (kappa) between the triage level specified in the patient file with the emergency severity index (ESI) triage level between physician and nurse was 58%, which was a significant difference (P<0.001). The relationship between correct triage leveling and patients’ complaints was significant (P<0.001). Conclusion: Nurses triage education about appropriate triage and ESI tool should be one of the most urgent topics and priorities of the studied hospitals. Over triage causes resources wasted and under triage may harm the patient’s health.

Abstract: Haemoglobin is the main component of red blood cells that serves as a transporter of oxygen and carbon dioxide in the blood. The normal range of haemoglobin values can be used to determine the degree of anemia according to age and gender. Geographical condition such as altitude influences the haemoglobin value. This study aimed to obtain the difference of haemoglobin levels between teenagers with good nutrition status who live at the highland and those at the seaside. This was an observational analytical study with a cross sectional design. Samples were students with good nutrition status of SMPN 3 Tomohon (living at the highland) and those of SMP Kristen Nazaret Tuminting (living at the seaside). There were 60 students who met the inclusion criteria as follows: adolescent, good nutrition status, healthy, aged 13-15 years, lived at the highland or at the seaside ≥ 6 month, and willing to be performed blood examinations, and had been approved by their parents. The Mann-Whitney test for the difference between haemoglobin levels of the two groups showed a p value < 0,001. Conclusion: There was a very significant difference between haemoglobin levels of good nutrition teenagers who lived at the highland and at the seaside. Keywords: haemoglobin levels, good nutrition, teenagers, highland, seaside  Abstrak: Hemoglobin merupakan komponen utama sel darah merah dan berfungsi sebagai transporter oksigen dan karbon dioksida dalam darah. Abstract Background Emergency care continues to be a challenge, since patients’ arrival is unscheduled and could occur at the same time which may fill the Emergency Department with non-urgent patients. Triaging is an integral part of every busy ED. The Canadian Triage and Acuity Scale (CTAS) is considered an accurate tool to be used outside Canada. This study aims to identify the chosen triage level and compare the variation between registered nurses, pediatric and adult emergency residents by using CTAS cases. Method This study was conducted at King Abdulaziz Medical City,Saudi Arabia. A cross-sectional self-administered questionnaire was used, and which contains 15 case scenarios with different triage levels. All cases were adopted from a Canadian triage course after receiving permission. Each case provides the patient’s symptoms, clinical signs and mode of arrival to the ED. The participants were instructed to assign a triage level using the following scale. A non-random sampling technique was used for this study. The rates of agreement between residents were calculated using kappa statistics (weighted-kappa) (95%CI). Result A total of 151 participants completed the study questionnaire which include 15 case scenarios. 73 were nurses and 78 were residents. The results showed 51.3, 56.6, and 59.9% mis-triaged the cases among the nurses, emergency residents, and pediatric residents respectively. Triage scores were compared using the Kruskal Wallis test and were statistically significant with a p value of 0.006. The mean ranks for nurses, emergency residents and pediatric residents were 86.41, 73.6 and 59.96, respectively. The Kruskal Wallis Post-Hoc test was performed to see which groups were statistically significant, and it was found that there was a significant difference between nurses and pediatrics residents (P value = 0.005). Moreover, there were no significant differences found between nurses and ER residents (P value> 0.05). Conclusion The triaging system was found to be a very important tool to prioritize patients based on their complaints. The results showed that nurses had the greatest experience in implementing patients on the right triage level. On the other hand, ER and pediatric residents need to develop more knowledge about CTAS and become exposed more to the triaging system during their training.

AbstractThere is growing evidence in Uganda that the non-attendance of antenatal care is largely influenced by the lack of decision-making autonomy, inadequate information and poor services offered in health facilities. Although previous studies have examined barriers and facilitators of antenatal care, a few of them have investigated the extent of decision making autonomy and ICT adoption among expectant mothers. A cross sectional design through focus group discussions and survey questionnaires was used to collect data. Three hundred and twenty households were randomly sampled in Kampala and Jinja districts. The Chi-square tests (χ2) for independence to analyze group differences among women’s socio-demographic characteristics and decision-making autonomy was used. Inclusion criteria included respondents aged 18 and 50 years, completion of primary school education, expectant mothers and mothers who gave birth two years prior to the study. A hundred and sixty-four respondents participated in this survey. About 59.5% of women lacked decision making autonomy. Midwives (37.6%) and village health teams (35%) were a major source of antenatal care information, and 49.5% of expectant mothers lacked ANC information. Ninety percent (90%) of mothers did not use any form of ICT’s to enhance their decisions yet 79% possessed mobile phones. We observed a strong association between antenatal care decision-making autonomy and women with higher education (χ2 = 8.63, ρ = 0.035), married (χ2 = 4.1, ρ = 0.043) and mature (36–50) (χ2 = 8.81, ρ = 0.032). The main findings in this study suggest that ICT adoption and decision making autonomy among expectant mothers is still low and less appreciated. Control measures and interventions should be geared towards empowering women to influence their decisions.

Background: The Emergency Severity Index (ESI) version 4 (v4) is a triage system based on vital signs, potential limb or organthreat, as well as expected resources needed in the emergency department (ED).Objective: The purpose of this study was to examine accuracy and misclassification rate of ESI triage over one year following implementation. Methods: This was a retrospective analysis of ED encounters from January 2011 to 2012. Charts were selected in one-week intervals every 12 weeks for one year (months 1, 3, 6, 9, and 12). Each encounter was reviewed to determine post hoc ESI level based on care in the ED. Descriptive statistics was used to compare the agreement between initial triage and post hoc ESI levels. Sensitivity and specificity for each level was determined. Kruskal Wallis test (KW) and Mann Whitney U (MWU) was used toassess differences in initial versus post hoc ESI levels by month to explore change in accuracy over time. Results: Five hundred and sixty separate ED encounters were included. Agreement was observed in 301 triage encounters (53.8%). Overestimation of the triage level occurred in 131 (23.4%) encounters, while the triage level was underestimated in 128 (22.9%) encounters. There was a significant decline in accuracy during the year (KW = 10.2; p = .037); with the greatest dropbetween month 1 and 9 months (MWU 4,859; p = .035). Sensitivity ranged from 24% to 76% and specificity ranged from 61% to 99%, based on ESI level. Conclusions: Enhanced education and quality improvement process is necessary to improve overall accuracy rates at this site.
