Objective: to determine the level of alpha-2 microglobulin of fertility in the serum and endometrium of female patients of reproductive age. Material and methods . 30 female patients of reproductive age were included in the study. According to the results of clinical and case history data, 20 (66.67 %) patients with reproductive dysfunction were included in the study group, 10 (33.33 %) healthy patients were included in the control group. The concentration levels of hormones (FSH, LH, testosterone, prolactin, estradiol, progesterone, fertility alpha-2 microglobulin) in the blood serum and the level of alpha-2 microglobulin of fertility in the endometrial tissue were determined by the method of enzyme immunoassay. Results . The female patients with reproductive dysfunction revealed changes in the concentration of sex hormones. The estradiol concentration was 0.22 (0.21-0.24) nmol/l in the study group of the patients, and 0.25 (0.24-0.28) nmol/l in the healthy patients (z = -3.37, p = 0.0007). The progesterone concentration was 9.57 (7.60-53.54) nmol/l in the study group of the patients and 5.37 (3.59-26.44) nmol/l in the healthy women (z = 1.78, p = 0.07). The level of alpha-2 microglobulin of fertility did not differ significantly in the patients of both the groups. The level of alpha-2 microglobulin of fertility in the endometrium was 0.87 (0.69-1.38) ng/ml in the female patients of the study group and 1.82 (1.38-5.81) ng/ml in the patients of the control group (z = -2.31, p = 0.02). Conclusion . The obtained results are indicative of the involvement of the endometrium in the development of reproductive dysfunction. The data on the levels of alpha-2 microglobulin of fertility in the endometrial tissue are promising for further study and determination of the boundaries of its normal values. Further research in this area will facilitate the development of a test to predict successful implantation.

The article presents the results of the study of the levels of steroid and gonadotropin-releasing hormones, prolactin, pro-inflammatory cytokines in female patients with reproductive disorders and those of the control group. Half of the patients of the study group revealed an increase in the threshold level of progesterone, and 32.14% of the female patients of the control group undergoing pregravid preparation detected hyperandrogenia. The level of proinflammatory cytokines was within the normal range in both the study and control groups.

Βackground: angiogenesis is seen during the multiple stages of carcinogenesis, aswell as during the process of surgical wound healing, a fact which has led tosubstantial debate over the last decades about the potential impact of surgery upon thefinal outcome of ceratin patients treated for breast cancer.Aim: the present research aims at investigating the potential effect of surgery on theprocess of angiogenesis, by studying a number of factors that are related to the latter,in patients suffering from breast cancer before and after the time of the procedure,whilst comparing these results with those of patients that were operated on their breastfor non-malignant disease.Material-Methods: blood from 10 female patients with breast adenocarcinoma(Study Group) was collected via venipuncture before surgery (labeled as PRO), aswell as on post-operative day 3 (labeled as D3) and day 7 (labeled as D7). Moreover,blood samples were also taken from 6 female patients with fibroadenoma (ControlGroup) before surgery (PRO) and on day 3 afetr surgery (D3). These samples weremeasured for detection of circulating levels of three established angiogenesisbiomarkers using ELISA (Enzyme-Linked ImmunoSorbent Assay): VascularEndothelial Growth Factor-A (VEFG-A), Interleukin-8 (IL-8) and basic FibroblastGrowth factor (bFGF or FGF-2). In addition, circulating transcripts of 84 agiogenesirelatedgenes were determined using RT-PCR (Real Time Polymerase ChainReaction). The two groups of patients were firstly compared to each other regardingtheir results. Also, patients belonging to the Study Group were analized at differenttime points regarding surgery. Finally, the results were investigated againstclinicopathological data and patient outcome.Results: using ELISA we were able to detect increased levels of circulating VEGF-Aand IL-8 in the Study Group patients compared to the Control Group patientspreoperatively (p=0,0381 and p=0,0218 respectively), while for bFGF there was nostatistically significant difference documented. Surgery resulted in a significantincrease in VEGF-A levels on D3 (p=0,0389) and D7 (p=0,0172) as compared toPRO levels. Perioperative kinetics of IL-8 showed a mild trend towards increase,which, however, was not statistically significant. Postoperative levels of bFGF wereslightly increased on D3, but on D7 they were even lower than preoperative values(p=0,0205). Using RT-PCR certain differences between the Study Group and theControl Group were recorded regarding the circulating transcripts of a great numberof angiogenesis-related genes preoperatively: upregulation of VEGF-C, EGF, IL-8,FGF-1, SPHK1, NRP1, LAMA5, COL4A3, TEK, EFNA3, EFNB2. AKT1, ITGB3,THBS1, CCL11, TIMP3 and downregulation of CXCL10. Moreover, mastectomyinduced an altered expression in several key-genes in breast cancer patients:upregulation of THBS1, COL4A3, BAI1, ITGB3 and downregulation of EREG,SERPIFN1, CXCL9, CXCL10, IL1B, CCL2, CXCL1, HIF1A, NOTCH4. Conclusions: patients suffering from breast cancer have a different angiogenic profilein comparison to patients with fibroadenoma, as documented through their differencesin circulating levels of angiogenic factors. These levels are greatly changed after thesurgical procedure. VEGF showed a transient increase, while bFGF initially increasedbut only to finally decrease to levels that were even lower than the preoperative ones.Moreover, mastectomy promoted a shift in the expression pattern of a broad panel ofangiogenesis-related gene transcripts.

Background: According to WHO 2009, 60-70% women use one of the method of contraception. 8.8% to 15.4% women use oral contraceptives. Objective of present study was to investigate the effect of oral contraceptives on androgen profile and tear film parameters in females within child bearing age.Methods: Present study involve 100 healthy women between 20-45 years, who presented in family planning clinic. Two groups were made according to the use of oral contraceptive pills. Study group consist of 50 women who were on OCPs (Oral contraception pills) and 50 as control group were not taking any hormonal contraceptives. Serum testosterone and DHEA levels of all subjects was done by Enzyme Immuno Assay on blood samples which were collected during 3th to 7th day of menstrual cycle. Quantitative test for tear secretion was done by Schirmer’s test. Stability of tear film was measured by Tear Breakup time (TBUT). Statistical analysis was done to determine the correlation between use of OCPs and androgen profile and tear film parameters.Results: Present results shows decreased androgen levels in women taking oral contraceptives as compared with age matched women who were not taking oral contraceptives. Tear secretion was significantly reduced in study group as indicated by decreased Schirmer’s test values in study group as compared to control group, the tear film stability was also significantly decreased in women taking oral contraceptives.Conclusions: Present study suggest that androgen profile decrease in women taking oral contraceptives. These results support that use of oral contraceptives may be an important etiological factor in pathogenesis of dry eye disease reproductive age group women.

AIM: The role of behavioural factors and sleep duration and quality is important in the pathogenesis of obesity. The aim of our study was to evaluate the effects of behavioural risk factors on melatonin secretion in women.SUBJECTS AND METHODS: In total, 120 female patients were enrolled in the study and divided into two groups according to the body mass index. Detailed history, anthropometric measurements, urine and blood samples were evaluated for each patient.RESULTS: Two groups significantly differed in weight, BMI, and waist circumference, and were 94.2 ± 14.9 kg, 33.4 ± 5.23 kg/m2 and 99.2 ± 12.6 cm for the study group and 56.0 ± 5.2 kg, 20.0 ± 1.8 kg/m2 and 60.1 ± 10.4 cm for the control group, respectively, sleep disruptions were detected in 48 patients from study group, with mean score 6.76 ± 3.6, and only 10 patients were detected in the control group, with mean score 4.42 ± 1.68. Eating disturbances were revealed in 66 patients from the study group and 21 patients from the control group. Melatonin levels were 17% higher in the study group, compared to control group.CONCLUSION: Higher melatonin levels in patients with obesity and concomitant behavioural impairments may be due to its protective effect to fight free radicals and to induce vasodilatation. Further studies are needed to confirm our finding.

Abstract Heavy Smokers appeared to be less resistant to infection, such as bacteria, viruses, and parasites. Many studies have examined the complement components concentrations than compared with normal people and ignored the functional sequencing of complement components, our study included the determination of complement activity by using Sheep red blood cells (SRBCs) as antigen and extracting the hemolytic activity (50%) of complement compounds, and because of difficulty of this method we using statistical analysis program (SPSS 23) and derived the inverse equation which gives the decomposition percentage (1-100%) of complement components by using five serum dilutions only. The total hemolytic complement activity (CH50) and its C3 and C4 fractions were determined in 30 heavy smokers. The results were compared with a control group that contained 30 persons matched in age and sex. Generally, both C3 and C4 concentrations were increased in smoker's individual in compared with the control group. However, when the independent t-test has applied the differences in the C3 and C4 levels in the control group (healthy individuals) and in the smoker group were found to be statistically insignificant but the inverse equation showed a 7% reduction in CH50 in smokers compared with the control group, where 18% reduction was observed. Our current study suggests that the complement components of the heavy smokers suffer from a significant dysfunction in the function, although the concentration of the basic components in the serum is parallel with normal values.

Aim. To evaluate the efficacy of haemorrhoidal artery ligation (HA) with a preliminary palpatory determination of its localization supplemented by mucopexy of haemorrhoids as a new surgical method in the treatment of haemorrhoidal disease (HD) and to compare it with HAL-RAR technology.Materials and methods. The randomized controlled clinical trial included patients over 18 years old with Goligher’s grade II, III or IV symptomatic HD. We operated on patients in the study group (n = 75) using palpatory determination of the localization of HA and subsequent mucopexia. In the control group (n = 75) we used HAL-RAR. The primary endpoint (25–30 days after surgery): recurrence rate of HD symptoms. Secondary endpoints: postoperative complication rate, pain intensity on a visual-analogue scale from 1 to 10 points, patient satisfaction with the treatment results on a 10-point scale.Results. According to the initial characteristics (age, gender, body mass index, stage of HD, frequency of clinical symptoms), the groups did not differ. Anal bleeding relapse developed: study group – 11%, control group –14%; relapse of haemorrhoids prolapse: 3% and 5% respectively (p > 0.05). Postoperative complications were noted in 6 (8%) in the study group and 4 (5%) in the control group (p > 0.05). The intensity of pain on the 2nd and 25–30 days after surgery was 6.3 [4.8; 7.4] and 1.2 [0.6; 2.5] points in the study group and 6.5 [4.9; 7.3] and 2.1 [1.9; 4.1] in the control group, respectively (p > 0.05). Patient satisfaction with the treatment results after 12 months was estimated at 8.7 [7.9; 9.2] and 9.4 [8.2; 9.6] in the study and control groups, respectively (p > 0.05).Conclusion. HA ligation with a preliminary palpatory determination of its localization and supplemented with mucopexy of haemorrhoids is no less effective than HAL-RAR in preventing haemorrhoidal bleeding and prolapse of the nodes.

Abstract An endometrial polyp is most commonly a benign, localized proliferation of the glands and the endometrial stroma, covered with epithelium and protruding above the level of the mucosa. These polyps are most often diagnosed during investigation into the causes of irregular menstrual bleeding or infertility. It is produced in the highest concentration during the secretory phase of the endometrial cycle. The level of glycodelin reaches its peak 12 days after ovulation. The aim of this paper was to determine the changes in the immunohistochemical expression of glycodelin at the level of the endometrium and in the tissue of the polyp, before and after hysteroscopic polypectomy, in infertile female patients with an endometrial polyp, and in the endometrial tissue of female patients without an endometrial polyp. The study included 82 infertile female patients. The infertile patients were divided into two groups. The first was the experimental group which included 56 infertile female patients who had an endometrial polyp. The second group was the control group, composed of 26 infertile female patients who did not have an endometrial polyp. The results obtained primarily indicate the existence of changes in the immunohistochemical expression of the cytokine glycodelin in the female patients from both the experimental and the control group, not only prior to but also after hysteroscopy. A larger number of patients who have an endometrial polyp show a lack of glycodelin expression, more pronouncedly so in the bioptate of the endometrium than in the endometrial polyp.

Introduction/Objective. Very little is known about the factors influencing women?s attitude towards colposcopy follow-up after cervical treatment. The aim of the study was to investigate the long-term attitude to follow-up colposcopy in women of reproductive age after cervical excision and to evaluate if their attitude was related to their anxiety and depression levels. Methods. Women treated with cervical excision were interviewed after a follow-up colposcopy visit. Their socio-demographic and clinical characteristics were recorded. All women filled in the Beck?s anxiety and depression inventory. Results. A total of 160 women were divided into the study group of 42 (26.3%) women who felt discomfort during follow-up colposcopy and the control group of 118 (73.7%) women who did not report such feelings. The mean age of the total sample was 35.3 ? 5.4 years with median time after treatment being five years (range: 2?18). Women in the study group had a significantly lower BMI values, had higher rates of nulliparity and nulligravidity, were more often single or living alone, and had significantly changed their attitude towards condom use after treatment. Beck?s anxiety and depression scores were significantly higher in the study group. Multivariate analysis showed that independent predictors of discomfort during follow-up colposcopy were anxiety levels (OR: 1.06; 95% CI: 1?1.12), living alone or without a partner (OR: 2.65; 95% CI: 1.08?6.55), and the change in their practice of condom use after treatment (OR: 2.69; 95% CI: 1.02?7.07). Conclusion. Almost one third of women after excisional treatment reported discomfort during their follow-up colposcopy. These women exhibited higher levels of anxiety.

Introduction. Psoriasis in women of reproductive age is one of the most pressing medical and social problems. The consequence of psoriasis disease is a significant decrease in the quality of life, disability of patients, difficulties in creating a family and deterioration of family relations, significant psychological discomfort. Objective of the study: to increase the effectiveness of treatment of women with psoriasis with menstrual irregularities by means of pathogenetically substantiated therapy based on the study of hormonal changes. Materials and research methods. The study included 130 women who were divided into three groups: the main group – 75 women with psoriasis with menstrual irregularities, who were randomized into 3 subgroups: 1a – 25 patients who will receive standard psoriasis therapy; 1b – 25 patients, in addition to the standard treatment of psoriasis, will receive gestagens for 3 months; 1c – 25 patients, in addition to the standard treatment of psoriasis, will receive a preparation of an dry extract of herb of creeping anchors for 3 months. The comparison group consisted of 25 women with psoriasis without hormonal disorders, comparable in age and social status with the study group. The control group – 30 apparently healthy women, comparable in age and social status with the study group. Research results. In patients of the main group who received standard psoriasis therapy (1a); who received progestogens for 3 months in addition to standard psoriasis treatment (1b) and, in addition to the standard treatment of psoriasis, who received the preparation of an dry extract of herb of anchors creeping for 3 months (1c), before the start of the treatment, the ratio of LH / FSH – luteinizing hormone (LH) and follicle-stimulating hormone (FSH) was below normal, which indicates a certain deficit in LH production relative to FSH values. In parallel, in these cohorts, the production of prolactin was increased, which suppresses the formation of FSH and LH in the pituitary gland. Changes in the production of gonadotropic hormones are a reaction to the existing deficiency or excess of sex hormones. In the main 1a, 1b and 1c in groups, estradiol values ​​were within the normal range with progesterone deficiency in the luteal phase. The testosterone value in all study groups did not deviate from the standard values. Conclusions. The obtained results of the survey indicate the need to increase the effectiveness of treatment of women with psoriasis with menstrual irregularities by involving measures aimed at correcting hormonal status in complex therapy.

Aim. To assess the influence of epilepsy and anticonvulsants on central control of hormone release and ovarian function in women of reproductive age. Methods. The reproductive and endocrine functions were assessed in 82 female patients with epilepsy, of whom 37 did not receive anticonvulsants (first group), 45 patients were on anticonvulsant treatment (second group). 17 healthy females (control group) were also assessed. Serum levels of estradiol, progesterone, prolactin, luteinizing hormone, follicle-stimulating hormone and dehydroepiandrosterone were estimated during follicular (n=48) and luteal (n=34) stages of menstrual cycle. Results. Estradiol serum level was below normal ranges, and prolactin serum level - above normal limits at follicular stage of menstrual cycle in female patients not treated with anticonvulsants. In luteal phase serum levels of estradiol, progesterone, dehydroepiandrosterone exceeded the reference ranges, and testosterone levels were lower than normal in both patients treated and not treated with anticonvulsants. Comparison of two groups showed that estradiol serum level was lower in untreated patients in both stages of menstrual cycle compared to patients treated with anticonvulsants. There was a marked difference in endocrine function at all levels of reproductive neuroendocrine regulation in untreated female patients compared to female patients treated with anticonvulsants with major transformations in female patients with epilepsy not treated with anticonvulsants. Conclision. Epilepsy and treatment with anticonvulsants can influence the central regulation of hormone release in females.
