Aim: To compare and evaluate clinical efficacy of corticosteroids in thrombocytopenia in patients suffering from moderate to severe Dengue. Methods: This randomized single blinded study was conducted at Furqan Clinic Gulbahar Peshawar from August 2021 to October 2021. 100 individuals of ages between 20 to 60 years irrespective of gender, with positive NS1 antigen test and no co-morbidities were included in study by convenient sampling technique. Patients with dengue and Covid-19 were excluded from the study. Randomization was done in four treatment groups. Consent was taken from all patients prior treatment with Cefixime, artemether/lumefantrine, IV Dexamethasone in adjunct to oral papaya leaf extracts. Duration of study was 3 months. Statistical analysis done using SPSS software version 24. Results: Group C and D showed the significant increase in platelet count as compared to group A and B. Pronounced effect was observed in group D. Conclusion: Intravenous Dexamethasone produced more beneficial effects in adjunct to oral carica papaya leaf extracts on thrombocytopenia with reduced risk of bleeding. Aim: To evaluate the role of platelet parameters in dengue fever and also to determine relationship of platelet parameters with platelet count and the severity of the disease. The platelet para Materials and methods: meters [platelet count, plateletcrit, mean platelet volume (MPV), platelet distribution width (PDW)] of 50 dengue infected children were calculated using BC 3000 plus Mindray Automated Hematology Analyzer. Normal range of MPV is 7.5-11.5 . Plateletcrit cut off value in thrombocytopenia is 0.2-0.36%. The PDW reported varies with reference intervals ranging from 8.3 to 56.6%. Subjects were divided into four groups according to their platelet count on day 3 of illness (<20,000 cells/cumm, 20,000 to 50,000 cells/cumm, 50,000 to 100,000 cells/cumm and >100,000 cells/cumm). Platelet indices were studied among these groups and also the indices were evaluated with respect to the severity of disease as group A (dengue without warning signs), group B (dengue with warning signs and group C (severe dengue). Results: Male: female ratio was 28:22. Mean age of presentation was 7.8 + 2.2 years. Thirty-ve patients belonged to Group A, 26 to group B and 2 belonged to group C. PLT (cells/cumm) on Day 3 and Day 7 was 91,828 ±33,532 vs 195,371 + 70,586 in Group A patients (p<0.0001), 69,076 ±45,904 vs 163,230 + 85,053 in Group B patients (p=0.0018) 38,500 + 38,890 vs 86,000 + 28,284 in Group C patients (p=0.29). PCT (%) on Day 3 and Day 7 was 0.09 + 0.04 vs 0.18 + 0.06 in Group A patients (p<0.0001), 0.07 + 0.04 vs 0.15 + 0.07 in Group B patients (p=0.0009) and 0.04 + 0.04 vs 0.08 + 0.28 in Group C patients (p=0.84). MPV () on Day 3 and Day 7 was 10.73 + 1.07 vs 11.34 + 1.09 in Group A patients (p=0.0212), 10.41 + 1.39 vs 10.87 + 0.99 in Group B patients (p=0.3389), 9.35 + 1.62 vs 10.3±1.41 in Group C patients (p=0.59). PDW () on Day 3 and Day 7 was 14.46 ± 1.35 vs 13.22 + 1.10 in Group A patients (p=0.0001), 14.61 + 1.36 vs 13.0 + 0.76 in Group B patients (p=0.0011) and 14.5 + 1.84 vs 13.5 + 0.71 in Group C (p=0.54). Though MPV was lower in patients with platelet count <20,000 cells/cumm (8.57 + 0.5 ) as compared to other groups, it was not statistically signicant (p=0.325). PCT increases with increase in platelet count on Day 3 and Day 7 (p=0.000). Though PDW was lower on Day 3 in patients with platelet count <20,000 cells/cumm as compared to the other 3 groups, it was not statistically signicant (p=0.0740). Conclusion: Lower platelet count and low PCT are seen with severe dengue, increasing platelet count and PCT suggest recovery phase of dengue. MPV had no correlation with severity of dengue or level of thrombocytopenia suggestive that there is no role to predict severity of dengue. PDW does not uctuate with severity of dengue or level of thrombocytopenia

Hormonal treatment of cows at the coastal region of Barisal district of Bangladesh was performed to assess the improvement of pregnancy rate. A total of 100 cows and heifers with irregular history of cyclicity were selected randomly and divided into five treatment groups. The groups were A (treated with anthelmintic), B (treated with anthelmintic, vitamin ADE and multivitamin powder), C (treated with PGF2α), D (treated with GnRH) and E (treated with GnRH and PGF2α). Each group comprised of 20 animals. The age, breed and parity of experimental cows were considered during treatment. In the study, the cows treated with both GnRH and PGF2α (group E) showed significantly (p<0.05) higher estrus (80%) and pregnancy rate (60%) than that of group A, B, C and D. The overall estrus rates of local and crossbred cows were 64% and 70%, respectively and the pregnancy rates were 40 and 52%, respectively. The crossbred cows responded significantly (p<0.05) to hormonal treatment than that of local cows. Parity-2 cows showed higher estrus sign than that of other parities. However, the pregnancy rates were higher significantly (p<0.05) in parity-2 and parity ≥4 cows than that of parity-0, parity-1 and parity-3 cows. The pregnancy rate was also found higher in case of 4 to <5 years old cows than that of 2 to <3, 3 to <4, 5 to <6, and ≥6 years old. It may conclude that the hormonal regimen increases the pregnancy rate as well as decreases the undesired waiting of estrus and conception. Further study with more sample size will reveal the more effective treatment for cows at the coastal areas of Bangladesh.

Plant-derived digestive enzyme inhibitors particularly those targeted to carbohydrate metabolism has been the focus of recent studies as natural supplements for weight control and diabetes. The present study explores the salivary amylase inhibition activity of Garcinia mangostana (Linn.) pericarp extracts and Carica papaya (Linn.) leaf extracts and fractions, as well as perform phytochemical screening and quantification, and thin layer – and high performance liquid chromatographic profiling. ­Results show that crude extracts and purified fractions were able to inhibit salivary amylase, with C. papaya fraction 1 being the most active at 30.89% inhibition. Phytochemical screening of all extracts tested ­positive for tannins, glycosides, phenolics, flavonoids and alkaloids. Quantification of phenolics showed that extracts contained high levels of phenolics, with C. papaya crude extract having the highest content with 219.0±12.7 mg GAE/g extract followed by G. mangostana crude extract with 247.1±18.0 mg GAE/g extract. Quantification of total flavonoids also showed C. papaya crude extract to contain the highest content with 55.12±0.679 mg QE/g extract. All extracts contained negligible alkaloid content, though. HPLC and TLC profiling showed several peaks and bands, when viewed in 210 nm and UV light, respectively. These results demonstrate in vitro the salivary amylase inhibitory activity of both plants and their potential as antidiabetic drug candidates; however, further studies need to be done, like isolation and structure elucidation of active components and toxicity assays. Keywords: Amylase inhibition, phytochemical quantification, Carica papaya, Garcinia mangostana

OBJECTIVE To compare the effectiveness of mobilization and self-exercises in the management of adhesive capsulitis of shoulder STUDY DESIGN Randomized Control Trial SAMPLE SELECTION 30 patients of adhesive capsulitis of shoulder from physiotherapy department of tertiary care hospitals of Karachi were selected through simple random sampling technique. PROCEDURE Treatment was continued for 5 days per week for the period of 3 weeks followed by assessment. Patients were randomly divided into two equal groups. Group A was treated with midrange mobilization while group B performed self-exercises. Both groups received TENS and hot pack prior to the exercises. Mean ± SD, frequencies and percentages were used for descriptive analysis. ROM via goniometry and pain intensity through VAS was analyzed by paired t-test within the groups and by independent t-test between the groups, using SPSS. P-value of less than 0.05 was considered significant. RESULTS 60% were females (n=18) and 40% were males (n=12) with mean age of 50.17±6.37 years. Significant improvement (p-value <0.05) in pain and shoulder ROM was observed among patients of Group A as compared to Group B. Pain intensity was decreased to 1.67 ± 0.62 in group A, whereas ROMs in these patients were also better than other group.

ABSTRAKPerkembangan (development) adalah bertambahnya kemampuan atau fungsi semua sistem organ tubuh sebagai akibat bertambahnya kematangan atau maturitas fungsi sistem organ tubuh (Dewi, 2013). Tujuan penelitian ini adalah mengetahui perbedaan efektivitas pemberian origami dan playdough terhadap perkembangan pada anak prasekolah kelompok A di TK Aisyiyah Bustanul Athfal tahun 2018.Desain penelitian yang digunakan adalah penelitian pre eksperiment dengan pendekatan pre-test and post-test Design. Populasi yang diteliti adalah seluruh anak kelompok A di TK Aisyiyah Bustanul Athfal berjumlah 56 anak dengan teknik purposive sampling diperoleh sampel 36 responden. Instrumen penelitian yang digunakan adalah lembar KPSP. Hasil penelitian kemudian dianalisa dengan menggunakan wilcoxon signed rank.Hasil penelitian yang dilakukan menunjukan bahwa perkembangan anak sebelum pelaksanaan pemberian Origami didapatkan setengahnya perkembangan anak meragukan, setelah pelaksanaan didapatkan hampir seluruhnya perkembangan anak sesuai. Perkembangan anak sebelum pelaksanaan pemberian Playdough didapatkan sebagian besar perkembangan anak meragukan, setelah pelaksanaan didapatkan sebagian besar perkembangan anak sesuai. Hasil analisis menunjukkan ada pengaruh pemberian permainan origami dan permainan Playdough terhadap perkembangan anak kelompok A di TK Aisyiyah Bustanul Athfal Tahun 2018 dengan hasil ρ-value = 0,001 ɑ = 0,05 dari kelompok origami dan ρ-value = 0,007 ɑ = 0,05 dari kelompok playdough, sedangkan hasil analisis perbedaan adanya perbedaan efektivitas pengaruh pemberian permainan origami dan playdough terhadap perkembangan anak pada kelompok A di TK Aisyiyah Bustanul Athfal Tahun 2018 dengan hasil ρ-value = 0,043 ɑ = 0,05.Berdasarkan hasil penelitian dapat disimpulkan ada pengaruh permainan origami dan playdough terhadap perkembangan anak pada kelompok A di TK Aisyiyah Bustanul Athfal Tahun 2018. The purpose of this research is to know the effectiveness difference of origami and playdough on development in preschoolers group A in Aisyiyah Bustanul Athfal Kindergarten in 2018.The research design used is research pre eksperiment with approach pre-test dan post-test. The population studied was all group A children in kindergarten Aisyiyah Bustanul Athfal amounted to 56 children with purposive sampling technique obtained sample 36 respondents. The research instrument used is KPSP sheet. The results were then analyzed by using wilcoxon signed rank.The results of the research show that the development of children before the implementation of giving Origami found half of child development doubt, after the implementation is obtained almost entirely the child's development accordingly. Child development prior to the implementation of Playdough gift obtained most of the development of children doubt, after the implementation is obtained most of the child's development accordingly.  The results of the analysis show that there is an effect of giving origami game and Playdough game to the child development group A in Aisyiyah Bustanul Athfal Kindergarten Year 2018 with the result ρ-value = 0.001 ɑ = 0.05 from the origami group and ρ-value = 0.007 ɑ = 0.05 of the playdough group, while the result of difference analysis that is difference between origami and playdough influence to children development in group A diiyah Aisyiyah Bustanul Athfal Year 2018 with result of ρ-value = 0,043 ɑ = 0,05.Based on the research results can be concluded there is influence of origami and playdough on the development of children in group A in Aisyiyah Bustanul Athfal Kindergarten Year 2018. Thrombocytopenia (platelet count less than 150 × 109/L) is a frequent complication of decompensate cirrhosis and is considered as an indicator of advanced disease. Carica papaya leaf juice has beneficial effect in thrombocytopenia associated with dengue. Tinospora cordifolia has been shown to prevent the fibrous tissue deposition of liver by modulation of kupffer cell activation. An attempt was taken to observe the usefulness of extract Carica papaya and Tinospora cordifolia in alcoholic decompensate cirrhosis. A market available product Cariden is easily available to the patients which contains Phyto extracts of Carica papaya 1100mg and Tinospora cordifolia 500mg. Phyto extracts of Carica papaya and Tinospora cordifolia can enhance the platelet count within 15 days and it can normalise the platelet within 90 days of therapy in all three cases. Further randomised control trial is suggested.

The study was to determine the presence of Classical Swine Fever virus (CSFv), in the meat of vaccinated pigs with the PAV-250 strain and then challenged using the same strain. Five treatment groups were established (each with four pigs). Group A: Pigs thatwere fed with processed hams from negative animals; Group B: Pigs that were fed with processed hams from commercial pigs inoculated with the ALD (reference strain) (titre of 104.0/ml); Group C: Pigs fed with processed hams from pigs infected with the virulent ALD strain (titre of 102.5/ml); Group D: Pigs fed with processed hams from pigs vaccinated with the PAV-250 strain and challenged with the ALD strain (titre of 101.1/ml); and Group E: Pigs fed with processed hams from pigs vaccinated with two doses of the PAV-250 strain and challenged with the ALD strain (negative). Blood samples were taken at d 1, 5, 10, 15 and 20 for biometric analysis. Groups B, C and D manifested clinical signs of CSFv: 40 °C temperature, anorexia, paralysis, vomiting, diarrhea, tremor, hirsute hair and cyanosis. Pigs were slaughtered and necropsies performed to identify lesions in tissues. Results of direct immunofluorescence testing of tissues were positive and the virus was recovered. Under these study conditions, it was found that CSFv resisted the cooking method at 68 °C for 40 min in hams from unvaccinated pigs, and that the virus was able to transmit the disease to healthy unvaccinated pigs, whereas the hams from the vaccinated animals did not transmit the virus.

The positive effects of nucleotide (NU) supplementation in milk replacer have been elucidated in infants and in dairy calves; however, NU addition to whole milk has not been evaluated previously. This study aimed to assess NU supplementation in the whole milk on calf growth and health. Thirty Holstein calves (body weight: 39.1 ± 1.0 kg; 3 d after birth) were randomly assigned to the following treatments: whole milk without any supplementation (NU0), whole milk + 0.5 g/d added a NU-containing supplement to whole milk (NUCS0.5), and whole milk + 1 g/d added a NU-containing supplement to whole milk (NUCS1). Calves were weaned at d 55 and stayed on study until d 75. Calves had free access to feed and water throughout the study. Dry matter intakes (DMI) were similar among treatments (p > 0.05) during the pre-weaning period; however, increasing NU resulted in a linear (p < 0.05) increase in DMI during the post weaning period (2158, 2432, and 2518 g/d for NU0, NUCS0.5, and NUCS1, respectively). Treatments did not affect body weight (BW) at the first and second month of study, but final BW linearly increased as NU was added (87.1, 90.6, and 95.4 kg for NU0, NUCS0.5, and NUCS1, respectively). Neither pre-weaning average daily gain nor post-weaning average daily gain was affected by treatments; accordingly, feed efficiency was similar among treatment groups. Days with loose fecal score were linearly decreased as NU was added to whole milk during the first month of life, while the fecal score did not differ among treatments until the end of the study. No difference was observed in the skeletal growth of calves in the current study. Therefore, it can be concluded that NU supplementation in the whole milk has some beneficial effects on calf performance in terms of final BW, post-weaning DMI, and less days with loose feces.
