Diabetic foot (DF) is one of the most common complications of diabetes. Diabetic foot is one of the main causes of disability and death of diabetic patients, and it is also a major public health problem that causes a heavy burden on society. Diabetic foot involves a variety of factors including peripheral nerve tissue lesions, ischemic lesions, and reduced body immunity. With the development of medical standards, clinical knowledge and treatment of diabetic foot are constantly improving. Early diagnosis and intervention is the key to reducing the incidence of diabetic foot and improving the cure rate. This chapter will briefly introduce the diagnosis, the treatment, the multidisciplinary collaborative therapy and prevention of diabetic foot.

Amebiasis caused by <em>Entamoeba histolytica</em> is a major public health problem in tropical and subtropical countries. Treatment failure with specific chemotherapy has been reported suggesting the possibility of drug resistance. This study investigated the anti-amoebic effects of four plant extracts on cecal amebiasis in rats. The cecal amebiasis was induced by the injection of 3.0×105 troph/mL of <em>E. histolytica</em> parasite directly into the rat’s caecum. A total of 137 rats were used for these studies; five rats in each group for both positive and negative control, 15 rats in each group to test the four plant extracts and metronidazole. The infected rats were treated for cecal amebiasis using each of the four plant extracts at graded doses of 100 mg/kg, 200 mg/kg and 400 mg/kg and with metronidzole at a dose of 62.5 mg/kg,100 mg/kg and 125 mg/kg for five consecutive days. The efficacy of the four plant extracts were evaluated based on Neal’s, 1951 method. The plant extracts of Garlic, <em>Guava</em>, Pawpaw and Pumpkin at 400 mg/kg and 200 mg/kg body-weight gave a cure rate of 80%, 100%, 60%, 40% and 40%, 80%, 40%, 0%, respectively. The mean parasite count in the cecal contents of the treated rats at a dose 400 mg/kg were 18.5±1.6, 0.0±0.0, 33.3±1.8 and 49.5±4.0, respectively. The difference was statistically significant (P&lt;0.05). This study has revealed that Guava at a high dosing level (400 mg/kg body weight) is as good as the standard drug in reducing the both parasite load (probably with limited side effect).

Background: Diabetes is global epidemic with devastating human, social and economic consequences. The disease claims as many lives per year as HIV/AIDS and places a severe burden on healthcare systems and economies everywhere, with the heaviest burden falling on low- and middle-income countries. Despite this, awareness of the global scale of the diabetes threat remains pitifully low, inappropriate diabetic foot care affects, feet health leading to callosities, cracks, fissures, fungal infections, ingrown toe nails and patients end up in ulcers and amputations. Objectives: To assess diabetic patients taking proper foot care according to International Guidelines and its impact on their foot health. Methods: A cross sectional study was conducted at outdoor patients department of Medicine, Liaquat University of Medical Sciences Hospital Hyderabad from 17th January 2008 to 16th January 2009. 100 diabetic patients were selected by non probability convenience sampling according to Performa having questions regarding diabetic foot care derived from American Diabetic Association Guidelines for Diabetic Foot care. Results: Diabetic patients taking proper foot care was only 6%. There were 45(45%) males and 55(55%) females. Mean age was 51.57+10.72 years. 38% patients knew about foot care. 17% used to inspect their feet daily, 20% washed their feet daily, while 73% washed their feet more than once. 23% patients dried their feet after every foot wash, 27% applied emollients, 25% checked shoes before wearing, 24% used to wear correct shoes, 8% used to wear cotton socks and 36% used to walk bare feet. Foot care practices on foot health has statistically highly significant association (p<0.01) e.g. number of foot washes with foot hygiene, fungal infections with proper foot drying, emollient application with skin texture, cracks and fissures. Associations of proper foot care were statistically significant with literacy status of patients and foot care teachings (p<0.05). Conclusion: Few diabetic patients are taking proper foot care. Proper Foot care practices were associated with provision of education of foot care and literacy status of patients. Community health education programs regarding diabetic foot care will likely to reduce diabetic foot complications. Diabetes mellitus (DM) represents a major public health problem, with yearly increasing prevalence. DM is considered a progressive vascular disease that develops macro and microvascular complications, with a great impact on the quality of life of diabetic patients. Over time, DM has become one of the most studied diseases; indeed, finding new pharmacological ways to control it is the main purpose of the research involved in this issue. Sodium–glucose cotransporter 2 inhibitors (SGLT-2i) are a modern drug class of glucose-lowering agents, whose use in DM patients has increased in the past few years. Besides the positive outcomes regarding glycemic control and cardiovascular protection in DM patients, SGLT-2i have also been associated with metabolic benefits, blood pressure reduction, and improved kidney function. The recent perception and understanding of SGLT-2i pathophysiological pathways place this class of drugs towards a particularized patient-centered approach, moving away from the well-known glycemic control strategy. SGLT-2i have been shown not only to reduce death from cardiovascular causes, but also to reduce the risk of stroke and heart failure hospitalization. This article aims to review and highlight the existing literature on the effects of SGLT-2i, emphasizing their role as oral antihyperglycemic agents in type 2 DM, with important cardiovascular and metabolic benefits.

Background:Diabetes mellitus is a major public health problem globally and in Indian population and diabetic foot is reported as the most common cause of non-traumatic amputation of the lower limbs in India. There is renewed interest in various inflammatory markers and their association with various chronic complications of diabetes mellitus. There is a scarcity of data on the subject in Indian population.Methods: The current study was a cross sectional study of 100 patients admitted to a Diabetic foot in Department of General Surgery, Stanley medical college and Hospital between January 2012-November 2012. The CRP level and fasting and plasma glucose levels were considered as the relevant variables for statistical analysis.Results:A total of 100 patients were included in the final analysis. The proportion of subjects with Wagners, grade 1, grade 2, 3, 4 and 5 ulcers were 48%, 27%, 16%, 3% and 6% respectively. Among the study population, 73% of patients had CRP value greater than 40 and 27% patients had CRP value less than 40. The proportion of subjects with Higher CRP level (>40) showed increasing trend with increasing level of fasting blood sugar and post prandial blood sugar in the study population. The proportion of people who underwent amputation was 27.5% in people with CRP value >40 and it was only 6.85% of people with CRP value <40, the association between CRP values and amputation was statistically significant. (P value 0.046).Conclusions:The study has established a strong positive association between poor blood sugar control and elevated CRP levels in the study population. The study has also documented a positive association between higher CRP levels and amputation.

Background: Diabetes mellitus is a major public health problem. Non communicable diseases like coronary artery disease (CAD), stroke, obesity are increasing in number and overtakes the communicable diseases with respect to morbidity and mortality. Diabetic populations have a higher risk of developing CAD than the general population. The aim of this study was to assess the risk of developing cardiovascular diseases among diabetic patients.Methods: This is a cross sectional study done among the urban adult population in Kancheepuram district. Sample size of 400 diabetic patients was studied by using consecutive sampling method. Data was collected by administering a pre tested structured questionnaire. Cardiovascular risk was assessed based on PROCAM scoring, a 10 years risk assessment score. The study was done for a period of three months from January to March 2018. Data analysis was done in SPSS software version 22.Results: Among the diabetic patients, 53.5% of them are in high risk category based on PROCAM score for developing cardiovascular disease. A statistical significant association was found between cardiovascular risk and factors like socioeconomic status and overweight/obesity.Conclusions: This study reveals that the diabetic populations are at higher risk of developing cardiovascular diseases. Hence awareness about the risk should be created and appropriate intervention at early stages should be implemented at primary health care level.

Background: Diabetic foot is a major public health problem and their complications are an imperative cause of morbidity and mortality in diabetes. Objective: To evaluate the rate of recurrence of foot ulcers post two years of follow-up, including the associated risk factors in the patients attending the diabetic foot clinic at the National Center for Diabetes, Endocrinology, and Genetics (NCDEG), Amman, Jordan. Methods: A historical cohort design was adopted for the patients who presented for the first time to the diabetic foot clinic at the NCDEG. Every patient who came to the clinic was reviewed by using his or her medical files with respect to diabetic foot-related complaints. Patients were classified under four categories: (1) Recurrent foot ulcers (2) chronic foot ulcer (3) free of recurrence, and (4) no foot ulcers. Among the four groups, group 1 (recurrent foot ulcers, n=76) and group 3 (free of recurrence, n=54) were included for the analysis. Results: Among the 141 patients who presented to the diabetic foot clinic during the two-year study period, 76 (53.9%) of them experienced ulcer recurrences, 54 (38.3%) were recurrence-free, and 11 (7.8%) had chronic ulceration. The two-year recurrence rate was 58.5%. The presence of deformity and osteomyelitis were the statistically significant independent risk factors for recurrent foot ulceration. Conclusion: This study of recurrences was clearly related to the type and complications of the ulcers rather than to the other variables. Recurrent foot ulceration is linked to the presence of osteomyelitis and/or deformities.

Abstract Background and objectives: Hypertension is a major public health problem with raising its prevalence and effect among adults overtime worldwide, especially in Bangladesh. The aim is to investigate the effect of diabetes on hypertension.Materials and methods: The dataset used in this study was extracted from Bangladesh Demographic Health Survey, 2017-18 having a total of 6,965 (male: 3,376 and female: 3,589) adults whose ages were B35 years. Bivariate analysis along with Pearson’s chi-square test was executed to observe the association between different selected factors and hypertension. Additionally, binary logistic regression was employed to investigate the effect of diabetes on hypertension based on adjusted odds ratio (AOR) along with p-value in Bangladesh.Results: The results of the study revealed that average age of the participants was 51.04a12.731 and a total of 34.7 percent participants were identified as hypertension. Logistic regression analysis demonstrated that diabetic patients were 1.280 times (95% CI of AOR: 1.107-1.479; p-value=0.001) higher risk of hypertension compared to non-diabetic. Furthermore, our finding’s also showed that diabetic patient who was 35–49 years age, 1.462 times (95% CI of AOR: 1.182-1.807; p-value=0.000) higher risk of hypertension compared to age groups ≥50 years.Conclusions: Based on the results, this study claimed that people with diabetes was significantly associated with hypertension. This study suggested greater attention of government and policymakers to make appropriate strategies to reduce hypertension as well as associated risk in Bangladesh.

Diabetes mellitus (DM) poses a major public health problem worldwide, with ever-increasing incidence and prevalence in recent years. The Institute for Alternative Futures (IAF) expects that the total number of people with type 1 and type 2 DM in the United States will increase by 54%, from 19,629,000 to 54,913,000 people, between 2015 and 2030. Diabetic Nephropathy (DN) affects about one-third of patients with DM and currently ranks as the first cause of end-stage kidney disease in the Western world. The complexity of interactions of Vitamin D is directly related with progressive long-term changes implicated in the worsening of renal function. These changes result in a dysregulation of the vitamin D-dependent pathways. Various studies demonstrated a pivotal role of Vitamin D supplementation in regression of albuminuria and glomerulosclerosis, contrasting the increase of glomerular basement membrane thickening and podocyte effacement, with better renal and cardiovascular outcomes. The homeostasis and regulation of the nephron’s function are absolutely dependent from the cross-talk between endothelium and podocytes. Even if growing evidence proves that vitamin D may have antiproteinuric, anti-inflammatory and renoprotective effects in patients with DN, it is still worth investigating these aspects with both more in vitro studies and randomized controlled trials in larger patient series and with adequate follow-up to confirm the effects of long-term vitamin D analogue supplementation in DN and to evaluate the effectiveness of this therapy and the appropriate dosage.

Introducción: El pie diabético es un problema de salud pública debido a su elevada frecuencia, severidad y a su impacto económico. En la etapa prepatogénica, el pie presenta alteraciones funcionales y estructurales que de no detectarse a tiempo pueden progresar a la úlcera. Sin embargo, el examen de los pies no se realiza en la mayoría de las consultas. La prevención primaria y la detección oportuna deben tener lugar en el primer nivel de atención.Objetivo: Desarrollar y validar un instrumento simple y práctico para clasificar de manera temprana el pie del paciente diabético en riesgo por parte de la enfermera entrenada antes de la visita a su médico tratante, teniendo en cuenta todos los elementos patogénicos.Métodos: Estudio transversal. Se creó una escala a partir de la experiencia clínica en donde se califican ocho factores de riesgo para desarrollar ulceración: tiempo de diabetes desde el diagnóstico, control metabólico, presencia de síntomas de neuropatía diabética, diagnóstico de la neuropatía por monofi- lamento, presencia de claudicación intermitente y ausencia de pulsos periféricos, presencia de deformidades, presencia de infecciones locales e historia anterior de úlceras que sanaron, amputaciones menores o pie de Charcot. Los puntajes fueron asignados según gravedad. Esta herramienta fue aplicada a un grupo de 204 diabéticos con ulceración y 207 pacientes diabéticos sin ulceración.Resultados: El promedio de edad fue de 65 años, el 42,8% de los pacientes eran hombres. La edad y todos los ítems de la escala, excepto hemoglobina glucosilada, fueron asociados con la presencia de úlcera. Usando un punto de corte de 10 puntos, la herramienta tuvo una sensibilidad del 100% y una especificidad del 73,4% para el diagnóstico de úlcera, con un área bajo la curva ROC de 0,992(IC 95%, 0,986 A 0,998).Conclusión: La herramienta “pie risk” es útil para la calificación y valoración del riesgo de pie diabético.Abstract Background: The diabetic foot is a public health problem due to its frequency, severity and economic impact. In prepathogenic phases, the foot presents structural and functional alterations that, if not detected opportunely, will progress to an ulcer. However, a thorough examination of the diabetic patient’s feet is often eluded in routine health controls. Objective: To develop and validate a simple and practical instrument for early classification of the diabetic foot risk that can be easily applied by a trained nurse before the medical visit. Methods: Cross-sectional study. Based on clinical expertise, a scale with eight risk factors for foot ulcer was developed: time from diagnosis, glycosylated hemoglobin, neuropathy symptoms, monofilament test, absent pulses or claudication; deformities, local infection and history of either amputation, ulceration or Charcot foot. Item scores were assigned based on severity. The tool was tested in a group of 204 ulcerated diabetic patients and 207 of non-ulcerated diabetic patients. Results: Median age was 65 years, 42.8% of patients were male. Age and all scale items except glycosylated hemoglobin were statically associated with ulcer presence. Using the proposed cut point of 10 points, the scale has 100% sensitivity and73.4% specificity for ulcer diagnosis, with an area under the ROC curve of 0.992 (95% CI 0.986 to 0.998). Conclusions: ‘Pie risk’ is a useful tool for the evaluation and risk assessment of the diabetic patient.

Background: Diabetes is a major public health problem both in developing and non-developing countries across the world. It is a chronic disease, which in long term causes several complications resulting in poly pharmacy for its management. Hence, this study was determined to analyze the drug utilization pattern for the management of type 2 diabetes with complications.Methods: A prospective, observational and non-interventional study was carried out in 100 diabetic patients with one or other complications admitted in medicine wards at Dhiraj Hospital. Patients who signed informed consent form were only included in the study. All the data were recorded from patients’ case files and analyzed.Results: Result of total 100 patients, maximum number 52 (52%) were falling in group of 61-70 kg and only 2 (2%) in 81-90 kg. Out of 100 diabetic patients, 40 (40%) were managed with insulin in addition to oral antidiabetic agents, 37 (37%) were managed with only Oral Hypoglycemic Agents (OHA) and 23 (23%) were managed with only insulin. The most commonly prescribed oral antidiabetic group of drug was Biguanides in 60 (60%) and most prescribed insulin was short acting Insulin in 40 (40%) patients.Conclusions: The diabetic patients are more prone to cardiovascular and other complications leading to a co morbid condition. The poly pharmacy is likely to occur in diabetic patients suffering with secondary complications. Therefore, intense blood sugar control with proper education can prevent the co morbid state and finally helps in reducing the economic burden.
