Closure of nasal defect remains a challenge for surgeons. There are several ways to do nasal reconstruction, including donor site for nasal reconstruction due to its vascularity that is superior to other areas. Methods: This is a case report of a 75-year old male patient with Squamous Cell Carcinoma on his left nostril who underwent wide excision. This action left a 4-cm defect, whilst the nasal septum remained intact. This defect closure was using V-Y advancement full thickness nasolabial flap in one step. This defect closure was using V-Y advancement full thickness nasolabial flap in one step. Results: The defect closure was performed with tum over local (nasobialis) flap from the left side of the nasal cartilage. The superiority of this flap, it is able to be performed bulking, so that closure with turn over local nasolabial flap has been a sole option in nostril reconstruction for decades, which gave very good cosmetic results. After being followed for 1 month after surgery, the flap was viable, the contour was well formed, the scar was minimal, and there was no respiratory disruption. The patient was satisfied by the results. Conclusion: Nasolabial turn over local flap can be used as an option to close a relatively wide nostril

Background: Anesthesia-related mortality refers to death resulting from the complications of anesthesia or death due to at least one anesthesia-related complication happening as a cause among the multiple causes of death. The aim of this study was to evaluate the incidence and causes of anesthesia-related mortality in pediatric patients, as experienced by a surgeon.  Materials and Methods: This was a retrospective study of children aged 15 years and younger who died at the pediatric surgery unit of Enugu State University Teaching Hospital (ESUTH) Enugu, Nigeria following administration of anesthesia between January 2010 and December 2019. Results: A total of 31,210 pediatric anesthesias/surgeries were performed during the study period; out of which 78 mortalities occurred due to anesthesia. There was male predominance and the median age of the patients was 4 years with a range of 1 week to 12 years. Mortality occurred more in neonates and during emergency surgeries. Airway complications and intussusception were the most common etiology of mortality and pathology in the children respectively. About two-thirds of the deaths occurred in the operating room before commencement of the surgery. Conclusion: Anesthesia-related deaths in children in middle income country like Nigeria are higher than what is obtainable in developed countries. The risk factors for anesthesia-related mortality include male gender, neonates and emergency surgeries. Airway problem is a common cause of mortality.

According to official sources, the coronavirus pandemic that emerged in China in December 2019 has already infected more than 17 million and killed more than 666,850 people. Less impacted than the rest of the world, the Africa continent has nearly 890,000 confirmed cases as of July 30, 2020. According to the African Bank of Development, nearly 50 million Africans will be victims of extreme poverty as a result of the coronavirus pandemic; and this, following the global economic crisis due to this pandemic. It is shown that between 2020 and 2021, Africa will lose in terms of economic income ¼ of trillion dollars and that a contraction of 3.4% of GDP would be noticed. The risk of decline is justified by the decline in oil production by the main exporting countries, notably Algeria, Nigeria and Angola; in addition to the decline in commodity prices on the world market, the volatility of global financial conditions and natural disasters. Even if some analysts believe that the effects of this pandemic are temporary on the economy of the states, it is clear to note that they are perceptible on the world economy in general. In Africa, the low rate of infection, which is 5% since the emergence of the pandemic until July 30, 2020, is a favorable opportunity for the continent to recover and accelerate the process of its economic and regional integration.

A trial was conducted in 2017 at the Higher Institute of Agronomic Studies (ISEA) in Bokonzi with the aim of setting up an egg incubator using palm oil-based combustion as an energy source to produce chicks. During 21 days of hen egg incubation in this tool, temperatures varied between 36.8 and 40°C with a minimum of 35.8°C (day eight) and a maximum of 40.5°C (day three and fifteen). Although the temperature values (40.5°C) are slightly high with consequences on egg hatchability, the range of temperatures recorded during this study is within the acceptable threshold previously outlined by other researchers for hen egg incubation. Thus, with the daily frequency of 3 to 4 egg turnings, the hatchability rate reached 75%. This result is consistent with the values previously obtained by other researchers, i.e. 65 to 75%.  In addition, in terms of production costs, the palm oil heated incubator proved to be superior, resulting in a 79.6% reduction in fuel costs compared to some types of oil heated incubators. This reflects a relative benefit of 875 Congolese Francs (or 0.53USD) per chick produced. This simple technology, using an available energy source, appears to be more adapted to the income of farmers and to the needs of rural communities where hydroelectric and thermal energy sources (oil and gas that produce heat energy) are not accessible at all.

The purpose of this study was to determine the effect of adding probiotic acidifier on the condition of the digestive tract, glucose levels and abdominal fat in broiler chickens. This research conducted in Takengon, Central Aceh District. The parameters observed were the condition of the digestive tract, blood glucose levels and the percentage of abdominal fat. The data obtained were analyzed using analysis of variance (ANOVA). The results showed the use of a 1: 1 mixture of probiotics and acidifier to level 0. The 3% added to the commercial ration which was substituted with 10% fermented feed ingredients (R-KFPA) had no significant effect (P <0.05) on weight and percentage of Gizzard, Gizzard pH and Intestine pH, Intestinal Length, Weight and Percent of Abdominal Fat, as well as blood glucose levels in broiler chickens. Although statistically not significantly different, giving a mixture of probiotics and acidifier at the level of 0.3% (R4) showed a positive effect on decreasing the percentage of abdominal fat from 1.52% (R1) to 1.35% (R4). The blood glucose obtained in this study was slightly higher than normal blood glucose levels in chickens as reported by Swenson weight and percentage of abdominal fat, as well as blood glucose levels of broiler chickens. The blood glucose obtained in this study was slightly higher than normal blood glucose levels in chickens as reported by Swenson (1993), which ranges from 13 to 27 ml / dl or 130 to 270 mg / dl. It is suspected that this is due to increased digestive activity in chickens so that more nutrients are absorbed.

Outbound Call Center PT. Infomedia, consumer profiling service PT. Telkom during the pandemic period divided its agents into 80% WFH agents (Work at Home) and 20% agents WFO (Work from Office). For the division of the working mechanism, it is necessary to measure its performance. In the discussion of this paper, we will discuss the measurement with the application of data mining using the K-Means method, so it is hoped that it will provide an overview, how the cluster of each WFH or WFO agent in terms of performance. The results of this discussion indicate that there is a significant difference between the performance of WFH and WFO Agents.

Thalassemia is a congenital blood disorder characterized by reduced production of one or more globin chains. Thalassemia patients lack healthy hemoglobin which the body needs to be properly oxygenated. Blood transfusion is the main treatment for thalassemia patients. Checking hemoglobin levels after transfusion is a common test, but until now there is no definite uniformity of time to carry out these tests, so it will be detrimental to the patient and increase the length of the patient in the hospital. Assessing the difference in time to increase in hemoglobin levels at 1 hour, 6 hours, and 12 hours after blood transfusion in thalassemia patients. This research is an observational analytic study with cross sectional design. This study was conducted by examining the hemoglobin levels of thalassemia patients after 1 hour, 6 hours, and 12 hours after transfusion in thalassemia patients aged 1-18 years with a total sample of 40 people. The statistical analysis used was paired t test. The results showed that 20 men and 20 women and Most were in the age group 10-13 (35%), with an average Hb level at admission of 7.38 g / dL (95% CI). At 1 hour post-transfusion, the patient's mean Hb level increased by 8.97 g / dL (8,59-9,35 g / dL), at 6 hours post transfusion, 8.95 g / dL (8,57-9,32 g / dL), at 12 hours post transfusion increased by 9.60 g / dL (9,17-10,03 g / dL). Significant increase in Hb levels occurred at 1 hour and 12 hours after blood transfusion.

Little is known about traditional bone setting (TBS) practise in Aceh, Indonesia despite of its common utilisation. The aim of this study was to explore the characteristics of TBS practise in Aceh, Indonesia. Thirty-nine participants from ten regencies in Aceh were interviewed from January to December 2019. Most of the participants were male (82.1%), and aged between 31-50 years old (79,5%). Half of the participants were elementary school graduates (51,3%). More than half (59%) of participants had been practicing bone setting for 5-10 years, and many of them (48.7%) used traditional massages combined with splints and herbal mixtures. None (0%) of the participant obtained legal permission to practice bone setting. More than half (71.8%) obtained the knowledge of TBS from their ancestral heritage, only one participant obtained it through apprenticeships. We concluded that TBS practices in Aceh were unregistered, and the practitioners were mostly untrained. There is a need to provide training and permits for TBS practise to ensure patients’ safety.
