Using Innovative Simulation Modalities for Civilian-based, Chemical, Biological, Radiological, Nuclear, and Explosive Training in the Acute Management of Terrorist Victims: A Pilot Study

AbstractObjectives:Chemical, biological, radiological, nuclear, and explosive (CBRNE) incidents are low frequency, high impact events that require specialized train-ing outside of usual clinical practice. Educational modalities must recreate these clinical scenarios in order to provide realistic first responder/receiver training.Methods:High fidelity, mannequin-based (HFMB) simulation and video clinical vignettes were used to create a simulation-based CBRNE course directed at the recognition, triage, and resuscitation of contaminated victims. The course participants, who consisted of first responders and receivers, were evaluated using a 43-question pre- and post-test that employed 12 video clinical vignettes as scenarios for the test questions. The results of the pre-test were analyzed according to the various medical training backgrounds of the participants to identify differences in baseline performance. A Scheffe posthoc test and an ANOVA were used to determine differences between the medical training backgrounds of the participants. For those participants who completed both the pre-course and post-course test, the results were compared using a paired Student's t-test.Results:A total of 54 first responders/receivers including physicians, nurses, and paramedics completed the course. Pre-course and post-course test results are listed by learner category. For all participants who took the pre-course test (n = 67), the mean value of the test scores was 53.5 ±12.7%. For all participants who took the post-course test (n = 54), the mean value of the test scores was 78.3 ±10.9%. The change in score for those who took both the pre- and post-test (n = 54) achieved statistical significance at all levels of learner.Conclusions:The results suggest that video clinical vignettes and HFMB simulation are effective methods of CBRNE training and evaluation. Future studies should be conducted to determine the educational and cost-effectiveness of the use of these modalities.

Hepatitis has become a health problem in the world. The hepatitis virus infected many people. According to the teacher of MTsN 02 Bondowoso more than 20 students have hepatitis A viral infection. The purpose of this research was to know the differences of students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study used pre-experimental (pre-post test design). This study used stratified random sampling technique, 127 students from 270 sample involved this research,and 143 students was excluded. We used questionnaires to collect data. The results showed that the mean value of the students 'knowledge about hepatitis A before counseling in MTsN 02 Bondowoso 2015 was 83.96 with the lowest value of 37.5 and the highest value was 100. The mean value of the students' knowledge about hepatitis A after counseling in MTsN 02 Bondowoso 2015 was 93.21 with the lowest value waf 62.5 and the highest value was 100. Paired t test showed that t (-9.07) > t table (1.98), the null hypothesis (H0) was rejected. There was a difference between students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study showed that routine counseling by healthcare provider was important to prevent hepatitis A infection.; Keywords: counseling, knowledge of students, hepatitis

Abstract Background and Aim: Small intestinal bacterial overgrowth (SIBO) is associated with gastrointestinal pathology and colonoscopy. This endoscopic investigation could cause changes in gut flora including the occurrence of SIBO. We looked in this study for the effect of colonoscopy (preparation and intubation) on the occurrence of SIBO. Materials and Methods: Prospective study including thirty patients with irritable bowel syndrome (IBS) diagnosed according to Rome III criteria. Two groups were designed: Twenty IBS patients that performed colonoscopy (G1) and ten IBS patients (G2) not referred to colonoscopy. All patients have been tested for the presence of SIBO using glucose hydrogen breath tests (GHBT) at the beginning of the study, on day 1. G1 patients have also been tested before colonoscopy (day 2) and 1 week after (day 9). G2 patients performed GHBT on day 1 and on day 9. Results: The peak value of expired H2 was assessed, and the mean value was calculated. There were no significant statistical differences between the mean H2 values in the 2 groups of patients on day 1. The mean level of H2 significantly decreased after preparing for colonoscopy in G1 patients (P < 0.0001). There were no significant statistical differences between the mean levels of H2 on day 2 versus day 9 in G1 patients (P = 0.176). The mean level of H2 1 week after performing colonoscopy (7.65 ppm) is higher than that obtained after preparing for it (6.3 ppm), but no statistical significance. Patients from G2 showed no statistical differences between the mean levels of H2 on day 1 versus day 9 (P = 0.6132). Patients in G1 had a significantly lower mean H2 level versus G2 patients on day 9. Conclusions: Colonoscopy does not produce SIBO. Preparing for colonoscopy influences the level of expired H2, it reduces the number of intestinal bacteria, probably trough a mechanic effect or by inflating air during the procedure. Performing GHBT too soon after colonoscopy might result in false negative results of GHBT.

Introduction: Basic first aid knowledge helps children to deal with emergency situations. Everyone needs to teach children about being mentally prepared for emergencies. Children should be taught about different first aid measures, both at home and at school, which helps emphasize the importance of child safety. This enables them overcome difficult situations like injuries, burns and outdoor emergencies. First aid is all about using common sense in the hour of need. Objectives of the study: To evaluate the effectiveness of self Instructional module by comparing the pretest and post test knowledge of school children. Method and Material: quantitative pre experimental one group pre test post test research design was used. by using Simple random sampling technique. This study was also done on 120 samples; data was collected by semi structured questionnaire to assess the knowledge regarding First aid and safety measure among Early Adolescent age (10-14 year). Results: The pre test means score was 10.58, standard deviation was 1.71 and the mean % was 52.9. The post test means score was 14.94, standard deviation was 1.39 and the mean % was 74.7. The difference in mean% was 21.8. The calculated ‘t’ value of 22.31 which showed high statistical significance at p<0.001 level. There was no significant association of post test level of knowledge with any of the demographic variables. Regarding First aid and safety measure. Conclusion: The present study assessed the knowledge of school children regarding first aid and safety measures, the school children had inadequate knowledge regarding first aid safety measures. After reading Self Instructional Module, there was a significant improvement in school children knowledge regarding first aid and safety measures.


The purpose of the research was to investigatethe effect of activity-based games on the academic achievement of graduate-level pupils in social sciences.An experimental research design with a pre-and post-test control group was used in this study. As pre-test and post-test, MCQ achievement tests containing 70 items were used as research methods for data collecting. The initial stage in this initiative was to collect data on what inspires children to learn. Both groups took a pre-test, and the results were tallied.The project's second phase was to study the effects of variousactivities on academic achievement. Both groups were given an MCQ performance test. The T-test was used to analyze the data. The findings of this study demonstrated that, compared to the control group, mostpupils'marks improved in the experimental group. The mean value showedthat experimental group participants scored 18.77on the post-test, while control group students scored 16.21. According to a post-lesson poll, most students regarded activity-based games to be more engaging than lecture-based instruction.

Objective: To evaluate the efficacy of ultrasound and calf stretching in subjects with gastrocnemius tightness in plantar fascitis to reduce pain and improve functional ability. Method: 30 patients with plantar fascitis selected from Saveetha college of physiotherapy and rehabilitation center (SPARC) based on inclusion and exclusion criteria. The patients were treated with ultrasound therapy and calf stretching. The pre and post test values of pain and functional ability was be calculated using Silfverskiold test and Foot Function Index as an outcome measure. Results: The mean value and standard errors were calculated for different variables and the difference in mean value was tested for statistical significance using paired t test. P value of <0.0001 was considered as statistically significant. Conclusion: From the statistical analysis and graphical interpretation the final derived results concluded that combined therapy of ultrasound and calf stretching is found to be effective in relieving gastrocnemius tightness in patient suffering with plantar fascitis and it can be used to improve the functional activities.

The aim of this study was to know the effectiveness of the use of alphabet cards game as interesting tool to explain vocabularies and it make it easier to remember their meanings. This study belongs to the quasi experimental research and presents pre-test, post test and to investigate the effect of using alphabet cards game on vocabulary mastery of commercial business administration students. The participants were the students from commercial business administration students at the third semester (N=54) of State Polytechnic Bengkalis and divided into two groups. Both groups were taught vocabulary, however, the experimental group students who are exposed to alphabet cards game and the controlled group students who received conventional method. Pre-test and post-test were given to both experimental group students and controlled group students. The score of the vocabulary tests were compared through independent sample t-test. The mean value of pre-test of experimental class was 61.35, while the control class was 52.23. After learning using alphabet card games in experimental class the mean value obtained in post-test was 78.85, while in control class was 59.04.  The improved of learning outcomes can be seen from the level of significance was 0.000 and it was lower than t test 0.000<0.05.  It means that the hypothesis null is not accepted. Therefore, it can be concluded that the score of the students in learning vocabulary between the experimental group and control group was significantly different. The experimental group students who learned vocabulary using alphabet cards game had increased their vocabulary mastery.

Background: The incidence of burns in Indonesia progressively increases with the increase in its population and industries. From January to September 2000, 158 patients were treated in the burn unit of Dr Soetomo Hospital with a mortality rate reaching 5,8%. Burns have a direct effect in causing both local and systemic changes in the body, not occurring in other injuries. In severe burns, a hypermetabolic state can occur, which increases cardiac workload and causes muscle atrophy and other morbidities. The purpose of this study is to examine the effect of propranolol on the hypermetabolic state in severely burned patients by measuring various clinical & laboratory parameters.Methods : This is an experimental study using pre and post test control group design with the objective of assessing the treatment outcome with oral propranolol given in 15 consecutive days for burn patients involving 25%- 60% TBSA. Measurements were taken three times, on day 0, 7 and 14.Results : Obtained 16 samples divided into 2 groups. In the treatment group, there was a significant decrease in CRP levels on days 0, 7 and 14 (p <0.05). The Mid Arm Circumference variable did not obtain a significant decreasing on days 0, 7 and 14. The albumin level studied showed a significant decreasing on day 0 & 7 days with a value of p = 0.045. From the comparison between the two groups, there were significant differences in CRP levels on days 0 & 14, with the mean value of the treatment group -5.12 + 2.88 and the mean value of the control group 2.86 + 7.37, and the value of p = 0.019.Conclusions: This study successfully proved that the administration of propranolol can overcome the effects of hypermetabolism which is characterized by decreasing in CRP levels.

This study aims to enhance students’ writing skills through Daily Journal writing. This research was divided into three steps: giving pre-test, giving treatment (writing daily journal), and giving posttest. The result showed significant difference between control and experimental group. Experimental participants was improved through the writing of daily journal, and the score gained by participants showed that there was a significant effect on students’ writing skills. The achievements showed that the mean difference of post-test scores between the two groups was -.45433 and for the standard error mean was .03840 with pValue .050 (see Table 3), means that Ha is accepted and Ho is not accepted. ABSTRACTBackground: Erythrocytes Sedimentation Rate (ESR) is a test to determine the rate of erythrocytes settling blood containing anticoagulants in a vertical tube within a certain time. ESR is generally used to check and monitor for tissue damage, inflammation and indicate disease. ESR examination can be done by manual and automatic methods. LED levels in pulmonary TB patients generally have increased.Objectives: Knowing the difference in the measurement results of the erythrocytes sedimentation rate (ESR) between the Westergen manual and automatic methods in patients with pulmonary tuberculosis.Methods: Laboratory experiments with a post-test-only approach. The number of samples was 30 people and was taken by purposive sampling. The research data were obtained from the results of the ESR examination using the Westergren manual and automatic methods. Results: Examination of the sedimentation rate of healthy respondents used the Westergren method had an average of 9,40 mm/hour while in the automatic method 10,15 mm/hour. The examination of sedimentation rate of pulmonary tuberculosis respondents with the Westergren method had an average of 66,13mm/hour, while the Automatic method was 67,80 mm/hour. There was no significant difference in the mean value of ESR between the measurement method used manual Westergren and Automatic methods in patients with pulmonary tuberculosis ( p = 0,878).   Conclusion: There is no significant difference in the mean value of ESR between measurements using the manual Westergren method and automatic in patients with pulmonary tuberculosis. Keywords: Erythrocytes Sedimentation Rate, Method, Westergren, Automatic  PERBANDINGAN NILAI LAJU ENDAP DARAH ANTARA PENGUKURAN METODE MANUAL WESTERGREN DAN ALAT AUTOMATIK PADA SAMPEL DARAH SITRAT PENDERITA TB PARU DI RSUD. Kadar LED pada penderita TB Paru umumnya mengalami peningkatan.Tujuan: Mengetahui perbedaan hasil pengukuran Laju Endap Darah (LED) antara metode manual Westergen dan Automatik pada Penderita TB Paru. Metodologi: Eksperimen laboratorik dengan pendekatan post test only. Jumlah sampel 30 orang dan diambil dengan purposive sampling. Data penelitian diperoleh dari hasil pemeriksaan LED menggunakan metode manual Westergren dan Automatik.  Hasil: Pemeriksaan Laju Endap Darah pada responden sehat metode Westergren memiliki rata-rata 9,40 mm/jam sedangkan pada metode Automatik 10,15 mm/jam. Pemeriksaan Laju Endap Darah pada responden TB Paru metode Westergren memiliki rata-rata 66,13 mm/jam sedangkan pada metode Automatik 67,80 mm/jam. Tidak terdapat perbedaan rata-rata nilai LED yang signifikan antara pengukuran menggunakan metode manual Westergren dan Automatik pada penderita TB Paru (p = 0,878). Kesimpulan: Tidak terdapat perbedaan rata-rata nilai LED yang signifikan antara pengukuran menggunakan metode manual Westergren dan Automatik pada penderita TB Paru.Kata kunci: Laju Endap Darah, Metode, Westergren, Automatik

Blockage of septic tanks is one of the problems in the community related to the discharge of toilet waste produced. The use of synthetic toilet cleaners that are not environmentally friendly is one of the factors that can inhibit the process of decomposing organic waste. The existence of microbial killer ingredients in toilet cleaning materials that come into the septic tank will actually inhibit the growth of essential microbes that play a role in the perfection of the decomposition process. Lack of understanding of the community related to microbial processes in the processing of toilet waste is one of the factors inability of the community to overcome these problems. This community service aims to disseminate information related to the processing of organic waste, the role of microbes in the processing of toilet waste, how to make microbial formulas that play a role in the processing of toilet waste, as well as efforts to increase the creativity of the community to process their own waste to improve environmental sanitation and improve community welfare. The results of the training showed that the average pretest and post test scores of the cadres of Tenggilis Surabaya village experienced a significant increase in value with a mean value of 37.78 (pretest) to 62.22 (post-test). Toilet waste processing cadres have been able to make biotoilet products that can be used to process limbat toilets in their homes. The informationand insights they receive can be passed on to the community from each cadre.AbstrakPenyumbatan septic tank merupakan salah satu permasalahan di masyarakat terkait dengan buangan limbah toilet yang dihasilkan. Penggunaan pembersih toilet yang berbahan sintetik yang tidak ramah lingkungan merupakan salah satu faktor yang dapat menghambat proses penguraian limbah organik. Keberadaan bahan pembunuh mikroba dalam bahan-bahan pembersih toilet yang ikut masuk ke dalam septik tank justru akan menghambat pertumbuhan mikroba esensial yang berperan dalam kesempurnaan proses dekomposisi. Kurangnya pemahaman masyarakat terkait dengan proses mikroba dalam pengolahan limbah toilet merupakan salah satu faktor ketidakmampuan masyarakat dalam mengatasi permasalahan tersebut. Pengabdian masyarakat ini bertujuanuntuk mensosialisasikan informasi terkait proses pengolahan limbah organik, peran mikroba dalam pengolahan limbah toilet, cara membuat formula mikroba yang berperan dalam pengolahan limbah toilet, serta upaya meningkatkan kreativitas masyarakat untuk mengolah limbahnya sendiri guna meningkatkan sanitasi lingkungan dan meningkatkan kesejahteraan masyarakat. Hasil pelatihan menunjukkan bahwa rerata nilai pretest dan post test kader kelurahan Tenggilis Surabaya mengalami peningkatan nilai yang cukup signifi kan dengan rerata nilai dari 37,78 (pretest) menjadi 62,22 (post test). Kader pengolahan limbah toilet telah mampu membuat produk biotoilet yang dapat dimanfaatkan untuk mengolah limbat toilet di rumah masing-masing. Informasi dan wawasan yang mereka terima dapat di teruskan kepada komunitas asal masing-masing kader.
