Background: Dolutegravir (DTG) plus lamivudine (2-DR) is suggested as an initial and switch option in HIV-1 treatment. Objective: To analyze the effectiveness, durability, and safety of 2-DR compared with DTG plus abacavir/lamivudine (3-DR). Methods: This was an observational, ambispective study that included all treatment-naïve (TN) and treatment-experienced (TE) patients who started 2-DR or 3-DR between July 1, 2018, and November 30, 2020. The primary end point was noninferiority, at 24 and 48 weeks, of 2-DR versus 3-DR regarding the percentage of patients with viral load (VL)≥50 and 200 copies/mL in TN (4% margin) and VL<50 and 200 copies/mL in TE (margin 12%). Durability of response, and safety were also measured. Results: 242 patients were included (53 TN and 189 TE). Two TN patients on 2-DR had VL≥50 copies/mL and 1 had VL≥200 copies/mL at week 24. In TE patients on 2-DR, 90.2% achieved VL<200 copies/mL at week 24 (difference: 3.8%; 95% CI = −6.3% to 14%) and 91.8% at week 48 (difference: 0.06%; 95% CI = −9% to 10%), meeting noninferiority criteria. Among the 53 TN patients, only 1 VF was observed in 2-DR. In TN patients, the risk of treatment discontinuation was similar between groups (hazard ratio [HR] = 0.37; P = 0.15); similar rates were also found in TE patients (HR = 0.94; P = 0.85). TE patients on 2-DR showed a better safety profile compared with 3-DR patients ( P<0.001). Conclusion and Relevance: Our results did not show noninferiority in terms of virological effectiveness. Nevertheless, all effectiveness measures support the use of 2-DR in a real-life cohort of TN and TE. Additionally, durability and safety of 2-DR were confirmed to be similar to that of 3-DR.

The aim of this study was to compare the efficacy of trabeculectomy (TE), single XEN microstent implantation (solo XEN) or combined XEN implantation and cataract surgery (combined XEN) in primary open-angle glaucoma cases, naïve to prior surgical treatment, using a monocentric retrospective comparative cohort study. Intraocular pressure (IOP) and the number of IOP-lowering drugs (Meds) were monitored during the first 24 months after surgery. Further disease progression was monitored using peripapillary retinal nerve fiber layer (RNFL) thickness examinations using spectral domain optical coherence tomography (OCT) as well as visual acuity (VA) and visual field (VF) tests. In the TE group (52 eyes), the mean IOP decreased from 24.9 ± 5.9 to 13.9 ± 4.2 mmHg (p < 0.001) and Meds decreased from 3.2 ± 1.2 to 0.5 ± 1.1 (p < 0.001). In the solo XEN (38 eyes) and the combined XEN groups, the mean IOP decreased from 24.1 ± 4.7 to 15.7 ± 3.0 mmHg (p < 0.001) and 25.4 ± 5.6 to 14.7 ± 3.2 mmHg (p < 0.001), while Meds decreased from 3.3 ± 0.8 to 0.8 ± 1.2 (p < 0.001) and 2.7 ± 1.2 to 0.4 ± 1.0 (p < 0.001), respectively. The VF and VA indices showed no sign of further deterioration, the RNFL thickness further decreased in all treatment groups after surgery. TE and XEN led to comparable reductions in IOP and Meds. Although the VA and VF indices remained unaltered, the RNFL thickness continuously decreased in all treatment groups during the 24-month follow-up.

The article analyzes the injuries in children received as a result of traffic accidents, depending on the injury mechanism. For those children, who were in the car, predominantly local injuries of the MSS were noted with a smaller number of associated injuries. For those children, who were hit by a car, severe polyfocal injuries of MSS with damage to internal organs were noted. The principles of providing qualified and specialized emergency medicine to the children with combined injuries are described, that improve the efficiency and adequacy of all treatment-diagnostic measures and their qualified implementation.

In recent years, cerebral vascular diseases have been increasingly detected in young patients. It is due not only to better physicians’ knowledge about this pathology, but also to the improvement of its diagnosis methods. Modern neuroimaging techniques allow us to clarify the nature of hemorrhage, to determine the volume and location of intracerebral hematoma, and to establish the degree of concomitant edema and dislocation of the brain. However, despite the high accuracy of the research, it is not always possible to establish the cause that led to a brain accident, which greatly affects the tactics of management and outcomes in this category of patients. A special feature of the structure of cerebrovascular diseases of young people is the high proportion of hemorrhagic stroke, the causes of which are most often arterio-venous malformations. Meanwhile, there are a number of other causes that can lead to hemorrhage into the brain substance. These include disorders of blood clotting, and various vasculitis, and exposure to toxic substances and drugs, and tumor formations (primary and secondary). All these pathological factors outline the range of diagnostic search in young patients who underwent hemorrhagic stroke. Diagnosis of these pathological conditions with the help of modern visualization techniques is considered to be easy, but this is not always the case. In this article, the authors give their own clinical observation of a hemorrhagic stroke in a young patient, which demonstrates the complexity of the diagnostic search in patients with this pathology.

Background: Evidence about the use of dolutegravir (DTG) and rilpivirine (RPV) as an antiretroviral therapy (ART) in treatment-experienced patients is scarce. Objective: To explore the effectiveness, safety, and costs of switching to a DTG plus RPV regimen in this population. Methods: This observational, prospective study included all treatment-experienced patients who switched to DTG plus RPV between November 2014 and July 2016. Patients were excluded if resistance mutations to integrase inhibitors or RPV were found. The effectiveness endpoint was the proportion of patients who achieved virological suppression (viral load [VL] <50 copies/mL) at week 48 (W48). Safety (incidence of adverse events leading to discontinuation and laboratory abnormalities), adherence, and costs were analyzed. Results: A total of 35 patients were included, and 91.4% were virologically suppressed at baseline. Patients were treated with ART for a median of 14 years (interquartile range = 7-20). At W48, 91.4% of patients were virologically suppressed (95% CI = 77.0-98.2). Two of the 3 patients not suppressed at baseline achieved undetectable VL at W48, and 2 patients discontinued DTG plus RPV (intolerance and a drug-drug interaction). None of the virologically suppressed patients at baseline showed virological rebound through W48. There were no significant changes in lipid, liver, and renal profiles. The proportion of patients with an ART adherence >90% increased from 65.6% to 93.8% ( P = 0.004). The annual per-patient ART costs dropped by €665 ( P = 0.265). Conclusions: Switching to DTG plus RPV seems to be an effective and safe strategy. Significant improvements in patients’ adherence and costs were achieved.

Sodium/hydrogen exchange (NHE) inhibitors show promise as potential therapeutic agents for the treatment of heart failure, but it is not known whether they can reverse the maladaptive remodeling that results in heart failure. We sought to determine the effect of the NHE-1-specific inhibitor EMD-87580 (EMD) on heart failure produced by myocardial infarction in the rat and to assess whether up to 4 wk of treatment delay results in beneficial effects. Male Sprague-Dawley rats were subjected to coronary artery ligation (or a sham procedure) and followed for up to 3 mo, at which time hypertrophy and hemodynamics were determined. EMD was provided in the diet, and treatment commenced immediately or 2–4 wk after ligation. EMD significantly reduced hemodynamic abnormalities, including the elevation in left ventricular end-diastolic pressure, and diminished the loss of systolic function with all treatment protocols. Left ventricular dilatation and hypertrophy, as assessed by heart weight, cell size, and atrial natriuretic peptide (ANP) expression, were similarly reversed to sham or near-sham levels. In addition, the increased plasma ANP and pro-ANP values were reversed to levels not significantly different from sham. Surprisingly, virtually all beneficial effects were identical with all treatment protocols. These effects were observed in the absence of infarct size reduction or blood pressure-lowering effects. Our results suggest that NHE-1 inhibition attenuates and reverses postinfarction remodeling and heart failure with a treatment delay of up to 4 wk after infarction. The effect is independent of infarct size or afterload reduction, indicating a direct effect on the myocardium.

The exogenous lipoic acid (LA) is successfully used as a drug in the treatment of many diseases. It is assumed that after administration, LA is transported to the intracellular compartments and reduced to dihydrolipoic acid (DHLA) which is catalyzed by NAD(P)H-dependent enzymes. The purpose of this study was to investigate whether LA can attenuate cardiovascular disturbances induced by ethanol (EtOH) and disulfiram (DSF) administration separately or jointly in rats. For this purpose, we measured systolic and diastolic blood pressure, recorded electrocardiogram (ECG), and estimated mortality of rats. We also studied the activity of aldehyde dehydrogenase (ALDH) in the rat liver. It was shown for the first time that LA partially attenuated the cardiac arrhythmia (extrasystoles and atrioventricular blocks) induced by EtOH and reduced the EtOH-induced mortality of animals, which suggests that LA may have a potential for use in cardiac disturbance in conditions of acute EtOH intoxication. The administration of EtOH, LA, and DSF separately or jointly affected the ALDH activity in the rat liver since a significant decrease in the activity of the enzyme was observed in all treatment groups. The results indicating that LA is an inhibitor of ALDH activity are very surprising.

SUMMARYExperiments, conducted over 3 years, investigated the effect of all combinations of two storage periods at low temperature (0 or 2 °C) and two storage periods at 10 °C on the sprout growth, numbers of stems per tuber, foliage development and subsequent tuber yield of the two maincrop varieties Maris Piper and Désirée.Although the number of day-degrees accumulated during sprouting was the same for all treatment combinations, there were large effects of treatments on the number of sproutlets per tuber and the total sprout length per tuber. However, there was no effect on the number of stems per tuber, foliage development or saleable ware yield inany year.

Micro-algae are to be an attractive way to produce bio-diesel due to high photosynthetic yields and lipid accumulation in cells. This high productivity combined with possibility to uptake CO2 stimulated its utilization as a biological mitigation method of CO2, at once as an alternative renewable source of energy. Growth characteristics and chemical composition of micro-algae can be altered by culture environment. Nutrient sufficiency,included magnesium element (Mg2+) is important factors on overall biochemical composition. In study, Nannochloropsis sp was cultivated in Erlenmeyer 250 ml containing 200 ml f/2 medium. There are three groups of treatment with different levelof magnesium (Mg2+), i.e. 0 (M0); 0.1mgL-1 (M1); and 1.0 mgL-1 (M2). All treatment was designed triplicate in batch system. Culture was then aerated continuously with sterile atmospheric air (1.5 L.min-1). Cells were harvested on 25th day after inoculation and analyzed. Data showed that Chlorophyll-a increased linearly with time and maximum at 18th days of growth period, i.e. 23.57; 26.44; and 27.74mgL-1, for M0; M1; and M2,respectively. Chlorophyll-a content decreased significantly when pH dropped to 5-6.Enrichment with Mg2+ increased the chlorophyll-a content 12.2-17.7%. Dry cell reached 375-400mgL-1 in all treatment. Lipid content of Nannochloropsis sp in control (M0) is 55.3%, higher than M1 and M2. Saturated fatty acid tends to increase from 80.70 (M0)to 96.70 (M1) and 94.53% (M2). Fatty acid of M0 and M1 was composed dominantly by palmitic acid (C16:0), i.e. 49.19-70.75% total fatty acids. Meanwhile, M2 treatment was dominantly by lauric acid (C12:0), i.e. 32.98%.Keywords: CO2 biological mitigation, chlorophyll-a, fatty acid, lipid, agnesium,microalgae, Nannochloropsis sp, photosynthesis.

EFFECT OF REPLACEMENT OF CORN WITH SORGUM Cv. KAWALI IN PELLET RANSUM ON LOCAL RABBIT PERFORM. The purpose of this study was to determine the effect of substitution corn with sorghum in rabbit rations on feed consumption, body weight gain, and ration conversion. The duration of the study was 28 days using 20 local rabbits. The statistical design used in this study was a completely randomized design, with 4 (four) treatment rations and 5 (five) replications. The four experimental rations, namely R0: 45% corn + 0% sorghum, R1: 30% corn + 15% sorghum, R2 15% corn + 30% sorghum, R3 0% corn + 45% sorghum. Variables were measured, viz: feed consumption, body weight gain and ration conversion. The results showed that the average of feed consumption, body weight gain, and feed conversion, were: 28.27–30.59 g /head /day, 15.76–21.55 g / head /day, and 1.42-1.80 g /head /day, respectively. Based on the analysis of variance, the treatments ration had no significant effect (P> 0.05) on consumption, but had a very significant effect (P <0.01) on body weight gain and feed conversion. The LSD test showed that all treatment rations using sorghum (R1, R2, and R3) gave results of body weight gain which were very significantly different (P <0.01) and higher than R0. Whereas, on ration conversion, treatments R1, R2, and R3 give very different results (P <0.01) lower than R0. It is concluded that the substitution of corn with sorghum at the level of 45% in the ration gives the highest results for body weight gain and conversion of rations for rabbits. Keywords: corn, sorghum, ration, performance, local rabbit
