Objective We aimed to evaluate the relationship between hyperglycosylated human chorionic gonadotropin (hCG-H) and placenta accreta spectrum (PAS) in the second and third trimesters of pregnancy. Study Design This was a case–control study of PAS and controls. hCG-H was measured in the second and third trimesters of pregnancy in women with pathologically confirmed cases of PAS and in gestational age-matched controls without PAS. We compared serum hCG-H levels in cases and controls, calculated summary statistics for diagnostic accuracy, and used receiver operating characteristic (ROC) curves to define an optimal cut-point for diagnosis of PAS using hCG-H. Results Thirty case samples and 30 control samples were evaluated for hCG-H. Mean hCG-H was lower in the case compared with control group (7.8 ± 5.9 μg/L vs. 11.8 ± 8.8 μg/L, p = 0.03). At an optimal cut-point for hCG-H of ≤7.6 μg/L, the sensitivity, specificity, positive likelihood ratios, negative likelihood ratios, and area under the ROC curve were 66.7%, 69.7%, 2.20%, 0.48%, and 0.68%, respectively. Conclusion Hyperglycosylated hCG levels in the second and third trimesters of pregnancy were lower in patients with PAS than in controls, but hCG-H showed only modest capability as a diagnostic test for PAS.

Abstract Background Placenta accreta spectrum is a serious obstetric disorder that is characterized by deeply penetrating villi which are abnormally attached to the myometrium of the uterus. This prevents its complete dissociation during the third stage of labor that may cause severe bleeding, with its marked risk on the mother life. All recent studies confirm that the occurrence of placenta accreta increased dramatically in the recent years with the increasing number of caesarean sections. The number of previous caesarean deliveries accompanied by placenta previa markedly increase the risk for placenta accreta. There is a continued need to study risk factors of abnormally adherent placenta to improve the antenatal diagnosis . Objective The current study aims to evaluate the effect of the short duration between the perior caesarean delivery and the current pregnancy on the occurence of placenta accreta spectrum. Patients and Methods a case control study which was performed in the Maternity Hospital of Ain Shams University. It compare the duration between the perior caesarean section and the current pregnancy in the placenta accreta cases and the matched controls to determine if it increases the occurrences of placenta accreta or not. It include all pregnant ladies with placenta accreta spectrum diagnosed radiologically or surgically in the period of the beginning of June 2019 to the beginning of December 2019 with in the inclusion criteria, also the matched controls. Results there was no difference with a statistical significance between control group and patients group regarding inter pregnancy interval. Conclusion this study reveals that there was no effect of the short duration between the previous caesarean delivery and the current pregnancy on the occurence of placenta accreta spectrum.

The incidences of pneumonia in infants Become the second highest cause mortality before reaching the age of 5 years and a health problem in Indonesia. The family of smoking and smoke from burning mosquito coils are the triggers for air pollution, which can lead to pneumonia in infants. The study aims to analyze the relationship between family smoking habits and the use of mosquito coils with the incidence of pneumonia in under-fives. The method of research applied analytic observational research using a case-control study approach. The number of samples in this study was 87 under-fives in the case group and 87 under-fives in the control group. Sampling was conducted by non-probability sampling. A questionnaire was used as a data collecting instrument. Data analysis used chi-square and multiple logistic regression. The results of this study Showed that there was a relationship between family smoking habits and the use of mosquito coils with the incidence of pneumonia in infants. In addition, there were other variables that had a relationship with the incidence of pneumonia in under-fives namely smoking location (OR = 3,046; 95% CI: 1429-6492, p = 0.003), the number of cigarettes (OR = 7.105 ; 95% CI: 3079-16394, p = 0.000), and length of time with smokers (p = 0.000). On the other hand, the variables of father smoker status and the frequency of using mosquito coils were not related to the incidence of pneumonia in under-fives (p value 0.05). It is concluded that family smoking habits, use of mosquito coils, the location of smoking, and number of cigarettes, and  length of time together with smokers were risk factors associated with the incidence of pneumonia in toddlers

Abstract Context.—Forensic pathologists regularly investigate the deaths of individuals with a history of drug abuse. Autopsy, including toxicology testing, reveals no cause for death in a subset of this cohort. Objective.—To determine whether deaths with an undetermined cause and manner of death are associated with a history of drug abuse. Design.—Retrospective matched case-control study of 52 decedents whose cause of death remained undetermined following autopsy, matched 1:2 to a control group of living patients admitted for cholecystectomy according to age and date of death or procedure. Results.—Individuals whose cause of death was undetermined were 5.3 times (95% confidence interval, 1.9– 14.5) more likely to have a history of drug abuse than were patients with cholecystitis. Conclusions.—Decedents with a history of chronic drug abuse appear to be at an increased risk of dying by their chronic drug abuse, even in the absence of any anatomical or toxicologic finding at autopsy to account for death.

ABSTRACT Chronic neurological disorders generate disabilities affecting multiple aspects of life, including sexuality. Objective To describe the presence of sexual dysfunction and comorbidities in a population with chronic neurological disorders. To analyze the relationship between disability and sexual dysfunction. Methods A cross-sectional case-control study was carried out. Patients with amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), Parkinson’s disease (PD), and stroke of at least one year since the onset of symptoms were included, and compared with controls with no neurological disease, matched by age and sex. Results We included 71 participants: 29 controls, with a mean age of 49.4 years, and 42 patients with a mean age of 53.8 years. Sexual dysfunction was present in 22.5% of the controls and 77.5% of the patients. A statistically significant relationship between sexual dysfunction and disability was found in the logistic regression analysis (OR = 20.38, 95%CI: 2.5 –165.86). Conclusions Disability proved to be the main variable related to the presence of sexual dysfunction. Patients with ALS had the worst rates of sexual dysfunction. Patients with MS were similar to the control group. As for the PD group, no patient had normal sexuality. Finally, in stroke patients, the presence of comorbidities and their treatment may have negatively influenced sexuality. These findings showed that patients with chronic neurological diseases have sexual dysfunction and underscore the need for neurologists to know and address this problem.

The aim of this study was to investigate the relationship between the combined effect of MTHFR C677T (rs1801133) and EPHX2 G860A (rs751141) polymorphism and ischemic stroke in Chinese T2DM patients. This case-control study included a total of 626 Chinese T2DM patients (236 T2DM patients with ischemic stroke and 390 T2DM patients without ischemic stroke). The rs1801133 and rs751141 were genotyped using real-time polymerase chain reaction. Statistical analysis was performed with SPSS 17.0. Results showed that the combined effect of MTHFR TT and EPHX2 GG or GA + AA genotype has a higher risk of ischemic stroke compared with the control group (combined effect of MTHFR CC and EPHX2 GA + AA genotypes; OR = 3.46 and OR = 3.42, resp.; P=.001 and P=.002, resp.). The A allele showed marked association with a lower risk of ischemic stroke in patients with the lowest Hcy levels under additive, recessive, and dominant genetic models (OR = 0.45, OR = 0.11, and OR = 0.44, resp.; P=.002, P=.035, and P=.008, resp.), which was not observed in medium or high Hcy level groups. In conclusion, the T allele of rs1801133 and the G allele of rs751141 may be risk factors of ischemic stroke in the Chinese T2DM population.

BACKGROUND: The incidence rate of hypertension is increasing in Indonesia concerning unhealthy behaviours such as unhealthy physical activity and eating pattern which trigger obesity. In Indonesia, the prevalence of hypertension in > 18-year-old people was 34.1% in 2018. AIM: The objective of the research was to find out the relationship between physical activity and obesity with the incidence of hypertension in adults (26-45 years old) in Medan. METHODS: The research was done in Medan, using a case-control study design. The samples were 150 hypertension patients, taken by using proportional allocation: 75 of them were in the case group and the other 75 of them were in the control group. The data were gathered by conducting interviews, measurement, and questionnaires and analysed by using simple logistic regression test. RESULTS: The result of the research showed that there was significant relationship of physical activity (p = 0.000; OR = 3.6; 95% CI, 1.802-7.270) and obesity (p = 0.000; OR = 4; 95% CI, 2.030-7.900) with the incidence of hypertension in 26-45-year-old respondents. CONCLUSION: Make a habit of a healthy lifestyle in their daily life by doing physical activity regularly and good eating pattern to forestall hypertension.

Though it is long known that irregular intake of antihypertensive may cause harm, but still there is much lack of awareness among the patients in this matter. This case controlled study was done in Dhaka Medical College Hospital, Dhaka, to determine the relationship between discontinuation of antihypertensive drugs and the risk of stroke among hypertensive patients. Number of cases was 72. Control group was selected via random sampling of stroke free treated hypertensive group, similar in age to stroke cases. Sampling technique was random systematic sampling. The relationship was determined employing statistical methods including Chi-square (?2) test using computer programs, and significant association between discontinuation of antihypertensive and development of stroke in hypertensive adult patients was found ?2(1,N= 103) = 32.637, p<0.001. e12005 Background: Neoplastic Meningitis (NM) is a complication of many common cancers that occurs with an incidence as high as 15% in some cancer types. Diagnosis remains elusive with poor screening tools and insensitive diagnostic options. Methods: A retrospective analysis of 341 consecutive patients with suspected NM was undertaken. Clinical presentation, demographic, cytologic and MRI data were recorded. Using cytology as the diagnostic gold standard, sensitivity, specificity, ROC curves and likelihood ratios were calculated. Results: 210 patients met inclusion criteria, 87 with primary brain tumors (PBT), 72 with non-CNS solid tumors, and 44 with lymphomas. 111 patients were male; 123 (58.6%) ultimately had a positive cytology. Only 59% of patients with positive cytology presented with symptoms concerning for NM. 32.9% of patients with PBTs and positive CSF cytology exhibited neurologic symptoms suggestive of NM. Patients with non-CNS solid tumors and positive CSF cytology presented with symptoms concerning for NM 80% of the time. Patients with cytology-positive, MRI-positive and symptom-positive NM had very different survivals. Potentially informative CSF values are in table below. Conclusions: A definitive diagnosis for patients with suspected NM remains elusive without a positive cytology, but some clinical, radiographic, and CSF findings are useful for raising diagnostic concern, and for screening in at-risk populations. Symptom-positive or MRI-positive NM may represent different diseases than cytology-positive NM. [Table: see text]

Abstract Purpose: To explore the relationship between thoracolumbar kyphosis (TLK), body mass index (BMI) and the content of lumbar posterior muscles in patients with degenerative thoracolumbar kyphosis (DTLK) combined with lumbar stenosis syndrome (LSS).Methods: 126 patients with DTLK and LSS (DTLK group) and 87 patients with simple LSS (control group) were retrospectively included with well-matched demographics. TLK and lumbar lordosis (LL) were obtained on the X-ray of the whole spine. Lumbar crossing indentation value (LCIV) was introduced to evaluate the content of the lumbar muscles, with was measured from T12-L1 to L4-L5 at T2-MRI axial imaging. Three subgroups of normal weight, overweight and obesity were respectively divided into in both groups according to BMI. Three subgroups of increased LL, normal LL normal and decreased LL were also divided into in DTLK group.Results: The mean LCIV (mLCIV) of the DTLK group was less than control group and LCIV showed an increasing trend in DTLK group from T12-L1 to L4-L5. Different from control group, there were no significances in gender and BMI distribution of mLCIV in DTLK group (P>0.05). LCIV in increased LL subgroup was larger than that of normal LL and less LL subgroup (P<0.01). There were no relationship between TLK and BMI in both groups. BMI was positively correlated with mLCIV in control group(P=0.004). TLK and LCIV were negatively correlated (P<0.001) in DTLK group with LCIV=13.75-0.48×TLK.Conclusion: LCIV in DTLK group was less than control group with no gender- and BMI-difference. LCIV and TLK were mutually predictable in DTLK with LCIV=13.75-0.48×TLK.
