Abstract Background In March 2019, the WHO published the “The Global Influenza Strategy for 2019-2030”, with the goal of strengthening seasonal prevention and control and preparedness for future pandemics. Accordingly, to the 2B, 2C, 3B goals, this study tries to assess the burden of the disease among the healthcare workers (HCWs). Our study aimed to estimate the impact of severe intensity season on absenteeism among HCWs in a large Italian hospital and if the vaccination rates would be associated with reduced sickness absence. Methods We investigated excess of absenteeism that occurred in a large Italian hospital, counting 5300 HCWs, during the flu season of 2017-2018. Data on influenza vaccination were gathered from the Occupational Health Unit, while absenteeism data from the hospital’s Personal Unit Database. The data collection ranged from July 2017 to June 2018, it was divided into two periods: epidemic and non-epidemic, the last one used as a baseline to calculate excess of absenteeism. We also gathered other variables in order to stratify the absenteeism among HCWs. Results The total lost days were 53.775. The average number of lost days per week during the epidemic period was 1263. Compared to the non-epidemic period, absenteeism increased by 65% (from 4.05 to 6.68 days/person). Vaccinated HCWs had a lower excess of absenteeism in comparison to non-vaccinated HCWs (1,74 vs 2,71 days/person). Nurses and allied health professionals registered the highest excess of absenteeism (3.16 days/person) while medical doctors showed the lowest (0.76 days/person). Conclusions A severe influenza epidemic impacts on the excess of absenteeism among HCWs. Although the low vaccination rate (6,8%), a positive effect on absenteeism is outlined and it may support health care facilities to promote and facilitate flu vaccination among HCWs. As literature states low absenteeism rate, as seen in medical doctors, could be an indicator of presenteeism that might lead to harm the inpatients. Key messages The flu vaccination has a positive effect on reducing the absenteeism rate in the HCWs during the epidemic period and should be promoted and advocated among european healthcare facilities. A low absenteeism rate may also be an indicator of presenteeism among HCWs, which may also be harmful to the inpatients.

Abstract Objectives: Iran is exposed to a wide range of natural and man-made hazards. Health-care facilities can play a significant role in providing life-saving measures in the minutes and hours immediately following the impact or exposure. The aim of this study was to determine the preparedness of health-care facilities in disasters and emergencies. Methods: This cross-sectional study was conducted in Damghan, Semnan Province, in 2019. The samples consisted of all the 11 health-care facilities located in Damghan County. A developed checklist was used to collect the data, including 272 questions in 4 sections: understanding threatening hazards, functional, structural, and nonstructural vulnerability of health-care facilities. The data were analyzed using SPSS 21. Results: The results revealed that the health-care facilities were exposed to 22 different natural and man-made hazards throughout the county. The total level of preparedness of the health-care centers under assessment was 45.8%. The average functional, structural, and nonstructural vulnerability was assessed at 49.3%, 31.6%, and 56.4%, respectively. Conclusions: Conducting mitigation measures is necessary for promoting the functional and structural preparedness. Disaster educational programs and exercises are recommended among the health staff in health-care facilities.

Background: Radio Frequency Identification (RFId) technology is rapidly integrating into hospital practices to track assets, identify patients and manage personnel; it is important to assess its impact on privacy, safety and quality of health services. Objective: This paper describes the methodological approach adopted and the tools used by the authors to evaluate the quality and effectiveness of applications and systems using RFId technology in health care facilities to solve problems due to lack of traceability. Methods: To analyse the actual and/or potential effects of this technology, the method adopted is taken from the multidisciplinary approach of Health Technology Assessment (HTA), with particular reference to the assessment of quality of services provided, of safety and of protection of personal and sensitive data. Tools developed during the research include survey, verbal questioning and interviews involving hospital staff members (Management, Technical, Clinical&Service Implementation), and audit in Italian healthcare facilities. Results: We identified the current applications and technical characteristics of RFId technology and we calculated priority levels to determine impact health indicators. Results were used for evaluate the diffusion, the applications and the characteristics of RFId technology and also for calculate its impact on healthcare processes. Conclusions: Using the tools described in this paper, health managers can evaluate the impact of the RFId-based applications in their healthcare structure.

Abstract Aims To describe the impact of the COVID-19 pandemic on the well-being of surgeons and allied health professionals across the world and assess the support provided by their institutions. Methods An online survey was distributed through medical organisations, social media platforms and collaborators from the 15th of July to the 15th of December 2020. Results 1590 responses were received. 80.0% were surgeons, 6.4% nurses, 5.3% assistants, 3.1% anaesthetists, 1.4% operating department practitioners and 3.8% classed as other. Of participants, 64.0% had found difficulty gaining access to personal protective equipment (PPE), 29.8% had not received training on its use and 32.0% had become physically ill since the start of the pandemic (59.9% of which were due to COVID-19 symptoms). 29.0% saw a decrease in salary and 35.2% a decrease in time spent with family. Between a time two weeks before the start of the pandemic and after, there was a significant increase in mean scores for depression (4.22 (CI = 3.98-4.46)) and anxiety (2.24 (CI = 2.01-2.46)), based on the Patient Health Questionnaire (PHQ-9) and Generalised Anxiety Disorder Assessment (GAD-7) respectively (p < 0.0001). In terms of support, only 36.0% had easy access to occupational health, 58.5% were able to take regular breaks, 16.5% had access to 24 hour rest facilities and 14.2% to 24 hour food and drink facilities. Conclusions This work has highlighted a need and ways in which to improve conditions for the health workforce. This will inevitably have a positive effect on the care received by patients.

ABSTRACTObjectivesThe objective of this study was to measure the impact of the Covid-19 pandemic on acceptance of flu vaccination in the 2020-21 season, including for those newly eligible for the UK National Health Service (NHS) free vaccination programme, extended this year to include an estimated 32.4 million (48.8%) of the UK population. Knowing intended uptake is essential to inform supply and steer public health messaging to maximise vaccination given the combined threats of both flu and Covid-19 — the unknown impact of which on both attitudes and the need for mass uptake yet again create the threat of ill-informed planning resulting in failure to meet necessary public health demand.MethodsAn online questionnaire posing question items on influenza vaccination was administered to registrants of the Care Information Exchange (CIE), the NHS’s largest patient electronic personal health record. This was part of a longitudinal study initiated during the Covid-19 pandemic lockdown. This analysis was limited to those who, in line with established NHS criteria, were previously or newly eligible but had not routinely received seasonal influenza vaccination in the past. Groups were stratified by response (yes/no) to intending to receive flu vaccination in 2020-21: Group 1.) Previously eligible now responding ‘yes’, 2.) Previously eligible still responding ‘no’, 3.) Newly eligible responding ‘yes’, and 4.) Newly eligible responding ‘no’. Within these groups, response by health worker status and each group’s inclination to vaccinate school age children was also measured. Summary statistics were reported alongside univariate and multivariable regression. Lastly, a network analysis visualised the frequency and co-occurrence of reasons qualifying response for or against influenza vaccination in 2020/21.FindingsAmong 6,641 respondents, 4,040 (61.1%) had previously routinely received the flu vaccination. 1,624 (24.5%) had been either previously eligible but not vaccinated (945, 58.2%) or newly eligible (679, 41.8%). Among the previously eligible participants who had not routinely received influenza vaccination 536 (56.7%) responded they would in 2020-21, increasing the vaccination rate in the entire previously eligible cohort from 79.6% to 91.2%, and 466 (68.6%) in the newly eligible.Multivariable logistic regression resulted in few substantial changes to effect estimates, with the exception of age, for which all estimates showed a stronger association with intention to receive the flu vaccine. In those who became newly eligible to receive the flu vaccine, there was an association between intention to receive the flu vaccine and increased age (OR = 1.07, 95% CI 1.03 to 1.12), IMD quintile, and considering oneself at high risk from Covid-19 (OR = 1.80, 95% CI 1.22 to 2.70).Network analysis showed the most frequent themes for previously eligible unvaccinated and newly eligible participants accepting vaccination in 2020/21 were: ‘precaution for myself’ (41.2% and 46.1%) and ‘Covid-19’ (27.4% and 27.1%), where the former was qualified by the latter in 36% and 29.1% of responses. Among the previously and newly eligible not intending to receive vaccination in 2020/21, misinformed themes of ‘makes me unwell’, ‘gives me flu’ and ‘vaccine doesn’t work’ were present across 37.4% and 21.9% of responses, respectively.Among participants with school age children, of those previously eligible who now intend to be vaccinated themselves, 82.5% also intend to vaccinate their children in 2020/21 compared to 25.8% of those who would not accept the influenza vaccine for themselves. Among the newly eligible respondents this was 82.1% and 43.5%, respectively. 49.9% of the previously unvaccinated healthcare workers would continue to decline the vaccine in 2020/21.InterpretationIn this UK-wide observational study, Covid-19 has increased acceptance of flu vaccination in 2020/21 from 79.6% to 91.2% in those previously eligible, and for the 69% of those newly eligible. This high anticipated vaccination rate (to 26 million (80%) of the UK population) requires appropriate planning, but can be further increased with effective messaging campaigns to address negative misconceptions about flu vaccination, which may also help in preparation for future Covid-19 vaccination. It remains of concern that 50% of healthcare professionals who refused it previously still do not intend to have the flu vaccine.

Objectives: 1) To explore the possible impact of the pandemic on the health seeking behavior of the patients, 2) To explore the relation of socio-demographics on the utility of health-care facilities. Methods: This cross-sectional study was conducted by enrolling all patients ≥15 years of age presenting to the Out-Patient-Department of three main public-hospitals after obtaining ethical committee approval. A questionnaire with validated Urdu translation was filled by each participant that included socio-demographic data, pre-Covid and Covid-19 era health seeking behaviors and the impact of the pandemic on the utilization of healthcare facilities. Data was analyzed using SPSS V.19. Results: A total of 393 patients were enrolled with a male preponderance (72%) and a median age range of 31-45 years. Fifty-eight percent of the study population was unemployed and 47.3% were seeking follow up care. The frequency of ER and multiple (>4 times) OPD visits were significantly decreased in the Covid-19 times whereas, the laboratory and radiology services were largely unaffected. A significant number of patients were not satisfied with the current healthcare facilities that was seen irrespective of the socio-demographic status. Emergency Room and radiology services were largely unaffected whereas, elective procedures and laboratory facilities were reported to be severely affected or delayed in relation to socio-demographic variables. Conclusions: Healthcare inequalities have widened and depression has shown a sharp rise during this pandemic. Digitalization is a megatrend, but there is relatively little knowledge about its consequences for service work in general and specifically in knowledge-intensive business services (KIBS). We studied the impact of digitalization on psychological consequences for employees in tax consultancies as a special case of KIBS. We compare two tax consulting jobs with very different job demands, those of tax consultants (TCs) and assistant tax consultants (ATCs). The results show that the extent of digitalization at the workplace level for ATCs correlates significantly positively with their job satisfaction. For TCs, the same variable correlates positively with their work engagement. These positive effects of digitalization are mediated in the case of ATCs by the impact on important job characteristics. In the case of TCs, which already have very good working conditions, the impact is mediated by the positive effect on self-efficacy. Theoretical and practical consequences of these results are discussed.

The effects of mechanical perforation densities by extracting soil cores through an aerator Vertidrain with a working width of 1.6 m and equipped with hollow tines spaced of 65 mm, were studied on a sandy soil of a grassy sward in the Golf Course El Kantaoui in Sousse (Tunisia). The mechanical aeration was performed at two densities: 250 and 350 holes/m2. The cone penetration resistance and soil water infiltration were measured. These parameters were performed at initial state before aeration (E0) and then on the 10th, 20th and 30th day after aeration. These results showed that perforation density of 350 holes/m2 had a positive effect on the soil by reducing its cone resistance to penetration compared to the initial state (Rp = 14.8 daN/cm2). At 5 cm depth the decrease in resistance to penetration was 34% and 43% on the 10th and 20th day after aeration, respectively. However, on the 30th day after aeration the soil resistance to penetration tended to grow and its value compared to the initial state decreased only by 21 and 26%, respectively, at 5 and 15 cm of depth only by 10% and 9% with 250 holes/m2 density. The soil water infiltration made a good improvement after aeration compared to the initial state. This parameter increased from 4.8 cm/h to 8.3, 10.9 and 13.1 cm/h with 250 holes/m2 density and to 10, 12.9 and 14.8 cm/h with 350 holes/m2 density on the 10th, 20th and 30th day following the aeration.


One of efficiency indicators of grain grinders is grain granulometric composition. The basis of mixed fodder is crushed grain, the particles of which must have a leveled granulometric composition for subsequent mixing and obtaining a high-quality feed mixture. In agricultural production, hammer crushers are widely used, in which the destruction of grain occurs due to the impact of a hinged hammer. The main disadvantage of these crushers is that not the entire surface of the hammers is involved in grinding, thus reduces grinding process efficiency. A slightly different principle of material destruction is laid down in the basis of the proposed design of the shock-centrifugal grinder. Main work is performed by flat impact elements located on the rotor, which serve to accelerate crushed particles with subsequent impact of them on the bump elements. An important step in the design of new constructions of shock-centrifugal grinders is to determine size and location of the impact elements on the rotor, without which the grinding process is not possible. In the calculation method presented, the dependencies for determining the velocities and angles of a single particle flight from the surface of a flat impact element for its specified dimensions are proposed. Two variants of an impact element location on the rotor are considered and analyzed: radial and at an angle in the direction of rotor rotation. As a result of research carried out, it is noted that in the case of inclined position of an impact element on the rotor an increase in flight speed and flight angles change in crushed particles, which gives the opportunity to have a positive effect on grinding process.

BACKGROUND It is necessary to cope with situations in daily life to prevent stress-related health consequences. However, coping strategies might differ in their impact on dealing with stressful situations in daily life. Moreover, the effect of coping strategies on situational coping might differ between women and men. OBJECTIVE The aim of this study was to investigate the impact of coping strategies on situational coping in everyday life situations and to investigate gender differences. METHODS An ecological momentary assessment study with the mobile health app TrackYourStress (TYS) was conducted with 113 participants. Coping strategies were measured at baseline with the coping scales Positive Thinking, Active Stress Coping, Social Support, Support in Faith, and Alcohol and Cigarette Consumption of the Stress and Coping Inventory (SCI). Situational coping was assessed by the question “How well can you cope with your momentary stress-level” (slider 0-100) in daily life over four weeks. Multilevel models were conducted to test the effects of the coping strategies on situational coping. Additionally, gender differences were evaluated. RESULTS Positive Thinking (P=.03) and Active Stress Coping (P=.04) had significant positive impacts on situational coping in the total sample. For women, only Social Support had a significant positive effect on situational coping (P=.046). For men, only Active Stress Coping had a significant positive effect on situational coping (P=.001). Women had higher scores on the SCI scale Social Support than men (P=.007). CONCLUSIONS These results suggest that different coping strategies could be more effective in daily life for women than for men, which should be considered in the development of interventions aimed at reducing stress consequences through coping. Interventions taking gender into consideration might lead to better coping-outcomes than generalized interventions.

The impact of World War II on cost accountancy in the U.S. may be viewed as a double-edged sword. Its most positive effect was engendering greater cost awareness, particularly among companies that served as military contractors and, thus, had to make full representation to contracting agencies for reimbursement. On the negative side, the dislocations of war, especially shortages in the factors of production and capacity constraints, meant that such “scientific management” techniques as existed (standard costing, time-study, specific detailing of task routines) fell by the wayside. This paper utilizes the archive of the Sperry Corporation, a leading governmental contractor, to chart the firm's accounting during World War II. It is concluded that any techniques that had developed from Taylorite principles were suspended, while methods similar to contemporary performance management, such as subcontracting, emphasis on the design phase of products, and substantial expenditure on research and development, flourished.
