The objective of the present study was to evaluate bone and cardiac abnormalities and symptoms and signs of thyroid hormone excess in women with subclinical hyperthyroidism (SCH) aged < 65 years. Forty-eight women with SCH were evaluated. The control group consisted of 48 euthyroid volunteers. The mean symptom rating scale score was significantly higher in patients. Cardiac involvement, both morphological and affecting systolic and diastolic functions, was also observed in patients. Women with SCH showed a significant increase in serum markers of bone formation and resorption. In addition, bone mineral density (BMD) was lower in the femoral neck but not in the lumbar spine in patients before menopause, whereas a lower BMD was observed at both sites in postmenopausal patients. SCH is not completely asymptomatic in women aged < 65 years, and is associated with heart abnormalities and with increased bone turnover and reduced BMD even before menopause.

Young women often feel primary dysmenorrhoea because the hormonal cycles experienced are not stable, this can disrupt the concentration and activity of young women. The principle of back to nature is increasingly popular today, the side effects of chemical drugs can cause new problems, it is one of the driving force of the development of traditional medicine. Papaya leaves contain Vitamin E which can reduce dysmenorrhea. In addition, turmeric acids also contain curcumine and anthocyanins that inhibit cyclooxygenase, thereby reducing the occurrence of inflammation during uterine contractions. The aim of this research is to know the effectiveness of papaya leaf stew with acidic turmeric to primary dysmenorrhea. Type of Research is an experimental study with a Pretest-Posttest Control Group Design design. The sample is 30 people. The research was conducted on December 27, 2017 s.d February 24, 2018 at SMP Negeri 46 Palembang. Instrument to measure pain Numeric Rating Scale. The statistical test used by Wilcoxon and Mann Whitney U. Average score of pain before papaya leaves stem 5.40 ± 0.73 while the mean score of pain after given turmeric acid 5.33 ± 0.61 The mean score of pain after being given papaya leaves stew 3.60 ± 0.91 while the mean score of pain after given turmeric acid 4.06 ± 0.79. There was a difference of mean score of dysmenorrhea pain before and after given papaya leaf stem p value = 0.000. There is difference of mean score of dysmenorrhea pain before and after given turmeric acid p value = 0,002. There was no difference of mean score of dysmenorrhea pain before and after given papaya leaf sting and turmeric acid p value = 0,217. The decoction of papaya leaf and turmeric acid have the same effectiveness in reducing primary dysmenorrhea pain.

<p>Merapi eruption in 2010 in Yogyakarta caused people losing their homes and triggering traumatic effect. Elderly is one of victims that vulnerable to psychological problem. The common psychological late respons due to disaster is insomnia. The impacts of insomnia are decrease the level of immunity, increase mortality and decrease quality of life, thus it needs to be solved. One of the solution is by listening murotal Al-Qur’an that will give relaxation effect. The study was aimed to know the effectiveness of listening murotal Al-Qur’an to the changing of elderly insomnia level in shelter Dongkelsari, Sleman, Yogyakarta.The design of study was a quasy experimental pre post test with control group. Technique of taking sample was probability sampling and it was found that there were 37 elderly who live in shelter Dongkelsari Sleman Yogyakarta that fulfilled the inclusion criteria. Research instrument which was used to measure insomnia level is KSPBJ questionnaire rating scale and to listen for murotal Al-Qur’an (Ar-Rahmaan) was used laptop and speaker. The duration of listening murotal Al-Qur’an was 8 days with 12 minutes for each day. Data analysis was used paired samples t test and independent samples t test. The level of insomnia for control group pre test and post test is in moderate insomnia by the mean ± SD each of them is 9.3 ± 3.7 and 10.3 ± 3.4 (p&gt;0.05) and for intervention group there was a changing from moderate insomnia to mild insomnia by the mean ± SD each of them is 10.9 ± 3.6 and 8.2 ± 4.2 (p&lt;0.05). There was a significant differences between the intervention group and control group by the mean ± SD each of them is -2.8 ± 3.0 and 0.9 ± 3.5 (p&lt;0.05). In conclusion, listening murotal Al-Qur’an was effective to reduce elderly insomnia level in shelter Dongkelsari Sleman Yogyakarta.</p>

Depressive disorders are a great burden for individual patients and society. Blood-based biomarkers are regarded as a feasible option for investigation of depressive disorders. Several potential biomarkers for depression were selected. We studied the following serum markers: cortisol, melatonin, brain-derived neurotrophic factor (BDNF), prolactin, insulin-like growth factor 1 (IGF-1), β-endorphin, orexin A. The patient sample consisted of 78 persons with depressive disorders. Patients were divided into two groups: 46 patients with a first depressive episode and 32 patients with recurrent depressive disorder. Control group consisted of 71 healthy individuals of corresponding age and sex. All markers were measured in serum using MILLIPLEX® MAP panels (Merck, Darmstadt, Germany) by analyzer MAGPIX (Luminex, USA). Statistical analyses were performed using SPSS software. Results were expressed as median and quartile intervals [Q1–Q3]. There was a significant increase of serum concentrations of cortisol (663.69 [467.5–959.49] nmol/L, Р < 0.001) and melatonin (66.31 [33.6–132.59] pg/mL, P = 0.029) in patients compared with the control group (526.1 [367,24–654,7] nmol/L and 45.11 [27.47–73.47] pg/mL). In addition, correlations were found between potential biomarkers, clinical indicators and treatment response measured by applying the Hamilton Depression rating scale and the Clinical Global Impression rating scales. A significant correlation was found between the concentration of prolactin and high response to pharmacotherapy (r = –0.267, P = 0.029). Identifying biomarkers that can be used as diagnostics or predictors of treatment response in people with depressive disorders will be an important step towards being able to provide personalized treatment.Disclosure of interestThe work is supported by the project of Russian Foundation of Basic Research No 14-04-01157a.

Context Osteoporosis is a silent disease until it is complicated by trivial fall fractures. There is an increasing interest within the orthopaedic community in the noninvasive cost-effective measurement of the bone mineral density. Aims The aim of the study is to assess whether urinary N-telopeptide level can be a new diagnostic tool in diagnosing osteoporosis. Methods and Material This prospective study was done at Sri Ramachandra Medical Centre (SRMC) hospital from October 2015 to October 2017. The study was conducted among patients who comes to SRMC as inpatient or outpatient with suspected osteoporosis and underwent dual-energy X-ray absorptiometry (DXA) scan and urinary N-telopeptide. The inclusion criteria were women aged 65 or older, women aged less than 65 with risk factors, younger postmenopausal women with one or more risk factors, men aged 70 or older, men less than 70 with risk factors, and any above group patients who comes within 24 hours following trivial fall fractures. The exclusion criteria were pathological fracture, history of any illness affecting bone metabolism. The results from DXA scan were taken as gold standard against urinary N-telopeptide. Then the patients were divided into two groups control and study. The control group contains patients who had normal DXA, while study group contains patients having either osteopenia or osteoporosis. Based on our inclusion and exclusion criteria, 110 persons were included in the study. We had 60 study and 50 controls patients. We had 88 females and 22 males. The results obtained were statistically analyzed. Statistical Analysis Used The collected data were analyzed with IBM SPSS statistics software 23.0 version. To describe about the data descriptive statistics frequency analysis, percentage analysis was used for categorical variables and the mean and standard deviation were used for continuous variables. To find the significant difference between the bivariate samples in independent groups, the unpaired sample t-test was used. To find the significance in categorical data, chi-square test was used. In both the earlier statistical tools, the probability value of 0.05 is considered as significant level. Results In our study, we had 18.2% osteopenic and 36.4% osteoporotic patients. The mean value of urinary N-telopeptide in control was 49.8 and in case was 182.5. The standard deviation of urinary N-telopeptide value in case was 159.9. Conclusion Urinary N-telopeptide can give reproducible results and be able to assist in the evaluation of the quantity as well as the quality and be a good judge of someone's risk of fracture. Hence, urinary N-telopeptide can be used as a diagnostic tool for diagnosing osteoporosis.

Playing The Interpersonal Game (IG) should result in (a) increased congruence between a player's self-concept and his self-ideal, between his ideal concept of the other player and the other player's self-ideal, and between his concept of the other player and the other player's self-concept, (b) lowered scores on the MMPI scales of Ai, D, Hy, Sc, Hs, and Pa, and an increased score on the MMPI Es scale, and (c) an improved relationship with the other player. Two groups were formed at random, an experimental group which played the IG consisting of 15 boy-girl pairs and a control group of 16 boy-girl pairs which did not play the game. Pre- and posttests, using a Self and Others Rating Scale, the MMPI, and an Interpersonal Relationship Rating Scale were administered to all Ss. Appropriate t tests comparing the mean change scores for the game group with the controls failed to confirm any of the hypotheses.


The aim of the study is to explore the effect of random excitement strategy on developing English conversation skills among ninth graders. The descriptive approach was used. The sample included (81) ninth graders and was chosen randomly from Al-Aysheya Basic Elementary School for Girls. To collect data, conversational skills rating scale and an oral conversation test were used. The results showed a significant difference between the mean scores of the experimental group which was taught through the random excitement and control group was taught through the traditional way in the posttest of the oral side. The results also proved that there is a significant difference between the mean scores in the experimental group (the random excitement) and the control group in the posttest of the performance aspect. This difference is due to the effect of using (random excitement). The study recommended creating managing materials to furnish educators with the required information to utilize dynamic learning techniques, and building up instructors' capacities in picking the perfect conversation exercise procedure to meet diverse learning styles. Moreover, the study recommended concentrating on all conversation skills such as speaking rate, volume, and accuracy in teaching conversation.

Background: Anxiety in stroke patients occurs as a normal reaction to stress with life changes; however, when it becomes excessive, It becomes disabling. Effort to deal with anxiety is needed and spiritual approach nursing care is considered useful in caring patients with stroke.Objective: To examine the effect of spiritual nursing care on anxiety in stroke patients in the inpatient ward.Methods: This study used a quasy experimental design with pretest-postest control group. Thirty respondents were selected using consecutive sampling, which 15 respondents assigned in the experiment and control group. The Hamilton Anxiety Rating Scale was used to measure anxiety. Data were analyzed using paired t-test and independent t-test.Results: The results showed that the mean level of anxiety in the experiment group before intervention was 29.33 and decreased to 9 after intervention, while in the control group the mean level of anxiety before intervention was 29.47 and decreased to 17.73 after intervention. Paired t-test obtained p-value 0.000 (<0.05), which indicated that there was a significant effect of spiritual nursing care on anxiety levels in patients with stroke.Conclusion: Spiritual nursing care could reduce anxiety in patients with stroke.

Abstract Context Bone mineral density is normal in acromegalic patients and the cause of increased fracture risk that characterizes active acromegaly is unknown. Objective This study compared serum sclerostin levels between patients with active acromegaly and healthy individuals. Design, Setting, and Participants The serum sclerostin levels of patients with active acromegaly were compared with those of healthy volunteers in a cross-sectional study. The mean age of the 30 acromegaly patients (male/female: 14/16) was 47.26 ± 12.52 years (range, 18–64 years) and that of the healthy volunteers (male/female: 17/13) was 44.56 ± 10.74 years (range, 19–62 years). IGF-1 and GH levels were measured using an electrochemiluminescence method, and serum sclerostin levels using an ELISA. The Mann-Whitney U test was used to compare sclerostin levels between the 2 groups. The correlations of sclerostin level with IGF-1 and GH were determined using Spearman’s test. Results The 2 groups did not differ in age or sex (P &gt; 0.05). The median GH and IGF-1 levels in the patient group were 2.49 ng/mL (range, 0.22–70.00 ng/mL) (interquartile range [IQR], 1.3–4.52) and 338.5 ng/mL (range, 147–911 ng/mL) (IQR, 250–426), respectively. The median GH and IGF-1 levels in the control group were 0.95 ng/mL (range, 0.3-2.3) and 144 ng/mL (range, 98–198), respectively. The median sclerostin level was 29.95 ng/mL (range, 7.5–78.1 ng/mL) (IQR, 14.37–37.47) in the acromegaly group and 22.44 ng/mL (range, 8.45–36.44 ng/mL) (IQR, 13.71–27.52) in the control group (P &lt; 0.05). There was a moderate positive correlation between the sclerostin and IGF-1 levels (rho = 0.54; P &lt; 0.01), and between the sclerostin and GH levels (rho = 0.41; P &lt; 0.05). Conclusions High sclerostin levels may contribute to the increased fracture risk seen in patients with acromegaly.

Background: Escitalopram is one of the most commonly used SSRIs at present, which has the characteristics of quick onset, less interactions with other drugs, and relative safety. Objective: This study aims to investigate the effects of escitalopram on neural functional prognoses and endothelial dysfunction after acute ischemic stroke. Methods: One hundred eligible patients afflicted with acute ischemic stroke were randomized into two groups: control and treatment groups. Patients in the treatment group received escitalopram in addition to the basic therapies in the control group over a period of 90 days. Neurological deficits were quantified using the National Institutes of Health Stroke Scale (NIHSS) score and Barthel index (BI) score, cognitive impairment was determined using the Mini-Mental State Examination (MMSE) score, depressive symptoms were measured using the 17-item Hamilton Depression Rating Scale (HAMD). Furthermore, post-stroke depression (PSD) was defined based on the Diagnostic and Statistical Manual of Mental Disorders (Fourth Edition), with a HAMD score ≥17. Flow-mediated vascular dilatation (FMD) of the brachial artery was use as a surrogate indicator for endothelial dysfunction assessment with ultrasound. Results: The mean NIHSS and HAMD scores on day 90 after treatment were significantly lower in the treatment group than in the control group (2.17±0.36 vs. 4.24±0.85; 5.81±1.35 vs. 10.43±4.91; P < 0.01), while the mean BI score and FMD were significantly higher in the treatment group (93.08±6.23 vs. 79.64±7.56, P < 0.01; 8.71±2.35 vs. 5.83±1.21, P < 0.05) than in the control group. The improvement in MMSE score was not significantly different between the two groups. Conclusions: Treatment with escitalopram early after ischemic stroke can improve neural functional prognoses and endothelial dysfunction. Escitalopram had less side effects, which is worthy of clinical prophylactic application.

This study aimed to investigate any inflammatory effect of nicotine on rat embryo by exposing their mothers to different dosages of nicotine during pregnancy. During this experimental study, 32 pregnant healthy Wistar rats were divided into 4 equal groups, including a control and 3 nicotine exposure groups. Injections were performed subcutaneously starting at the first day of pregnancy until parturition. As the dosages of nicotine were increased, the weight gain by pregnant rats and the mean weight of their newborns were significantly reduced. Mean ± SD of hs-CRP was significantly higher among groups exposed to various dosages of nicotine (2, 4, and 6 mg/kg) compared to the control group (P<0.0001) and its increasing rate was also dose dependent. Mean ± SD serum level of IL-6 and TNF-αamong all groups exposed to nicotine, except for 2 mg/kg nicotine injected group, was increased significantly (P<0.0001). Mean ± SD of serum level of TGF-βand nitrite oxide among exposure groups showed significant differences compared to the control group only at the dosage of 6 mg/kg (P<0.0001). The current study showed that exposing pregnant rats to nicotine causes a dose dependent increase in the rate of all the studied inflammatory serum markers among their newborns.
