Introduction: Due to the side effects of overuse of antibiotics, the aim of study was to evaluate the optimal antibiotic use in children with gastroenteritis. Methods: This descriptive-analytical study was conducted on 178 children with gastroenteritis from January to December 2018. Data including age, gender, clinical symptoms and laboratory findings were extracted from records. Results: Out of 178 cases of diarrhea, 32 cases were due to bacteria and 146 cases were due to viruses. The most clinical symptoms in the patients were fever (34.40 %), vomiting (33.2 %) and coryza (13.1%). The most frequency of viral (46.2%) and bacterial diarrhea (32.2%) were seen in autumn season. Antibiotics were used mostly in spring (39 %) and autumn (25 %) seasons. In 14% of cases, the appropriate treatment was not selected. In addition, 50.56 % of treated children were less than 1 years old. No significant relation was also seen between season with bacterial or viral cases (p>0.05). Significant relation was seen between taking antibiotics at admission and clinical signs such as abdominal pain and suspicious food (p<0.01). Data were analyzed throgh SPSS, version16. Chi Square test was used for analysis of data Conclusion: Virus is the main cause of diarrhea in children with gastroenteritis. Moreover, the most frequency of diarrhea was seen in autumn and spring season. However, there was no significant relation between seasons with bacterial and viral cases. It indicates that bacterial and viral diarrhea was not affected by season. For preventing microbial resistance, antibiotic therapy should be administered more carefully in children with diarrhea.

Background and Aim: Horses have a strategic and vital role to play in the lives of the people of Sumba Island, East Nusa Tenggara Province. They act as social animals that are involved in death ceremonies, horse races, and during pasola, thereby supporting tourism, and are given away as dowry in wedding ceremonies. This study aimed to investigate the prevalence of trypanosomiasis among horses in four districts of Sumba Island by examining clinical symptoms and detecting parasites, antibodies, and other factors that are related to Trypanosoma evansi infection in horses. Materials and Methods: We studied a total of 211 horses that belonged to 88 clinical hobby breeders. Giemsa-colored smears and serum were examined in order to detect antibodies using card-agglutination tests (CATT). The study was conducted during the rainy season that lasted from January to March 2017. Potential risk factors such as the species, sex, origin of the livestock, how the livestock were maintained, and the farmers' knowledge concerning trypanosomiasis were recorded using questionnaires. Data were collected annually for three years from 2010-2012 and repeatedly analyzed by a Chi-square test. Results: Clinical signs of trypanosomiasis were found in 34 horses; blood smears were examined using Giemsa staining and negative preparations were obtained at a frequency of 0.0% (0/211). The CATT results generally showed that 13.3% (28/211) of the samples were seropositive for antibodies to T. evansi; the highest percentage, 16.67% (8/48), of seropositivity was found in the West Sumba District, and the lowest, 12.0% (5/50), was found in Southwest Sumba. The incidence of trypanosomiasis was higher (75% [21/28]) among female hip horses; horses with 1-5 years of experience were more susceptible to a T. evansi infection (46.4% [13/28]). In general, farmers on Sumba Island knew of trypanosomiasis (89.8% [79/88]), and 69.3% (61/88) of the farmers reported that their livestock was sick. This study was the first serological study conducted on trypanosomiasis in horses of Sumba Island after the surra outbreak in 2010-2012. There were 3% of farmers who were willing to provide the government with information on implementing a prevention program and controlling the spread of surra on the island. Conclusion: The diagnoses of surra disease were made based on clinical symptoms and parasitological examinations. CATTs could be used to diagnose T. evansi infection in horses.

INTRODUCTION: One of the important current problems in HIV/AIDS infection is the establishment of epidemiological and laboratorial prognostic parameters during patient follow-up. This study aimed at analyzing the evolution of laboratory tests: CD4 lymphocyte count, viral load, hemoglobin (Hb), aspartate aminotransferase (AST), alanine aminotransferase (ALT), and the epidemiological variables sex and age as prognostic factors for survival in progression to death among AIDS patients. METHODS: A retrospective study was conducted using analysis of medical records, and prospective 24-month follow-up of patients with HIV/ AIDS attended at the President Vargas Hospital Outpatient Clinic, a reference center in HIV/ AIDS attendance in the State of Maranhão, Brazil. The study analyzed patients aged 10 to 60 years old, who manifested AIDS and who were not using antiretroviral therapy or had used it for less than 5 years. The Chi-square test was used for statistical analysis. RESULTS: The sample included 100 patients - 57 were current outpatients, and 43 had died. The variables viral load (p=0.726), ALT (p=0.314), sex (p=0.687), and age (p=0.742) were analyzed, and no evidence of association between them and worst prognosis was observed. CONCLUSIONS: A significant relation was verified between low Hb levels (p=0.000) and CD4 (p=0.000) and shorter survival.

Background: Umbilical cord stump is a budding point for bacterial colonization subsequently leading to sepsis that contributes to high neonatal morbidity and mortality, if not properly managed. Antiseptic care can significantly reduce omphalitis and ultimately improve newborn survival. Objective of this study was to see the efficacy of 4% chlorhexidine use to prevent umbilical cord infection in neonates.Subjects and methods: It was a comparative analytical study conducted in Neonatal unit, Sir Ganga Ram Hospital (SGRH) Lahore from July, 2016 till January, 2017. One hundred neonates were enrolled and randomized into two equal group by simple random method (50 each). In one group, nothing was applied to cord while in chlorhexidine group, 4% chlorhexidine gel was applied on umbilicus and around it, once daily for 7 days or till cord detached whichever came early. First application was done by a nurse followed by duly trained mother/caregiver. The signs of omphalitis (redness, pus or localized oedema) were observed and recorded for each neonate in both groups. Chi square test was used to see the difference in omphalitis in these groups with p˂ 0.05 considered as statistically significant result.Results: Out of 100 neonates, 29 (58%) and 23 (46%) males while 21 (42%) and 27 (54%) females neonates belonged to dry care and chlorhexidine group respectively. Nineteen (38%) neonates with dry cord had omphalitis compared to only 5 (10%) in chlorhexidine group (p 0.001). Neonates with chlorhexidine application showed prolonged mean cord separation time (7.9±1.5 days) compared to dry care (6.1±1.8 days). Conclusion: The use of 4% Chlorhexidine was effective to lower omphalitis compared to neonates with dry cord care.

Research: Pre-eclampsia is a disease with signs of hypertension, edema, and proteinuria arising from pregnancy. This disease generally occurs in the third trimester of pregnancy, but can occur earlier, for example in hydatidiform mole (Wiknjosastro, 2005: 282). Preeclampsia and Eclampsia are one of the main causes of maternal mortality in Indonesia. The incidence of preeclampsia ranges from 5-15% of all pregnancies in the world. Based on the 2003 Indonesian Demographic and Health Survey (IDHS), the incidence of preeclampsia in Indonesia is estimated at 3.4–8.5%. The research method used analytical observational method through a case control approach, which is an analytical study concerning how risk factors are studied using a retrospective approach. In other words, the effect is identified now, then the risk factor is identified as having existed or occurred in the past. Research Results The results of the significance test using the Chi-Square test showed a significant relationship to the occurrence of low birth weight (LBW) in mothers with preeclampsia. This is indicated by the value of p = 0.000 which is smaller than α = 0.05, so Ho is rejected. While the results of the calculation of the Odds Ratio show that preeclampsia is one of the causes of LBW, this is indicated by the OR value of 3.028. Conclusion: There is a relationship between preeclampsia and low birth weight in dr. Soedarso Pontianak in 2018 with a probability value (ρ) = 0.000.

Abstract Background Non-prescription antibiotic use at community is a main driver of antimicrobial resistance. Cough is a common condition and prevalent in all communities, including China. This study aims to investigate the non-prescription antibiotic use for cough in China and explore to which extent antibiotic use knowledge was correctly instructed in communities.Methods A probability-proportionate-to-size (PPS) sampling method was adopted to survey from all 14 communities in Yiwu city, China. All participants were investigated by face-to-face interview on Portable Android Devices (PADs). The continuous variables were presented by mean and standard deviation (SD) or medium and inter-quartile range (IQR). The categorical variables were presented using percentage or constituent ratio. Chi-square test for univariate analysis and logistic regression for multivariate analysis were conducted to assess the odds ratios (ORs) and 95% confidence intervals (CIs), respectively.Results A total of 3034 respondents across the 14 communities and the 50 natural villages/streets completed all key items of the questionnaire. Of 2400 (79.10%) respondents stated that they experienced cough in the past 12 months with the medium age of 36.5 (IQR: 26-49) and 12.21% (293/2400) respondents had the non-prescription antibiotic use behavior. Among those 293 respondents, the proportion of non-prescription antibiotic use for cough peaked at around 16% among people aged 30-39 years old. The major sources of antibiotics were pharmacy (77.70%) and/or family storage (43.92%). As for antibiotic knowledge in 3034 participants, 61.8% participants had minimal knowledge on broad-spectrum antibiotic and 53.76% were not familiar about the effects of joint use.Conclusions Non-prescription antibiotics use for cough is prevalent in the community, especially among people in their thirties. Strengthened drug purchase regulation and well-trained professional pharmacists would be promising alternatives to ameliorate AMR. Moreover, penetrating antibiotics knowledge to common citizens and is an urgent task to alleviate antimicrobial resistance. Therefore, proactive policies and regulations should be made to improve current situations.

Objective: To determine relationships of body mass index with plantar fasciitis and foot functions index and how many activities of daily living were limited with plantar heel pain due to overweight in population of Government Hospitals of Faisalabad City. Study Design: Cross-sectional study. Setting: Government Hospitals of Faisalabad City (Allied Hospital & District Headquarter Hospital). Period: March to May 2018. Material & Methods: 140 patients included both males and females. Convenient sampling technique was used in selection of study sample. Calculate BMI (kg/m2) and FFI questionnaire used in this study to find how many activities of daily living was limited with plantar heel pain due to overweight. Data was analyzed by using statistical package for social sciences (SPSS) version 20. Chi square test was applied. Results: There were 70 males and 70 females. Data was collected to overweight population (mean age, 42.15; mean BMI, 29.52 kg/m2; and mean height, 1.68m). Out of 140 overweight population was (66.4%) and obese population (33.6%). The windlass test showed (87.9%) positive and (12.1%) negative. Chi-square test revealed no significant relation between rise in BMI and plantar heel pain (p = 0.105). However it shows significant relation between rise in BMI and FFI (p = 0.000). Conclusion: There is no statistical significant relation between body mass index and plantar fasciitis. According to foot function index patients significantly limit activities of daily living with plantar heel pain due to overweight.

 Background: Acne beyond 25 years of age is frequently associated with hormonal derangement in women. Hormonal association provides the impetus for hormonal therapy as well as underpins the need for blood investigations in this population. Hence, we aim to estimate the presence of hormonal derangement and lipid alteration in female adult acne.Methods: A prospective, observational study was conducted in Dhulikhel Hospital from July 2015 to February 2016. Females older than 25 years with acne were taken in the study after informed consent. Total 100 patients were enrolled aftersample sizeestimation. Hormonal paneland lipid profile were measured. Hormones tested were androgens, C-peptide and thyroid stimulating hormone. Data analysis was done with SPSS-23. Bivariate analysis was done by chi-square test for categorical data.Results: In this study, majority of patients were younger than 30 years (70.5%) and perioral area most commonly involved. Hormonal alteration was seen in 37.2% patients, among which 17.9% had hyperandrogenism, 15.4% had abnormal thyroid level and 10.3% had high C-peptides respectively. Lipid profile was altered in 15.4% patients. Hormonal alteration had significant association with irregular menstruation (P<0.05) but not acne severity.Conclusions: We observed hormonal alteration frequently in females with adult acne, which comprised of various hormonal parameters including hyperandrogenism. Hormonal alteration reflects deranged metabolic milieu and we suggestthat wide hormonal panel should be done in female adult acne. Relationship of hormones with menstrual irregularity but not with acne severity, suggest that clinical symptoms should lead hormonal investigations in all grades of acne.

Background: Diarrhea is an illness which characterized by changed in shape, consistency of the stool and with excessive bowel frequency (more than 3 times within a day). Diarrhea disease is the third contributor of children’s morbidity and mortality rate around the world. Purpose: This study aims to analyzing factors that caused diarrhea’s cases. This research were an observational analytic type. Sampling technique were used random sampling / probability sampling. Methods: Data were analyzed by chi-square test. Data collection techniques were divided into primary data (direct observation and questionnaire) and secondary data (journals and articles). Results: On this research showed theres no significant correlation between sex, age, education, income, hand washing behavior using soap, clean water facility and trashbin condition with diarrhea occurrence during last 3 months in RW.VI Kelurahan Rangkah Buntu, Surabaya. There were a significant relation between cleaning up the environment, making and consuming oralit, and the behavior of handwashing with soap before meals at the rate of diarrhea cases during the last 3 months in RW VI Kelurahan Rangkah Buntu, Surabaya. Conclusion: theres no significant correlation between social demography, behavior using soap, clean water facility and trasbin and a significant relation between clean up the environtment, behavior handwashing, making and consuming oralit during the last 3 month in RW VI Kelurahan Rangkah Buntu, Surabaya.

Abstract: Sudden death could occur in anyone regardless of gender either male or female. This study was aimed to determine the relationship between gender and the incidence of sudden death at Prof. Dr. R. D. Kandou Hospital Manado. This was a descriptive analytical study with across sectional design using total sampling method. The results showed 345 cases of sudden deaths consisted of 209 males and 136 females. The age ranges were 0-1 years (7 cases), 1-18 years (7 cases), 18-40 years (38 cases), 40-60 years (138 cases), and >60 years (155 cases). Coronary heart disease was the leading cause of sudden deaths (40 cases). Data analyzed by using Chi-square test showed a P value of 0.838. Conclusion: There was no relationship between gender and the incidence of sudden death. Sudden deaths were most common among males, age >60 years, with coronary heart disease as the most common cause of sudden death.Keywords: gender, sudden death Abstrak: Kematian mendadak dapat terjadi pada siapa saja, tidak memandang jenis kelamin baik laki-laki maupun perempuan. Penelitian ini bertujuan untuk mengetahui hubungan antara jenis kelamin dan kejadian kematian mendadak di RSUP Prof. Dr. R. D Kandou Manado. Jenis penelitian ialah deskriptif analitik dengan desain potong lintang menggunakan metode total sampling. Hasil penelitian mendapatkan 345 kasus kematian mendadak terdiri dari 209 laki-laki dan 136 perempuan. Rentang usia 0-1 tahun berjumlah 7 kasus, usia 1-18 tahun berjumlah 7 kasus, usia 18-40 tahun berjumlah 38 kasus, usia 40-60 tahun berjumlah 138 kasus, dan usia >60 tahun berjumlah 155 kasus. Penyakit jantung koroner merupakan penyebab kematian mendadak terbanyak dengan jumlah 40 kasus. Analisis data menggunakan uji chi-kuadrat mendapatkan P=0,838. Simpulan: Tidak terdapat hubungan antara jenis kelamin dengan kejadian kematian mendadak. Sebagian besar kematian mendadak ditemukan pada laki-laki, kelompok usia >60 tahun, dengan penyebab penyakit jantung koroner.Kata kunci: jenis kelamin, kematian mendadak

AbstrakHipertensi merupakan masalah kesehatan yang banyak dijumpai pada masyarakat dan berkorelasi dengan penyakit lainnya. Banyak faktor penyebab terjadinya hipertensi, salah satunya adalah gangguan profil lipid. Profil lipid dapat memicu terjadinya hipertensi melalui berbagai mekanisme, baik secara langsung atau tidak langsung. Penelitian ini bertujuan untuk meneliti lebih lanjut tentang hubungan obesitas dengan kejadian hipertensi. Penelitian dilakukan pada masyarakat Etnik Minangkabau di 8 kelurahan di kota Padang. Ini adalah studi komparatif menggunakan desain cross sectional study, dengan jumlah subjek 160 responden. Pengumpulan data responden dilakukan dengan wawancara. Pengukuran tekanan darah dilakukan berdasarkan cara yang direkomendasikan WHO dan penetapan nilainya berdasarkan JNC VII. Profil lipid diukur dilaboratorium dengan acuan penilaian berdasarkan NCEP ATP III. Analisis statistik yang digunakan adalah uji chi square. Hasil penelitian ini menunjukkan bahwa sebagian besar gangguan kadar kolesterol terdapat pada penderita hipertensi dari pada normotensi. Uji statistik chi square menunjukkan hubungan yang bermakna antara kadar kolesterol dan trigliserida dengan kejadian hipertensi (total kolesterol p < 0,05; OR = 2,40, trigliserida p< 0,05; OR = 2,49). Kadar HDL dan LDL tidak menunjukkan hubungan yang bermakna terhadap kejadian hipertensi (p > 0,05). Dari hasil tersebut dapat disimpulkan bahwa sebagian fraksi profil lipid mempengaruhi kejadian hipertensi pada etnik Minangkabau yang berada di kota Padang, yaitu kadar kolesterol total dan trigliserida.Kata kunci: Hipertensi, kolestertol total, HDL, LDL, Trigliserida, etnik Minangkabau.AbstractHypertension is a common disease in the community and have correlation with other diseases. Many factor can leads hypertension, such as disturbance of lipid profile. Lipid profile can lead hypertension through a variety of mechanism, either directly or indirectly. This study aims to investigate the lipid profile relation with the incident of hypertension. The research was conducted at the Minangkabau ethnic communities in 8 district in Padang city. This was s a comparative study using a cross sectional study in 160 respondents. Data was collected by interview to get respondent characteristics. Measurement of blood preasure waist and carried out by the WHO recommended and appointment value based on JNC VII. Profile lipid was measured in laboratory and appointment value based on NCEP ATP III. The statistical analysis used chi square test. The result found that the disturbance of total cholesterol and trigliceride more existed in hypertension than normotension population. Chi square statistical test showed there was relation between total cholesterol and triglycerides with hypertension incident (total cholesterol p < 0,05; OR = 2,40 , triglycerides p < 0,05 ; OR = 2,49 ). HDL and LDL had not significant relation with hypertension incident (p > 0.05). The conclusion of this study is some of profil lipid effected hypertension incident at Minangkabau Ethnic in Padang, especially total cholesterol and triglycerides.Keywords: Hypertension, total cholesterol, HDL, LDL, triglycerides, Minangkabau Ethnic
