ABSTRACT Introduction Propofol is the drug of choice for induction of anesthesia because of its rapid onset, easy titration, and short duration of action. However, pain on propofol injection is an unpleasant experience felt by many of our patients. Many factors have been investigated to decrease the pain on propofol injection but it is still a mystery. Aim To compare the effect of two doses of ephedrine (7.5 and 15 mg) and lignocaine on pain during injection of propofol and to compare their hemodynamic parameters and note for any complications. Materials and methods This study was a prospective, randomized, double-blinded, single-center study on 150 adult patients of both the sexes belonging to American Society of Anesthesiologists (ASA) grades I and II for elective surgery under general anesthesia. The aim was to compare the analgesic effect of lignocaine, ephedrine 7.5 and 15 mg in ameliorating propofol injection pain. About 50 patients were allocated to each of the three groups receiving lignocaine 2% (1.5 mL), ephedrine 7.5 mg, and ephedrine 15 mg intravenously admixed with propofol. The intensity of pain at the time of propofol injection was assessed using verbal rating scale (VRS) 4-point scale before the patient lost consciousness. Statistical analysis Statistical analysis was done by using Statistical Package for the Social Sciences (SPSS) version 22.0 software. The qualitative data were summarized as frequency and percentages. The Fischer exact test was used to analyze the data. The continuous data were summarized as mean and standard deviation. The group and intergroup analyses were done using repeated measures analysis of variance (ANOVA) test. A p-value <0.05 was considered significant. Results Median (interquartile range) of the pain score was 1 (2) in group E-7.5 mg, 0 (1) in group E-15 mg, and 0(1) in group L. Results were statistically significant. Conclusion Adding 15 mg ephedrine was as effective as adding lignocaine for the prevention of propofol injection pain and ephedrine had a better and stable hemodynamic profile. BACKGROUND Topical anaesthesia is fundamental in enhancing pain control during the process of dental injections. The study compared the effect of cooled and uncooled topical anaesthetic gel before infiltration anaesthesia in assessing pain perception in paediatric patients undergoing dental treatment procedures. METHODS This is a split-mouth study where 16 children aged between 8 and 10 years who required bilateral local anaesthesia administration for various dental procedures were selected. Before infiltration anaesthesia, topical anaesthetic gel cooled to 40 C was applied on one side in the first visit followed by application of uncooled gel on the contralateral side in the subsequent visit. The patients were asked to individually rate their pain experience on each side using the Wong-Baker Faces Pain Rating Scale (WB-FPRS). Anxiety was assessed using pulse oximeter and FLACC scale (face, legs, activity, cry, and consolability). Data was analysed statistically using the paired ‘t’-test and a P value less than or equal to 0.05 was considered as statistically significant. RESULTS Patients reported less pain on Wong-Baker Faces Pain Rating Scale by using cooled topical anaesthetic gel. Discomfort and anxiety levels were also seen to be low when assessed using pulse oximeter and FLACC Scale for the same group (P < 0.05). CONCLUSIONS Cooling the soft tissue site with anaesthetic gel helped reduce pain perception during infiltration in children and was seen to be a more feasible technique. Hence this technique can be used as a successful adjunct to the local anaesthesia administration prior to dental procedures. KEY WORDS Anaesthesia, Cooling, Topical Gel, Injection, Pain

Background: There is evidence regarding beneficial use of Transcutaneous Electrical Nerve Stimulation (TENS) on post Lower Segment Caesarean Section (LSCS) incision pain. However, efficacy of different types of TENS following C section pain has not yet been explored adequately. Methods: 96 women who had recently undergone LSCS were included for the study. The subjects were in the age group of 20 to 40 years (25.84±3.96); having pain intensity 4 or more on Numerical Pain Rating Scale (NPRS). They were divided into three groups by random allocation method; namely Group A: Acupuncture (Low/Motor) TENS, Group B: Conventional (High/Sensory) TENS and Group C: Control group. Group A and B received specific type of TENS twice a day for 15 minutes. Control group C did not receive any TENS intervention. All subjects received standard post-operative medications and physiotherapy. Pain intensity was recorded on NPRS pre and post intervention. Results: Both Acupuncture TENS and Conventional TENS significantly decreased post-operative pain intensity as compared to control group (p value <0.0001).Conclusions: Both, acupuncture and conventional TENS are equally effective in reducing post LSCS incision pain at a strong and non-painful intensity.

Introduction: Technical skills training is fundamental for clinical practice although poorly emphasised in undergraduate medical curricula. In these circumstances, Peer Assisted Learning methodology has emerged as a valid alternative to overcome this insufficiency. The purpose of this study is to evaluate the impact on students of a Peer Assisted Learning program in basic surgical skills, regarding technical competences and knowledge improvement.Material and Methods: A total of 104 randomly selected third year medical students participated in a workshop delivered by fifth year students. From that total, 34 students were assessed before and after the workshop, using the Objective Structured Assessment of Technical Skills instrument, that consists of a global rating scale and a procedure-specific checklist. Sixth year students (control group) were also assessed in their performance without participating in the workshop. Before workshop versus after workshop Objective Structured Assessment of Technical Skills results were compared using Wilcoxon and McNemar tests. After workshop versus control group Objective Structured Assessment of Technical Skills results were compared using Mann-Whitney, qui-squared test and Fisher’s exact test. Results: For the global rating scale, students obtained an after the workshop score (29.5) that was significantly higher than the before the workshop score (15.5; p-value < 0.001), but no significant differences were found between after the workshop and control group scores (p-value = 0.167). For the procedure-specific checklist, 3rd year students had a substantial positive evolution in all parameters and obtained higher rates of correct achievements compared to the control group.Discussion: The final outcomes demonstrated a significant qualitative and quantitative improvement of knowledge and technical skills, which is in accordance with other literature. Conclusion: This Peer Assisted Learning program revealed promising results concerning improvement of surgical skills in medical students, with little staff faculty contribution and extension to a much broader number of students.

Abstract A comparative study has been carried out between fishing lines and nylon thread in regard to their applicability as surgical thread. Four groups of 10 fishing lines in each of the following diameters = 0.15, 0.20, 0.25 and 0.30 mm = were subjected to the following treatments: group I, control; group II sterilized with formalin tablets; group III, autoclaved 30 minutes at 121oC; group IV, autoclaved twice 30 minutes at 121oC. The following parameters were then considered: length, caliber, resistance at the knot, and elongation. Ten samples of each surgical 0, 1.0, 2.0, 3.0, 4.0 and 5.0 nylon threads were used in comparatively similar experiments as the ones carried out with the fishing lines. According to the statistical analyses of the experimental results, it has been found that the sterilization by means of the humid heat exerts an adverse effect on the resistance of the fishing line. However, no differences were found between the effect displayed by two cycles sterilization compared with the fishing lines subjected to only one cycle. The threads maintained in formalin kept the majority of their original properties of resistance, length, diameter and elongation when compared with the control group. It is known, however that formaldehyde is an irritant chemical towards the skin and the mucous membranes. It was possible to conclude: a. fishing lines display characteristics similar to that ones of the nylon surgical thread; b. they can be maintained in formalin solution; c. autoclave sterilization alters some of their physical properties.
