The medical record department is one of the most important parts in the hospital's effort to provide excellent service to patients. The medical records section is indeed a part that is not directly involved in patient care, but other health workers need a medical record section in order to serve patients. The purpose of this study was to analyze internal customer satisfaction related to the service of the medical records department at Muhammadiyah Hospital Ponorogo. The research design used a descriptive quantitative research design. The sampling technique used was snowball sampling. The results showed that several obstacles were found, namely the speed of providing medical records, the accuracy of providing medical records, the management of KLPCM (Incomplete Filling Of Medical Records) and medical record officers who had medical record competence were still very limited. Improved services provided by the medical record department can increase internal customer satisfaction, and of course will have a direct impact on service to patients. nurses and hospital BPJS healthcare officers.

Abstract Internal patient transfer is the process of transferring patients from one room to another in a hospital while still being oriented towards quality and patient safety. The results of the observations showed that the filling of the internal patient transfer form was not optimal so that there were still incomplete forms due to the large number of patients and the weak coordination between health workers. To determine the effect of the completeness of filling out the internal patient transfer form on the quality of medical records. The research method used is quantitative with a descriptive approach. Data collection techniques used are observation, questionnaires and literature study. The sampling technique was simple random sampling technique so as to obtain a sample of 91 internal patient transfer forms. From the results of observations, 22% of the internal patient transfer forms were found that were not completely filled in, especially in the signature and clear name. The two variables have a strong relationship. The effect of the variable completeness of the internal patient transfer form on the medical record quality variable is 90.1% and the remaining 9.9% is influenced by other factors. Based on these studies, it can be concluded that the lack of accuracy and coordination of nurses, doctors and other officers in filling out internal patient transfer forms so that this affects the quality of medical records in the aspect of accuracy. Keyword : Completeness, Internal Patient Transfer Form, Medical Record Quality   Abstrak Transfer pasien internal merupakan proses pemindahan pasien dari satu ruangan ke ruangan yang lain di dalam satu rumah sakit dengan tetap berorientasi pada mutu dan keselamatan pasien. Hasil observasi menunjukkan bahwa belum optimalnya pengisian formulir transfer pasien internal sehingga masih terdapat formulir yang tidak lengkap disebabkan oleh faktor dari banyaknya pasien dan lemahnya koordinasi antara tenaga kesehatan. Untuk mengetahui pengaruh kelengkapan pengisian formulir transfer pasien internal terhadap mutu rekam medis. Metode penelitian yang digunakan yaitu kuantitatif dengan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah observasi, kuesioner dan studi pustaka. Teknik pengambilan sampel adalah teknik simple random sampling sehingga memperoleh sampel sebanyak  91 formulir transfer pasien internal. Dari hasil observasi ditemukannya formulir transfer pasien internal yang belum terisi lengkap sebanyak 22% terutama pada tandatangan dan nama jelas. Kedua variabel memiliki hubungan yang kuat. Pengaruh variabel kelengkapan formulir transfer pasien internal terhadap variabel mutu rekam  medis sebesar 90,1% dan sisanya 9,9% dipengaruhi oleh faktor lain. Berdasarkan penelitian tersebut dapat disimpulkan bahwa kurangnya ketelitian dan koordinasi perawat, dokter dan petugas lain dalam pengisian formulir transfer pasien internal sehingga hal ini mempengaruhi mutu rekam medis pada aspek keakuratan. Every health facility, whether it is primary, secondary, tertiary, is required to maintain medical records in order to achieve administrative order. Incomplete (incomplete) medical records will affect the service process provided by health workers and have an impact on the quality of service of a hospital. This study aims to determine the factors causing the incomplete filling of inpatient medical records. This study uses a literature review method. The strategy in searching literature reviews is using Google Scholar. In the search phase, articles are limited to publications from 2015-2020. The keywords used are the factors causing incomplete medical record filling. The search results obtained 10 articles and then 5 articles were taken. The results of the literature review show that the factors causing the incompleteness of filling in medical records as a whole can be seen from the lack of knowledge, motivation and awareness of medical personnel about medical records. The meeting as a means of communication between caregivers and management has not yet been implemented to discuss evaluation and monitoring as well as sanctions for officers who do not complete medical records. lack of socialization on filling out medical records. Unsystematic arrangement of medical record forms. Limited availability of funds or budget to support medical record service activities. Conclusion Hospitals need to pay attention to the factors causing the incompleteness of filling in medical records so that filling in medical records is complete according to standards. So that the quality of service, especially the quality of patient medical records.AbstrakSetiap fasilitas kesehatan baik tingkat primer, sekunder, tersier wajib menyelenggarakan rekam medis agar tercapainya tertib administrasi. Ketidaklengkapan (Incomplete) rekam medis akan berpengaruh terhadap proses pelayanan yang diberikan oleh petugas kesehatan dan berdampak pada kualitas pelayanan suatu rumah sakit. Penelitian ini bertujuan untuk mengetahui faktor penyebab ketidaklengkapan pengisian rekam medis rawat inap. Penelitian ini menggunakan metode literatur review. Strategi dalam pencarian literatur review menggunakan Google Scholar. Pada tahap pencarian artikel dibatasi terbitan dari tahun 2015-2020. Kata kunci yang digunakan adalah Faktor Penyebab ketidaklengkapan pengisian rekam medis. Hasil penelusuran artikel didapatkan 10 artikel dan selanjutnya diambil 5 artikel. Hasil dari literatur review didapatkan faktor penyebab ketidaklengkapan pengisian rekam medis secara keseluruhan, penyebabnya dapat dilihat dari kurangnya pengetahuan, motivasi dan kesadaran dari petugas rekam medis tentang rekam medis. Belum terlaksananya rapat sebagai wadah komunikasi antara pemberi asuhan dan manajemen yang membahas evaluasi dan monitoring serta sanksi bagi petugas yang tidak mengisi rekam medis dengan lengkap. kurangnya sosialisasi pengisian rekam medis. Susunan formulir rekam medis yang tidak sistematis. Terbatasnya ketersediaan dana atau anggaran untuk mendukung kegiatan pelayanan rekam medis. Kesimpulan Rumah sakit perlu memperhatikan  faktor penyebab ketidaklengkapan pengisian rekam medis sehingga pengisian rekam medis menjadi lengkap sesuai dengan standar. Sehingga  mutu dari pelayanan terutama mutu rekam medis pasien.

Abstrak: Perdarahan post partum adalahperdarahanlebihdari 500-600 ml selama 24 jam setelah anak lahir termasuk perdarahan karena retensio plasenta. Perdarahan post partum adalah perdarahan dalam kala IV  lebih dari 500-600 cc dalam 24 jam setelah anak dan plasenta lahir (Mochtar, 2008).Tujuan penelitian ini adalah untuk mengetahu ifaktor-faktor  yang berhubungan dengan perdarahan post partum pada ibu bersalin di RSUD Pringsewu Tahun 2016. Jenis penelitian ini adalah penelitian kuantitatif, Rancangan penelitian menggunakan Crosssectional. Dalam rancangan penelitian ini mempunyai 560 populasi ibu bersalin yang melakukan persalinan di RSUD PringsewuTahun 2016. Teknik pengambilan sampel pada penelitian ini menggunakan random sampling, dan diperoleh sampel sebanyak 222 responden. Cara ukur dalam penelitian ini menggunakan rekam medik. Analisa data yang digunakan adalah analisa univariat dan analisa bivariat dalam bentuk table distribusi frekuensi. Hasil uji Chi-Square didapatkan bahwa pada atonia uteri P-Value = 0,009 (0,009 < 0,05). OR = 7,296, pada retensio plasenta P-Value = 0,000 (0,000 < 0,05). OR = 13,788, dan pada laserasi jalan lahir P-Value = 0,000 (0,000 < 0,05). OR = 9,118. Maka dapat disimpulkan bahwa Ha diterima ada hubungan antara variabel independen dan variabel dependen. Disarankan bagi seluruh tenaga kesehatan untuk dapat meningkatkan pemberian pelayanan khususnya pada ibu bersalin agar dapat menurunkan Angka Kematian Ibu (AKB) akibat perdarahan khususnya di Provinsi Lampung Abstract: Post partum bleeding is bleeding more than 500-600 ml for 24 hours after child have born, including bleeding because retensio  placenta. Post partum pleading is bleeding in more than 500-600 cc in 24 hours after child and placenta was born (Mochtar, 2008). The aim of the research is to know the factors related to post partum bleeding mother maternity at RSUD Pringsewu in 2016. This research type is quantitative research, research design used crossectional. In this research design have 498 population of maternity that do deliveries at RSUD Pringsewu in 2016. Sampling technique counted 222 respondents. Measure method in this research used medical record. The data analysis that used is univariate analysis and bivariate analysis in the form of frequency distribution table. The result of chi-square test showed that in uterine atonia P-Value = 0,009 (0,009 < 0,05). OR = 7,296, padaretensioplasenta P-Value = 0,000 (0,000 < 0,05). OR = 13,788, danpadalaserasijalanlahir P-Value = 0,000 (0,000 < 0,05). OR = 9,118. So, it can be concluded that ha is accepted and the relationship between the independent variabel and the dependent variable. Recommended for all the health workers to can increase the provision of service, especially to mother maternity in order to can reduce the rate mortality to mother, especially in Provinsi Lampung.

Background: The hospital is a health service institution that plays a role in efforts to improve the degree of public health. One of the important roles in a hospital is the medical recorder installation. One of the minimum service standards for medical records at a hospital is the length of time to provide medical records for outpatient services, which is less than 10 minutes. The purpose of this study was to determine the length of time and to analyze the factors causing the length of time for the provision of medical records for inpatients at Bhayangkara Hospital in 2019.Methods: This research is a combination of research with a sequential explanatory design conducted at the Bhayangkara Hospital in Padang in November 2018 to June 2019. In quantitative research, the sampling technique was by means of the accidental sampling technique and descriptive analysis. While the qualitative research techniques for determining informants were used by purposive sampling and analysis by using content analysis techniques.Results: Quantitative research results, obtained an average time for providing medical records for outpatients is 9 minutes 6 seconds. The qualitative research results obtained that the input of the implementation of the medical record service system is not optimal and the process of implementing the medical record is also not well implemented.Conclusions: The time for providing outpatient medical records is fast according to the minimum hospital medical record service standard, but the implementation is not in accordance with standard operating procedures.

Preliminary: A medical resume is a summary of medical service activities provided by health workers, especially doctors during the treatment period until the patient is discharged either in good health or in death. Incomplete filling of medical resumes can be caused by negligence of officers or lack of understanding of the importance of completing patient data. This research is a descriptive research with a qualitative approach. In this study there were 4 medical recorder and 50 medical resume form. Method: The sampling technique used is the total sample. The data analysis used is descriptive data analysis. Results: Based on the results of the study, the incomplete medical resume form for inpatients in 2021 with a sample of 50 medical resume forms, the average percentage of medical resume completeness was 66.7%, while the incomplete medical resume was 33.3% of the 50 samples of medical resume forms. The highest percentage of incompleteness is the doctor's signature (52%). And the highest percentage of completeness is no medical record (76%). The problem that usually arises is that there are still many incomplete components of a medical resume. Conclusion: to overcome the problem usually requires more optimal socialization and collaboration between doctors and medical recorders, to further improve health services regarding the completeness of filling out medical record documents in order to improve the quality of health services, and the head of medical records needs to hold meetings or directives to the unit concerned so that the resume is filled out. can be carried out properly. The aim of the study was to investigate the relationship among Narcissism, personality traits and conspicuous consumption of brands in youth. This used quantitative research design with a sample consisting of 50 Men and 50 Women. The age ranged between 18 – 22 years. A purposive sampling technique was used to select participants. The findings revealed that there was a relationship among conspicuous consumption and traits of personality. It was also uncovered that there are gender differences in conspicuous consumption of brands, narcissism and personality traits. Further it was concluded that narcissism is positively associated with conspicuous consumption of brands. Narcissism was likely to be a positive predictor of conspicuous consumption of brands and personality traits are likely to be a predictor of conspicuous consumption of brands.

Data collection techniques are conducted through document studies, conducted by reviewing documents on positive law. Furthermore, the method of data analysis is done through normative qualitative.The results of the study indicate: Legal aspects of Medical Record or Electronic Medical Record   in Teurapetik Transactions related to: first, the obligation of health workers in coaching and health services to make Medical Record or Electronic Medical Record correctly and responsible for secrecy because it is the opening of Medical Record or Electronic Medical Record without With the permission of the patient having the consequences of criminal law. The absence or misuse of the Medical Record or Electronic Medical Record means that health workers may be subject to criminal, civil and administrative sanctions. Second, the position of  Medical Record or Electronic Medical Record is evidence in the form of a letter (if given outside the court), and expert information (if delivered in court).

The coronavirus disease (COVID-19) pandemic in Indonesia has a psychological impact among health workers who are working in hospital and caring for patients with positive COVID-19. This study aimed to examines the psychological impact among health workers in effort to facing the coronavirus pandemic (COVID-19) in Indonesia. Snowball sampling technique was employed to gather the sample in the eight islands. The health workers were responded to the depression anxiety stress scales (DASS-42) questionnaire, with the total of 644 respondents. The results of this study showed that about 65.8% of respondents experienced anxiety due to COVID-19 outbreak, There were 3.3% experienced extremely severe anxiety and 33.1% experienced mild anxiety. Whereas those experienced stress due to COVID-19 outbreak were 55%, extremely severe stress levels were 0.8% and mild stress were 34.5%. Health workers who experienced depression were 23.5%. There were 0.5% with very severe depression and mild depression was 11.2%. There is a closed correlation between anxiety (r=0.152 p=0.000), stress (r=0.086 p=0.029), and depression (r=0.111 p=0.005), to the worried of being alienated if infected coronavirus (COVID-19). The Indonesian government has to handle the anxiety, stress and depression that occurs in health workers in particular and can provide protection to medical personnel so that they do not feel alienated by those around them.

ABSTRACTBackground: Dysmenorrhea is a painful sensation, cramping in the lower abdomen which is often accompanied by other symptoms, such as sweating, headache, nausea, diarrhea, and tremors, all of which occur before or during menstruation. Teenage girls who experience dysmenorrhea can interfere with social or physical activities because when they are in pain, sufferers tend to be silent and even don't want to interact with other people, they tend to be more emotional. Emotional embodiment part of what a woman feels, a reaction to a certain event or situation. Emotional status and dysmenorrhea in women is a conscious experience that influences bodily activities and is psychologically able to influence a woman's emotions. Objective: To identify emotional status and to analyze the relationship between emotional status and the degree of dysmenorrhea in young girls. Methods: Quantitative research, descriptive analytic research design with cross sectional research design with proportionate stratified random sampling technique, the sample of this study was 54 students of class X SMKN 12 Loa Buah Samarinda who experienced dysmenorrheaResults: Variable emotional status obtained positive emotional classification 33 (61, 1%) and negative emotions 21 (38.9%) respondents. Variable The degree of dysmenorrhea was classified as mild 35 (64.8%), moderate 12 (22.2%), severe 5 (9.3%) and unbearable 2 (3.7%). The test used Pearson Chi-square results obtained P value = 0.402, the significant level (α) is 0.05, then p> from α. This result means that Ho is accepted, there is no relationship between emotional status and the degree of dysmenorrhea in young girls at SMKN 12 loa buah samarinda. Conclusion: There is no relationship between emotional status and the degree of dysmenorrhea in adolescents at SMKN 12 Loa Buah Samarinda

Background: Incomplete filling of medical record files for inpatients at Dr. Reksodiwiryo hospital medical records will be describe health services and the quality of medical record services. Medical record quality services include the completeness of medical record files, accuracy in providing diagnosis and diagnosis codes, as well as speed in providing service information. The requirements for quality medical records must be accurate, complete, reliable, valid, timely, usable, common, comparable, guaranteed, and easy.Methods: This research method is a descriptive with a retrospective approach or looking at existing data. This study was carried out in September 2021. The population was 70 files cases of inpatient digestive surgery. Samples were taken from 27 files of inpatients with appendicitis cases.Results: From the research that has been done, the highest percentage of incomplete identification components is found on the gender item about 81.48%, the highest percentage of incomplete important report components is obtained on the medical resume and informed consent items about 11.1%. The highest percentage of incomplete authentication components was obtained in the nursing degree about 96.3%. The highest percentage of the components of the recording method was obtained by 59.3%, there are several blank sections about 16 files. The percentage of incomplete diagnostic codes and procedures is 100%  Conclusions: the researcher suggested that the hospital can have an Operational Standart on filling out the completeness of medical records files
