Background: Hypertension is one of the major risk factors for cardiovascular disease, especially for coronary artery disease. Healthy lifestyles will delay the progression of coronary artery disease.Objectives: To find out the life style pattern on prevention of coronary artery disease among hypertensive clients.Methodology: This is a descriptive; cross sectional research design based on quantitative approach. Non probability purposive sampling was used to select 202 respondents attending Outpatient Department at Manmohan Cardiothoracic Vascular and Transplant Centre, Kathmandu. Data was collected by interview technique using semi-structured questionnaire by researcher herself. Descriptive and inferential statistics were used to analyze the data.Results: Findings of the study showed that 7.90% of hypertensive clients consumed ≥ 5 servings of fruits and vegetables per day, whereas 97.52% were engaged in vigorous intensity work and current tobacco and alcohol consumers were 28.57% and 33.80% respectively. Adherence to all four healthy lifestyle pattern (Diet, Physical Activity, Tobacco Use and Alcohol Consumption) was 32.7%, whereas more than half (58.9%) of the clients adhered to three lifestyle pattern. There was significant association between dietary pattern and economic status (p = 0.018) and ethnicity (p = <0.001); and tobacco use and sex (p = <0.001).Conclusion: Adherence to healthy lifestyle pattern of hypertensive clients was inadequate. Thus, health education and other interventions to promote healthy lifestyle pattern are suggested.

Introduction: Coronary Artery Disease (CAD) impairs all aspects of the patient's life due to the decrease in physical function and lower quality of life, indicating an overall decreased health status. Self-efficacy as a psychological factor plays an important role in individuals maintaining a healthy lifestyle and improving their health status. The aim of this study was to analyze the correlation between self-efficacy and the health status of coronary artery disease patients.Methods: This study used an observational analytic research design with a cross-sectional approach. This study involved 112 respondents who were coronary artery disease patients in RSD Dr. Soebandi Jember obtained through the incidental sampling method. The data was collected using Cardiac Self-Efficacy (CSE) and Seattle Angina Questionnaire (SAQ). The data analysis used the Spearman rank test with a 95% CI.Results: The results showed that there was a significant relationship between self-efficacy and health status (p = 0,001, r = 0.307, α = 0,05). Self-efficacy was in good category (71,41 points) while health status was also in the good category (79,56 points).Conclusion: The low positive correlation between the two variables shows that the higher the value of self-efficacy, the higher the value of health status. Good self-efficacy through healthy living behaviors can increase the health status of coronary heart disease patients. It is important for nurses to improve the psychological aspect of the patients including self-efficacy when optimizing their self-care and health status.

Background: The Indian sub-population is the biggest among the foreigners in Kuwait. Due to the harsh weather conditions most of the time in the year there is less inclination towards a healthy lifestyle. The present study was conducted to determine the extent of the Indian population living in Kuwait having the risk factors for coronary artery disease (CAD).Methods: A cross-sectional survey was conducted including 400 subjects aged 18 years or more (346 males and 54 females). They were subjected to general physical examination, and were asked to respond to a standard questionnaire.Results: Significantly more females than males were found to be overweight (63% vs. 43.9%, p<0.01) or obese (16.7% vs. 4.3%, p<0.005). Females also had higher proportions than males in having inadequate physical activity (79.6% vs. 53.2%, p<0.001). Significantly more males were having an inadequate intake of fruits and vegetables as compared to females (34.7% vs. 18.5%, p<0.005). There was no significant difference between the genders in proportions of hypertension, smoking, and diabetes. Overweight, hypertension, and inadequate physical activity showed a rising trend with increasing age. The studied population was compared with several geographically different populations, as well as the different populations living in India.Conclusions: As the stay and the jobs of Indians in Kuwait depend upon their health-status, prevention of CAD is of paramount importance. Looking at the levels of the prevalence of several risk factors, multilevel interventions are needed for reducing the CAD-morbidity and mortality.

Aim: Some studies have linked the present of chronic obstructive oulmonary disease (COPD) to coronary artery disease (CAD). Low grade systemic inflammation occurs in patients with COPD as well as patients with CAD. This study was designed to find out the concentration differences of hs-CRP and TNF-a in patients having both chronic obstructive pulmonary and coronary artery diseases with those having either. Methods: A cross - sectional descriptive study was conducted in 200 patients undergoing a coronary artery angiography in the Heart Institute, Thong Nhat Hospital and 115 People Hospital. COPD was diagnosed using GOLD classification. Result: Our study had shown that the levels of hs-CRP and TNF-a were statistically increased in patients with COPD, CAD as well as in patients who had COPD with CAD (p<0,05). The levels of hs-CRP were higher in CAD than in COPD nad the levels of TNF-a were higher in COPD than in CAD. In patients with COPD and CAD, there were increased the levels of both hs-CRP and TNF-a in serum. Conclusion: Systemic inflammation presents in both COPD and CAD. Abstract Objective To identify redundant clinical trials evaluating statin treatment in patients with coronary artery disease from mainland China, and to estimate the number of extra major adverse cardiac events (MACEs) experienced by participants not treated with statins in those trials. Design Cross sectional study. Setting 2577 randomized clinical trials comparing statin treatment with placebo or no treatment in patients with coronary artery disease from mainland China, searched from bibliographic databases to December 2019. Participants 250 810 patients with any type of coronary artery disease who were enrolled in the 2577 randomized clinical trials. Main outcome measures Redundant clinical trials were defined as randomized clinical trials that initiated or continued recruiting after 2008 (ie, one year after statin treatment was strongly recommended by clinical practice guidelines). The primary outcome is the number of extra MACEs that were attributable to the deprivation of statins among patients in the control groups of redundant clinical trials—that is, the number of extra MACEs that could have been prevented if patients were given statins. Cumulative meta-analyses were also conducted to establish the time points when statins were shown to have a statistically significant effect on coronary artery disease. Results 2045 redundant clinical trials were identified published between 2008 and 2019, comprising 101 486 patients in the control groups not treated with statins for 24 638 person years. 3470 (95% confidence interval 3230 to 3619) extra MACEs were reported, including 559 (95% confidence interval 506 to 612) deaths, 973 (95% confidence interval 897 to 1052) patients with new or recurrent myocardial infarction, 161 (132 to 190) patients with stroke, 83 (58 to 105) patients requiring revascularization, 398 (352 to 448) patients with heart failure, 1197 (1110 to 1282) patients with recurrent or deteriorated angina pectoris, and 99 (95% confidence interval 69 to 129) unspecified MACEs. Conclusions Of more than 2000 redundant clinical trials on statins in patients with coronary artery disease identified from mainland China, an extra 3000 MACEs, including nearly 600 deaths, were experienced by participants not treated with statins in these trials. The scale of redundancy necessitates urgent reform to protect patients.

Introduction: Interleukin L-17 is produced by Th 17 cells and other cells. There is a debate if IL 17 is atherogenic or atheroprotective. The true role of this interleukin during the development and progression of the coronary artery disease is not known. Objective: To evaluate if there are differences between the IL17 A serum levels according to clinical presentation of the coronary artery disease. Methods: This is a cross sectional study which enrolled 101 patients with acute coronary syndrome (ACS), 100 patients with chronic coronary syndrome (CCS) and 100 healthy volunteers. Blood samples were taken from patients ( at admission) and controls to analysis the level of IL17A. Clinical characteristics were collected through questionnaires. This research was approved by ethical committee. Results: Comparisons of the clinical characteristics between patients with ACS and CCS revealed: mean age ( 62 ± 12.4 vs 63.3 ± 9.8, p = 0.4 ), male (63.4% vs 58%, p = 0.4) hypertension (85.1% vs 79%, p = 0.1) , disyipidemia (48% vs 31%, p =0.01), Diabetes Mellitus (47.5% vs 41%, p = 0.3), previous myocardial infarction (57.4% vs 40%, p = 0,01), smoking (29.7% vs 38%, p = 1). The peripheral concentrations of IL17A according to ACS, CCS and controls were: 5.36 ± 8.83 vs 6.69 ± 17.92 vs 6.26 ± 11.13, p = 0.6. Besides, the comparison between ACS and CCS showed: 5.36 ± 8.83 vs 6.69 ± 17.92, p = 0.3. Conclusion: The main finding os this study was that the circulating IL 17 concentrations were similar in patients with ACS, CCS and healthy volunteers). Besides, there was no difference between patients with ACS and CCS. Therefore, our hypothesis is that in patients with ACS and CCS the circulating IL 17 A concentrations are low or undetectable.

Background: The superiority of the GRACE and TIMI risk scores in predicting the angiographic severity of coronary artery disease in patients with non ST-elevation myocardial infarction (NSTEMI) has not yet been established. This study was done to compare the GRACE and TIMI risk scores in predicting the angiographic severity of coronary artery disease in this group of patients.Method: The cross sectional study done in the Department of Cardiology, NICVD, Dhaka. The patients admitted with NSTEMI were evaluated to calculate the GRACE and TIMI risk score from April, 2015 to April, 2016.Coronary angiogram was done during index hospitalization and the severity of the coronary artery disease was assessed by vessel score and Gensini score.Results: Of 115 patients assessed, a positive correlation of the vessel score and Gensini score was observed with both the GRACE and TIMI risk scores (p=<0.001) and the GRACE score (r=0.59) correlated better than the TIMI score (r=0.52). The GRACE score presented area under the Receiver Operating Characteristic (ROC) curve of 0.844(95% CI = 0.774 – 0.914) significantly superior to the area under the ROC curve of 0.752(95% CI =0.658– 0.846) of the TIMI score for the difference between the two scores.Conclusion: Both the GRACE and TIMI scores had good predictive value in predicting the severity of coronary artery disease in the patients with NSTEMI but when both the scores were compared, the GRACE score was found to be superior and correlated better with the severity of coronary artery disease.Cardiovasc. j. 2017; 10(1): 45-51

OBJECTIVE: To determine the association between non alcoholic fatty liver disease and coronary artery disease. METHODOLOGY: This cross sectional study is conducted from July 2016 to December 2016, in cardiology unit, Lady reading hospital. By using non probability consecutive sampling, patients of all age groups and either gender, presenting to cath: lab for coronary angiography, indicated for angina CCS III, were included in the study. All patients fulfilling inclusion and exclusion criteria were subjected to screening for NAFLD by using ultrasonography. Patients were classified into having no, mild, moderate and sever NAFLD. Correlation between NAFLD and CAD, confirmed on cath: studies, was done using Spearman’s rho test. RESULTS: Total of 370 patients with mean age of 55.36 ± 10.07 years were enrolled in the study, of which 44.6% were females. Known risk factors for CAD like Diabetes mellitus, hypertension, and smoking were present in 63.5%, 64.9% and 23% respectively. 28.4% of patients had no NAFLD, 28.4% had mild, 28.4% had moderate and 14.4% had sever NAFLD. 12.2% had no CAD while mild, moderate and sever disease was present in 36.5%, 31.1% and 2.3% respectively. By using chi square test co relation co efficient between NAFLD and CAD was calculated and came out to be 285.536 ( p value <0.000). NAFLD also increased the odds of having CAD by 2.9 times with a p value for odd ratio <0.000. CONCLUSION: NAFLD is strongly associated as an independent risk factor with CAD and increases the odds of having CAD. Coronary artery disease (CAD) is a very ordinary health problem around the globe. CAD is affected by numerous factors like diabetes, obesity, smoking, gender, and diet. However, the association of CAD with diet is understudied in our region that’s why our study was aimed to evaluate the effect of diet on CAD incidence. This cross-sectional study was completed among local population of Rawalpindi, Pakistan in almost 6 months from November 2020 to April 2021. People were taken in our study via set criteria. Data was collected by self-structured proforma. Data analysis was performed by applying IBM SPSS version 25. Descriptive statistics were applied for quantitative variables. Statistical tests like Chi-square test and Spearman correlation analysis were applied to know the association, direction, and strength of study variables. Our current study displays overall high prevalence CAD (17.48%) among study population. Mean value of age for study participants was 45.42 with SD of ±8.01years. Prevalence of CAD was more common among females, faster food eaters and fresh fruit and raw vegetables non-eaters. Gender, fast food, and fresh fruits and vegetables were associated with CHD significantly with p-values 0.030, 0.0007, and 0.0006 respectively. Strength and direction for fast food and CAD association was very strong and positive while for association between fresh fruits and raw vegetables and CAD was very strong and negative with correlation coefficient +0.812 and -0.831 respectively. In a nutshell, current study indicates high prevalence of CAD especially among females, fast food eaters and fresh fruits and raw vegetables non-eaters. Higher consumption of fast foods leads to higher incidence of CAD and vice versa while higher consumption of fresh fruits and raw vegetables bring down the incidence of CAD incidence and vice versa. So, we need to create awareness among people about the impact of food on CAD and gender specific interventions for each gender to lower down the CAD incidence.
