Introduction.Nowadays, coronavirus disease is reported a systemic disease, which impairs immune function, lung and gastrointestinal damage. The latter, along with the respiratory system, can serve as the entry gate to infection. Chronic diseases of the gastrointestinal tract are considered as risk factors for more severe course of the disease. Objective: to study the frequency of gastrointestinal symptoms and liver functioning in patients with coronavirus disease. Materials and methods: 55 medical records of inpatients with coronavirus disease and pneumonia (20 women and 35 men), who were treated at the infectious department of the Central Municipal Clinical Hospital, Ivano-Frankivsk. The mean age of female patients was 57.1 ± 2.62 years and 59.2 ± 2.59 years for male patients. All patients underwent polymerase chain reaction testing and enzyme-linked immunosorbent assay (IgM SARS COV 2), general clinical examinations (general blood test, biochemical analysis of live damage markers, glycemia level), chest CT. We determined inflammatory biomarkers as well. Results and discussion. The patients with coronaviral disease and pneumonia were found to have various comorbidities, including hypertension, obesity, diabetes, chronic kidney disease, gastroesophageal reflux disease, chronic viral hepatitis B and others. The most common gastrointestinal symptoms included anorexia detected in 50 patients (90.9%), nausea in 11 (20%) patients, diarrhoea in 6 (10.9%) patients, vomiting in 2 (363%) patients, abdominal pain in 4 (7.27%) patients, and constipation in 1 (1.8%) case. Diarrhea usually did not pose a serious threat, and was reversible in the course of pneumonia therapy. Thrombocytopenia occurred in 10.9% of patients. The level of C-reactive protein was high in 90.9% of patients. A third of the patients demonstrated as increase in the activity of aspartate aminotransferase by 30.9% and alanine aminotransferase by 32.7%. Conclusion. Coronavirus can induce inflammation and change the permeability of mucous membranes, possibly receptor-mediated penetration into the cells of the body; we can suggest the virus affects the composition of the intestinal microbiota, disrupts the interaction between organs "intestinal-lung" that may contribute to the progression of respiratory symptoms.

Abstract Background: The objective of this study was to retrospectively compare clinical features and prognostic values between the patients who were referred to the general surgery clinic of our hospital with the presumed diagnosis of acute appendicitis and underwent positive or negative appendectomy. Methods: Patients were divided into two groups as positive (PA) (n:362) and negative appendectomy (NA) (n:284) and the data obtained were compared between these two groups.Laboratory investigations were performed in all patients, and white blood cell (WBC), mean platelet volume (MPV), neutrophils count (NEU), neutrophils (%) (NEU%), C-reactive protein (CRP) and total bilirubin (TBIL) values were studied. Results: The mean MPV value was found as 7.88 fl in PA groups and 8.09 fl in NA group, and the mean MPV value was not statistically significantly difference in PA group, compared to NA groups (p=0.012). Laboratory parameters were also compared between genders. Accordingly, the mean MPV value was statistically significantly higher in female patients compared to male patients in PA group (p = 0.04). The mean TBIL value was 0.97 mg/dl in PA group and 0.69 mg/dl in NA group, and the mean TBIL value was statistically significantly higher in PA group (p< 0.001). Finally, TBIL value was statistically significantly lower in female patients compared to male patients in NA and PA group (p < 0.05).Conclusions: According to the results of our study, MPV and T. BIL values differ in PA and NA groups depending on gender. Therefore, these values may not be used as specific biomarkers in predicting positive acute appendicitis. We believe that these results will contribute to the literature and will be guiding for future studies.

Aim. While there are many well-defined serological markers for inflammatory bowel disease (IBD), there is limited evidence that they positively affect clinical outcomes. This study aimed to evaluate the correlation between hepcidin serum levels and disease activity in IBD.Materials and Methods. Eighty-five consecutive IBD patients were enrolled in the study. Hepcidin serum levels were assessed using an enzyme-linked immunosorbent assay (ELISA) and were compared with disease activity as well as the interleukin-6 (IL-6) and C-reactive protein (CRP) levels.Results. The mean hepcidin serum levels in Crohn’s disease (CD) patients in remission and in the active phase were3837±1436and3752±1274 pg/mL, respectivelyP=0.613. The mean hepcidin serum levels in ulcerative colitis (UC) patients in remission and in the active phase were4285±8623and3727±1176 pg/mL, respectivelyP=0.241. Correlation analysis between inflammatory markers and hepcidin serum levels indicated that there was no correlation between hepcidin levels and IL-6P=0.582or CRPP=0.783.Conclusion. As an acute-phase protein, hepcidin seems to have a lower efficacy than other parameters in the detection of activation in IBD.

Background: One of the conditions on premenopausal period is the increase in body fat, among which are caused by levels of the fluctuating estrogen hormone. Increase fat affects the body fat percentage and adipsin levels. Adipsin plays a role in the stimulation of insulin secretion by producing C3a, in which C3AR1 (C3a Receptor 1) fragments interact with beta cells to secrete insulin. Objective: The purpose of this study is to determine correlation of body fat percentage with adipsin levels on premenopause.Design: This was an analytical study with cross-sectional design. Body fat percentage was obtained by examination using Bioelectrical Impedance Analysis (BIA). Venous blood was taken at the mediana cubiti area used for serum adipsin levels. Adipsin levels were analyzed by Enzyme-linked Immunosorbent Assay (ELISA) technique. Results: The mean of body fat percentage on premenopausal was 36,18 ± 8,35%. The mean of adipsin levels on premenopausal was 117,05 ± 87,93 ng/ml. Result of Pearson test analysis got value r = 0,632 and value p = 0.0005 (p <0,05).Conclusions: There is a positive significant correlation between the percentage of body fat with adipsin levels on premenopause.

Objectives: This study investigated the relationship between viral load and the incidence and recovery of olfactory and gustatory dysfunction (OD and GD), incidence of respiratory and gastrointestinal symptoms in COVID-19 patients. Design: A Retrospective Cohort Study. Setting and Participants: In total 599 outpatients’ cases polymerase chain reaction (PCR)–confirmed COVID-19-positive patients in Golestan province were included in the study. Main Outcome Measures: The incidence of OD, GD, their severity and the time of recovery was determined. The association of these variables with cycle threshold (CT) values of SARS-CoV-2 polymerase chain reaction was assessed. Results: The mean age of patients was 38.27 ± 13.62 years. The incidence of general symptoms included myalgia 70.1%, headache 51.8%, fever 47.7%, and dyspnea 21.4%. 41.9% of patients had gastrointestinal symptoms, including abdominal pain 26.5%, diarrhea 25.2%, nausea 20.5%, and vomiting 12.9%. 12.2% of patients had comorbidity. The trimester recovery rates of OD and GD were 93.94% and 94.74%, respectively. The mean recovery time of OD and GD was 14.56±13.37 and 13.8±3.77 days, respectively. The mean CT value in all patients was 27.45±4.55. There were significant associations between CT value with headache (P=0.04), GD (P=0.002) and OD (P=0.001). Conclusions: The intensity and the recovery of OD and GD in Covid-19 patients may be affected by initial viral load. Unlike to respiratory and gastrointestinal symptoms, the OD and GD were associated with lower viral load. Therefore, it may be recommended to use these clinical symptoms as an indicator in the initial screening of patients during pandemics.

The molluscum contagiosum virus (MCV) is a dermatotropic poxvirus. The causative agent of molluscum contagiosum (MC) is nonlethal, common and worldwide. Additionally, little inflammation is associated with MC papules. The present study aims to evaluate the immune status of MC patients by measuring the level of immunoglobulins IgG and IgM by using the radial immune diffusion assay (RIA) and the level of interleukin 18 receptor 1 (IL-18R1) by the Enzyme-linked immunosorbent assay (ELISA).The study is conducted during November 2013 to April, 2014 in outpatient clinic of Baquba Teaching Hospital. There are 75 patients, diagnosed with clinical lesions of MCV on different areas of the body, whose age is ranged between 2-50 years including 40(53.3%) males and 35(46.7%) females. The study includes 15 healthy persons age between 2-50 years. The level of IL 18R1 were significantly elevated in patients (677.15±874.22) compared with control (178.46±31.79 ng/ml). There is also a significant elevation in the mean level of serum IgM, where it is 1946.6±825.6 mg/dl while in control group is 140.1±68.7mg/dl. By contrast in patients with lower levels of IgG than the control, the mean serum IgG level in patient is 221.9±96.7 mg/dl while in the control is 1229.9±299.7 mg/dl. Finally, there is no significant difference between MC patients from rural area and urban area.

The problem of human adaptation to extreme environmental factors, in particular to the cold, which is one of the most common, is relevant from general biological and medical points of view. In this case, morphological reorganization takes place in various organs and tissues, and sometimes the changes become irreversible. That’s why from a scientific and practical point of view it is important to assess the role of the cold factor, the influence of which is experienced by most living beings, in particular on the adrenal glands, which are among the first to react to changes in the internal environment of the body. The purpose of the study is to follow the dependence and to study the morphological and functional state of the adrenal glands at various stages after the action of general deep hypothermia. The morphological and functional state of the adrenal glands of white outbred adult male rats was studied using histological, electromicroscopic, enzyme-linked immunosorbent assay, fluorometric, and statistical methods of research in various periods after exposure to general deep hypothermia. The morphometric analysis of the thickness of the adrenal cortex zones, the diameter of the microvessels, the area of cells and their cores, both cortical and brain, was performed using the software VIAVISION Version 2. The average arithmetic mean (M) and the mean arithmetic mean (m) error using the software were determined. “Statistica-5,0”. The probability of the difference between the mean values and their errors was determined by the criterion of the Student. It has been established that at the height of the cold action, the levels of cortisol and adrenaline increase, along with hypertrophy of the cells of the fasciculate zone and the adrenal medulla. At the stages of the posthypothermic period, one more rise of corticosteroids is observed on the 7th day, while the concentration of catecholamines decreases. The hypertrophy of the adrenocorticocyte of the fasciculate zone persists until the 30th day, and the adrenocyte of the medulla lasts up to 7 days after the action of general deep hypothermia. On the 14th day, in the structural components of the hemomicrocirculatory bed, next to intracellular compensatory-adaptive phenomena, single microvessels with destructive signs of endothelial cells are noted. Until the 30th day, the vast majority of microvessels acquire the characteristic structure of the norm. These changes indicate a close relationship of the sympathoadrenal and hypothalamic-pituitary-adrenocortical systems. The results of the conducted research expand modern ideas about the features of angioarchitecture and the submicroscopic structure of the adrenal parenchyma, reveal the structural and functional bases of their restructuring after exposure to general deep hypothermia.

Background. Hyperammonemia resulting from chronic liver disease (CLD) can potentially challenge and damage any organ system of the body, particularly the brain. However, there is still some controversy regarding the diagnostic or prognostic values of serum ammonia in patients with over hepatic encephalopathy, especially in the setting of acute-on-chronic or chronic liver failure. Moreover, the association of serum ammonia with worsening Child-Pugh grade of liver cirrhosis has not been studied. Objective. This study was conducted to solve the controversy regarding the association between hyperammonemia and cirrhosis, especially hepatic encephalopathy in chronically failed liver. Material and Methods. In this study, 171 cirrhotic patients had their serum ammonia measured and analyzed by SPSS version 16. Chi-squared test and one-way ANOVA were applied. Results. The study had 110 male and 61 female participants. The mean age of all the participants in years was 42.33±7.60. The mean duration (years) of CLD was 10.15±3.53 while the mean Child-Pugh (CP) score was 8.84±3.30. Chronic viral hepatitis alone was responsible for 71.3% of the cases. Moreover, 86.5% of participants had hepatic encephalopathy (HE). The frequency of hyperammonemia was 67.3%, more frequent in males (N=81, z-score = 2.4, and P<0.05) than in females (N=34, z-score = 2.4, and P<0.05), and had a statistically significant relationship with increasing CP grade of cirrhosis (χ2(2) = 27.46, P<0.001, Phi = 0.40, and P<0.001). Furthermore, serum ammonia level was higher in patients with hepatic encephalopathy than in those without it; P<0.001. Conclusion. Hyperammonemia is associated with both increasing Child-Pugh grade of liver cirrhosis and hepatic encephalopathy.

Background:Interleukin 23 (IL-23) is a pivotal pro-inflammatory cytokine in the Th17/IL-23 axis which became the center of attention in researches during these last decades, especially during spondyloarthritis (1).Objectives:We aimed to study the diagnosis value of IL-23 serum in spondyloarthritis. Methods:We conducted a case-control study, including 144 subjects divided into 2 groups:-G1: 72 patients meeting the Assessment of SpondyloArthritis International Society (ASAS) criteria for spondyloarthritis (SA)-G2: 72 healthy controls matched for age and sex.For each SA patient we collected the following parameters: BASDAI (Bath Ankylosing Spondylitis Disease Activity Index), ASDAS (Ankylosing Spondylitis Disease Activity Score), BASFI (Bath Ankylosing Spondylitis Functional Index), and BASRI (Bath Ankylosing Spondylitis Radiology Index). The IL-23 level was measured using Enzyme-linked immunosorbent assay (ELISA).Erythrocyte Sedimentation rate (ESR) and C-reactive protein (CRP) were also measured. We performed a ROC analysis and computed the air under the curve (AUC) at IL-23 to diagnose SA patients.Statistical analysis was performed using “IBM SPSS Statistics” software version 25.Results:The study included 57 men and 15 women. The mean age was 44.84 ± 13.42 years. The mean age at the onset of the disease was 35.97 ± 12.88 years. The disease duration was 8.54 ± 7.7 years.Seventy-nine per cent of our patients had axial radiographic spondyloarthritis (n=57). Peripheral involvement was found in 45.8% (n=33). Eighteen patients had both axial and peripheral involvement concomitantly. Psoriasis was found in 36.1% of the cases (n=26).The mean BASDAI and ASDAS-CRP were 3.21 ± 1.64 and 3.05 ± 1.51, respectively. The mean was BASFI 3.88 ± 2.69. The mean was BASRI 5.26 ± 4.14.The mean ESR and CRP were 36.74 ± 29.38 mm/hr and 20.45 ± 25.19 mg/dL, respectively.IL-23 level was significantly higher in patients compared to healthy controls (23.1 ± 2.72 pg/mL and 5.02 ± 0.59 pg/mL, respectively, p<0.0001).As shown in Figure 1, the AUC value to distinguish between spondyloarthritis and healthy control was 0.705 (p<0.0001). IL-23 cut-off was 7.96 pg/mL (Sensibility= 69.4%, specificity=98.6%).Figure 1.AUC at IL-23 between SA patients 0.705 (p<0.0001) Nevertheless, no correlation was found between serum IL-23 levels and the following parameters: ESR, CRP, BASDAI, ASDAS-CRP, BASFI and BASRI.Conclusion:As reported to previous studies, our study showed that IL-23 is significantly higher in SA patients (2).Interestingly, IL-23 was able to distinguish between SA patients and healthy controls with a cut-off of 7.96 pg/mL. This finding suggests that IL-23 may be practical for the diagnosis of SA.References:[1]K V, D E.
