Gastroesophageal reflux disease (GERD) is one of the most common gastrointestinal diseases encountered in primary care and gastroenterology clinics. Most cases of GERD can be diagnosed based on clinical presentation and risk factors; however, some patients present with atypical symptoms, which can make diagnosis difficult. An esophagogastroduodenoscopy can be used to assist in diagnosis of GERD, though only half of these patients have visible endoscopic findings on standard white light endoscopy. This led to the development of new advanced endoscopic techniques that enhanced the diagnosis of GERD and related complications like squamous cell dysplasia, Barrett's esophagus, and early esophageal adenocarcinoma. This is conducted by improved detection of subtle irregularities in the mucosa and vascular structures through optical biopsies in real-time. Management of GERD includes lifestyle modifications, pharmacological therapy, endoscopic and surgical intervention. Minimally invasive endoscopic intervention can be an option in selected patients with small hiatal hernia and without complications of GERD. These endoscopic interventions include endoscopic fundoplication, endoscopic mucosal resection techniques, ablative techniques, creating mechanical barriers, and suturing and stapling devices. As these new advanced endoscopic techniques are emerging, data surrounding the indications, advantages and disadvantages of these techniques need a thorough understanding.

Abstract:: There is universal agreement that when lifestyle modifications are inadequate to control the hyperglycemia, metformin should be considered as the first line pharmacological therapy. However, when metformin monotherapy fails, there is no consensus as to which drug should be added. Many new classes of drugs that are currently available including DPP-4 inhibitors, GLP-1 agonists and SGLT-2 inhibitors have undergone cardiovascular outcome trials which had been made mandatory by the regulatory authority and thereby established their cardiovascular safety or at least neutrality. Though sulfonylureas are one of the widely prescribed drugs both in developed and developing countries and have proven their efficacy for glycemic control and prevention of microvascular complications, there is considerable uncertainty about its cardiovascular safety which has been going on for nearly five decades. In this review we will critically analyze the efficacy and cardiovascular safety of sulphonylurea, based on the latest available literature to clarify their role in our day-to-day clinical practice.

Endoscopy remains the most important diagnostic and monitoring modality in the management of inflammatory bowel disease. Advances in imaging have progressively added new tools into the armamentarium of endoscopists with the goal of more accurate, sensitive, and accessible visual diagnoses for the benefit of patients with gastrointestinal diseases. Here, we review the relevant literature regarding commonly used endoscopic techniques (dye-based and digital chromoendoscopy, high-definition endoscopy, capsule endoscopy, and endosonography), as well as advanced and experimental technologies (full-spectrum endoscopy, endocytoscopy, autofluorescence, laser endoscopy, and endomicroscopy, including molecular imaging), applicable to inflammatory bowel diseases and emerging for implementation into everyday practice. Additionally, we discuss future directions and techniques as candidates for a superior inflammation imaging in the diagnosis and prediction of therapeutic response.

This study analyses the advantages and disadvantages of cooperative learning type Numbered Head Together (NHT). The method in this research is a literature study based on previous research. The analysis technique used in this research is descriptive qualitative obtained from books and journals. Based on literature from 26 articles in scientific journals, it was found that the advantages of this type of NHT cooperative learning model are to make students more active, make students more interested in learning, think critically and responsibly and can improve student learning outcomes. While the obstacle in this learning model is the lack of time management) so that not all students can present the results of the discussions obtained during the learning process

Background: Elderly as one of vulnerable population has experienced several risk of cardiovascular system disturbance such as hypertension. Hypertension be non communicable disease most suffered elderly in some countries. Objective: This literature review aimed at describing the effect of traditional music therapy on blood pressure among elderly with hypertension. Method: This study carried out from databases such as EBSCOhost, Scopus, Pubmed, ProQuest, Science Direct and Sage with limited years of an article published in the last ten years (2008-2018). Result: Fifteen most relevant articles were extracted in this study and found that traditional music therapy provide a positive effect to decrease blood pressure’s elderly with hypertension and this finding explained clearly with Roy’s Adaptation Model. Conclusion: Traditional music therapy provides a positive effect in lowering blood pressure’s elderly with hypertension. This therapy can be given as single intervention or collaboration with other non pharmacological therapy such as calisthenics, breathing exercises, muscle relaxation, lifestyle modifications and walking-based exercise.

TO EVALUATE THE DIAGNOSIS AND TREATMENT OF EARLY GASTRIC CANCER AT HUE CENTRAL HOSPITAL Objectiva: to evaluate the diagnosis and treatment of early gastric cancer at Hue Central Hospital. Material and methods: A retrospective study on patients with suspected lesions on white light endoscopy were performed magnifying endoscopy at Hue Central Hospital from January 2017 to December 2018. Patients who were diagnosed with early cancer will be removed with ESD treatment if indicated and surveiled after 06 months of treatment. Results: We collected and analyzed data from 104 endoscopic patients who screening for early cancer by magnifying endoscopy, 24 patients were diagnosed with early cancer, 15 men, 9 women;12 patients were treated with the Endoscopic Submucosal Dissection (ESD) method from January 2017 to December 2018. There are no specific symptoms for early stomach cancer. Superficial depressed-type (0-IIc) and Differentiated type account for the highest proportion. The patients in the study were treated with ESD, had a hemorrhage patient in the procedure and had to intervene with Coagrasper Hemostatic Forceps, without perforation complications during and after the procedure. The Diagnosis before and after treatment is consistent. During follow-up, no patients were found to have recurrence or distant metastasis. Conclusion: Early gastric cancer can still be treated well with endoscopic techniques. Multi-disciplinary coordination is needed to develop early diagnosis and treatment of cancer. Abstract The study presents a new innovative approach to the accepted principle of agile enterprise management by using the method of rationalization of the structure of administrative and office work (DZA). Agile enterprise management focused on minimizing costs induces permanent necessity to look for new methods and techniques to achieve the assumed cost targets. Contemporary enterprises have access to a wide range of activities and instruments affecting the formation of costs while running their business. In connection with the above, a research question was posed in the study; how to implement the concept of agile enterprise management based on methods of rationalization of the structure of administrative and office work and lean management? The aim of the paper is to learn and evaluate the applied method of analysis of run times in the system of administrative and office work (DZA Dürchlaufzeitenanalyse) in the context of agile management of the enterprise. The study indicates the advantages and disadvantages of implementing this concept for the enterprise, thus the paper bridges the gap between theory and practice in the selection of instruments to realize of the concept of agile business management. The applied research method is literature studies and descriptive analysis in the field of application of the method of minimizing time of runs. Descriptive analysis was developed based on an example of the activity of a chemical industry enterprise operating in Poland. The research issues are important and up-to-date due to the constant need to maintain cost discipline in the enterprise and the implementation of innovative approaches to seek ways to reduce costs.

Object Even with good results, conventional disc operations may result in subsequent damage due to trauma. Endoscopic techniques have become the standard in many areas because of the advantages they offer intraoperatively and postoperatively in rehabilitation. The goal of this prospective study was to examine the expanded technical possibilities of full-endoscopic transforaminal and interlaminar resection of herniated lumbar discs in which the authors used newly developed optics and instruments. The focus was on questions of achieving sufficient decompression, as well as the advantages and disadvantages of the minimally invasive procedure. Methods Two hundred thirty-two patients underwent follow up for 2 years after undergoing surgery. In addition to general and specific parameters, the following measuring instruments were used: a visual analog scale, the German version of the North American Spine Society instrument, and the Oswestry Disability Index of low-back pain disability. Postoperatively 84% of the patients no longer had leg pain, and 12% had only occasional pain. The results of decompression were equivalent to those of conventional procedures. The incidence of traumatization was reduced. Epidural scarring was minimized. The recurrence rate was 6.0%. No serious surgical complications were observed. Resection of the herniated disc was technically possible in all cases in which the new instruments were used. Conclusions The authors view the aforedescribed techniques, which offer the advantages of a truly minimally invasive procedure, as a sufficient and safe supplementation and alternative to conventional procedures, when the appropriate indication criteria are heeded. The new endoscope with its 4.2-mm working channel and corresponding instruments significantly reduced the technical problems.

Introduction: Hypertension is a widely recognized public health problem worldwide. Its further complications lead to cardiovascular diseases, stroke, retinal and renal diseases. DASH diet has been recommended to control raised blood pressure among hypertensive patients. However; limited evidence exists for it in Asian countries. This study aimed to review published research articles on DASH diet for hypertension control. Methods: Studies on hypertension and DASH diet (N=54) were searched using PubMed Central from 2012 to 2019 including all countries. The search strategy consisted of keywords "hypertension OR raised blood pressure OR blood pressure" and "Dietary Approaches to Stop Hypertension OR DASH diet". Whereas; no limitation was used for searching the literature and further references of the relevant studies were also analyzed. Full text articles of 31 researches were retrieved and analyzed for this review article. Results: Findings of this review suggests that DASH diet is significantly effective in lowering the blood pressure as well as for prevention of cardiovascular diseases (CVDs). In addition, a low-sodium diet is beneficial for individuals and hypertensive patients in reducing CVD related events. However; evidence suggests that adherence to DASH diet for a longer duration is effective along with lifestyle modifications in the population. Conclusion: Consuming a DASH diet as recommended can be a useful preventive measure to reduce blood pressure. Country-specific dietary recommendations are thus required. DASH diet along with pharmacological therapy and lifestyle modifications are proven to be effective. Further, longitudinal studies establishing temporal associations between consumption of DASH diet and its effect on blood pressure will be essential for clinical and public health practice.

Polycystic Ovarian Disease (PCOD) is a reproductive metabolic disorder caused by hormonal imbalances within women of fertile age. Mainstay for treating patients with PCOD includes pharmacological therapy and lifestyle modifications. Lifestyle modifications that play a key role in the management of PCOD are weight management, reduction in stress, physical activity, body mass index and dietary changes. There is sparse information regarding the impact of these interventional parameters among the PCOD women in the literature. This study aims to determine the impact of healthy lifestyle modifications in the management of PCOD among young adults and adolescents. It was a prospective interventional study conducted in the Gynaecology outpatient department in Government General Hospital, Kadapa over the time of 6 months from June 2019 to November 2019. Counseling on lifestyle changes and implementation of healthy lifestyle interventions were given to the study population by using the standard questionnaire forms and post counseling changes were collected after 90 days of visit to the clinic. The statistical significance was done by using the unpaired t-test and graph pad version 8.3.0. The total sample of 30 PCOD patients was compared to before patient counseling on lifestyle modifications and after follow-up. The total population was categorized into 2 groups as adolescents (10%) and young adults (27%). The current study showed that there is a significant association between dietary intervention (p <0.0113) and physical activity scores (p<0.029) among the PCOD patients. In PCOD patient’s healthy lifestyle modifications like dietary interventions and Physical activity serves as an effective treatment strategy adjunct to pharmacological therapy and improved the patient reproductive health.

Bio metric technology is now used for employees time management attendance system because it has ability to recognize peoples unique physiological characteristics. Because of multiple benefits of Bio metric attendance system it has gained high popularity in the market. Bio metrics attendance system is extremely useful in helping business community feel secure by eliminating employees time theft, as it relies on ones personal characteristics that vary between individuals. Since bio metrics characteristics cannot be duplicate, it prevents punching on behalf of a co-worker working in the same company. The manual practice of attendance system is time consuming and needs full time personal monitoring whereas bio metric attendance saves employee time, decreases staffing overhead and provides accurate labor data to payroll system to effectively manage business operations and thereby increase productivity. Moreover the bio metric attendance system has brought transparency and enhanced the work culture in the system. We will explore various advantages and disadvantages of implementation of the bio metric attendance system to a big organization, or small entrepreneur or unit and the necessary measures for its efficient implementation.
