Obesity remains a major public health problem due to its increasing prevalence. Natural products have become common as adjunct therapeutic agents for treating obesity and preventing metabolic diseases. Cocoa and its products are commonly consumed worldwide. Dark chocolate, a rich source of polyphenols, has received attention lately for its beneficial role in the management of obesity; however, conflicting results are still being reported. This scoping review aims to provide a comprehensive understanding of the existing literature on the relationship and effects of cocoa and dark chocolate intake among obese adults. We searched multiple databases for research investigating the consumption of cocoa and/or dark chocolate in managing obesity among adults. This review includes epidemiological and human studies that were published in English over the last 10 years. Our review of the current literature indicates that epidemiological and human trials with obese adults have shown inconsistent results, which may be due to the different populations of subjects, and different types of cocoa products and doses used for intervention. Studies among obese adults are mainly focusing on obese individuals with comorbidities, as such more studies are needed to elucidate the role of cocoa polyphenols in weight control and preventing the risk of chronic diseases among obese individuals without comorbidities as well as healthy individuals. Careful adjustment of confounding factors would be required. The effects of cocoa and dark chocolate intake on obese adults were discussed, and further research is warranted to identify the gaps.

The emergence in China of 2019 of severe acute respiratory syndrome coronavirus2 (SARS-CoV-2) previously provisionally names 2019-nCoV disease (COVID19) caused major global outbreak and is a major public health problem. On 30 January 2020, the WHO declared COVID19 to be the sixth international public health emergency. This present pandemic has engrossed the globe with a high rate of mortality. As a front line practitioner, physiotherapists are expected to be getting in direct contact with patients infected with the virus. That’s why it is necessary for understanding the many aspects of their role in the identification, contains, reduces and treats the symptoms of this disease. The main presentation is the involvement of respiratory system with symptoms like fever, cough, sore throat, sneezing and characteristics of pneumonia leads to ARDS(Acute respiratory distress syndrome) also land up in multiorgan dysfunction syndrome. This text describes and suggests physiotherapy management of acute COVID-19 patients. It also includes recommendations and guidelines for physiotherapy planning and management. It also covers the guidelines regarding personal care and equipment used for treatment which can be used in the treatment of acute adult patients with suspected or confirmed COVID-19.

AbstractObjectiveTo evaluate the awareness and knowledge about weight status and its management.DesignA 1 d cross-sectional survey. Basic anthropometric assessments (weight, height, BMI and waist circumference) and a self-administered questionnaire were considered.SettingNineteen Clinical Nutrition or Endocrinology and Metabolic Disorders Units or Dietetics Services in the Italian region of Lombardy.SubjectsAll adults attending the ‘Obesity Day’ initiative.ResultsA total of 914 participants (605 female and 309 male) were recruited. Although most of the participants (83·5 %) considered obesity to be a disease, 38·5 % were likely to misperceive their weight status. In particular, 38·8 % of normal-weight adults believed themselves to be overweight, whereas 71·1 % and 37·5 % of classes I and II/III obese adults classified themselves as being overweight and mildly obese, respectively. However, most of the overweight (90·2 %), mildly (96·8 %) and moderately/severely obese adults (99·1 %) recognized the need to lose weight. In all, 37·8 % of the sample underestimated the role of physical activity in weight management. Interestingly, only 17·2 % of dieters (previous or current) declared being advised by their doctor to lose weight. Multivariate models revealed that higher age, low education and higher BMI were important determinants of poor weight control and management. In addition, previous dieting appeared not to provide better knowledge, whereas the role of physical activity was recognized mainly by those practising it.ConclusionsThe present study suggests that in Italy knowledge about weight management should be improved not only in the general population but also among health-care professionals. To confirm this finding, there is now the rationale for a nationally representative survey. New educational programmes can be designed on the basis of the information collected.

Opioid use disorder is a major public health problem, and opioid replacement therapy with buprenorphine (BPN) is a clinically effective and evidence-based treatment for it. To deter misuse of the tablet through the injecting route, BPN coformulated with naloxone (BNX) in 4:1 ratio is available in many countries. Despite this, significant diversion and injecting use of the BNX combination has been reported from across the world. In this article, the pharmacological properties of BPN and BNX and the evidence for their diversion are reviewed. Also, a critical examination is made of the evidence supporting the role of naloxone in reducing the agonist effects of BPN when used through the injecting route. Based on this evidence, a hypothesis explaining the continued diversion of BNX has been proposed.

Introduction: Pulmonary embolism (PE) is the third most common acute heart disease after myocardial infarction and stroke and is a major public health problem. PE is a disease that has high morbidity and mortality, yet it is challenging to obtain a diagnosis. Objective: The study aims to assess the role of MDCT-PA in the diagnosis of pulmonary embolism, and it's associated conditions in suspected cases of pulmonary embolism. Materials and Methods: The study was conducted in the department of radiology in KRISHNA INSTITUTE OF MEDICAL SCIENCES DEEMED TO BE UNIVERSITY situated in KARAD equipped with a 16 slice MDCT scanner (EMOTION) manufactured by Siemens. Results: Through the study period of one and half year and have met the inclusion criteria, a total of 90 consecutive patients who presented with suspicion of PE were referred for MDCT-PA in the department of radio-diagnosis, Krishna Hospital, Karad. A review of all the cases was done, and the results were presented in the form of tables. Conclusion: Multidetector computed tomography pulmonary angiography can be used to confidently diagnose pulmonary embolism in which it is not contraindicated. MDCT-PA is the investigation of choice because it is a rapid, non-invasive study, highly sensitive and specific.

Introdução: Atualmente, a obesidade é considerada um grave problema de saúde pública, pois além de ser uma doença, é um fator de risco para progressão e desenvolvimento de outras doenças crônicas não transmissíveis. Objetivo: Compreender a percepção de indivíduos com obesidade quanto à participação em papeis ocupacionais, grau de importância atribuído a cada papel e padrão de desempenho de papeis ocupacionais ao longo da vida. Método: Configura-se como um estudo transversal do tipo exploratório de caráter descritivo, com análise quantitativa dos dados. Como instrumento, utilizou-se a Lista de Identificação de Papéis Ocupacionais. Resultados: A amostra foi composta por 65 indivíduos com obesidade, 87,7% (n=57) era do sexo feminino e 12,3% (n=8) do sexo masculino, tendo como média de idade 42 anos e 9 meses. Constatou-se que o papel mais comum no passado foi o de estudante e com relação ao presente e futuro, os papeis mais citados foram os de serviço doméstico e membro da família. Quanto ao grau de importância, identificou-se que os indivíduos valorizam os papeis ocupacionais, atribuindo maior valorização aos papeis de membro da família e trabalho, e menor proporção de valor ao papel de participação em organizações. Conclusão: A condição de obesidade pode interferir no modo como o indivíduo se envolve e desempenha suas ocupações, evidenciando a terapia ocupacional como uma estratégia importante no enfrentamento da obesidade. AbstractIntroduction: Currently, obesity is considered a serious public health problem, since it is also a disease, it is a risk factor for progression and development of other chronic non-communicable diseases. Objective: To understand the perception of individuals with obesity regarding the participation in occupational roles throughout life, about the degree of importance attributed to each role, and in a complementary way to identify the performance pattern of occupational roles in Long life. Method: It is configured as a cross-sectional exploratory study of descriptive character, with quantitative analysis of the data, as an instrument used the identification list of occupational roles. Results: The sample consisted of 65 obese individuals. Of these, 87.7% (n = 57) were females and 12.3% (n = 8) were males, with a mean age of 42 years 9 months. It was found that the most common role in the past was that of student and in relation to the present and future the most cited roles were those of domestic service and family member. As to the degree of importance, it was identified that the individuals value the occupational roles, attributing greater value to the roles of family members and work, and less appreciation of the role of participation in organizations. Conclusion: The condition of obesity can interfere in the way the individual engages and performs his occupations, thus placing the occupational therapy professional as an important member in coping with obesity.Key words: Obesity, Activities of Daily Living, Occupations, Occupational Therapy.ResumenIntroducción: Actualmente, la obesidad se considera un grave problema de salud pública, ya que es también una enfermedad, es un factor de riesgo para la progresión y el desarrollo de otras enfermedades crónicas no transmisibles. Método: se configura como un estudio exploratorio transversal de carácter descriptivo, con análisis cuantitativo de los datos, como instrumento utilizó la lista de identificación de roles ocupacionales. Resultados: la muestra consistió en 65 individuos obesos. De ellos, 87,7% (n = 57) fueron hembras y 12,3% (n = 8) fueron varones, con una edad media de 42 años 9 meses. Se descubrió que el papel más común en el pasado era el de los estudiantes y en relación con el presente y el futuro, los papeles más citados eran los del servicio doméstico y el miembro de la familia. En cuanto al grado de importancia se identificó que los individuos valoran los papeles ocupacionales, atribuyendo mayor valoración a los papeles de miembro de la familia y trabajo, y menor valorización al papel de participación en organizaciones. Conclusión: la condición de la obesidad puede interferir en la forma en que el individuo se involucra y realiza sus ocupaciones, colocando así al profesional de la terapia ocupacional como un miembro importante para hacer frente a la obesidad.Palabras clave: Obesidad, Actividades Cotidianas, Ocupaciones, Terapia Ocupacional.

Dengue is a major public health problem in Indonesia. The program of elimination of mosquito breeding places is still low. This study was aimed to analyze the effectiveness of village surveillance officer for decrease container index. Study design was quasy experiment. The intervention included: community workshops; community involvement in clean-up campaigns; and distribution of information, education and communication materials in the village surveillans officer. Data were analyzed with t test, and path way analyzed. There is significantly differences knowdlege, attitude, behaviour, sanitation, and container index p<0.05. Based on the path analysis was concluded that village surveillance officer increased knowledge, attitude, behavior, sanitation and container index.Surveillance village officer is concluded to be more effectively decreased of larva index through comunity behavior. Surveillance village officer is important because it effectively the coverage of larva index through community behaviour participation.

Cytomegalovirus infection is a major public health problem, because annually there are born increasingly more children with this infection. Numerous studies have shown that cytomegalovirus infection is an important cause of hearing loss. Cytomegalovirus infection specific symptoms present at birth are a strong predictor for hearing loss, even in populations with low maternal seroprevalence rate. The severity of deafness due to cytomegalovirus infection is variable. Pathophysiology of deafness caused by cytomegalovirus infection is not well known, the mechanisms possible involved being impaired endolymphatic structures, cytopathic effect of the virus and the host response to the inner ear structures. The relationship between viral load and increased likelihood of deafness in infants supports the role of antiviral therapy in decreasing the incidence and the severity of deafness caused by cytomegalovirus. We consider that monitoring children with congenital cytomegalovirus infection should include also hearing monitoring.

Current COVID-19 has become a major public health problem because of its pandemicity, with wide spectrum of disease manifestation. SARS-COV-2 can have a varied clinical manifestation ranging from asymptomatic, mild symptomatic to severe disease like acute respiratory distress syndrome, cytokine storm, and multiorgan dysfunction. It has been described in literature that cytokine storm/hyperinflammation arises as result of dysregulated immune response leading to excessive release of various cytokines which causes multiorgan dysfunction. But there is paucity of literature describing the immune response and hyperinflammation in mild disease which may cause unremitting symptoms. Here we describe a case series of three patients with mild disease, who had persistent symptoms beyond 1 week and managed with low dose steroid after confirming it to be hyperinflammation. So it is imperative to detect the hyperinflammatory phase to halt the disease progression. Also we have discussed the role of immune system and role of steroid in COVID-19.

<p class="abstract">Osteoporosis is a major public health problem, affecting many individuals. The aging population will require problem related prevention, education, and treatment to maintain bone density and reduce the bone fractures and falls. Inadequate intake of nutrients increases the risk for bone loss and subsequent osteoporosis. Protein, calcium and vitamin D are vital nutrients for optimal bone health. Adequate calcium is essential for bone maintenance. There are several other vitamin and mineral needed for metabolic processes related to good bone health, including manganese, copper, iron, zinc, vitamin A, vitamin K, vitamin C and the B vitamins. Related macro- and micronutrients play an important role in bone mass integrity and quality. Adequate nutrition for older adults needs to be encouraged to promote and maintain bone health.</p>

Hepatitis C virus (HCV) is a major public health problem. HCV is a hepatotropic and lymphotropic virus that leads to hepatocellular carcinoma (HCC) and lymphoproliferative disorders such as cryoglobulinemic vasculitis (CV) and non-Hodgkin’s lymphoma (NHL). The molecular mechanisms by which HCV induces these diseases are not fully understood. MicroRNAs (miRNAs) are small non-coding molecules that negatively regulate post-transcriptional gene expression by decreasing their target gene expression. We will attempt to summarize the current knowledge on the role of miRNAs in the HCV life cycle, HCV-related HCC, and lymphoproliferative disorders, focusing on both the functional effects of their deregulation as well as on their putative role as biomarkers, based on association analyses. We will also provide original new data regarding the miR 17-92 cluster in chronically infected HCV patients with and without lymphoproliferative disorders who underwent antiviral therapy. All of the cluster members were significantly upregulated in CV patients compared to patients without CV and significantly decreased in those who achieved vasculitis clinical remission after viral eradication. To conclude, miRNAs play an important role in HCV infection and related oncogenic processes, but their molecular pathways are not completely clear. In some cases, they may be potential therapeutic targets or non-invasive biomarkers of tumor progression.
