Chronotype (CT) has been associated with predisposition to chronic noncommunicable diseases (CNCDs), such as diabetes mellitus and obesity. However, the effects of CT on individuals assisted by public health systems (PHSs) in middle-up economies are still poorly explored. The objective of this study was to evaluate the relationship between CT and clinical, sociobehavioral and nutritional aspects in adults assisted by a PHS in Brazil. This is a population-based cross-sectional study. The sample consisted of 380 individuals, selected through probabilistic sampling by clusters, in all health units in a city of approximately 100 thousand inhabitants. Data collection was performed during home visits, by means of general and nutritional interviews, anthropometric measurements and the Morningness–Eveningness Questionnaire (MEQ). Statistical analysis comprised chi-square test and principal component analysis (CPA) followed by Fisher’s discriminant analysis to determine aspects associated with each CT (morning, evening or intermediate). With the aim of explaining the variation in the CT scores, the consumption of micronutrients (corrected to the total energy intake) and other individual and sociodemographic variables were used as explanatory factors in the adjustment of a linear regression model. The morning group was characterized by older men, with less than eight years of schooling, with low body mass index (BMI) and with low intake of omega-6, omega-3, sodium, zinc, thiamine, pyridoxine and niacin. The evening group, on the other hand, was composed of younger individuals, with a high consumption of these same nutrients, with high BMI and a higher frequency of heart diseases (p < 0.05). It was concluded that most morning CT individuals were elderly thin males with lower consumption of omega-6 and -3, sodium, zinc, thiamine, pyridoxine and niacin, whereas evening individuals were younger, had higher BMI and had higher consumption of the studied micronutrients. The identification of circadian and behavioral risk groups can help to provide preventive and multidisciplinary health promotion measures.

AbstractLong-Chain Polyunsaturated Fatty Acid (LCPUFA) is essential throughout pregnancy, since deficiency of LPUFA may linked to obstetrical complications. This study aimed to investigate LCPUFA status in severe preeclampsia and preterm birth. A cross sectional study was conducted in 104 pregnant women, which divided into normal pregnancy, severe preeclampsia and preterm birth groups. Serum percentage and concentration of total LCPUFA, omega-3, alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), omega-6, linoleic acid (LA), and arachidonic acid (AA) were measured using gas chromatography/mass spectrometry. Receiver operating characteristic (ROC), bivariate and multivariate analysis were performed. Severe preeclampsia showed the highest concentration of total PUFA and the lowest DHA percentage, with significantly higher Omega-6/Omega-3 ratio (p = 0.004) and lower omega-3 index (p < 0.002) compared to control. Preterm birth showed the least omega-3 concentrations, with significantly low omega-6 derivates (LA (p = 0.014) and AA (p = 0.025)) compared to control. LCPUFA parameters have shown to increase the risk in both conditions, particularly ALA ≤ 53 µmol/L in preeclampsia with OR 5.44, 95%CI 1.16–25.42 and preterm birth with OR 4.68, 95%CI 1.52–14.38. These findings suggest that severe preeclampsia and preterm birth have an imbalance in LCPUFA status.

Global morbidity associated with noncommunicable diseases (NCDs) has increased over the years. In Thailand, NCDs are among the most prevalent of all health problems, and affect both Thai citizens and non-Thai residents, such as expatriates. Key barriers to NCD health service utilization among expatriates include cultural and language differences. This study aimed to describe the situation and factors associated with NCD service utilizations among expatriate patients in Thailand. We employed a cross-sectional study design and used the service records of public hospitals from the Ministry of Public Health (MOPH) during the fiscal years 2014–2018. The focus of this study was on expatriates or those who had stayed in Thailand for at least three months. The results showed that, after 2014, there was an increasing trend in NCD service utilizations among expatriate patients for both outpatient (OP) and inpatient (IP) care. For OP care, Cambodia, Laos PDR, Myanmar, and Vietnam (CLMV) expatriates had fewer odds of NCD service utilization, relative to non-CLMV expatriates (p-value < 0.001). For IP care, males tended to have greater odds of NCD service utilization compared with females (AdjOR = 1.35, 95% CI = 1.05–1.74, p-value = 0.019). Increasing age showed a significant association with NCD service utilization. In addition, there was a growing trend of the NCD prevalence amongst expatriate patients. This issue points to a need for prompt public health actions if Thailand aims to have all people on its soil protected with universal health coverage for their well-being, as stipulated in the Sustainable Development Goals. Future studies that aim to collect primary evidence of expatriates at the household level should be conducted. Additional research on other societal factors that may help provide a better insight into access to healthcare for NCDs, such as socioeconomic status, beliefs, and attitudes, should be conducted.

Objective: This study aimed to evaluate knowledge and attitude toward medication error (ME) among pharmacists working in public health care institutions. Methods: A cross-sectional study was conducted among pharmacists working in public health care institutions. Respondents were randomly recruited from 5 hospitals and 25 primary healthcare clinics in the state of Pahang, Malaysia. A set of self-administered questionnaires was used to assess their knowledge and attitude, distributed as a web-based survey. Knowledge and attitude toward ME reporting were assessed using five-point Likert-scale. This study was conducted between May and July 2019. Results: A total of 186 respondents participated in the study. A majority of respondents were female (n = 144). About 90% of the respondents had good score on knowledge on ME. Only 25.4% of the respondents had favorable attitude toward ME reporting. Female pharmacists ( P = .001), more experienced pharmacists ( P = .012) and those working in primary health clinics ( P = .014) were associated with more favorable attitude. Knowledge did not correlate well with attitude toward ME reporting (r = 0.08, P = .29). Conclusion: Despite having good knowledge on ME, the attitude toward ME reporting was still very poor among the pharmacists.

Fatty acids have a great metabolic and structural importance. Evaluation of fatty acid composition of peppers is still incomplete. Pulps and seeds from six varieties of the genus Capsicum were evaluated in this work with respect to their contents in fatty acids. A total of 25 different fatty acids, including some with odd number of carbons were identified in the samples. The most abundant fatty acids were palmitic (16:0), oleic (18:1n-9) and linoleic (18:2n-6) acids. The polyunsaturated:saturated fatty acid (PUFA/SFA) ratios for all peppers were high due to the elevated amounts of polyunsaturated acids, particularly linoleic acid. In the pulps, the omega-6/omega-3 ratios ranging from 1.28 to 4.33, were relatively adequate if one considers that ratios between 0.25 and 1.0 in the human diet are regarded as highly appropriate. In the seeds, the levels of omega-3 were very low whereas the levels of omega-6 were high, leading to very inadequate omega-6/omega-3 ratios ranging from 74.2 to 279.6. Principal component analysis (PCA) explained 93.49% of the total variance of the data. Considering the PUFA/SFA ratio and omega-6/omega-3 ratio, our data suggest that, among the peppers of the genus Capsicum evaluated in this work, the bell pepper and orange habanero pepper present the best nutritional characteristics concerning fatty acid composition.

Introduction: Fast Food (FF) consumption reduces the nutritional quality of daily diet. FF is even more detrimental to human health, if preferred during night-time meals, because metabolism is slower at end of the day. Quality of food and time of eating are nutritional determinants of the health. Aim: To study the association of anthropometric and societal factors such as age, Body Mass Index (BMI), health awareness with frequency of FF and preference for meal timings. Materials and Methods: Through a cross-sectional Google-form food preference survey of 2887 people, it was sought to uncover a possible association between the perception and frequency of FF and preference for mealtimes. Unadjusted associations of people’s eating preferences with age, BMI and health awareness with FF intake was analysed using Spearman’s correlation coefficients, Cronbach’s α, Eigen values, odd ratios, relative risk factors and χ2-tests. Results: The high odd ratio revealed greater FF popularity in adolescents and children as compared to adults. Principal component analysis revealed four important factors (Eigen value >0.9; factor weight >12%) viz., age, busy life, body weight and weekend drive. Taking FF as a food quality index, there was food quality jetlag between week days and weekends. Psychometric analysis revealed a positive association between preferred mealtime and FF intake. Conclusion: Mealtimes regulate the human circadian system; therefore, health consequences of FF consumption ensconce other determinants of public health like night eating and lifestyle. The deteriorating effects of FF are associated with circadian disruption. More research is needed to highlight associativity of different lifestyle factors detrimental to circadian health.

Background: Hypertension is one of the major risk factors for cardiovascular disease. Decreased intake of omega-3 and increased intake of omega-6 can increase the risk of hypertension. Hypertension cases in Indonesia are highest in women especially aged 30 - 50 years. This study aimed to determine correlation of omega-3 and omega-6 intake with blood pressure in women aged 30-50 years.Method : This was an observational research with cross-sectional study design. Fifty four subjects were selecting using consecutive sampling. Intakes were assessed by food recall 2x24 hours. Blood pressure levels were measured by Sphygmomanometer. Data were analyzed by Chi Square and Fisher Exact.Result : Systolic and diastolic blood pressure mean were 115.92 ± 14.5 mmHg and 75 ± 7.45 mmHg, while omega-3 and omega-6 mean were of 3. 1.03 ± 0.52 g and 14.17 ± 5 , 8 g. More than one third of the subjects (37.9%) with pre-hypertension/hypertension had omega-3 intake less than 1.1 g and 52.7% of them had omega-6 intake less than 12 g.There was correlation of omega-6 intake with systolic blood pressure (p <0.05) but there were no correlation between omega-3 intake with systolic blood pressure, omega-3 and omega-6 intake with diastolic blood pressure (p >0,05). Conclusion: There was correlation of omega-6 intake with systolic blood pressure, there were no correlation between omega-3 intake with systolic blood pressure, omega-3 and omega-6 intake with diastolic blood pressure

Background and aims: Behavioral risk factors of noncommunicable diseases (NCDs) are established during early age and continued into adulthood. In Bangladesh, the scenario of NCD risk factors among students was inadequately studied. The aim of this study was to assess and compare the prevalence of behavioral risk factors of NCDs between undergraduate medical and nonmedical students. Methods: This cross-sectional study was conducted among 280 undergraduate students (equal number of students from medical and nonmedical backgrounds). Respondents were selected purposively from 5 purposively-selected institutions. Modified WHO STEPS instrument was used for data collection. Respondents were asked about their behavioral risk factors (tobacco use, insufficient fruit and vegetable intake, inadequate physical activity, and alcohol consumption) by face-to-face interviews. Results: Men students were more in both groups. The mean age of the medical and nonmedical students was 22.1±2.0 and 21.8±1.9 years, respectively. Tobacco use was more among nonmedical students than that among the counterpart (22.1% vs. 15.7%). Medical students were more used to take insufficient fruits and vegetables compared to nonmedical students (97.9% vs. 93.6%). Equal proportions (71.4%) of students in both groups were used to perform inadequate physical activity. Alcohol consumption was observed more among nonmedical students (12.9% vs. 8.6%). Conclusion: Behavioral risk factors of NCDs were remarkable among students of both groups, mainly among nonmedical students.

Abstract Background High omega-6/omega-3 intake ratio in westernized world is of concern. FADS genes variants are associated with plasma long-chain polyunsaturated fatty acids (LC-PUFA) in diverse ethnicities and might modulate plasma omega-6/omega-3 net balance. Therefore, the objective of this study was to evaluate the relationships between FADS genetic variants with dietary fat and macronutrient intake, plasma phospholipid fatty acid profile, estimated plasma desaturase activity and cardiometabolic risk factors, in a sample of Serbian subjects.Methods Non-smoking adult volunteers (>28 years), free of acute or chronic disease were included. Food and nutrient data were compiled through 24h recalls for non-consecutive days. Plasma phospholipid fatty acid content was assessed by gas-chromatography. Selection of FADS2 variants (rs174593, rs174616 and rs174576) was based on its positional and functional aspect, and evidence-based data. Genotyping was performed by using Real-Time PCR. Estimated desaturase activities were calculated as conversion rates towards LC-PUFA in omega-6 pathway. Multivariable-adjusted general linear were applied and the contribution of minor alleles to the variability of physiological parameters was analyzed by multivariable hierarchical multiple regression models.Results Sample included 34 men and 54 women, mean age=40±7years, with 70% being overweight (BMI>25). Minor allele frequencies were 33%, 36% and 51% for rs174593, rs174576 and rs174616, respectively, in line with other populations. None of the three variants was associated with food or nutrient intake, serum lipids, or obesity (p>0.05). Irrespective of gender, age, total daily polyunsaturated/saturated fatty acid intake and obesity, rs174593, rs174616 and rs174576 were associated with lower arachidonic acid (AA, C20:4 n-6, p<0.001) and estimated desaturase-5 activity (p<0.001) in plasma phospholipids. The rs174576 associations with AA withstood multiple testing and additional adjustments for other variants.Conclusion We observed inverse associations between FADS2 variants and plasma phospholipid AA but not omega-3 fatty acids in a sample of Serbian adults, and larger cohorts should confirm the associations. In our study FADS2 rs174576 exhibited the strongest associations, and future gene-dietary studies with varying omega-6/omega-3 intake should validate its suitability for precision nutrition strategies aimed at PUFA recommendations in Serbian population.Trial registration This is a cross-sectional study, forming part of larger intervention study registered at ClinicalTrials.gov as NCT02800967.

Abstract Objectives There has been evidence suggesting the effect of omega-3 fatty acids (FA) in promoting and preserving lean body mass. Western diets are deficient in omega-3 FA, while having excess amounts of omega-6 FA. A high omega-6/omega-3 FA ratio promotes the pathogenesis of several diseases, whereas an omega-6/omega-3 FA ratio suppresses inflammation. The objective of this study was to report on omega-3 FA supplementation and lean body mass (LBM) among athletes. Methods Female and male athletes who were 18 years of age and older were recruited for this cross-sectional study. Body weight (kg) and height (cm) were measured using a SECA scale. LBM (kg) was measured using a bio-electrical impedance analyzer. Dietary consumption data were measured using a Block Food Frequency Questionnaire. A univariate analysis of variance was used to report the interaction between omega-3 FA supplementation and LBM. In addition, a linear regression was used to analyze the omega-6/omega-3 ratio and LBM. The significance level was set a priori at P &lt; 0.05. Results A total of 300 athletes (145 women and 155 men) were included in the study. Participants were 34.76 ± 11.5 years of age, with an average LBM of 57.19 ± 11.24 kg. The average daily consumption of omega-6 and omega-3 FA were 14.58 ± 6.21 g and 1.56 ± 0.75 g, respectively. The average ratio of omega-6/omega-3 was 9.56 ± 1.90 (a ratio of £3). Of the 300 athletes, 98 chose to supplement their diet with omega-3 FA. There was a positive interaction between omega-3 FA supplementation and LBM (P = 0.007). There was no significant relationship between omega-6/omega-3 FA ratio and LBM. Conclusions We found a positive interaction between omega-3 FA supplementation and LBM. However, these athletes did not have a favorable omega-6/omega-3 FA ratio, and hence, may be susceptible to inflammatory responses and other chronic diseases. Therefore, it may be beneficial to include servings of foods and supplements that are high in omega-3 FA to mitigate these effects. Intervention studies should further investigate the potential of a diet rich in omega-3 FA in preserving LBM in athletes. Funding Sources This project was unfunded.
