Aim: The aim of this study was to develop, implement and evaluate a Quality Assurance (QA) programme for use by nine rehabilitation departments in the Midlands province of Zimbabwe.Methodology: A participatory methodology was implemented to:1. Define and implement the parameters of quality for a rehabilitation service.2. Design standardised assessment forms for the different conditions managed.3. Develop, pilot and evaluate a QA measuring instrument to use in defining the QA profile of a rehabilitation service, and4. Determine patient satisfaction with the rehabilitation service. Administering a simple questionnaire, whose design was based on the Zimbabwe Patients Charter, accomplished this.Results: The process of developing, implementing and evaluation of the QA programme took six years. One year after implementation of the QA, all (J00%) patients coming into contact with the service had a rehabilitation record. The QA profile for the Midlands province rose from 55% in 1994 to a record high of 92.1% in 1998, and 89.4% in 1999. The patient satisfaction profile for each indicator also demonstrated a linear increase with areas such as satisfaction with service rising from 86% in 1997 to 100% in 2000, and satisfaction with benefit from the service rising from 76.6% in 1997 to 100% in 2000. Other qualitative benefits not depicted by the QA measuring instrument or the patient satisfaction instrument, but which were observed and expressed by rehabilitation staff as measures of success of the QA programme are discussed.Conclusion: This paper concludes that significant improvements in clinical documentation arising from the QA programme, translated to an overall increase in the service providers’ professional skills and knowledge base, and ultimately rehabilitation outcomes. The success of this process is attributed to its participatory and empowering nature.

Background/aims People undergoing inpatient rehabilitation largely spend their day by the bedside, inactive and alone. Increasing patients' physical, cognitive and social activity levels may improve rehabilitation outcomes for both the individual and the service. The aim of this study was to trial an activity programME that aimed to increase inpatient physical, cognitive and social activity levels, provided within existing resources whilE maintaining or increasing patient satisfaction. Methods A mixed-methods approach was used to develop and evaluate a multi-component activity programme for patients undergoing inpatient rehabilitation. Results Patient activity across the unit did not change but the amount of time spent alone during a weekday was significantly reduced. Patient satisfaction levels remained high. Each new therapy group was well received by patients and the volunteers who supported implementation. Novel programmes included table tennis coaching and independent gym access for selected inpatients, which were well used and not resource intensive. Therapist prescription of independent practice programmes remained low. Broader initiatives such as patient education folders require revision and further investment to succeed. Conclusions It is possible to change and evaluate multiple therapy services concurrently. The absence of change in overall activity levels demonstrates a need to establish the most successful components, harness support from the organisation to address some of the physical/environmental barriers and expand smaller programmes, and invest more time to consolidate the whole-of-service changes required to push the rehabilitation service in the direction of greater patient activity and ownership. Clinicians should pursue, implement, evaluate and revise novel programmes to engage their patient population.

Abstract Background Home rehabilitation is a growing rehabilitation service in many countries, but scientific knowledge of its components and outcomes is still limited. The aim of this study was to investigate; 1) which changes in functioning and self-rated health could be identified in relation to a home rehabilitation program in a population of community-dwelling citizens, and 2) how socio-demographic factors, health conditions and home rehabilitation interventions were associated to change in functioning and self-rated health after the home rehabilitation program. Method The sample consisted of participants in a municipal home rehabilitation project in Sweden and consisted of 165 community-dwelling citizens. General Linear Models (ANOVA repeated measures) was used for identifying changes in rehabilitation outcomes. Logistic regressions analysis was used to investigate associations between rehabilitation outcomes and potential factors associated to outcome. Result Overall improvements in functioning and self-rated health were found after the home rehabilitation program. Higher frequencies of training sessions with occupational therapists, length of home rehabilitation, and orthopaedic conditions of upper extremities and spine as the main health condition, were associated with rehabilitation outcomes. Conclusion The result indicates that the duration of home rehabilitation interventions and intensity of occupational therapy, as well as the main medical condition may have an impact on the outcomes of home rehabilitation and needs to be considered when planning such programs. However, more research is needed to guide practice and policymaking.

This study utilized a new type of detector, the CROSS II (Liverage Biomedical Inc., Taiwan), to perform a beam quality assurance (QA) procedure on a Sumitomo (Sumitomo Heavy Industries, Inc., Japan) pencil beam linear scanning proton therapy machine. The Cross II can monitor proton Pristine Bragg peak range, beam width, beam size, beam position, and scanning speed. All the data presented here were collected during a time span of over one year. The accuracy of the QA program could be verified if all the QA items were tested stably and within the programmed tolerances. Our results showed that the proton range remained within the [Formula: see text] mm tolerance, with the majority of measurements within [Formula: see text] mm, [Formula: see text] mm for spot size, 1.5 mm for spot position, and [Formula: see text]% for scanning speed. We found that the CROSS II detector is in high precise and steady state with highly efficient. Our proton therapy system was also proven to be in an accurate and reliable condition according to our QA results.

Aims The primary aim of this study was to assess whether non-fatal postoperative venous thromboembolism (VTE) within six months of surgery influences the knee-specific functional outcome (Oxford Knee Score (OKS)) one year after total knee arthroplasty (TKA). Secondary aims were to assess whether non-fatal postoperative VTE influences generic health and patient satisfaction at this time. Methods A study of 2,393 TKAs was performed in 2,393 patients. Patient demographics, comorbidities, OKS, EuroQol five-dimension score (EQ-5D), and Forgotten Joint Score (FJS) were collected preoperatively and one year postoperatively. Overall patient satisfaction with their TKA was assessed at one year. Patients with VTE within six months of surgery were identified retrospectively and compared with those without. Results A total of 37 patients (1.5%) suffered a VTE and were significantly more likely to have associated comorbidities of stroke (p = 0.026), vascular disease (p = 0.026), and kidney disease (p = 0.026), but less likely to have diabetes (p = 0.046). In an unadjusted analysis, patients suffering a VTE had a significantly worse postoperative OKS (difference in mean (DIM) 4.8 (95% confidence interval (CI) 1.6 to 8.0); p = 0.004) and EQ-5D (DIM 0.146 (95% CI 0.059 to 0.233); p = 0.001) compared with patients without a VTE. After adjusting for confounding variables VTE remained a significant independent predictor associated with a worse postoperative OKS (DIM -5.4 (95% CI -8.4 to -2.4); p < 0.001), and EQ-5D score (DIM-0.169 (95% CI -0.251 to -0.087); p < 0.001). VTE was not independently associated with overall satisfaction after TKA (odds ratio 0.89 (95% CI 0.35 to 2.07); p = 0.717). Conclusion Patients who had a VTE within six months of their TKA had clinically significantly worse knee-specific outcome (OKS) and general health (EQ-5D) scores one year postoperatively, but the overall satisfaction with their TKA was similar to those patients who did not have a VTE. ‘Skipped generation’ families, consisting of grandparents and grandchildren with parents absent from the home, are frequently served in social work agencies. These families have unique multigenerational patterns and family structures that are important for service providers to recognize. This paper uses a multigenerational systems perspective to highlight the diversity among grandparent-headed households. Twenty families who were previously part of a larger study of stress, well-being, and life satisfaction among caregiving grandparents constituted a follow-up case study involving videotaped family interviews one year after the first study. Three families representing the range of diversity among the twenty are described with accompanying genograms. Differences in structure, interactional processes, and links with prior generations are identified in each case. These examples reveal the strengths and vulnerabilities, as well as the diversity, of grandparent-headed families.

Background and purpose: Patient satisfaction, including on radiography services, is an important component of health care provision. Surveys on patient satisfaction have been carried out particularly in government hospitals, but a more detailed study to determine the relationship between patient satisfaction with the responsiveness of service providers has yet to be carried out. This study aims to determine the relationship between the six domains of responsiveness of service providers and outpatient satisfaction at three conventional radiography facilities in Denpasar.Methods: Study was conducted by cross-sectional survey of 158 respondents selected consecutively in three conventional radiography services, namely a government hospital, a private hospital and a private radiology clinic. Data were collected through interviews using modified questionnaires on responsiveness from WHO. Data were analyzed by univariate, bivariate (chi-square test) and multivariate logistic regression method.Results: Univariate analysis indicated that patient satisfaction was 69% but still below the minimum service standards (>80,0%). Multivariate analysis showed that factors associated with satisfaction were the domain of attention (AOR= 3.77; 95%CI: 1.62-8.76), the quality of infrastructure (AOR=4.57; 95%CI: 1.61-12.93) and communication (AOR=6.30; 95%CI: 1.75-22.64).Conclusion: Patient satisfaction was generally still below the standard. Domains of responsiveness associated with patient satisfaction were the quality of infrastructure, level of attention and communication skills of service providers.

ABSTRACT Excellent service is the attitude or way of service providers who play a major role in creating satisfaction for patients, so the hospital's effort to improve the quality of service and patient satisfaction is to implement service excellence. Factors that must be applied by nurses in providing excellent service, including abilities, attitudes, appearance, attention, action and accountability. This study aims to determine the description of excellent service nurses based on patient perceptions in the Talaga Bodas inpatient (Class 3 Male Disease) Kindergarten II Hospital 03.05.01 Dustira Cimahi City in 2019. The study design was an analytic survey. The total sample of 149 respondents using purposive sampling technique with inclusion and exclusion criteria. Data collection was performed using a Likert scale questionnaire method. Analysis of data through two stages, namely univariate to see the frequency distribution. The results showed as many as 80 respondents (53.7%) felt excellent service. It is recommended that each inpatient room has a standard operating procedure standard (SOP) so that all nurses know the steps - good steps to perform excellent service to increase patient satisfaction. Abstract.Introduction: Patient satisfaction is an essential indicator for assessment of the quality of care and there is evidence of its correlation with health outcomes. Satisfaction with health care is a multidimensional concept which considers aspects such as access, organization and patient-professional interactions. We believe that the nursing care, in particular, are critical in the health/disease process.Objectives: Validate a scale to assess the satisfaction of users with regard to nursing care, adapted from EUROPEP instrument and assess the satisfaction of users of the primary health care of the central region of Portugal.Methods: Cross-sectional study with a sample of 827 adult users (female 64.4%) with a mean age of 50,08±18,58 years. Data were collected through a questionnaire consisting of socio-demographic variables, the instrument EUROPEP (Ferreira, 1995) to assess satisfaction with primary health care and to assess satisfaction specifically with the nursing team we have created issues grouped in the help relationship, interpersonal and instrumental dimension. Internal consistency, reproducibility and content analysis were evaluated using the SPSS 23.0; considering the consistency acceptable to an α of Cronbach’s alpha > 0.70. The coefficient for each item is presented with a confidence interval of 95%.Results: In all dimensions of EUROPEP questionnaire, the highest percentage of satisfaction with the care was between "good" and "very good". The dimensions created to evaluate specifically the nursing care showed a Cronbach's alpha coefficient of α total of 0.972.Conclusions: These results suggest that the dimensions created to evaluate nursing care will be useful for research on the Portuguese population. User satisfaction is crucial to the quality and efficiency of care, requiring the commitment of all service providers on implementation of systematic management practices leading to satisfaction, giving particular attention to the continuous improvement of organizational processes.Keywords: Patient satisfaction; primary health care; nursing care, adult, PortugalResumo.SATISFAÇÃO DOS UTENTES DOS CUIDADOS DE SAÚDE PRIMÁRIOS COM OS CUIDADOS DE ENFERMAGEM – AMOSTRA DA REGIÃO CENTRO DE PORTUGALIntrodução: A satisfação dos doentes constitui um indicador indispensável para a avaliação da qualidade dos cuidados e há evidência da sua correlação com os resultados em saúde. Consideramos que os cuidados de enfermagem, em particular, são fundamentais no processo saúde/doença.Objetivos: Validar uma escala para avaliar a satisfação dos utentes face aos cuidados de enfermagem, adaptado do instrumento EUROPEP e avaliar a satisfação dos utentes dos cuidados de saúde primários da região centro de Portugal.Material e métodos: Estudo transversal, com uma amostra de 827 utentes adultos (maioria do sexo feminino 64,4%) com uma média de idade de 50,08±18,58 anos. Os dados foram recolhidos através de um questionário, constituído por variáveis sociodemográficas, o instrumento EUROPEP (Ferreira, 1995) para avaliar a satisfação com os cuidados de saúde primários e para avaliar a satisfação especificamente com a equipa de enfermagem elaboramos questões adaptadas do instrumento EUROPEP e agrupadas nas dimensões relação de ajuda, dimensão interpessoal e instrumental. A consistência interna, reprodutibilidade e análise de conteúdo foram avaliados com recurso ao SPSS 23.0; considerando a consistência aceitável para um de Cronbach > 0,70. O coeficiente para cada item é apresentado com um intervalo de confiança de 95%.Resultados: Em todas as dimensões do questionário EUROPEP, a maior percentagem de satisfação com os cuidados situou-se entre “boa” e “muito boa”. As dimensões criadas para avaliar especificamente os cuidados de enfermagem apresentaram um coeficiente de α de Cronbach total de 0,972.Conclusões: Estes resultados sugerem que as dimensões criadas para avaliar os cuidados de enfermagem serão úteis para a investigação na população Portuguesa. Objectives: One of the most essential needs of children with Autism Spectrum Disorder (ASD) is using rehabilitation services. However, these services are associated with numerous challenges. The present study aimed to identify the perceptions of the parents of children with ASD and rehabilitation service providers about the challenges of using rehabilitation services in Iran. Methods: This was a qualitative study. In total, 27 participants were selected from the parents of children with ASD and rehabilitation service providers to these children, based on purposive sampling method; we considered maximum variation and sampling continued until data saturation. The necessary data were collected by semi-structured and in-depth interviews. The interviews lasted from July to December 2020 and MAXQDA was used for data analysis. Results: The challenges of children with ASD in accessing and using rehabilitation services were identified and categorized into 6 main classes, as follows: the lack of a systematic support system; the lack of commitment to observing professional ethics; neglecting the structure and policy of rehabilitation; the lack of comprehensive ASD diagnostic and rehabilitation protocols; inadequate knowledge and attitude, and the lack of feasible access to services. Discussion: Policymakers, service providers, and managers must be aware of the challenges of rehabilitation services for ASD; subsequently, they are expected to pay special attention to this issue. This study provided evidence for the above-mentioned groups for accessing and using rehabilitation services among ASD. Besides, the obtained data could help to improve the services they receive and their living conditions; ultimately, they help improve their quality of life.

This perspective article provides an example of a study planned using guidelines for comprehensive rehabilitation outcomes research, an approach that is believed to give service providers meaningful evidence to support practice. This line of investigation has been guided by the World Health Organization's International Classification of Functioning, Disability and Health. The short title of a study under way is Move & PLAY (Movement and Participation in Life Activities of Young Children). The article briefly describes the conceptual model, provides guidelines on how indicators and measures are selected, alludes to the details of selected measures, and describes processes of preparing for data collection, including obtaining ethics approval, preparing data collection booklets, training assessors and interviewers, and sampling. The aim of this investigation is to gain a better understanding of the multiple child, family, and service factors associated with changes in mobility, self-care, and play of preschool children with cerebral palsy as a result of using this research method. Comprehensive rehabilitation outcomes research holds promise in providing evidence that supports the complexities of planning rehabilitation services with clients with chronic conditions, such as children with cerebral palsy.
