Contamination of currency notes by pathogenic microorganism is a public health concern. An improved understanding of the bacteriological quality of currencies would help us better understand the role of money as a medium of transmitting infections. This study investigates the bacteriological quality of Nigerian currencies circulating in Aminu Kano Teaching Hospital (AKTH) in Kano metropolis. A total of 128 Naira note samples containing 16 pieces of each denomination of 5, 10, 20, 50, 100, 200, 500 and 1000 were aseptically collected from different cash units of the hospital and subjected to standard microbiological methods for the enumeration and isolation of bacteria. The susceptibility of the bacterial isolates to antibiotics was investigated using disk diffusion method. The mean aerobic mesophilic bacterial count of the currency notes range between 3.10 x104 cfu/mlto 5.25 x104 cfu/ml. Different bacterial species were isolated which include E. coli, Klebsiella spp, Staphylococcus aureus and Coagulase negative Staphylococci, with S. aureus having the highest frequency of occurrence of 37 (46.2 %). Findings of antibacterial susceptibility test indicated that100% of the isolated Gram negative bacteria were susceptible to Meropenem, followed by susceptibility to Gentamycin and Ciprofloxacin. However, the isolates were found to be highly resistant to Augmentin (55%) and Cefuroxime (32%). On other hand, the Gram positive bacterial isolates were highly susceptible to Erythromycin (84%), Gentamycin(83%), Clindamycin (79%),and Oxicillin (78%) while few of the isolate showed resistance to Penicillin(24%), Oxacillin (22%), and Clindamycin, (16%). The study reveals that currency notes in circulation at AKTH may serve as vehicles for the spread of disease causing organisms. Cashless transactions and use of hand sanitizers is therefore recommended to reduce the risk of infection. Key words: Naira notes, Contamination, Hospital environment

This study investigated the prevalence and concentration of antibiotic residues in twenty brands of milk. Determination and quantification of antibiotic residues was done using High Performance Liquid Chromatography. Aerobic plate count was carried out and the associated bacteria isolated. Identification of bacteria was done using standard microbiological methods. Molecular characterization and identification of bacterial isolates was done using 16S rRNA gene sequencing method. Antibiotic sensitivity was carried out on bacterial isolates using disc diffusion method. Plasmid profile of drug resistant isolates was done using alkaline analysis method. Comparison of means was done using Analysis of Variance. Antibiotic residues were detected in 10 (50%) of the 20 milk brands analyzed. The residual levels of tetracycline, oxytetracycline and chlortetracycline ranged between 5ng/kg and 1569ng/kg while none of the samples had doxycycline residues. The prevalence of tetracycline residues in evaporated milk brands and powdered milk samples were 100 and 23 % respectively. The aerobic plate counts ranged from 2.5 × 102 to 6.5 × 102 CFU∕mL for evaporated milk and 2.5 × 101 to 6.0 × 101 CFU∕mL for powdered milk. A total of ten bacterial species were isolated and identified. Susceptibility result showed that 95% of the isolates were sensitive to erythromycin, cefuroxime and gentamycin; 12.5% were resistant to tetracycline and ampicillin, 10% were resistant to ofloxacin and 7.5% to doxycycline and penicillin. Five of the bacterial isolates were resistant to more than one class of antibiotics. Resistant isolates subjected to plasmid profiling had detectable plasmids with estimated sizes between 120bp and 1000bp. The obtained results provide evidence that the presence of antibiotic residues in evaporated and powdered milk is an indication that the public is exposed to the harmful effects of the residues.

Bacteria including the pathogenic species have been isolated from fomites, these organisms are sometimes multidrug resistant and are of public health concern. It is therefore important to isolate and identify potential bacterial pathogens associated with the internal surface of ladies handbags, in Umuahia, Abia state. One hundred and forty swabbed samples were collected from the ladies hand bags in different groups of individuals which include; Nurses, civil servants, students and market women. Also the handbags from which the samples were collected includes: Leather, Cotton, Nylon and Polyester and velvet handbags. The bags were swabbed with sterile swab sticks and inoculated on different types of culture media and incubated at 37o C for 24 hours. Bacterial isolates were identified using standard microbiological methods including biochemical tests before subjecting isolates to different antimicrobial sensitivity test that was carried out by disc diffusion method. The following bacteria were isolated from the internal surface of the handbags, Coagulase Negative Staphylococci 6(2.6%), Escherichia coli 36(15.7%), Klebsiella spp. 14(6.1%), Staphylococcus aureus 49(21.3%), Bacillus spp. 48(20.9%), Pseudomonas aeruginosa 5(2.2%), Proteus spp. 5(2.2%), streptococcus spp. 31(13.5), Micrococcus spp. 20(8.7%), Salmonella spp. 3(1.3%) and Enterococcus faecalis 13(5.7%). Most of the isolates were sensitive to levofloxacin, gentamicin, norfloxacin, ciprofloxacin and resistant to ampiclox, chloramphenicol and erythromycin. Potentially pathogenic bacteria resistant to multiple antibiotics can be spread by hand contact from ladies handbags. Keywords: Bacterial pathogens, ladies handbags, antibiogram

Background: At present, drug-resistant pathogens are considered one of the major increasing causes of morbidity and mortality around the world. The data on microorganisms' resistance assist define the best available treatment for patients. Therefore, this study aimed to screen the antimicrobial-resistant profile of different drugs in major clinical pathogens of urine, ear and wound infections. Methods: This study was conducted in Al-Shomali General Hospital, Southern Babil, Iraq from October 2019 to May 2020. Totally 67 clinical specimens obtained from the wound, urine, and ear discharge collected from hospitalized patients as well as 30 healthy individuals participate in this study. Then, the standard microbiological methods carried outperformed to the isolated and identified bacterial species. Antimicrobial susceptibility tests were performed using different antimicrobial discs by applying the Kirby–Bauer disc diffusion method. Results: Totally, 67 bacterial isolates were obtained from 44 (66%) female and 23 (34%) male patients. Staphylococcus aureus and E. coli were the most common predominant organisms. All isolates were showed a high rate of resistance to evaluated cephalosporins 100% and 87% to cefotaxime and ceftriaxone respectively, while very low resistance recorded in Aminoglycosides 22% and 12% to Gentamicin and amikacin, respectively. Conclusion: These results suggest a constant screening for the detection of antibiotic resistance, as well as developing antimicrobial stewardship programs in Babil, Iraq. Moreover, these bacterial isolates have shown multidrug resistance, mainly to commonly administered drugs that could cause therapy ineffective. Therefore, in clinical use, appropriate treatment should be chosen based on the results obtained from antimicrobial susceptibility tests.

Background: Multi drug resistant bacterial agents that contaminate seafood cause several diseases in humans and are widely documented as a global public health challenge. Methods: This study evaluated the microbiological and antimicrobial resistance genes profiles of bacterial Isolates from shellfish vended at Iko and Douglas Creeks of Cross River State, Nigeria. A total of 540 shellfish (117 clams, 88 oysters, 136 periwinkles) samples were collected from various vendor at the two Creeks were analyzed. The samples were processed using standard microbiological methods to identify bacterial pathogens. Antimicrobial susceptibility was assayed using the Kirby-Bauer disk diffusion method. Isolates were screened for antimicrobial resistant genes using polymerase chain reaction. Results: Overall, a total of 135 bacteria isolates were identified. The most common isolate was Alcaligenes species 53(39.2%) followed by Pseudomonas species 44(32.6%), Providencia species 25(18.5%), Vibrio species 6(4.4%), and Paenalcaligenes species 7(5.2%). The isolates showed varying susceptibilities to Imipenem (36%) and amikacin (28%) but were all resistant to Trimethoprim-Sulfamethoxazole. Fifty-three isolates had a multiple antibiotic resistance index (MARI) of ≥0.9 - 1.0. Most of the bacterial isolates were detected with TEM genes (82.2%), SHV (51.8%,), VIM (50.3%) resistance genes. None of the isolates expressed Veb gene. Only 40.7% of the isolates expressed QnrB gene while none expressed QnrA and QnrS. Conclusion: The detection of these multidrug resistant clinically relevant bacterial species suggests a significant linkage of commonly consumed seafood in the community and environmental spread of MDR bacteria.

The discharge of untreated hospital waste water into the environment is a major public health concern as this could result in the environmental spread of multidrug-resistant (MDR) bacteria. The dissemination of such MDR bacteria in waste water remains unexamined in most geographical area. This study assessed the prevalence of multidrug-resistant bacterial isolates in waste water from hospital environment in Umuahia, Nigeria. Exactly 200 waste water samples were collected from various diagnostic laboratory units of Michael Okpara University of Agriculture, Umudike (MOUAU) Health Centers and Federal Medical Center (FMC), Umuahia in 250 ml screw-capped, heat-sterilized bottles. Water samples were analyzed using standard microbiological techniques. Bacterial isolates from water samples were identified with API-20E test kit. Antibiotic susceptibility test was done using Kirby–Bauer disc diffusion method. Multiple antibiotic resistance index (MARI) of isolated bacteria was determined using standard formulae. A total of 147(73.5 %) bacterial species such as S. aureus, Shigella spp, E. coli, Enterobacter spp, Proteus mirabilis, and Arizona spp were identified from the waste water samples. Bacterial isolates exhibited resistance to tetracycline, trimethoprim-sulfamethoxazole, clindamycin, nalixidic acid, vancomycin, and chloramphenicol but very susceptible to imipenem. All isolates were multidrug-resistant with MARI values ranging from 0.5 - 0.8. This study revealed the presence of MDR bacteria in hospital waste water samples in Umuahia, Abia state, Nigeria. The threat and risk of exposure to such MDR bacteria is of public health significance and raises concern over poor management and disposal of hospital waste water or effluents.

Background: Cleanliness and dirt are a dichotomy to categorize a particular environment especially hospital settings in which cleanliness pave a great linkage to patient satisfaction on quality of care and reduction of infection. Dirt creates negative thought on the quality of services being rendered to patients in the hospital environment. Objectives: To assess the perception of patients and their relatives on the level of cleanliness in radiology department of Usmanu Danfodiyo University Teaching Hospital (UDUTH) Sokoto and to determine which amongst the diagnostic rooms is the cleanest (Computed Tomography (CT) room, Routine X-rays room or contrast exams room (fluoroscopy)). Materials and Method: A descriptive cross-sectional study was conducted, 100 questionnaires were distributed to the respondents of which 66 in Routine xray room, 23 in CT suit and 11 in contrast examination room. Out of the 100 questionnaires nine were not returned and the remaining 91 were analysed. The questionnaire contains fifteen statements in total apart from the socio-demographic component. The count of responses was considered and for each type of response (SA, A, N, D, and SD) the percentage were calculated using statistical package for social sciences (SPSS) version 23.0. Results: Sixty percent of the respondents were patients while 40% were relatives of patients that were critically ill. The modal age range of the respondents was 25-34years, 58% were married, while 48.4% were civil servants. The result of our study shows that the respondents have adequate knowledge on cleanliness with a mean score of 4.3, they perceived radiology department as a clean environment for diagnosis and visitation but need further improvement. According to the findings, CT room was the cleanest (87.0%) followed by contrast examination room 64.0% and routine X-rays room was the least 47.0%. It also revealed that lack of manpower is the cause of inadequate sanitation. Conclusion: Radiology Department in Usmanu Danfodiyo University Teaching Hospital is clean and the CT suit is the cleanest.

Abstract Background Recent emergence of Candida auris as a multidrug resistant fungal pathogen, is a serious concerns for public health. However, there is a paucity of reported cases from Oman. Literature search resulted in finding only 7 cases from Oman, reporting C. auris infections in the articles first published in 2017. However, the rate of isolatin is increasing. Methods In this study, we included the results of all positive blood cultures of C. auris in Suhar teaching hospital from May 2018 (date of first detection) till end of April 2019. Further confirmation of the species was performed by MALDI-TOF and antibiotic susceptibility test (AST) by Vitek 2 in central public health laboratory (CPHL) of Oman. Results We detected 13 patients (9 females, 4 males). The mean age was 58.61% years (28–76 years). All candidemic patients had serious underlying conditions, including prolonged hospital stay or extensive and prolonged antimicrobial exposure or medical comorbidities (8 of 13). The time from hospital admission to onset of C. auris candidemia was 8–49 days, with a median of approximately 27 days. The most common isolated co- pathogen from blood culture was K. pneumonia (without regard to Coagulase-negative staphylococci). As average, every patient received 4.8 kind of different antibiotics in mean 88 doses before candidemia developed and piperacillin–tazobactam was the most common used antibiotics. AST was done just for 5 patients and revealed high-level resistance to fluconazole and Amphotricin B while, Echinocandins (anidulafungin, caspofungin) were fully sensitive and voricunazole had intermediate sensitivity. Mean duration of anti-fungal treatment was 12.5 days (5 – 26 days). 8 patients treated by Echinocandins (4/8 died), 4 by Fluconazole (3/4 died) and one without treatment discharged. 30-day all-cause mortality was 61.5%. Conclusion In Oman, C. auris has been reported from many hospitals. Resistance to several antifungal agents and persistence in the hospital environment make this organism a potential menace for the treating physician and the infection control personnel. In our hospital, every candidemic patient should be treated with Echinocandins and assumed to be resistant to Fluconazole until proven otherwise according to results of AST. Background: Otitis Media is a prevailing and notorious infection in developing countries causing serious local damage and threating complication. Mainly in developing countries like Nepal, Otitis Media results because of illiteracy, poverty and poor hygiene. The aim of this study was to determine the profile of Otitis Media, its causative agents and their antibiotic susceptibility pattern.Methods: The study included 263 pus samples from 240 patients attending ENT department of Bharatpur hospital from May 2015 to January 2016. Samples were processed in microbiology department for bacteria using standard operating protocol. Antibiotic susceptibility testing was performed for all bacterial isolates by Kirby Bauer disc diffusion method and the results were interpreted according to clinical and laboratory standard institute (CLSI) guideline.Results: Out of 240 patients, 121 were female and 119 were male. Highest incidence of Otitis Media was observed in 1-10 year age group. Out of 263 samples taken from 240 patients, 216 showed bacterial growth. Gram negative bacteria predominated and the most common bacteria isolated were Staphylococcus aureus 36.11% followed by Pseudomonas aeruginosa 33.33% and Coagulase Negative Staphylococci 8.08%. All bacterial isolates were sensitive to gentamycin. Staphylococcus aureus was sensitive to Amikacin and gentamycin. All gram negative bacterial isolates were sensitive to Imipenem and gentamycin. 100% of Pseudomonas aeruginosa was sensitive to Imipenem.Conclusions: Staphylococcus aureus was the most predominant organism isolated from the pus swab followed by Pseudomonas aeruginosa and all the isolated organisms were sensitive to Gentamycin.

Abstract Background Bacterial opportunistic infections are quite common in HIV patients. Besides HIV-TB coinfection, lower respiratory tract infections by multidrug-resistant bacteria cause significant morbidity and mortality among HIV patients. This study was done to evaluate the bacterial coinfection of LRT and detect plasmid-mediated blaTEM and blaCTX−M genes among Extended-Spectrum β-Lactamase (ESBL) producing isolates from sputum samples in HIV patients. Methods A total of 263 sputum samples from HIV-positive cases were processed with standard microbiological methods to isolate and identify the possible pathogens. The identified bacterial isolates were assessed for antibiotic susceptibility pattern by using modified Kirby Bauer disk diffusion method following Clinical Laboratory Standard Institute (CLSI) guidelines. Plasmid DNA was extracted from multidrug-resistant and ESBL producers for screening of ESBL genes; blaCTX−M and blaTEM by conventional PCR method using specific primers. Results Of 263 sputum samples, 67 (25.48%) were culture positive showing Klebsiella pneumoniae; 17(25.37%) as the most predominant one. A higher rate of infection (4/8, 50%) was observed among old-aged people of 61 -70 years, whereas no infection was observed below 20 years. About 30.0% (15/50) of smokers, 32.86% (23/70) cases with previous pulmonary tuberculosis and 52.38% (11/21) with CD4 count <200 cells/µl were found to be susceptible to LRTIs. Among 53 bacterial isolates, 52.83% (n=28) were multidrug-resistant and 43.4% (n=23) were ESBL producers. All ESBL producers were sensitive to Colistin and Polymyxin B. Of 23 ESBL producers, 47.83% (11/23) and 8.6% (2/23) possessed only blaCTX−M and blaTEM genes respectively and 43.48% (10/23) possessed both ESBL genes. Conclusion The increasing rate of MDR bacterial infections mainly ESBL producers of LRTIs causes difficulty in the management of diseases leading to high morbidity and mortality of HIV patients.
