Coronavirus Disease 2019 (COVID-19) is a disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Severe cases of SARS-CoV-2 infection are characterized by a systemic inflammatory response in acute respiration. The inflammatory response can be checked by laboratory tests with C-Reactive Protein. The purpose of this study was to determine CRP levels in elderly patients with comorbidities who were confirmed positive for Covid-19. This type of research is descriptive quantitative with a cross-sectional design using accidental sampling technique obtained a sample of 56 patients. This research was conducted at Pasar Rebo Hospital. Based on the univariate statistical test, the frequency distribution of males was 29 people (51.8%) while women were 27 people (48.2%), the elderly group with the highest group was in the elderly group with an age range of 65-69 years as many as 22 people (39.3%), elderly patients with comorbid Hypertension dominate as many as 18 people (32.1%). The results of laboratory examinations showed an increase in levels of C-Reactive Protein which varied, this was accompanied by the condition of patients who had clinical comorbidities. Covid-19 in the elderly also worsens the symptoms and course of Covid-19, due to decreased immune function, the immune system of organs and the many clinical comorbidities that occur.

Abstract: Cigarette are the most cause of death around the world. Smoking cigarrete is harmful to the organs because it contains many toxic chemical that can stimulate inflammatory process. Smoking cigarrete is a risk factors for heart disease and chronic pulmonary disease (COPD). C-reactive protein (CRP) are a non spesific inflammatory marker that can elevated in both local and systemic disease. Beside biomarker, CRP also use as prognostic marker for inflammation. The purpose of this study was to identify the level of serum CRP of smokers aged >40 years old. Twenty eight smokers were eligible to this criteria and participated in this study. Cross-sectional design with a descriptive method was employed in this study. Sampels were analysed in laboratory for serum CRP levels. The results showed that 23 subjects (82%) had normal (negatif) serum CRP level and five subjects (18%) had positive serum CRP level. It can be concluded that serum CRP level of >40 years old smokers in Kolombo village, west Bitung two were mostly in normal level. Keywords: c-reactive protein (crp), active smoker, at age >40 years oldAbstrak: Rokok menjadi salah satu penyebab kematian terbesar di dunia. Abstract Objective:To explore the value of C-reactive protein (CRP) and lymphocyte (L) in the assessment of disease severity and prognosis of elderly COVID-19 patients. Methods: A total of 194 positive COVID-19 patients were collected from Tianyou Hospital and Puren Hospital, affiliated hospital of Wuhan University of Science and Technology. Their demographic characteristics were analyzed. The dynamic changes of CRP and L in peripheral blood were retrospectively studied. Results: (1) There were significant statistical differences in CRP, L in clinical typing and clinical outcome in patients over 60 years old compared with those under 60 years old. Survival analysis showed that the risk of death was greater in patients over 60 than in those under 60.(2)In 125 patients over 60 years old, the hospitalized patients with severe or critical types of disease had significantly higher CRP than those with moderate type (p<0.01). In the outcome of the elderly patients, the CRP of the patients with the outcomes of discharge, improvement, aggravation and death increased successively (p<0.01). According to the analysis of Logistic regression model, the increase of CRP constitutes a risk factor for death in elderly patients. (3) In the ROC curve analysis to distinguish the death outcome and non-death outcome of COVID-19 patients, the area under the curve (AUC) of CRP and L was 0.751 and 0.720 respectively. CRP and L had good diagnostic accuracy for the death outcome of patients. (4) Changes in CRP were correlated with changes in CT imaging and were consistent with changes in the course of the disease. Conclusions: (1) The data collected in this research showed that the cumulative survival rate of patients over 60 years old was lower than that of patients under 60 years old. With the increase of age, the CRP of patients showed an increasing trend, and the L of patients showed a characteristic lower than the normal reference interval. (2) CRP and L are important monitoring indicators of COVID-19 in elderly patients. Combined with CT examination and observation of their dynamic changes, CRP and L are of important clinical guiding value for the judgment of disease severity and the evaluation of prognosis.

Accurate diagnosis and timely treatment of pneumonia, as one of the most common infectious diseases in elderly patients, require careful attention to the clinical and paraclinical findings, which may be different between the elderly and non-elderly patients. The aim of the present study was to compare the clinical, laboratory, and radiological findings of elderly and non-elderly patients with pneumonia and pneumosepsis. This cross-sectional study was performed on 97 elderly and 93 non-elderly patients with pneumonia, admitted to Sina hospital in Hamadan, west of Iran, in 2017. Patients in both groups were also compared in terms of the underlying diseases, sepsis rate, electrolyte disturbances and CURB-65 criteria. All underlying diseases, except for HIV infection, in addition to clinical findings such as tachypnea, lethargy, decreased consciousness, hypotension, and respiratory alkalosis were significantly more common in the elderly, compared to the non-elderly group. In the elderly group, the average length of hospital stay, sepsis rate, and mortality rate were 9.4 days, 74.2%, and 21.7%, respectively versus 6.2 days, 46.3%, and 3.2%, respectively in the non-elderly group. Patients in the two groups were significantly different in terms of CkkURB-65 criteria and radiological findings. For the timely diagnosis of pneumonia and pneumosepsis in the elderly, it is necessary to consider any alteration in respiratory rate and consciousness status. Also, for proper treatment, the clinicians should pay attention to the existence of any comorbidities and electrolyte disturbances. © 2019 Tehran University of Medical Sciences. All rights reserved.
