Objective: This pilot study explored the attitudes, beliefs, and intentions of a group of chiropractic interns concerning health promotion, wellness, and preventive services before and after a series of brief educational interventions. Methods: Interns completed a survey before (n = 37) and after (n = 22) the interventions. The survey included 12 Likert scale questions about attitudes and intentions toward wellness and health promotion models. The interventions consisted of classroom lectures, clinical training, and online information pertaining to health promotion and wellness. Results: The interns initially favored wellness models, perceived a need for them, and felt partially prepared to administer them, with mean Likert scores 4 or greater on a 1 to 5 scale. Afterward, the average scores were higher and the interns reported some benefit from this short course of training. Conclusion: The initial survey demonstrated that interns had some understanding of wellness, health promotion, and preventive services, and favored utilization of these services. The follow-up survey suggested that a short educational intervention could have a positive impact on these attitudes and future utilization of wellness procedures in their practices.

Abstract Adolescents are an important target group for sexual health promotion, and there are numerous programs and interventions carried out in this field. The aim of this study is to describe adolescents’ attitudes, knowledge and sexual behavior before and after a sexual health promotion intervention. The intervention was developed in the study and consisted of three elements: (i) class-room session, (ii) information materials and (iii) free condom distribution. The study was carried out in eight randomly selected vocational schools in Finland. The participants were first year students aged 15–19 years. The data were collected using an electronic questionnaire before intervention (intervention baseline n = 500, control baseline n = 183) and two times after the intervention (intervention first follow-up n = 173/second follow-up n = 202, control first follow-up n = 115/second follow-up n = 46). There were significant differences before and after the intervention concerning better knowledge and more frequent testing for sexually transmitted infections (STIs). Schools are an important environment to reach adolescents during the phase where their sexual health is developing and there is an increased risk of STI transmission. More school-based interventions are therefore needed, and the results of this study can be utilized when developing sexual health promotion interventions among adolescents.

Background: Unhealthy diet and physical inactivity contribute to the growing burden of cardiovascular diseases in Nepal. Lifestyle is formed in childhood and in the Nepalese context influenced mainly by mothers, it is to date unknown how influential mothers are. Objectives: To assess changes in children’s diet and physical activity as perceived by their mothers after a health promotion intervention. Methodology: The Heart-Health Associated Research, Dissemination and Intervention in the Community is a community trial conducted in the Jhaukhel-Duwakot Health Demographic Surveillance site, in Bhaktapur district of Nepal. We conducted a health promotion intervention on diet and physical activity targeted at mothers with children aged one to nine years old in August-November 2016. Duwakot was randomized as the intervention site and Jhaukhel as the control. We conducted a follow-up study after three months to determine the outcome of the intervention. Nine trained enumerators conducted door-to-door visits to all households with eligible mothers. We calculated mean, frequency and percent changes for children’s behavior. Results: As responded by mothers, children in Duwakot consumed more healthy snacks after the intervention compared to Jhaukhel. Children in Duwakot increased consumption of water and milk. Children’s consumption of packet juices and soft drinks was decreased by 30% and 4% respectively. There was 21% increment in the duration of outdoor playing among the children at Duwakot during follow-up. Conclusion: The Heart-Health Associated Research, Dissemination and Intervention in the Community that focused on mothers showed indirect positive impact on their young children’s diet and physical activity behavior. In future, the longterm effects of such intervention should be assessed.

Abstract Survival among children and adults with SCD in Belgium published in 2015 showed a low mortality rate (0.25/100 patient years (PY)) among the 469 patients included (5,110 PY) with no significantly increase above 18 years of age. Hydroxyurea (HU) had a positive impact on patients' survival rate when compared to those without disease-modifying treatments (DMT) or transplanted. This led us to compare the incidence of acute complications and hospitalizations among children and adults to see if significant change occurred. Complications before and after the age of 18 for 84 patients with severe genotype followed after 18 years of age and not transplanted for whom the data during childhood were recorded in the registry are detailed in Figure 1 and Table I. Data on the pre- and post-18 years' events for the entire severe genotype cohort are detailed in Table II and include data from the patients under 18 years and data at the pediatric age of those more than 18 years. Data was censored from the time of the transplantation. Furthermore, data from adult patients before age 18 and beyond with severe genotype treated or not with HU are given in Table III. There were 139 patients (1759 PY) more than 18 years who had at last follow up a median age of 21 years (range: 18-53 years). 42% (59/139) of the patients were treated with HU. The median number of vaso-occlusive crisis (VOC) was statistically lower during adulthood (0 vs 29.6/100 PY; p= 0.007) and the median number of acute chest syndrome (ACS) was not statistically higher in adult period compared to their follow-up during childhood. Overall, severe infections as well as hospitalizations number (19 vs 63/100 PY; p=0.0006) and days (119 vs 259/100 PY; p=0.021) are significantly higher in children. The comparison of adults under HU and without DMT showed a significant higher incidence of acute events in HU patients (Table III). This is true for follow-up in childhood and beyond the age of 18 (Table III). Patients without DMT obviously seems to have attenuated symptomatology (clinical phenotype not very severe despite their severe genotype) and were therefore not considered benefiting from DMT. In the latter, no ACS was recorded during their follow-up in adulthood (Table III). The increase in CVO and ACS incidence with age is well described in the literature but not found in our results. Higher number of hospitalizations and infections before 18 years of age are observed. This can be explained by the fact that children consult more quickly and frequently and they are more readily monitored in hospital compared to adults. They also live in community, which increases the risk of seasonal viral infections. Compared with data available in the literature, our adult patients, most of whom are treated with HU, have fewer acute complications. Nevertheless the comparison is jeopardized because of different methodological approaches. In addition, our cohort of adult patients is limited, has a relatively short follow-up and consists mainly of adults under 21 at the last follow-up. The shorter length of follow up and the underrepresentation of older adults who pay a heavier toll on SCD because of co-existence of chronic complications, may explain the reduced incidence of acute complications in our cohort. In conclusion, the incidence of events observed in our adult population is lower than that described in the literature. This difference is probably related to a larger proportion of HU patients in our cohort. But these results should be confirmed by data from a larger number of adult patients with longer prospective follow-up. Disclosures No relevant conflicts of interest to declare.

The American Diabetes Association recommends that patients with type II diabetes and atherosclerotic cardiovascular disease be prescribed an SGLT-2 inhibitor or GLP-1 agonist for cardioprotective benefit. This project assessed the use of these medications in this patient population in a rural clinic by measuring prescribing rates of SGLT-2/GLP-1 therapy before and after pharmacist interventions. Of the 60 patients identified at baseline, 39.39% (13/33) managed by a pharmacist were prescribed SGLT-2/GLP-1 therapy compared to the 14.81% (4/27) who had not seen a pharmacist (p = 0.025). Of the 43 patients that were not on SGLT-2/GLP-1 therapy at baseline, 13 were lost to follow-up and 13 had contraindications. For the 17 remaining patients, pharmacists recommended initiating SGLT-2/GLP-1 therapy and were able to successfully initiate therapy for 9 patients (52.94%). Pharmacist interventions improved the prescription rates from a baseline of 36.17% (17/47) to 55.3% (26/47) (p = 0.002), with SGLT-2/GLP-1 therapy contraindicated in 27.66% (13/47) of patients. This suggests that patients managed by a pharmacist have medication regimens that were optimized at a greater rate and pharmacists can have a positive impact on the appropriate medication usage in this population.

Introdução: O seguimento farmacoterapêutico (STF) de pacientes com HIV tem grande importância, pois contribui para adesão ao tratamento. Objetivos: Avaliar adesão e qualidade de vida em pacientes em uso de terapia antirretroviral através de questionários padronizados, durante a assistência intensiva da equipe farmacêutica. Métodos: Foram acompanhados 15 pacientes HIV positivos que frequentam o Centro de Testagem e Aconselhamento. Cada paciente recebeu o seguimento por sete meses: 1º Encontro: aplicação dos questionários de adesão e qualidade de vida; aplicação do formulário de atenção farmacêutica; análise dos exames laboratoriais (carga viral e contagem de linfócitos T Cd4). 2º Encontro: apresentação do plano de metas e proposta de diário para o registro de tomada da medicação. 3º Encontro: acompanhamento do plano de metas; verificação de problemas relacionados aos medicamentos (PRM); intervenções educativas. 4º Encontro: acompanhamento do plano de metas; verificação de PRM; intervenções educativas sobre a medicação. 5º Encontro: aplicação dos questionários de adesão e qualidade de vida; análise dos exames laboratoriais. Resultados: A média de idade foi de 42,5 ± 8,8 anos. Em relação à contagem de linfócitos T-CD4 antes e após o STF, houve aumento estatisticamente significativo (p=0,0048) deste parâmetro. A carga viral de 13 dos 15 pacientes se tornou indetectável ou reduziu. Não houve diferença estatisticamente significativa na adesão e qualidade de vida quando comparados o antes e o depois do seguimento. Conclusão: A atenção farmacêutica mostrou-se importante para os pacientes com HIV, pois os mesmos apresentaram melhora significativa em seus parâmetros clínicos.Palavras-chave: Atenção farmacêutica; Seguimento farmacoterapêutico; HIVABSTRACTIntroduction: Pharmacotherapeutic follow-up study of HIV infected patients is of great importance as it contributes to treatment adherence. Aims: To evaluate adherence and quality of life in patients using antiretroviral therapy through standardized questionnaires, during the intensive care of the pharmaceutical team. Methods: Fifteen HIV positive patients attending the Testing and Counseling Center were followed. Each patient was followed-up for seven months: 1st Meeting: application of adherence questionnaires and quality of life; application of the pharmaceutical care form; analysis of laboratory tests (viral load and CD4 T lymphocyte counts). 2nd Meeting: presentation of the goals' plan and proposal of a diary for the registration of medication taking; 3rd Meeting: follow-up of the goals' plan; checking of problems related to medicines (PRM); educational interventions on medication. 4th Meeting: follow-up of the targets plan; PRM checking; educational interventions on medication. 5th Meeting: application of adhesion questionnaires and quality of life; analysis of laboratory tests. Results: The mean age was 42.5 ± 8.8 years. Regarding the T-CD4 lymphocyte count before and after pharmacotherapeutic follow-up, there was an increase in this statistically significant parameter (p=0.0048). The viral load of 13 of the 15 patients became undetectable or reduced. There was no statistically significant difference in adherence and quality of life when compared to before and after follow-up. Conclusion: Pharmaceutical Care has proved to be an important practice for HIV patients, for they have shown a significant improvement in their clinical parameters.Keywords: Pharmaceutical care; Pharmacotherapeutic follow-up; HIV

A national survey of 591 community-based youth smoking cessation programs provided an opportunity to assess the sustainability of health promotion programming over a 3-year period. Initial survey questions were mapped to five sustainability domains (local ownership, organizational alignment, resources, demand, and standard operating procedures) and examined to identify correlates of sustained operation. Follow-up surveys were completed with 305 programs. Assuming loss to follow-up indicated failure to sustain, the overall rate of program continuation was 32%. Baseline correlates of sustaining operation included the following: serving more youth, training staff in smoking cessation, longer time in operation at the initial survey, and receiving state funding as a sole source of support. Primary reasons for discontinuation related to lack of funding, insufficient enrollment, change of focus from tobacco cessation, and scheduling difficulty. Replication of studies like this survey of youth smoking cessation programs with other types of health promotion and public health programming can further test sustainability frameworks, provide validated measures, and ultimately inform a robust and replicable array of lasting, effective, evidence-based public health programs.

Introduction — Burning is one of the most accidents with a higher rate of morbidity and mortality. The aim of this study was to investigate the status of nursing cares in the burning ward to improve the quality of nursing service following the education. Methods — An after-before intervention-educational audit study was conducted on burning ward nurses in Iran. The data were collected using a researcher-made checklist based on the Ministry of Health standards with “Yes/No” questions. The data were analyzed using SPSS software version.16 through descriptive statistics and Mc-Nemar nonparametric metric test. Results — The population consisted of 20 nurses working in the burning ward of the Sina Hospital in Tabriz/Iran. Presentation of nursing services was good in terms of the primary care (80.5%) and during hospitalization (83.4%); and it was weak in terms of the secondary care (42.5%). A significant improvement was observed following the educational intervention according to the results of the Mc-Nemar test performed before and after intervention (P<0.001). Conclusion — Audit education improved the nursing secondary cares and follow- up care after patients discharge. It is necessary to hold educational workshops for improving the nurses̓ performance in the other fields.

BackgroundAlternatives to face-to-face consulting (F2F) with patients have been developed including telephone consulting and e-consulting. One area of concern about both alternatives is that they may act to increase total consultation time by generating additional F2F consultation activity.AimTo determine the proportion of clinical e-consults where a GP requested a follow-up F2F consultation and to determine whether e-consultation training reduced the proportion of GP requests for a follow-up F2F consultation.MethodA longitudinal study was conducted observing ‘remote closure’ rates (the proportion of clinical e-consults with no F2F follow-up appointment) before and after the launch of a dedicated at scale electronic consultation ‘eHub’ with a bespoke technical and clinical training programme. Remote closure rates were available for 42 months prior and 22 months following the intervention. A follow-up GP consultation was defined as the doctor requesting the patient to make a F2F appointment.ResultsThe remote closure rates were 63% (3019/4778) before and 87% (6957/7993) after the intervention. χ2 with Yates correction 1024.9; P<0.001.ConclusionThe consultation as a core competency of GP training requires specific skills for e-consulting. We have devised a training package for e-consultation training. Following implementation of a package of intensive e-consultation training, remote closure rates of 87% were obtained. Some of the observed increase in remote closure rates may have been attributable to increased familiarity of both GPs and patients with new models of consultation. We plan to test our e-consulting educational package in other settings with low e-consultation closure rates.

Nutritional problems occur in almost all age groups including children. Pasaman Barat district is one of the stunting focus locations in West Sumatra with an incidence rate of 32.09% Therefore this community service held in Pasaman Barat District. This health promotion activity aimed to increase mother’s knowledge and skill to overcome stunting incident on children. Method used in this activity is counseling and demonstration. Health promotion was attended by 45 women of childbearing age consisting of 15 pregnant women, nursing mothers and toddler mothers each. Counseling held separately for each group meanwhile in the demonstration session all of participants were joining together to compete on making the nutritious supplementary food. This activity succeeded in having a positive impact on the target group. This can be seen from the enthusiasm of the target in participating at each stage of the activity and in discussion session. The activity was closed by making a commitment to follow up the activity together with the district health office, Parit Primary Health Care and Parit Village party.

Hypertension is a condition which blood pressure is higher than 140/90 mmHg. Uncontrolled hypertension can cause a variety of diseases to the point of death. However, prevention of hypertension can be done early. Health Promotions aim to increase the knowledge of students in Ciamis District about prevention of hypertension. Health Promotion were carried out by explanation and discussion of 130 students in Ciamis District who were collected at WISMA GURU Ciamis on March 6, 2018. Before and after Health Promotion, participants were given a knowledge questionnaire and the results were processed using SPSS Version 16. Health Promotion instruments used flyers and power point presentation. Health Promotion of prevention of hypertension early has done successful and has a positive impact on increasing the knowledge of students and society.
