AbstractObjectiveCardiorespiratory fitness is known to be cardioprotective and its association with the components of the metabolic syndrome in children is becoming clearer. The aim of the present study was to examine the extent to which cardiorespiratory fitness may offset the weight-related association with mean arterial pressure (MAP) in schoolchildren.DesignCross-sectional study.SettingsSchoolchildren from the East of England, UK.SubjectsA total of 5983 (48 % females) schoolchildren, 10 to 16 years of age, had height, weight and blood pressure measured by standard procedures and cardiorespiratory fitness assessed by the 20 m shuttle-run test. Participants were classified as fit or unfit using internationally accepted fitness cut-off points; and as normal weight, overweight or obese based on BMI, again using international cut-off points. Age-adjusted ANCOVA was used to determine the main effects and interaction of fitness and BMI on MAP Z-score. Logistic regression models were used to estimate odds ratios of elevated MAP.ResultsPrevalence of elevated MAP in schoolchildren was 14·8 % overall and 35·7 % in those who were obese-unfit. Approximately 21 % of participants were overweight and 5 % obese, while 23 % were classified as unfit. MAP generally increased across BMI categories and was higher in the aerobically unfit participants. Obese-fit males had lower MAP compared with obese-unfit males (P < 0·001); this trend was similar in females (P = 0·05).ConclusionsIncreasing fitness level may have a positive impact on the weight-related elevations of MAP seen in obese and overweight schoolchildren.

The peroxisome proliferator-activated receptor gamma 2 (PPARγ2) genotypes are related to obesity and the metabolic syndrome (MetS). A low level of cardiorespiratory fitness is also a strong determining factor in the development of MetS. This cross-sectional study was performed to investigate the influence of the interaction between the PPARγ2 genotype and cardiorespiratory fitness on the risk of MetS. Healthy Japanese men ( n = 211) and women ( n = 505) participated in this study. All subjects were divided into 8 groups according to sex, fitness level (high and low fitness groups), and age (younger, age < 40 yr; middle-aged/older, age ≥ 40 yr). The PPARγ2 genotypes (Pro12Ala and C1431T) were analyzed by real-time PCR with Taq-Man probes. Two-way ANCOVA with adjustment for age as a covariate indicated that fitness and the CC genotype of C1431T in the PPARγ2 gene interacted to produce a significant effect on MetS risk in younger men and that the risk of MetS in the CC genotype group with low cardiorespiratory fitness was significantly higher than that in the corresponding CT+TT genotypes or in the high fitness groups. There was no significant interaction between fitness and genotype in determining MetS risk in middle-aged/older men or in women in any group. With regard to the Pro12Ala genotype of the PPARγ2 gene, there were no significant differences in fitness or genotype effects nor were there any interactions between measurement variables. We concluded that the CC genotype of C1431T in the PPARγ2 gene together with low cardiorespiratory fitness may increase the risk of MetS in younger men (age < 40 yr), even with adjustment for age.

The aims of this study were a) to assess whether obesity acts as a mediator between i) cardiorespiratory fitness (CRF) and mean blood pressure; and ii) between between physical activity (PA) and mean blodd pressure in children and adolescents. A cross-sectional study was conducted with a 632 children and adolescents. It was measured mean blood pressure, body mass index, fat mass and waist circumference. CRF and PA was assessing with Course Navette test and ActiGraph. The analysis of the mediation was performed using Process macro for SPSS. The results indicate that obesity acts as a partial mediation in the association between CRF and mean blood pressure in 10-12 years old children (z=from -5.81 to -5.40; all p˂0.000). These results indicate that obesity acts as a complete mediator in the association between PA and mean blood pressure in 10-12 years old children (z=from -4.49 to -1.94; all p˂0.000). Our result reinforces the relevance of prevent weight increse and improve cardiorespiratory fitness level since erly age in children and adolescents to prevent high mean blood pressure. Increasing the level of physical activity can influence on obesity and cardiorespiratory fitness.

Aim: To describe the distribution of ocular perfusion pressure hypertensive patients. Design: Cross-sectional observation study. Materials and Methods: Hospital-based, case control cross-sectional study conducted on 100 patients. Systolic and diastolic blood pressure (SBP and DBP) was measured with a Mercury Sphygmomanometer. Mean ocular perfusion pressure (MOPP) = ⅔ (mean arterial pressure − IOP), where mean arterial pressure (MAP) = DBP + ⅓ (SBP − DBP), systolic perfusion pressure (SPP) = SBP – IOP and diastolic perfusion pressure (DPP) = DBP − IOP was calculated. Results: High values of diastolic BP (>90 mmHg) and low values of OPP (<40 mmHg) were associated to an increased risk of confirmed POAG. Conclusion: There is a close relationship between OPP and confirmed glaucoma in hypertensive patients.

Information regarding the prevalence of features of Metabolic Syndrome (MS) in Malaysia has not been previously reported. There are also difficulties in using Western cut-off criteria to define MS in this Asian population. The aim of this study was to determine the prevalence of features of MS in normal body weight normoglycemic Malay subjects. An additional aim was to determine the optimal waist circumference cut-off which best identified MS in this population. We used data from a cross-sectional study carried out in Malay descendants in Kota Bharu a city on east coast peninsular Malaysia. The prevalence of different features of MS was determined using the AHA/NHLBI and IDF criteria. The results showed that even after excluding subjects with diabetes mellitus, impaired glucose tolerance (IGT) or impaired fasting glucose (IFG), hypertension, a first degree relative with diabetes mellitus and obesity (BMI >25 kg/m2), the features of metabolic syndrome were still highly prevalent in this population. Furthermore, the features of the metabolic syndrome were associated with reduced insulin sensitivity in an additive manner. The metabolic syndrome is a biologically relevant construct for the identification of the insulin resistant individual in a Malaysian population.Bangladesh Journal of Medical Science Vol.15(1) 2016 p.123-128

Background:The main goal of this study was to analyze the associations between cardiorespiratory fitness (CRF) and body mass index (BMI) with self-rated health (SRH) of adolescent girls.Methods:This was a cross-sectional study of 533 adolescents girls, aged from 10 to 18 years old. CRF was predicted by maximal multistage 20-m shuttle-run test according to procedures described from FITNESSGRAM. Girls’ obesity status was classified according to International Obesity Task Force and Self-rated health (SRH) was assessed by questionnaire.Results:The findings showed that among adolescent girls 23.2% had negative SRH. Girls who were classified as unfit were more likely to report negative SRH in both univariate logistic (OR: 3.05; CI: 1.91−4.87; P < .05) and multivariate (OR: 2.93; CI: 1.82−4.72; P < .05) regression analyses compared with their fit peers. Obese girls were more likely to report negative SRH (OR: 2.30; CI: 1.14−4.62; P < .05) compared with their normal-weight counterparts. However such association was lost in multivariate analyses suggesting an effect of CRF.Conclusions:Negative perception of health was associated with lower CRF and weight status although such association it is mediated by CRF condition.

Objective To compare the total and differential leukocyte count in obese and normal-weight adolescents, and to verify their possible relations with cardiorespiratory fitness and adiposity indicators.Methods A cross-sectional study conducted with 139 adolescents (107 obese and 32 normal weight) aged between 13 and 18 years. Cardiorespiratory fitness was determined by direct gas analysis during an incremental treadmill test. Total leukocytes and subsets were estimated by flow cytometry. Body composition was assessed by dual-energy X-ray absorptiometry. Thet-test for independent samples was used for comparison between groups. The relation between leukocytes, cardiorespiratory fitness and adiposity indicators was verified by Pearson’s correlation and multiple linear regression (adjusted for age and body mass index) tests.Results Obese adolescents had higher leukocyte (8.12±2.36u/L x 103; p=0.001), neutrophil (4.33±1.86u/L x 103; p=0.002), and monocyte (0.70±0.22u/L x 103; p=0.002) counts compared to the levels of normal weight subjects. After the necessary adjustments, cardiorespiratory fitness had a negative association with leukocytes, neutrophils, and monocytes in boys.Conclusion Obese adolescents had higher total and differential leucocyte count when compared to normal weight individuals. We also observed a weak positive association between adiposity and total leukocyte, monocyte, and neutrophil counts, and in boys, a negative association between cardiorespiratory fitness and total count of leukocytes, monocytes, and neutrophils.

It has been recognised recently that obese individuals have lower concentrations of micronutrients and this may affect the concentrations of inflammatory cytokines. A cross-sectional study was carried out to evaluate the association of specific micronutrients' status with chronic inflammation caused by obesity in 280 women (36·1 (sd7·5) years) from seven rural communities in Mexico. Measurements of weight, height and waist circumference were made on all women and body composition was determined by dual-energy X-ray absorptiometry. Concentrations of the cytokines IL-1, TNF-α, IL-6, IL-10 and IL-12, lipid profile, and the micronutrients Zn and vitamins A, C and E were determined in fasting blood samples. Ordered logistic regression models were used to determine associations between categorised cytokine levels and micronutrients. It was found that 80 % of women were overweight or obese, and had significantly higher concentrations of C-reactive protein than normal-weight women (P= 0·05). The risk of higher levels of TNF-α, IL-6, IL-10 and IL-12 was reduced significantly among women with higher Zn concentrations (OR 0·63, 95 % CI 0·42, 0·96,P= 0·03; OR 0·57, 95 % CI 0·39, 0·86,P= 0·025; OR 0·63, 95 % CI 0·41, 0·96,P= 0·04; OR 0·62, 95 % CI 0·41, 0·95,P= 0·03, respectively). Higher concentrations of vitamin A were slightly associated with reduced risks of higher levels of IL-1 and IL-12 (OR 0·97, 95 % CI 0·95, 0·99,P= 0·03; OR 0·97, 95 % CI 0·94, 0·99,P= 0·03, respectively); when adjusting for BMI, this association was lost. No associations were found between vitamin C or vitamin E:lipids concentrations and inflammatory cytokines. In conclusion, higher Zn concentrations are associated with reduced risks of higher concentration of inflammation markers in a population of women with a high prevalence of obesity.

Few studies have assessed the association between the rs1414334 C/G polymorphism in the HTR2C gene and the development of the metabolic syndrome in patients treated with atypical antipsychotics. To provide further evidence, a cross-sectional study was conducted in Spain between 2012 and 2013 in 166 patients with these characteristics. In these patients, the association between the polymorphism and the presence of the metabolic syndrome was determined by implementing binary logistic regression models adjusted for variables associated with the metabolic syndrome. We did not confirm previous claims that the C allele of the polymorphism was linked to the metabolic syndrome: the association was in the opposite direction and non-significant. This conclusion held after taking gender and lifestyle variables into account.

The aim of this research was to study the impact of the unitary prevention delegates (UPDs) on the Spanish working environment. To this end, a cross-sectional study was carried out using microdata from the National Survey on Health and Safety Management in Companies (ENGE-2009) with a sample of 5147 work centres. To measure the relationship between the presence of UPD in workplaces with preventive management indicators and damage to health, individual and multiple logistic regression models were carried out, calculating the crude (cOR) and adjusted (aOR) odds ratios by sociodemographic covariates, with their corresponding 95% confidence intervals (95% CI). Ambivalent results were obtained. On the one hand, a positive impact of the UPDs was found, in the management of prevention showing a higher probability of prevention plans being carried out (aOR = 3.97; 95% CI: 3.26–4.83), risk assessments (aOR = 5.96; 95% CI: 4.44–8.01) and preventive actions were planned (aOR = 3.01; 95% CI: 2.55–3.56), as well as 1.56 times less likely to register minor occupational accidents (aOR = 0.64; 95% CI: 0.53–0.76). On the other hand, the presence of the UPDs did not promote the activation of a participatory culture and did not reduce the probability of suffering serious and fatal accidents at work. In conclusion, UPDs need to activate workers’ participation to improve results.
