The dual nature of obesity in metabolic programming: quantity versus quality of adipose tissue

Abstract The global prevalence of obesity has been rising at an alarming rate, accompanied by an increase in both childhood and maternal obesity. The concept of metabolic programming is highly topical, and in this context, describes a predisposition of offspring of obese mothers to the development of obesity independent of environmental factors. Research published in this issue of Clinical Science conducted by Litzenburger and colleagues (Clin. Sci. (Lond.) (2020) 134, 921–939) have identified sex-dependent differences in metabolic programming and identify putative signaling pathways involved in the differential phenotype of adipose tissue between males and females. Delineating the distinction between metabolically healthy and unhealthy obesity is a topic of emerging interest, and the precise nature of adipocytes are key to pathogenesis, independent of adipose tissue volume.

AbstractOver-conditioned dairy cows, classified by body condition score (BCS) and backfat thickness (BFT) are less able to metabolically adapt to the rapidly increasing milk yield after parturition. Based on serum metabolome and cluster analyses, high BCS cows (HBCS) could be classified into metabotypes that are more similar to normal (NBCS) cows, i.e., HBCS predicted normal (HBCS-PN) than the HBCS predicted high (HBCS-PH) cows—similar to the concept of obese but metabolically healthy humans. Our objective was to compare muscle metabolome and mRNA abundance of genes related to lipogenesis and lipolysis in adipose tissue between HBCS-PH (n = 13), HBCS-PN (n = 6), and NBCS-PN (n = 15). Tail-head subcutaneous fat was biopsied on d −49, 3, 21, and 84 relative to parturition. Potential differences in the oxidative capacity of skeletal muscle were assessed by targeted metabolomics in M. semitendinosus from d 21. Besides characteristic changes with time, differences in the mRNA abundance were limited to lipogenesis-related genes on d −49 (HBCS-PH > HBCS-PN). The HBCS-PH had more than two-fold higher muscle concentrations of short (C2, C4-OH, C6-OH) and long-chain acylcarnitines (C16, C18, and C18:1) than HBCS-PN, indicating a greater oxidative capacity for fatty acids (and utilization of ketones) in muscle of HBCS-PN than HBCS-PH cows.

The quality of human life is linked to the exploitation of mining resources. The Exploitability Index (EI) assesses the actual possibilities to enable a mine according to several factors. The environment is one of the most constraining ones, but its analysis is made in a shallow way. This research is focused on its determination, according to a new preliminary methodology that sets the main components of the environmental impact related to the development of an exploitation of industrial minerals and its weighting according to the Analytic Hierarchy Process (AHP). It is applied to the case of the ophitic outcrops in Cantabria (Spain). Twelve components are proposed and weighted with the AHP and an algorithm that allows for assigning a normalized value for the environmental factor to each deposit. Geographic Information Systems (GISs) are applied, allowing us to map a large number of components of the environmental factors. This provides a much more accurate estimation of the environmental factor, with respect to reality, and improves the traditional methodology in a substantial way. It can be established as a methodology for mining spaces planning, but it is suitable for other contexts, and it raises developing the environmental analysis before selecting the outcrop to be exploited.

Studies over the past 30 years have revealed that adipose tissue is the major endocrine and paracrine organ of the human body. Arguably, adiopobiology has taken its reasonable place in studying obesity and related cardiometabolic diseases (CMDs), including Alzheimer’s disease (AD), which is viewed herein as a neurometabolic disorder. The pathogenesis and therapy of these diseases are multiplex at basic, clinical and translational levels. Our present goal is to describe new developments in cardiometabolic and neurometabolic adipobiology. Accordingly, we focus on adipose- and/or skeletal muscle-derived signaling proteins (adipsin, adiponectin, nerve growth factor, brain-derived neuroptrophic factor, neurotrophin-3, irisin, sirtuins, Klotho, neprilysin, follistatin-like protein-1, meteorin-like (metrnl), as well as growth differentiation factor 11) as examples of metabotrophic factors (MTFs) implicated in the pathogenesis and therapy of obesity and related CMDs. We argue that these pathologies are MTF-deficient diseases. In 1993 the “vascular hypothesis of AD” was published and in the present review we propose the “vasculometabolic hypothesis of AD.” We discuss how MTFs could bridge CMDs and neurodegenerative diseases, such as AD. Greater insights on how to manage the MTF network would provide benefits to the quality of human life.

Abstract Autism is a set of heterogeneous neurodevelopmental conditions, characterised by early-onset difficulties in social communication and restricted, repetitive behaviour and interests. The worldwide population prevalence is about 1% with an increasing incidence and prevalence rates. Autism affects more male than female individuals, and comorbidity is common (>70% have concurrent conditions). Determinants of these changes in incidence and prevalence rates may also be related to exposure to environmental factors and to modifications in diagnostic concepts and criteria. In spite of the uncertainty in determinants of incidence of autisms, there is evidence that environmental characteristics play a significant role both as autism risk factors and as potential obstacles that influence the capabilities of autonomously and fully “using” everyday spaces. The workshop aims to provide a framework on risk factors of autism and explore the relationship with the built environment, focusing on the quality of the everyday spaces and projecting the effects that it could have in the long term on achieving a desirable level of quality of life. The 11th Sustainable Development Goals of United Nations “Make cities inclusive, safe, resilient and sustainable” underlines the necessity of designing policies and projects acting to enhance and promote healthy cities and communities by addressing the needs of the most vulnerable groups of inhabitants. Herewith we bring together the discipline of Public Health and Urban Design to promote an interdisciplinary debate on a little explored topic investigating how the approaches adopted during childhood to promote the wellbeing of people with ASD can be related or strengthen by focusing also on built environment design intervention to pursue and reach the same objectives even during adulthood. The workshop will consist of four presentations. The first focuses on giving an overview on current knowledge of intervention for people with autism, presenting also criteria for evidence-based interventions. The second explores the relationship between autism and built environment by providing an exhaustive framework of the available research literature in order to identify a first set of spatial requirements for autism friendly cities. The third examines the impact of built environment on ASD users with the aim of developing a specific evaluation tool for healthcare spaces and best practices formulation according to the specific sensorial hypo- or hyper-activation of people with autism. Finally, the fourth reports the results of a two years Research & Development project called “GAP REDUCE” finalized at developing an Assistive Technology tool to support people with ASD, adult and high-functioning, to plan urban itineraries towards daily destinations. Key messages World's incidence of autism is about 1% with an increasing incidence whose determining rates may also be related to environmental factors and to modifications in diagnostic concepts and criteria. Environmental characteristics play a significant role also as potential obstacles that influence the capabilities of people with autism of autonomously and fully “using” everyday spaces.

Abstract Introduction The relationship between H. pylori infection and obesity development has remained controversial among various studies. The aim of this study was to clarify the pooled effect of H. pylori infection on the development of obesity and vice versa. Methods We searched international databases including Medline (PubMed), Web of sciences, Scopus, EMBASE, Cochrane, Ovid, and CINHAL to retrieve all case–control studies reporting the effect of H. pylori on obesity and vice versa, which had been published in English between January 1990 and June 2019. The quality of included studies was assessed by the Modified Newcastle–Ottawa Scale for Case–Control studies. The logarithm of the odds ratio (OR) and its standard error was used for the meta-analysis. Results Eight case–control studies with 25,519 participants were included for qualitative and quantitative analyses. The pooled analysis showed that obese participants had a higher risk of H. pylori infection than lean participants with an odds ratio of 1.46 (95%CI: 1.26, 1.68). Also, the pooled analysis revealed that participants infected by H. pylori had a higher risk of obesity than non-infected participants with an odds ratio of 1.01 (95%CI: 1.01, 1.02). Conclusion The results of this meta-analysis showed that there was a positive correlation between the risk of H. pylori infection and the prevalence of obesity development. Thus, H. pylori positive patients were more likely to be obese, and obese individuals had higher risks of H. pylori infection.

Rotator cuff (RC) disease is a common musculoskeletal disorder of the shoulder entailing pain, with reduced functionality and quality of life. The main objective of this study was to present a perspective of the current scientific evidence about personalized, predictive, participatory, precision, and preventive approaches in the management of RC diseases. The personalized, predictive, participatory, precision and preventive (P5) medicine model is an interdisciplinary and multidisciplinary approach that will provide researchers and clinicians with a comprehensive patrimony of knowledge in the management of RC diseases. The ability to define genetic predispositions in conjunction with the evaluation of lifestyle and environmental factors may boost the tailoring of diagnosis and therapy in patients suffering from RC diseases.

Abstract Aims/hypothesis Levels of the microRNA (miRNA) miR-126-3p are programmed cell-autonomously in visceral adipose tissue of adult offspring born to obese female C57BL/6J mice. The spectrum of miR-126-3p targets and thus the consequences of its dysregulation for adipocyte metabolism are unknown. Therefore, the aim of the current study was to identify novel targets of miR-126-3p in vitro and then establish the outcomes of their dysregulation on adipocyte metabolism in vivo using a well-established maternal obesity mouse model. Methods miR-126-3p overexpression in 3T3-L1 pre-adipocytes followed by pulsed stable isotope labelling by amino acids in culture (pSILAC) was performed to identify novel targets of the miRNA. Well-established bioinformatics algorithms and luciferase assays were then employed to confirm those that were direct targets of miR-126-3p. Selected knockdown experiments were performed in vitro to define the consequences of target dysregulation. Quantitative real-time PCR, immunoblotting, histology, euglycaemic–hyperinsulinaemic clamps and glucose tolerance tests were performed to determine the phenotypic and functional outcomes of maternal programmed miR-126-3p levels in offspring adipose tissue. Results The proteomic approach confirmed the identity of known targets of miR-126-3p (including IRS-1) and identified Lunapark, an endoplasmic reticulum (ER) protein, as a novel one. We confirmed by luciferase assay that Lunapark was a direct target of miR-126-3p. Overexpression of miR-126-3p in vitro led to a reduction in Lunapark protein levels and increased Perk (also known as Eif2ak3) mRNA levels and small interference-RNA mediated knockdown of Lunapark led to increased Xbp1, spliced Xbp1, Chop (also known as Ddit3) and Perk mRNA levels and an ER stress transcriptional response in 3T3-L1 pre-adipocytes. Consistent with the results found in vitro, increased miR-126-3p expression in adipose tissue from adult mouse offspring born to obese dams was accompanied by decreased Lunapark and IRS-1 protein levels and increased markers of ER stress. At the whole-body level the animals displayed glucose intolerance. Conclusions/interpretation Concurrently targeting IRS-1 and Lunapark, a nutritionally programmed increase in miR-126-3p causes adipose tissue insulin resistance and an ER stress response, both of which may contribute to impaired glucose tolerance. These findings provide a novel mechanism by which obesity during pregnancy leads to increased risk of type 2 diabetes in the offspring and therefore identify miR-126-3p as a potential therapeutic target. Vitamin D deficiency and the rapid increase in the prevalence of obesity are both considered important public health issues. The classical role of vitamin D is in Ca homoeostasis and bone metabolism. Growing evidence suggests that the vitamin D system has a range of physiological functions, with vitamin D deficiency contributing to the pathogenesis of several major diseases, including obesity and the metabolic syndrome. Clinical studies have shown that obese individuals tend to have a low vitamin D status, which may link to the dysregulation of white adipose tissue. Recent studies suggest that adipose tissue may be a direct target of vitamin D. The expression of both the vitamin D receptor and 25-hydroxyvitamin D 1α-hydroxylase (CYP27B1) genes has been shown in murine and human adipocytes. There is evidence that vitamin D affects body fat mass by inhibiting adipogenic transcription factors and lipid accumulation during adipocyte differentiation. Some recent studies demonstrate that vitamin D metabolites also influence adipokine production and the inflammatory response in adipose tissue. Therefore, vitamin D deficiency may compromise the normal metabolic functioning of adipose tissue. Given the importance of the tissue in energy balance, lipid metabolism and inflammation in obesity, understanding the mechanisms of vitamin D action in adipocytes may have a significant impact on the maintenance of metabolic health. In the present review, we focus on the signalling role of vitamin D in adipocytes, particularly the potential mechanisms through which vitamin D may influence adipose tissue development and function.

The aim of the study was to evaluate associations of emotional state and quality of life with lipid concentration, duration of the disease, and the way of treating the disease in males and females with type 2 diabetes mellitus. A total of 53 persons with type 2 diabetes mellitus (27 males and 26 females; mean age, 58.7±8.9 years) and 56 healthy persons (26 males and 30 females; mean age, 54.7±8.3 years) participated in the study. Emotional state was evaluated by means of Profile of Mood State and quality of life by means of WHO Brief Quality of Life Questionnaire. Emotional state and quality of life were significantly worse, tension-anxiety and fatigue-inertia were significantly higher, vigor-activity was significantly lower in male patients with type 2 diabetes mellitus than in healthy males. In females, no significant differences in emotional state and quality of life comparing type 2 diabetes mellitus group and controls were detected. In females with type 2 diabetes mellitus, emotional state and quality of life were significantly better, scores of tension-anxiety, depression dejection, anger-hostility, and fatigue-inertia were significantly lower, and score of vigor-activity was significantly higher than in males with type 2 diabetes mellitus. Some significant correlations were found. In males, vigor-activity correlated with total cholesterol level and negatively correlated with triglyceride level. In females, significant correlations were found between scores of emotional state (tension-anxiety, depression-dejection, confusion-bewilderment, and total score of emotional state) and lipid levels (total cholesterol, triglyceride, and low-density lipoprotein cholesterol levels). There were no significant associations of emotional state and quality of life with duration of the disease in males and females with type 2 diabetes mellitus. No significant differences in emotional state and quality of life were found between males and females with type 2 diabetes mellitus, who were treated with oral antidiabetic preparations and insulin preparations.

The prevalence of obesity, especially in women of child-bearing age, is a global health concern. In addition to increasing the immediate risk of gestational complications, there is accumulating evidence that maternal obesity also has long-term consequences for the offspring. The concept of developmental programming describes the process in which an environmental stimulus, including altered nutrition, during critical periods of development can program alterations in organogenesis, tissue development, and metabolism, predisposing offspring to obesity and metabolic and cardiovascular disorders in later life. Although the mechanisms underpinning programming of metabolic disorders remain poorly defined, it has become increasingly clear that low-grade inflammation is associated with obesity and its comorbidities. This review will discuss maternal metainflammation as a mediator of programming in insulin sensitive tissues in offspring. Use of nutritional anti-inflammatories in pregnancy including omega 3 fatty acids, resveratrol, curcumin, and taurine may provide beneficial intervention strategies to ameliorate maternal obesity-induced programming.
