AbstractGrowing by an alarming rate in the Western world, obesity has become a condition associated with a multitude of diseases such as diabetes, metabolic syndrome and various cancers. Generally viewed as an abnormal accumulation of hypertrophied adipocytes, obesity is also a poor prognostic factor for recurrence and chemoresistance in cancer patients. With more than two-thirds of the adult population in the United States considered clinically overweight or obese, it is critical that the relationship between obesity and cancer is further emphasized and elucidated. Adipocytes are highly metabolically active cells, which, through release of adipokines and cytokines and activation of endocrine and paracrine pathways, affect processes in neighboring and distant cells, altering their normal homeostasis. This work will examine specifically how adipocyte-derived factors regulate the cellular metabolism of malignant cells within the tumor niche. Briefly, tumor cells undergo metabolic pressure towards a more glycolytic and hypoxic state through a variety of metabolic regulators and signaling pathways, i.e., phosphoinositol-3 kinase (PI3K), hypoxia-inducible factor-1 alpha (HIF-1α), and c-MYC signaling. Enhanced glycolysis and high lactate production are hallmarks of tumor progression largely because of a process known as the Warburg effect. Herein, we review the latest literature pertaining to the body of work on the interactions between adipose and tumor cells, and underlining the changes in cancer cell metabolism that have been targeted by the currently available treatments.

With approximately two-thirds of the United States adult population classified as overweight or obese, obesity remains a critical public health concern. Obesity not only contributes to several health complications including type 2 diabetes mellitus and cardiovascular disease, but the condition is also associated with sexual dysfunction in both women and men. Despite evidence linking obesity and its concomitant pathophysiology to sexual problems, the potential roles of psychosocial factors such as body image are understudied. This narrative review evaluates the research linkages between obesity and sexual dysfunction, with particular attention to the potential effects of body image dissatisfaction. A literature search of biomedical and psychological databases was used to identify research pertaining to obesity, sexual function, and/or body image constructs. The pathophysiological effects of obesity on sexual function are well-documented in mechanistic studies and animal trials, often with corroboration in human clinical samples. However, very few studies examine obesity, body image, and sexual function in tandem. Body image dissatisfaction appears to independently impinge upon the sexual response cycle and mental health outcomes, irrespective of body weight. While obesity is often associated with negative body image appraisal, it is unclear whether these constructs exert additive, synergistic, or antagonistic effects on sexual responsivity. Additionally, overweight/obese individuals who exhibit higher levels of body image satisfaction or self-confidence appear to be protected from the deleterious effects of obesity on sexual satisfaction, at least to some extent. Greater reliance upon conceptual/theoretical models from the body image literature may better clarify the relationships between these constructs.

The “easy” diseases have pretty much been beaten in the Western world, leaving doctors to contend with the more complex illnesses that stealthily overrun the body. Two-thirds of the deaths in the United States are now attributable to cancer or coronary disease. By the time these conditions manifest themselves, it’s often too late to intervene. Treatment is only likely to succeed if early signs of cancerous growth or clogging arteries can be detected. A tumor measuring a few millimeters across is plainly less threatening than one the size of a tennis ball, not least because there is less risk of metastasis at an early stage. The focus is therefore on enhancing rapid diagnosis, which in turn means improving medical imaging. Eighty percent of all diagnoses are based on images. Yet many small but life-threatening physical processes are still missed by the scanners, echographs, and other devices that peer inside our bodies. Growths measuring less than a centimeter tend to be overlooked, so scientists are constantly working on techniques capable of offering a more detailed internal picture. Breakthroughs in imaging technology can mean the difference between life and death. They’ll enable us to intervene sooner, boosting the patient’s survival chances. Little more than a generation ago, X-rays were the only means we had of looking inside the human body. The images they produce are flat, however, and lacking in depth information, which can make them hard to interpret. An ingenious technique was therefore devised in the 1970s that allowed a single three-dimensional image to be created by combining a series of X-ray photographs. The CT (computerized tomography) scan was the first technique to produce a genuine three-dimensional image of our insides. Doctors could now tell, for instance, whether an abnormality was located on top of a bone or beneath it. Several other techniques for producing three-dimensional images of the body have since become available, some of which require patients to be injected with a contrast agent to highlight specific parts of the body.

This paper examines the shifting paradigms of language used in HIV/AIDS life writing. As a testimonial genre, military metaphors have played a crucial role in mobilising communities and revealing how discourses around chronic illness inscribe themselves on the body. Through a textual analysis of three memoirs, Douglas Wright's Ghost Dance (2004) and Terra Incognito (2006), and David Caron's The Nearness of Others (2014), I argue that these texts represent a shift that instead engage metaphors of terrorism and security to convey meaning of lived experience and negotiate the precariousness of ongoing survival. Simultaneously, Wright and Caron maintain their health through protease inhibitors and reflect on the national anxieties produced by the September 11 terrorist attacks in the United States of America. Both writers draw on the language of terrorism, especially the images taken from the Abu Ghraib prison, to inform their own experiences as HIV-positive white, middle-upper class, gay men. The significance of these metaphors can be found in their individual struggles with depression. What this paper contributes to is an understanding of what it means to think about HIV after the pharmaceutical turn when HIV is no longer considered a death sentence in the Western world, how discourses of terror inform public and personal understandings of chronic illness and mental health, and how embodied experience informs autobiographical modes of expression, and vice versa.

Cancer mortality is primarily attributed to metastasis and the resulting compromise of organs secondary to the initial tumor site. Metastasis is a multi-step process in which the tumor cells must first acquire a migratory phenotype and invade through the surrounding tissue for spread to distant organs in the body. The ability of malignant cells to migrate and breach surrounding tissue/matrix barriers is among the most daunting challenges to disease management for men in the United States diagnosed with prostate cancer (CaP), especially since, at diagnosis, a high proportion of patients already have occult or clinically-detectable metastasis. The interaction between hepatocyte growth factor (HGF) secreted by the stroma, with its receptor c-Met located in the epithelium, must occur for epithelial CaP cells to become migratory. We studied the effects of grape-derived phytochemical resveratrol on the transition of epithelial tumor cells from sedentary to a mobile, penetrant phenotype. A time lapse microscopy assay was used to monitor the acquisition of the migratory phenotype by resveratrol. The results show that resveratrol inhibits HGF-mediated interaction between the stroma and epithelium and suppresses epithelial CaP cell migration by attenuating the control of epithelial-to-mesenchymal transition (EMT).

Tumor cells possess a high metabolic plasticity, which drives them to switch on the anaerobic glycolysis and lactate production when challenged by hypoxia. Among the enzymes mediating this plasticity through bidirectional conversion of pyruvate and lactate, the lactate dehydrogenase A (LDHA) and lactate dehydrogenase B (LDHB), are indicated. LDHA has a higher affinity for pyruvate, preferentially converting pyruvate to lactate, and NADH to NAD+ in anaerobic conditions, whereas LDHB possess a higher affinity for lactate, preferentially converting lactate to pyruvate, and NAD+ to NADH, when oxygen is abundant. Apart from the undisputed role of LDHA and LDHB in tumor cell metabolism and adaptation to unfavorable environmental or cellular conditions, these enzymes participate in the regulation of cell death. This review presents the latest progress made in this area on the roles of LDHA and LDHB in apoptosis and autophagy of tumor cells. Several examples of how LDHA and LDHB impact on these processes, as well as possible molecular mechanisms, will be discussed in this article. The information included in this review points to the legitimacy of modulating LDHA and/or LDHB to target tumor cells in the context of human and veterinary medicine.

The 2010 Centers for Disease Control and Prevention (CDC) report indicates that 63.4% of Delaware’s adult population is overweight and 28% is obese. Here, the authors reveal analyses acquired from detailed investigations about the importance of gender, and other lifestyle factors and behaviors on the Body Mass Index (BMI) trends amongst an indiscriminate sample of the Wesley College (Wesley) undergraduate population. A 25-question paper-format survey was distributed to 307 randomly chosen Wesley undergraduates. The accrued qualitative (or categorical) data were transferred to an Excel spreadsheet to construct and observe frequency distributions. A Chi-square test of independence (χ2) was performed between BMI status (normal, overweight, obese) and the following factors: gender, diet plan, adherence to the United States Department of Agriculture (USDA) MyPlate nutrition guide, use of the seasonal flu shot, weekly workout schedule, supplement usage, participation on athletic teams, questioning of label nutritional facts, and the use of added salt in food. A 2-sample proportion test was performed between students who were overweight or obese for the same factors. Also performed were t-tests for mean BMI for those who followed USDA MyPlate guidelines and for those who did not. An analysis of 278 completed surveys show that 29.5% of the Wesley respondents are overweight and 19.8% are obese. The mean BMI for males was statistically higher than the mean BMI for females. The mean BMI for students living on-campus was statistically higher than the mean BMI for students living off-campus. The results also demonstrate that adhering to the USDA dietary recommendations for fruit and dairy can be important factors in reducing the risk of obesity.

BACKGROUND Though public health systems are responding rapidly to the COVID-19 pandemic, outcomes from publicly available, crowd-sourced big data may assist in helping to identify hot spots, prioritize equipment allocation and staffing, while also informing health policy related to “shelter in place” and social distancing recommendations. OBJECTIVE To assess if the rising state-level prevalence of COVID-19 related posts on Twitter (tweets) is predictive of state-level cumulative COVID-19 incidence after controlling for socio-economic characteristics. METHODS We identified extracted COVID-19 related tweets from January 21st to March 7th (2020) across all 50 states (N = 7,427,057). Tweets were combined with state-level characteristics and confirmed COVID-19 cases to determine the association between public commentary and cumulative incidence. RESULTS The cumulative incidence of COVID-19 cases varied significantly across states. Ratio of tweet increase (p=0.03), number of physicians per 1,000 population (p=0.01), education attainment (p=0.006), income per capita (p = 0.002), and percentage of adult population (p=0.003) were positively associated with cumulative incidence. Ratio of tweet increase was significantly associated with the logarithmic of cumulative incidence (p=0.06) with a coefficient of 0.26. CONCLUSIONS An increase in the prevalence of state-level tweets was predictive of an increase in COVID-19 diagnoses, providing evidence that Twitter can be a valuable surveillance tool for public health.

The body weight of U.S. adults and children has risen markedly over the past three decades. The physical health consequences of obesity are widely documented, and emerging research from the Midlife in the United States study and other large-scale surveys reveals the harmful impact of obesity on adults’ psychosocial and interpersonal well-being. This chapter synthesizes recent research on the psychosocial implications of body weight, with attention to explanatory mechanisms and subgroup differences in these patterns. A brief statistical portrait of body weight is provided, documenting rates and correlates of obesity, with a focus on race, gender, and socioeconomic status disparities. The consequences of body weight for three main outcomes are described: institutional and everyday discrimination, interpersonal relationships, and psychological well-being. The chapter concludes with a discussion of the ways that recent integrative health research on the psychosocial consequences of overweight and obesity inform our understanding of population health.

BACKGROUND: Vertigo and dizziness are extremely common conditions in the adult population and therefore place a significant social and economic burden on both patients and the healthcare system. However, limited information is available for the economic burden of vertigo and dizziness across various health care settings. OBJECTIVE: Estimate the economic burden of vertigo and dizziness, controlling for demographic, socioeconomic, and clinical comorbidities. METHODS: A retrospective analysis of data from the Medical Expenditures Panel Survey (2007–2015) was performed to analyze individuals with vertigo or dizziness from a nationally representative sample of the United States. Participants were included via self-reported data and International Classification of Diseases, 9th Revision Clinical Modification codes. A cross-validated 2-component generalized linear model was utilized to assess vertigo and dizziness expenditures across demographic, socioeconomic and clinical characteristics while controlling for covariates. Costs and utilization across various health care service sectors, including inpatient, outpatient, emergency department, home health, and prescription medications were evaluated. RESULTS: Of 221,273 patients over 18 years, 5,275 (66% female, 34% male) reported either vertigo or dizziness during 2007–2015. More patients with vertigo or dizziness were female, older, non-Hispanic Caucasian, publicly insured, and had significant clinical comorbidities compared to patients without either condition. Furthermore, each of these demographic, socioeconomic, and clinical characteristics lead to significantly elevated costs due to having these conditions for patients. Significantly higher medical expenditures and utilization across various healthcare sectors were associated with vertigo or dizziness (p <  0.001). The mean incremental annual healthcare expenditure directly associated with vertigo or dizziness was $2,658.73 (95% CI: 1868.79, 3385.66) after controlling for socioeconomic and demographic characteristics. Total annual medical expenditures for patients with dizziness or vertigo was $48.1 billion. CONCLUSION: Vertigo and dizziness lead to substantial expenses for patients across various healthcare settings. Determining how to limit costs and improve the delivery of care for these patients is of the utmost importance given the severe morbidity, disruption to daily living, and major socioeconomic burden associated with these conditions.
