Analysis of morbidity and mortality of cervical cancer in Zabaikalskii krai

Relevance. Currently, noncommunicable diseases are responsible for the majority of deaths worldwide. Cervical cancer (СС) is actual medical and social problem. Comparative analysis of morbidity, mortality and state of oncological care in Zabaikalskii krai and Russian Federation was purpose of this study. Aim. Estimation of morbidity and mortality from cervical cancer in the Zabaikalskii krai in dynamics over 10 years. Materials and methods. The assessment was carried out on the basis of an analysis of statistical data concerning the burden of morbidity and mortality from cancer in the territory of the Zabaikalskii krai and the Russian Federation. Statistical analysis was performed using the Statistica 10 software. Standardized indicators were used to analyze morbidity and mortality. Differences were considered statistically significant at an error level of p0.05. Results. Analysis cervical cancer incidence of the Zabaikalskii krai and the Russian Federation in dynamics from 20072017 (100 000 female population) demonstrates a significant increase in the incidence of cervical cancer. On the territory of the Russian Federation, there is a steady increase on average by 2.26% annually, the growth dynamics over this period amounted to 25.81%, and in 2017 amounted to 15.76. In the Zabaikalskii krai in 2017, the incidence of cervical cancer was 41.54. In dynamics over the past 10 years, this indicator increased by 46.42%, which is significantly higher than in the Russian Federation (p0.01). The analysis of standardized indicators of mortality from cervical cancer in the structure of mortality from cancer in Russia in 2017 amounted to 5.18%, which corresponds to 10th place. Mortality in the first year since diagnosis was 14.6%. In the group of women of reproductive age 3039 years, mortality from cervical cancer was also maximum (23.6%) compared with other age groups. Mortality from cervical cancer in the Zabaikalskii krai was 7.17 in 2017. This indicator is higher than the all-Russian values by 38.41% (p0.01). In dynamics over 10 years, a decrease in mortality in the Zabaikalskii krai by 6.39% is noted. Conclusion. It should be noted that a significant increase in the incidence of cervical cancer, and a decrease in mortality from this pathology, may indicate some success in the diagnosis and treatment of this pathology. In this regard, the search for new diagnostic, prognostic and predictive markers at this stage is relevant in solving this all-Russian medical and social problem.

Introduction. The article is devoted to the analysis of the state of cardiovascular morbidity (CVD), mortality of the population and the demographic situation, as well as determining the prevalence of CVD as a medical and social problem at the federal and regional levels. The analysis of the implementation of the policy of providing medical care to patients with CVD, modern scientific publications on the indicated problem, and the search for ways to solve the medical and social problem of high morbidity and mortality from CVD are carried out.Materials and methods. The data of official statistical information on the morbidity and mortality of the adult population of the Russian Federation and the Voronezh region for 2010–2019 were used, the works of modern domestic and foreign scientists dealing with the solution of this problem were studied.Results. It has been established that high CVD and mortality are an acute medical and social problem. Mortality from CVD in Russia in 2019 amount to 46.8% of the total number of deaths. The mortality rate from CVD in Russia exceeds that in the developed countries of the European Union. Despite the presence of positive trends in the fight against this disease, the mortality rate from CVD in the Voronezh region is higher than the average for the Russian Federation (601.8 and 573.2 per 100 thousand population, respectively). The deterioration of the demographic situation, the presence of active targeted projects in this area are the basis for conducting scientific research to find ways to duce CVD and mortality in the adult population.Conclusion. The high morbidity and mortality from CVD requires a search for new ways to solve the problem, since the existing approaches to its solution are not holistic and universal. It is urgent to develop an integrated approach that takes into account the medical and social characteristics of the adult population of the region.

Introduction. The study was conducted to identify the main trends in reproductive, maternal, and newborn’s health to justify additional measures to achieve sustainable demographic development of industrial regions in the Arctic zone of the Russian Federation (AZRF). Material and methods. The study was carried out using reporting and statistical materials characterizing the main demographic and health statistics of the reproductive health in the female population of Vorkutinsky municipal district (16 settlements), summarized in the “Demographic Yearbook of the Komi Republic” (2015-2019), and the annual reports of medical organizations providing obstetric and gynaecological medical care to the population of Vorkuta. Results. As in many other areas of the Russian Arctic, in the city of Vorkuta, there were observed: a population decline from 60.4 in 2015 to 54.2 thousand people in 2019, a decrease in the number of women at reproductive age from 19.9 to 19.2 thousand people, as well as a significant increase in the frequency of preterm birth (PB) (from 2,2% to 8,4%) mainly due urogenital infection (42,3%), chronic placental insufficiency (CPF) (27%), multiple pregnancies (11,5%), preeclampsia (7,7%), cervical incompetence (7,7%), uterine scar inconsistency (3,8%). The incidence of PB among primiparous women was significantly lower than that among multiparous women - 34.6% and 65.3%, respectively. The frequency of abortions increased from 15.4 to 20.9 per 1000 women of reproductive age, and there was an increase by 16.3% in the rate of spontaneous abortions among women 18-44 years of age at the pregnancy terms from 12 to 22 weeks. Perinatal mortality during the study period increased from 7.5 ‰ to 12.7 ‰ in 2019. (on average in Russia 7.23 ‰). The stillbirth rate was 9.5 ‰ (on average in Russia 5.51 ‰). The main cause of stillbirth in the study period was intrauterine asphyxia due to the decompensated chronic placental insufficiency. Among the possible reasons for increased fetal infantile losses is a significant proportion in the general population of the Vorkuta of workers exposed to adverse occupational risk factors (25.7% compared to 14.1% in the Russian Arctic as a whole). Conclusion. Among the population living in the area of the Pechora coal basin, the risk remains for an increase in the demographic crisis phenomena mainly due to the rise in the frequency of pregnancy disorders and, above all, a high level of fetal-infantile losses. To solve one of the main tasks of national security to prevent further depopulation of the Arctic regions, set in the Decree of the President of the Russian Federation*, it is necessary besides socioeconomic measures to preserve the population number, to develop and implement programs to effectively reduce fetal-infantile losses, including those potentially associated with adverse occupational exposure to reproductive risk factors.

Исследование смертности занимает важное место в демографии в целом и в особенности в демографии старения. России свойственна неоднородность демографического развития, в частности региональная дифференциация показателей смертности населения. Работа посвящена анализу зависимости смертности в регионах России в старших возрастных группах от социально-экономических показателей. Исследование проводили с использованием методов регрессионного анализа, где в качестве единиц наблюдения выступали субъекты РФ, в качестве объясняемой переменной - смертность мужского (женского) населения старшего (60+) возраста, представленная в виде стандартизованных по структуре населения коэффициентов. В результате работы выявлены значимые социально-экономические факторы, позволяющие объяснить различия уровней смертности в субъектах РФ. Studies of old-age mortality are an important part of demography, especially the demography of ageing. Demographic development of Russia is characterized by heterogeneity including regional differences in mortality. The aim of the paper is to analyze the dependence of mortality at old-age groups on socio-economic indicators. The study is conducted using methods of regression analysis; the units of observation are the regions of the Russian Federation, the explained variable is the mortality rate of male (female) population at older (60+) ages standardized by the population-age structure. As a result, the significant socio-economic factors, explaining the differences of mortality rates in the regions of the Russian Federation, were identified.

The results of a study of the concentration of vitamin D in blood serum of 3455 residents of Ufa are presented. Physiological values of 25(OH)D were observed in 34.9 % (274) of men and 31.7 % (847) of women, which is 32.4 % (1121) of all examined. Vitamin D deficiency was detected in 67.1 % of the examined. The level of 25(OH)D in the blood serum of residents is on average 75 nmol/L, which is the upper limit of vitamin D deficiency. Vitamin D deficiency is extremely common among residents of this region of the Russian Federation (67.1 %).

Рак печени (РП) - тяжелейшая онкопатология. В России смертность превышает число первично-учтенных больных в среднем. Ежегодно в России регистрируют более 8 тыс. (8 810 - в 2018 г.) случаев рака печени и более 10 тыс. (10 018 - в 2018 г.) случаев смерти от этой причины. Использованы стандартные методы медицинской статистики, все зарегистрированные первичные случаи РП в России. Углубленная разработка материала проведена на основе базы данных Популяционного ракового регистра Северо-Западного Федерального округа РФ. Проанализированы данные всего комплекса доступной информации в целом по России. Установлен прирост заболеваемости РП населения России за 18 лет в «грубых» показателях и абсолютных числах на 20% и в стандартизованных - на 1,22%. У женщин отмечено снижение учтенных случаев РП на 4,55%. Смертность населения России в стандартизованных показателях снизилась за 18 лет практически на 3%. Превышение числа умерших над заболевшими от РП существенно возрастает в старших возрастных группах. Индекс достоверности учета (ИДУ) - это отношение абсолютных чисел или «грубых» показателей смертности населения к заболеваемости. Для всех злокачественных новообразований (ЗНО) величина в среднем по России составила 0,47, практически совпадающая со среднеевропейской. ИДУ у мужчин - 0,55, выше расчетных показателей для женщин - 0,40, что в первую очередь связано с более благоприятной структурой заболеваемости женщин ЗНО. Медиана выживаемости первичных больных РП составляет немногим более 3 мес. Пятилетняя выживаемость больных РП даже на ранних стадиях составляет у лиц моложе 60 лет с I стадией - 25,3%, старше 60 лет - только 17,2%, со II стадией - 17,9 и 11% соответственно. Liver cancer is a severe oncopathology. Mortality exceeds the number of primary-accounted patients in Russia on average. Every year in Russia more than eight thousand (8 810 in 2018) cases of liver cancer and more than 10 000 (10 018 in 2018) deaths from this cause are registered. Used Standard methods of medical statistics, all registered primary cases of liver cancer in Russia. In-depth development of the material was carried out on the basis of the database of the Population cancer register of the North-Western Federal district of the Russian Federation. The analysis of the data of the whole complex of available information in Russia as a whole is carried out. The increase in the incidence of liver cancer in Russia over 18 years in «rough» indicators and absolute numbers by 20 % and in standardized by 1,22 % was established. In women, there was a decrease in reported cases of liver cancer by 4,55 %. The mortality rate of the Russian population in standardized indicators has decreased by almost 3 % over 18 years. The excess of the number of deaths over those from liver cancer increases significantly in older age groups. The index of reliability of accounting is the ratio of absolute numbers or «rough» indicators of mortality to morbidity. For all malignant tumors, its value on average in Russia was 0,47, almost coinciding with the average European. The index of reliability of accounting for men is 0,55, higher than the calculated indicators for women - 0,40, which is primarily due to a more favorable structure of the incidence of malignant tumors in women. The median survival of primary liver cancer patients is a little more than three months. The fi ve-year survival rate of patients with liver cancer even in the early stages is in the first among persons younger than 60 years - 25,3 %, over 60 years - only 17,2 %, in the second, respectively, 17,9 and 11%.

The article offers a socio-psychological analysis of religious identity structure of Muslim youths who live in the Russian Federation. The research was conducted in the Chechen Republic (city of Grozny). The two groups of respondents were selected, the high school pupils (9–10th grades) and students (1–2 year). In order to study their religious identity was applied a “Religious identity components” questionnaire. The original version of this tool was developed by D. Van Camp, it was adapted to the Russian circumstances by V. A. Shorokhova and subsequently developed by O. S. Pavlova. The results of the study prove that religious identity of the two age groups in both cases remains the same, i.e. the four-factor model, regardless the respondents’ age. However, there are still some specific features as applied to each group.

Aim. To analyze the demographic indices in connection with aging of the population in Perm Krai and the Russian Federation and as a result – to single out problems and ways of solution. Materials and methods. Calculation and dynamic analysis of reproduction of the population in Perm Krai and the Russian Federation over the period of 2005 – 2017 was performed using official statistic data in connection with gender-and-age-specific characterization of the population. Results. The processes of aging of the population were estimated from demographic positions. Socioeconomic problems, associated with them, were singled out, the ways of their solving were determined in accordance with national projects regarding health and health care. Conclusions. At the expense of growing index of expected average life, decrease in fertility and birth rate there is observed accumulation of older age groups of the population, i.e. process of aging. It, naturally, results in growth of chronic pathology, mortality not only in the older age periods but also in the able-bodied part of the population, thus, reducing working potential.

The relevance of the direction chosen for research is multifactorial. First, there is a steady increase in attacks on property by deception or abuse of trust. Second, the emergence of new forms of fraudulent activities requiring a symmetrical response from government agencies. Third, the offensive, not always error-free development of criminal policy in the form of the creation of new legal and technical mechanisms to counter the considered type of criminal deviant behavior of selfish orientation. All this prompted us to identify and consider the most significant methodological problems in the area taken for research in the form of their demonstration, as well as proposals for directions for their solution. The first criminal law flaw in the state strategy in the fight against fraud is the fallacy in the systematization of the crimes reflected in Art. 159-159of the Criminal Code of the Russian Federation. The substitution of the significance of public relations protected by the norms included in these articles caused the imbalance in the Special Part of the Criminal Code. RF. The second methodological problem is the imbalance in the cost criteria of Art. 159-159of the Criminal Code of the Russian Federation, which form the basis for their criminalization and differentiation. The third problem is the fact that there are separate elements of fraud with their fixation in separate articles of the Criminal Code of the Russian Federation, depending on the areas of encroachment. In the article, on the basis of the conducted critical analysis and the presented argumentation, directions for resolving the noted methodological problems, theoretical, applied and legislative format, are proposed.

Aim. Conduct a comparative assessment of the main indicators of the incidence of cervical cancer in Khabarovsk Krai (20092019) аnd the prevalence of types of human papillomavirus among the female population. Materials and methods. A comparative analysis of the data of the official statistics of the Ministry of Health of Khabarovsk Krai, using the Rosstat database for the period 20092019, taking into account the incidence, mortality, prevalence of types of human papillomavirus, was carried out. The dynamics trend was determined in the process of graphical analysis of the diagram and by modeling trends. The results of the studies were subjected to the methods of statistical information processing. Results. The results revealed in our study indicate that the increase in the incidence of cervical cancer in Khabarovsk Krai from 2009 to 2019 was 44.4% (with an average annual growth rate of 4.5%). Most often, cervical cancer in Khabarovsk Krai is detected in urban residents. The share of urban women with this pathology in 2019 reached 77.2%. The proportion of stage III cancer detected exceeds the proportion of stage IIIIV cancer by 2.5 times. Mortality in patients with cervical cancer in Khabarovsk Krai exceeds that in the Russian Federation. Of the 1617 residents of the city of Khabarovsk of reproductive age, 883 (54%) women had the presence of human papillomavirus. For our region, most often, women had highly oncogenic types of human papillomavirus 16, 56, 51. Conclusion. Statistical analysis based on the results of diagnosing early and advanced stages of cervical cancer, mortality from this nosological form, and the spread of the human papillomavirus remain among the most important criteria. The data obtained make it possible to assess the effectiveness of the chosen tactics in the regions and take timely corrective measures aimed at both early detection of cervical cancer and a decrease in the persistence of the human papillomavirus in women with background cervical pathology.
