Background: Type 2 diabetes mellitus (DM) is now considered as a growing global public health concern due to cost associated with diabetic micro and macrovascular complications. Diabetic retinopathy (DR) is one of the leading causes of vision loss. Accurate estimation of prevalence of DR among diabetic patients and associated risk factors are of crucial importance to plan and execute preventive strategies in the community. Aims and Objectives: The present cross sectional hospital based observational study was undertaken to determine the prevalence of DR in diabetic patients at their first ophthalmological contact and toevaluate associated risk factors. Materials and Methods: In this cross sectional hospital based observational study, all diabetic patients visiting ophthalmology clinic for the first time after being diagnosed as diabetic were enrolled. After recording demographic data and biochemical findings, each patient was investigated for DR and correlated with associated risk factors. Results: Of 1699 patients, majority (68.9%) of them had come to ophthalmology clinic with complaint of decreased vision. Only 16.12 % (274) patients had been referred for retinal exam by treating physician. DR was prevalent in 242(16.98%) non-referred and 31(11.31%) referred patients. Mean age and mean duration of diabetes was significantly higher in non-referred patients. Vision threatening DR was also significantly higher in nonreferred patients. Prevalence of DR was significantly correlated with age at presentation, duration of diabetes, rural background, uncontrolled diabetes, systolic blood pressure and high BMI. Conclusion: Low referral by treating physician and lack of awareness among patient accounts for late presentation of diabetic patients to ophthalmologist. Beyond just developing strategies to promote screening programs for early detection and management of DR, education of the patients,comprehensive planning and coordination between ophthalmologist and physician can go a long way to decrease the economic and social burden of preventable blindness due to diabetic retinopathy.

Background: Early detection of diabetic retinopathy (DR) is the most important factor in reducing the blinding complications due to diabetes. Study of the various socio-demographic factors affecting awareness of DR will help us to formulate effective screening programs for early detection. Objectives of the study were to find the sociodemographic determinants associated with awareness of DR and to evaluate the association of stage of DR with awareness about diabetic retinopathy.Methods: A cross sectional study was conducted among 384 patients who had been diagnosed with diabetes mellitus in various medical camps and diabetic clinics at Thiruvananthapuram. Socio demographic variables and awareness were assessed using structured self-administered questionnaire. Dilated fundus evaluation was done and retinopathy classified. Data entered to excel sheet and analysis done using statistical software (SPSS version 20).Results: Among the 384 patients, 44.9% were diagnosed to have diabetic retinopathy. Among the diagnosed 41.6% had mild non proliferative DR (NPDR), 33.0% had moderate NPDR, 11.45% had severe NPDR. 13.4% of subjects were diagnosed to have proliferative DR. 53.1% of the subjects with diabetic retinopathy had clinically significant macular edema. There was no statistically significant association between the stage of DR to awareness. Among the socio demographic variables, significant positive association with level of awareness was obtained for duration of diabetes (OR=10.96; p=0.004)Conclusions: Duration of diabetes was significantly associated with level of awareness. There was no statistically significant association between the stage of diabetic retinopathy to awareness. This signifies the urgent need to intensify our diabetic retinopathy awareness programs.

AbstractA cross-sectional study was carried out to determine the seroprevalence of Toxoplasma gondii and associated risk factors in pigs in the largest pork-producing region in Cuba. Serum samples from 420 pigs, including 210 sows and 210 post-weaning pigs, were tested for antibodies against T. gondii using a commercial indirect enzyme-linked immunosorbent assay. Anti-T. gondii antibodies were detected in 56 animals (13.3%, 95% CI: 10.1–16.6). A generalized estimating equations model revealed that the risk factors associated with higher seropositivity in pigs were altitude (higher in farm’s location < 250 m above sea level (masl) versus ≥ 250 masl) and age (higher in sows compared to post-weaning pigs). The results indicated that this protozoan parasite is widely distributed on pig farms in the study area, which is a public health concern since the consumption of raw or undercooked pork meat products containing tissue cysts is considered one of the main routes of T. gondii transmission worldwide. Control measures should be implemented to reduce the risk of exposure to T. gondii in pigs in Cuba.

Background: Diabetes is a major public health concern that affects nearly 463 million (9.3%) of global adult population. Diabetic retinopathy, which affects around 35% of all diabetic patients, is the fifth leading cause of preventable global blindness. This study was done to determine the status of diabetic retinopathy screening and the factors that influence its uptake among diabetic patients attending a tertiary care setting in Kerala, India.Methods: 200 patients with diabetes mellitus on physician care were enrolled for a questionnaire-based survey which collected information on patient demographics, education, occupation, patient’s awareness of retinopathy, screening, diabetic blindness and their source of such knowledge.Results: 83% were aware that diabetes can result in vision loss. 61% were aware that diabetic blindness is preventable. 42% patients were aware of screening options for retinopathy. The awareness of retinopathy screening was significantly associated (p=0.0001) only with duration of diabetes.Conclusions: Awareness of diabetic retinopathy among diabetic patients in Kerala was sub optimal. Better patient education and use of mass media can increase awareness on diabetes retinopathy screening programs.

Uncontrolled blood pressure is a threat to diabetic patients’ life. The aim of this study was to identify risk factors of hypertension among diabetic patients at different stages from Yaoundé Central Hospital and Etoug-Ebe Baptist Health Center of Cameroon. A hospital-based cross-sectional study was conducted for 6 months, and 109 participants (types 1 and 2), aged 24–81 years, were enrolled using simple random sampling. A pretested structured questionnaire was used to collect sociodemographic data, habitual behaviors, clinical history blood pressure, and anthropometric measures. The prevalence of hypertension was 86.2%. Of the total, 13.8% participants were normotensive, 32.1% stage 1 hypertensive, and 54.1% stage 2 hypertensive. Being a male (p=0.046) and not smoking (p=0.036) were negatively associated with stage 1 hypertension whereas eating less than 3 times (p=0.046) and duration of diabetes greater than 9 years among women (p=0.039) were positively associated. Age above 40 years (p=0.002) was negatively associated with stage 2 hypertension. However, age above 40 years had a negative effect among Christian, less educated diabetics, people having diabetes for more than 9 years, and those on medical treatment (5.556≤specific OR≤10.278). Duration of diabetes (age-adjusted OR=1.155; p=0.003) and abnormal waist circumference (crude OR=4.074; p=0.024) were positively associated with stage 2. Abnormal waist-to-hip ratio (crude OR=3.773; p=0.028) and feeding rate greater than 2 times a day (WHR-adjusted OR=3.417; p=0.046) were positively associated with hypertension (stages 1 and 2). This study suggests that hypertension, present at its two stages, is a serious health issue among diabetic patients. Thus, appropriate intervention should be put in place to prevent and control hypertension by managing identified risk factors.

Background: Retinopathy is the leading cause of blindness in persons with diabetes. Strict monitoring and maintenance of normal blood glucose specially HbA1c and prevention of different risk factors can prevent and delay the diabetic retinopathy. The purpose of the study was to explore the factors influencing or related to the development of the diabetic retinopathy with spcial concern to the HbA1c levels.Materials and Methods: We studied 400 type 2 diabetic patients in this cross-sectional study which was conducted in the out-patient department of BIRDEM hospital, Bangladesh. The randomly selected patients were evaluated for the presence of retinopathy through the review of their registered diabetic guide book. We included sociodemographic information, blood pressure, anthropometry (height, weight, BMI) and lipid profile of the patients. Glycaemic status was assessed by HbA1c (HbA1c was categorized into 3 groups) and plasma glucose levels. We used Student's t-test, Chi-square test and logistic regression analysis to determine and quantify the association of diabetic retinopathy with various risk factors specially HbA1c.Results: 400 type 2 diabetic patients (male 166 and female 234) were studied. The prevalence of retinopathy was 12.3%; male 12.7%, female 12.0%. Increasing HbA1c categories above 7.0% were significantly associated with increased prevalence of retinopathy (4.2 vs 12.3 vs 18.1%;c2 = 12.529, p < .01). Logistic regression models of univariate analysis showed that the risk of retinopathy at HbA1c categories >7.0% was (OR = 3.22; 95% CI: 1.12-9.25) and the risk was strongly increased at the HbA1c categories 8% (OR = 5.07; 95% CI: 1.90-13.50). Advanced age (OR = 2.92; 95% CI: 1.44-5.91), longer duration of diabetes (OR = 3.08; 95% CI: 1.49-6.37), presence of hypertension (OR = 2.42; 95% CI: 1.14-5.16), FBG (OR = 1.139; 95% CI: 1.036-1.251), blood glucose 2 hours ABF (OR = 1.124; 95% CI: 1.046-1.207) and SBP (OR = 1.033; 95% CI: 1.011-1.056) had significant association with retinopathy.Conclusions: HbA1c categories >7.0% is an important risk factor for the development of retinopathy. Poor glycaemic control, advanced age, longer duration of diabetes, hypertension are other significant risk factors of diabetic retinopathy.KYAMC Journal Vol. 6, No.-2, Jan 2016, Page 614-619

Abstract Objectives To assess the prevalence and risk factors for fundus status among patients with type 2 diabetes mellitus (T2DM) using fundus screening in four towns of Quzhou city, Zhejiang province of China. Methods This cross-sectional study included 230 T2DM patients of four towns in Quzhou city, Zhejiang province of China. Participants were examined for the diabetes related fundus abnormalities and possible risk factors. Results Almost half of the T2DM patients (53.04%) reported with diabetic retinopathy (DR). Patients with diabetic retinopathy had longer duration of diabetes (P < 0.001) and higher HbA1c (P < 0.01). Risk factors for development of diabetic retinopathy included duration of diabetes and HbA1c. The prevalence of DR increased with the prolongation of the disease duration. The prevalence of DR was 28.79% in the group of disease duration < 5 years, 46.25% in the group of disease duration 5–10 years, 72.92% in the group of disease duration 10–15 years and 88.57% in the group of disease duration ≥ 15 years. What’s more, the prevalence of DR also increased with the increment of HbA1c levels. The prevalence of DR was 44.62% in the group of HbA1c < 7%, 53.13% in the group of HbA1c 7%-8%, 62.92% in the group of HbA1c ≥ 8%. Conclusion The prevalence of DR was disturbingly high. Risk factors for DR were similar to other studies and included duration of diabetes and HbA1c. Thus, good glycemic control remains the core foundation of managing DR.

Syphilis in pregnant women caused by Treponema pallidum remains a major cause of reproductive morbidity and poor pregnancy outcomes in developing countries. Severe neonatal infections, stillbirths, perinatal deaths, and low birth weight babies are common among mothers with syphilis infection. The aim of the study was to assess the prevalence and risk factors associated to syphilis in pregnant women. A cross-sectional and retrospective studies were conducted among pregnant women who attended the prenatal service at the Gisovu health center. Participants were pregnant women who tested positive for syphilis based on lab results and responses collected from anonymous questionnaires completed with sexual behaviors, demographics, sexual partners, history of abortion, and knowledge about STDs. The prevalence of syphilis was 5.74%. Syphilis was associated to the history of previous abortion (P = 0.005 < 0.05), a low level of education (P = 0.049 < 0.05), and marital status (P = 0.044 < 0.05). The main associated behavioral factor was women who had sex with different partners using condom and did not acquire syphilis infection (P = 0.00 < 0.05). Syphilis is still a public health concern in patients and especially in pregnant women as shown by the findings of this study. It is very important to screen all pregnant women for syphilis and to strengthen the existing antenatal care services and health education on transmission and prevention of the disease.

Microalbuminuria is the most leading cause for cardiovascular disease, diabetic nephropathy, end stage renal diseases, and death among diabetic patients. The study carried out                                                                                       to find out the prevalence of microalbuminuria in a sample of Sulaimani diabetics and to determine its relation to different associated risk factors. The first part of study was a cross-sectional study conducted through examination of records of diabetic patients registered in Sulaimani Diabetic Center from June 2010 to May 2012. The second part was a case- control study that conducted in the same center, including 50 cases of diabetics with positive microalbuminuria, and 50 controls with negative one, that were registered in the same center and matched by age and gender. A specially designed questionnaire was used by researcher to collect information from the records. SPSS version 21.0 was used for data analysis. Males accounts for half of cases and controls, highest proportion of sample was found among age group (60-69) years about 34%. The prevalence of microalbuminuria was 29%. The study showed a statistical significant association of microalbuminuria with obesity, smoking, hypertension, retinopathy, ischemic heart disease, poor glycemic control, and dyslipidemia.  In conclusion moderate prevalence of microalbuminuria was found among diabetic patients, the prevalence was more common in poor glycemic control, and hypertensive diabetics. There was significant relation between microalbuminuria and diabetic nephropathy.
