scholarly journals The Relationship between Phimosis, Smegma, and Preputial Bacteria with Inflammatory Status of Circumcised Patient

Phimosis is the inability of the penis to potrude from the prepuce either partially or completely and the preputial retractability increases with age. Circumcision is believed by various parties as a form of defense against pathogens that may cause various abnormalities. Smegma is whitish lump that formed from desquamation of preputial epithelial cells. Smegma and bacteria in the prepuce may cause abnormalities such as genital infections, urinary tract infections, and even malignancy. This study aims to determine the presence between phimosis, smegma, and preputial bacteria, and its relationship with inflammatory status of circumcised patient. A cross sectional analytical study for boys who underwent circumcision at Saiful Anwar General Hospital Malang. The data will be analyzed using chi-square and will be calculated for the relative risk. There were 76 patients who underwent circumcision from March 2018 until August 2018. The result was found that the presence of phimosis was significantly associated with the presence of smegma and preputial bacteria (p <0.01) with a risk of 30x and 8x respectively. However, the presence of smegma and preputial bacteria in the prepuce was not significantly associated with the inflammatory status in the prepuce (p = 0.541 & p = 0.084). In conclusion, the presence of phimosis has a significant risk in the formation of smegma and the growth of preputial bacteria.

Background: Urethral catheter placement is a form of nursing care performed by nurses, a standard for catheter placement is required. Urinary tract infections often occur around 40% of all hospital infections each year. In addition, from some researchers, 80% of urinary tract infections occur due to the use of catheterization instruments, because almost 10% of all hospitalized patients, at Bantul Hospital in 2017 There are 21% of patients are infected with the urinary tract. Nurse compliance uses standards based on various factors. Research Objectives: To determine the factors associated with the participation of nurses in the implementation of the standard installation of urethral catheters Research Methods: Descriptive analytic research by discussing the cross-sectional study. The sample used was total sampling with a sample of 43 nurses according to inclusion criteria. Data collection tools using questionnaires and observation sheets. Chi-Square data analysis with α <0.05. Results: The gender of female respondents was 76.7%. Age of respondents received 18-40 years 76.7%. The level of knowledge of respondents in the good category is 65.1%. The attitude of respondents in the positive category was 83.7%. The working period of respondents <5 years 51.2%. There is a significant relationship to age with the implementation of the standard implementation of urethral catheters, (P = 0.003 with an OR value of 2.54). There is a significant relationship between knowledge and the implementation of standard urethral catheter placement (P = 0,000 with an OR value of 19.50). There is a significant relationship between attitude with the agreement on the implementation of the standard urethral catheter installation (P = 0.007 with an OR value of 0.361). There is a significant relationship between a work period and the implementation of the standard implementation of urethral catheters (P = 0,000 with an OR value of 20.40). Conclusion: There is a significant relationship between age, knowledge, attitudes, and years of service to the approval of nurses in the implementation of the standard installation of urethral catheters in care in Bantul Hospitall Keywords: Working period of a nurse, and catheter installation standards

Abstrak: Infeksi saluran kemih (ISK) adalah keadaan adanya infeksi (ada pertumbuhan dan perkembangan bakteri) dalam saluran kemih yang meliputi infeksi di parenkim ginjal sampai infeksi di kandung kemih dengan jumlah bakteriuri bermakna yaitu ≥ 100.000 koloni / ml urin segar, infeksi ini sering di temukan pada anak dan merupakan penyebab kedua terbanyak mordibitas penyakit infeksi pada anak sesudah infeksi saluran napas. Sebelum usia 1 tahun, ISK lebih banyak terjadi pada anak laki-laki sedangkan setelahnya anak perempuan lebih dominan, rasio ini terus meningkat hingga di usia sekolah. Salah satu faktor penyebab ISK adalah  sirkumsisi, dimana anak laki-laki yang sudah disirkumsisi resiko ISK menurun dari 0,2 - 0,05% dari anak laki-laki yang tidak disirkumsisi. Anak laki-laki yang tidak di sirkumsisi, ISK terjadi karena daerah di bawah kulit prepusium sangat peka terhadap mikrolesi dan lingkungan yang lembab sehingga dapat memudahkan terjadinya infeksi. Jenis penelitian ini bersifat observasional analitik dengan rancangan cross sectional. Penelitian ini mengenai hubungan sirkumsisi dengan infeksi saluran kemih pada anak sekolah dasar Madrasah Ibtidayah yang menggunakan uji chi-square (x2) dan koefisien korelasi pada tingkat kemaknaan 95% (α0,05). Kesimpulan: Dari hasil penelitian yang dilakukan didapatkan bahwa tidak adanya hubungan yang bermakna antara sirkumsisi dengan kejadian ISK pada anak sekolah dasar. Kata kunci: Sirkumsisi, Infeksi Saluran Kemih (ISK), Siswa     Abstract: Urinary Tract Infection (UTI) is an infection state (there is growth and development of bacteria) in the urinary tract which include in the kidney parenchyma to infection in the bladder with a significant amount of bacteria that is ≥ 105 colonies / ml of fresh urine, this infection is often found in children and is the second most common cause of infectious disease morbidity in children after respiratory infection. Before the age of 1 year, UTI is more common in boys, while girls are more dominant thereafter, this ratio continued to increase until at school age. One of the causes of UTI is circumcision, where the boys were already circumcised risk of  UTI decreased 0,2 to 0,05% of the boys who are not circumcised. The boys who are not circumcised, UTI occurs because the area under the foreskin is very sensitive to mikrolesi and humid environment so as to facilitate the infection. The type of study is observational analytic cross sectional design. The study about a circumcision relationship with Urinary Tract Infections (UTI) in primary school children Madrasah Ibtidayah with using chi-square test (x²) and the correlation coefficient at 95% significance level (α0,05). Conclusion: Based on the results of the study it can be concluded that there was no significant correlation between circumcisions with incidence of UTI in elementary school children. One of the main complications that cause maternal death is preeclampsia/eclampsia. Activation of the systemic inflammatory response is thought to play a role in the pathogenesis of preeclampsia. Urinary tract infections (UTI) can cause activation of the systemic inflammatory response, so that it is considered capable of causing preeclampsia. Procalcitonin is known as a marker of infection and very stable in serum, so it is a promising biomarker in detecting UTI. The investigators intend to investigate the association between UTI and preeclampsia through exploration of procalcitonin levels in preeclampsia and non-preeclampsia mothers. This study was a cross-sectional method, accident sampling technique. Respondents in this study amounted to 65, preeclampsia criteria were 40, and not preeclampsia were 25. The results showed no relationship between urinary tract infections during pregnancy and preeclampsia P = 0.074. Even so, procalcitonin levels in preeclampsia mothers were found to be higher than mothers without preeclampsia, with a value of P = 0.005. Which means that the inflammation that occurs in preeclampsia mothers is not caused by urinary tract infections. It is hoped that further research can be carried out on the relationship of urinary tract infections and preeclampsia by using a larger sample.

The current paradigm of urinary tract infection (UTI) pathogenesis takes into account the contamination of the periurethral space by specific uropathogens residing in the gut, which is followed by urethral colonization and pathogen ascension to the urinary bladder. Consequently, studying the relationship between gut microbiota and the subsequent development of bacteriuria and UTI represents an important field of research. However, the well-established diagnostic and therapeutic paradigm for urinary tract infections (UTIs) has come into question with the discovery of a multifaceted, symbiotic microbiome in the healthy urogenital tract. More specifically, emerging data suggest that vaginal dysbiosis may result in Escherichia coli colonization and prompt recurrent UTIs, while urinary microbiome perturbations may precede the development of UTIs and other pathologic conditions of the urinary system. The question is whether these findings can be exploited for risk reduction and treatment purposes. This review aimed to appraise the three aforementioned specific microbiomes regarding their potential influence on UTI development by focusing on the recent studies in the field and assessing the potential linkages between these different niches, as well as evaluating the state of translational research for novel therapeutic and preventative approaches.

Abstract: Nicotine stomatitis could be found among heavy smokers. This study was aimed to determine the relationship between smoking habits and the incidence of lesions suspected as nicotine stomatitis among villagers of Ongkaw Dua. This was an analytical study with a cross sectional design. Population consisted of 183 smokers aged >15 years at Desa Ongkaw Dua and the subjects were 65 smokers. The chi-square showed a p-value of 0.592 for the relationship between the duration of smoking and the occurence of lesion supspected as nicotine stomatitis. Moreover, the chi-square showed a p-value of 0.005 for the relationship between the number of cigarettes consumed per day and the occurence of lesion suspected as nicotine stomatitis. In conclusion, there was no relationship between the duration of smoking and the occurence of lesion suspected as nicotine stomatitis, but there was a significant relationship between the number of cigarettes consumed per day and the occurence of lesion suspected as nicotine stomatitis.Keywords: smoking habit, nicotine stomatitis Abstrak: Stomatitis nikotina dapat dijumpai pada perokok berat. Penelitian ini bertujuan untuk mengetahui hubungan antara kebiasaan merokok dengan angka kejadian lesi yang diduga stomatitis nikotina pada masyarakat desa Ongkaw Dua. Jenis penelitian ialah analitik observasional dengan desain potong lintang. Populasi penelitian ini yaitu 183 perokok berusia >15 tahun di Desa Ongkaw Dua dan yang menjadi subyek penelitian berjumlah 65 orang. Hasil uji chi-square terhadap hubungan lama merokok dengan angka kejadian lesi yang diduga stomatitis nikotina mendapatkan p=0,592. Hasil uji chi-square terhadap hubungan antara jumlah rokok yang dihisap setiap hari dengan angka kejadian lesi yang diduga stomatitis nikotina mendapatkan p=0,005. Simpulan penelitian ini ialah tidak terdapat hubungan antara lamanya merokok dengan angka kejadian lesi yang diduga stomatitis nikotina, tetapi terdapat hubungan bermakna antara jumlah rokok yang dihisap setiap hari dengan angka kejadian lesi yang diduga stomatitis nikotina.Kata kunci: kebiasaan merokok, stomatitis nikotina

Background: A urinary tract infection (UTIs) is a common type of infection caused by bacteria that travel up the urethra to the bladder. Globally, it is reported that 150 million people are diagnosed with a UTI annually, costing the world economy over 6 million US dollars in treatment and work loss. Studies conducted in the in-patients pediatric ward of Muhimbili Hospital in Tanzania and Mulago National Referral Hospital in Uganda between five to ten years reported the prevalence of UTI to be 16.8% and 14.6% respectively. On average at Kam Medical Consult Clinic, patients who turn up in the laboratory are about 20%, and 50% of patients present with recurring UTIs clinically. Therefore,  there is a need to establish and investigate the prevalence of bacterial pathogens associated with UTI and multiple pathogens that are not known. Methodology: The cross-sectional study was carried out at Kam medical consult clinic (KMCC) located in Kafeero zone Mulago II Kawempe division. The study population was adults and children presenting with symptoms of urinary tract infection. 120 Midstream urine Samples were collected from patients presenting clinical signs of UTI. The diagnosis was done through macroscopy dry chemistry, microscopy, and culture. Results Among the sampled population, 33% of males had urinary tract infections while 67% of females had urinary tract infections.  88.5% were outpatients while 11.5 % were in-patients The overall prevalence of UTI among patients was 63.3% with children aged (1-10) having a prevalence of 5.8 %. Conclusion and recommendation: E.coli is an etiological agent causing UTIs in male and female patients presenting UTI at Kam Medical Consult Clinic. Other etiological agents included Klebsiella, Coliform, Enterococcus species, staphylococcus aureus, pseudomonas species, and candida species. There is a need to monitor the profile of etiological bacteria of UTI through culture and sensitivity regularly.

BACKGROUND Urinary tract infections (UTI) constitute a common cause of morbidity in infants and children. When associated with abnormalities of urinary tract, they may lead to long-term complications including renal scarring, loss of function and hypertension. Most urinary tract infections remain undiagnosed if investigations are not routinely performed to detect them. Prompt detection and treatment of urinary tract infections and any complicating factors are important. The objective of the study is to know the clinical, epidemiological and bacteriological profile (i.e. clinical signs and symptoms, age, sex, family history, associated urinary tract abnormalities, & causative organisms) of urinary tract infections in febrile children with culture positive urinary tract infection. METHODS This descriptive, cross sectional observational study was conducted at outpatient clinics of our “child health clinics” between May 2016 and April 2017 (one year). All children aged 0 to 12 years with culture positive urinary tract infections were included in this study to evaluate the clinical, epidemiological and bacteriological profile. RESULTS A total of 69 children with culture positive urinary tract infections were included in this study. Out of 69 children included in this study, 36 (52.2 %) were females and 33 (47.8 %) were males. Overall female preponderance was seen and the M: F ratio was 0.9:1. But during first year of life in our study group we had more boys (10, 14.49 %) affected with urinary tract infection than girls. 49.3 % of urinary tract infections in the present study belonged to lower socio-economic status. Most common organism causing urinary tract infection in our group was E. coli (56.5 %). Fever (100 %), anorexia or refusal of feeds (52.2 %), dysuria (46.4 %), vomiting (46.4 %) and abdominal pain (39.1 %) were the predominant clinical manifestations observed in our study. CONCLUSIONS Urinary tract infection is a common medical problem in children and it should be considered as a potential cause of fever in children. As febrile children with urinary tract infection usually present with non-specific signs and symptoms, urine culture should be considered as a part of diagnostic evaluation. Urinary tract infections (UTIs) are one of the major causes of morbidity and comorbidities in patients with underlying conditions, and it accounts for the majority of the reasons for hospital visit globally. Sound knowledge of factors associated with UTI may allow timely intervention that can easily bring the disease under control. This study was designed to determine the prevalence of UTI by isolating and characterizing the different bacterial etiological agents and to evaluate the factors associated with UTI. In this cross-sectional study, a total of 267, clean catch midstream urine (MSU) samples were collected aseptically and analyzed using standard microbiology methods. Data for the factors associated with UTI were obtained by use of questionnaires and standard laboratory tests for selected underlying conditions. The study revealed 86/267 (32.2%) UTI prevalence among patients attending hospitals in Bushenyi District, Uganda. Escherichia coli was the most prevalent bacterial uropathogen with 36/86 (41.9%) followed by Staphylococcus aureus 27/86 (31.4%), Klebsiella pneumoniae 10/86 (11.6%), Klebsiella oxytoca 6/86 (7.0%), Proteus mirabilis 3/86 (3.5%), Enterococcus faecalis 3/86 (3.5%), and Proteus vulgaris 1/86 (1.2%). This study has demonstrated that age ≤19 years, female gender, married individuals, genitourinary tract abnormalities, diabetes, hospitalization, indwelling catheter <6 days, and indwelling catheter >6 days had statistically significant relationships (p<0.05) with UTI. Screening for UTI in hospitalized patients, female gender, married individuals, genitourinary tract abnormalities, indwelling catheter, and diabetics should be adopted.

MotivationCatheter-associated urinary tract infections (CAUTI) are a common and serious healthcare-associated infection. Despite many efforts to reduce the occurrence of CAUTI, there remains a gap in the literature about CAUTI risk factors, especially pertaining to the effect of catheter dwell-time on CAUTI development and patient comorbidities.ObjectiveTo examine how the risk for CAUTI changes over time. Additionally, to assess whether time from catheter insertion to CAUTI event varied according to risk factors such as age, sex, patient type (surgical vs medical) and comorbidities.DesignRetrospective cohort study of all patients who were catheterised from 2012 to 2016, including those who did and did not develop CAUTIs. Both paediatric and adult patients were included. Indwelling urinary catheterisation is the exposure variable. The variable is interval, as all participants were exposed but for different lengths of time.SettingUrban academic health system of over 2500 beds. The system encompasses two large academic medical centres, two community hospitals and a paediatric hospital.ResultsThe study population was 47 926 patients who had 61 047 catheterisations, of which 861 (1.41%) resulted in a CAUTI. CAUTI rates were found to increase non-linearly for each additional day of catheterisation; CAUTI-free survival was 97.3% (CI: 97.1 to 97.6) at 10 days, 88.2% (CI: 86.9 to 89.5) at 30 days and 71.8% (CI: 66.3 to 77.8) at 60 days. This translated to an instantaneous HR of. 49%–1.65% in the 10–60 day time range. Paraplegia, cerebrovascular disease and female sex were found to statistically increase the chances of a CAUTI.ConclusionsUsing a very large data set, we demonstrated the incremental risk of CAUTI associated with each additional day of catheterisation, as well as the risk factors that increase the hazard for CAUTI. Special attention should be given to patients carrying these risk factors, for example, females or those with mobility issues.
