Background Tuberculosis (TB) infection is highly prevalent in Indonesia. The source of transmission of TB to a child is usually via an adult with sputum smear-positive pulmonary tuberculosis. The Mantoux test is a diagnostic tool for tuberculosis infection. The BCG vaccine has been used for the prevention of TB, but its efficacy is still debated. Objective To assess for an association between Mantoux test results and BCG vaccination in children who had contact with adult pulmonary tuberculosis and to assess for differences in Mantoux test induration with regards to nutritional status, age, type of TB contact, and time duration since BCG vaccination in BCG-vaccinated and BCG-unvaccinated children. Methods A cross-sectional study was conducted in FebruaryMarch 2011 on infants and children (aged 3 months to five years), who had household contact with adult pulmonary TB. We performed tuberculin (Mantoux) skin tests to detect TB infection in the children. Subjects were consisted of two groups: BCG-vaccinated and BCG-unvaccinated. Results Subjects were 100 children (50 BCG-vaccinated and 50 BCG-unvaccinated subjects). Positive Mantoux test results were observed in 9 vaccinated subjects and 33 unvaccinated subjects. The mean diameters of induration in the vaccinated and unvaccinated groups were 7.6 mm and 9.6 mm, respectively (95%CI of difference -4.25 to 0.20; P=0.074). In children who had household contact with sputum smear-positive adult pulmonary TB, BCG vaccination was a protective factor against TB infection, with an odds ratio (OR) of 0.113 (95%CI 0.045 to 0.286; P=0.0001). There were no significant differences in Mantoux test induration associated with nutritional status, age, type of TB contact, and duration since BCG vaccination, between the two groups. Conclusion BCG vaccination has a protective effect on TBexposed children, based on Mantoux test results. However, there are no differences in Mantoux test induration associated with nutritional status, age, type of TB contact, or duration since BCG vaccination, between the BCG-vaccinated and BCGunvaccinated groups.

Tuberculosis (TB) is considered as the leading killer among infectious diseases because of bacteria resulted in a high number ofmorbidity and mortality world wide. One risk factor for tuberculosisis diabetes mellitus (DM). The prevalence of TB and DM jump together globally, especially in developing countries, including Indonesia. Early and accurate diagnosis of TB cases in the enforcement is sputum smear examination. Monitoring of TB seen through sputum conversion. The purpose of this studyis to determine the duration of sputum conversion difference new case smear positive pulmonary TB patients with and without DM in hospital Prof. Dr.Margono Soekarjo (RSMS) Purwokerto. Thestudy design is observational analytic with cohort retrospective study. Sample of this study was patients who checked their health in Lung Clinis of RSMS Purwokerto obtained total sample of 44 people, consisting of 22 patients clinically diagnosed new cases smear-positive pulmonary TB with DM and 22 patients without DM in 2009 until 2012.Results of this study were analyzed using Fisher Exact Test. pvalues obtainedof 0.000 (p <0.05) (95% CI = 0,288-0,718) with relative risk 0,455. The conclusion is there were significant differences in sputum conversion time between the group of patients suffering from pulmonary uberculosis with and without DM. The conversion of sputum in patients with pulmonary tuberculosis who suffered DM is longer than patients without DM. Relative risk value which is equal to 0.455 which means that patients with DM have pulmonary tuberculosis risk by 0.455 times more likely to experience delays in sputum conversion than TB patients without DM

Kata kunci: status nutrisi, ibu menyusui, kecukupan ASIABSTRACTProblem of gizi in Indonesia and developing countries in general still predominated by problem less energi protein, iron anaemia, trouble effect of iodine insuffiency, lacking of vitamin of A and problem of obesitas especially in metropolis. Anaemia of gizi is generally met at gristle faction of gizi that is pregnant mother, mother suckle, child of balita and also schoolchild. Anaemia at mother suckle will cause trouble of nutrisi production and irrigate mother milk become less. this clear generate growth trouble for baby which in giving irrigate mother milk. At mother with ugly gizi generally produce slimmer irrigate mother milk in number, while the qualities of depend on food which is eating. Generally there are degradation of fat rate, vitamin and carbohydrate. This study aims to know the relationship between the status of breastfeeding mothers with the adequacy of breastfeeding. This research is analytical research with longitudinal design up (cohort). Based on the results of test statistics with the mann whitney u test on the 95% level of confidence between the nutritional status of breastfeeding mothers with the adequacy of breastfeeding showed a significant relationship (p = 0,009). This explains that breastfeeding mothers who have poor nutrition affect the adequacy milk product. With the results of the research above, it is expected that the health through the village midwifes for more attention on the status of maternal nutrition on the nutritional needs of breastfeeding so that breastfeeding mothers can be improved by providing more counseling related to maternal nutrition and breastfeeding, such as the provision of vitamin tablets to the blood. Keywords: nutritional status, mother suckle, breastfeeding

Identification of malnutritional risk and nutrition status evaluation of pediatric cancer patients with chemotherapy treatmentBackground: Hospitalized childhood cancer patients had a high risk for malnutrition, either caused by the disease or effects of cancer treatment. Malnutrition in cancer patients gives negative impacts on treatment outcomes in the form of increasing morbidity and mortality rates. Nutrition screening for identifying malnutrition risks could prevent malnutrition in hospitals. Objectives: Investigating the influence of malnutrition risk during hospitalization on the changes in the nutritional status of childhood cancer patients with chemotherapy treatment. Methods: This research was observational research with the nested case-control design. The research subjects were childhood cancer patients aged 2-18 years old meeting the inclusion criteria. They were 64 in number consisting of the case group involving 32 patients and a control group involving the rest. During hospitalization, analyses of nutritional intake, change in body weight, nutritional status, and hospitalization period. Furthermore, the analyses of the influence of malnutrition risk on the outcome between those two groups were then compared. Results: There was a significant influence of malnutrition risk on less energy intake (p<0.001), less protein intake (p=0.002), weight loss >2% (p<0.001), poor nutritional status based on the BMI/U (p=0.011), and longer hospitalization (p=0.034). The group of patients with malnutrition risks had risks of 15.5 (CI 95%: 3.991-63.359) times higher for less energy intake, 6.12 (CI 95%: 1.675-24.906) times higher for less protein intake, and 45.3 (CI 95%: 5.666-1940.768) times higher for weight loss > 2% than the group of patients without malnutrition risks. Conclusions: Patients with a significant risk of malnutrition had less energy and protein intake, weight loss > 2%, poor nutritional status based on BMI/U, and longer hospitalization.

Purpose – Identifying and addressing poor nutritional status in school-aged children is often not prioritized relative to HIV/AIDS treatment. The purpose of this paper is to elucidate the benefits of integrating nutrition (assessment and culturally acceptable food supplement intervention) in the treatment strategy for this target group. Design/methodology/approach – The authors conducted a randomized, double blind pre-/post-intervention trial with 201 HIV-positive children (six to 15 years) in Botswana. Eligibility included CD4 cell counts < 700/mm3 (a marker for the severity of HIV infection), documented treatment with antiretroviral (ARV) drugs, and no reported evidence of taking supplemental food products with one or more added nutrients in the six-month period prior to the study. The intervention (12 months) consisted of two food supplements for ethical reason, one with a higher protein content, bean (bean-sorghum based) group (n=97) and a cereal (sorghum) group (n=104) both of which contained added energy- and micro- nutrients. Anthropometric and biochemical nutritional status indicators (stunting, wasting, underweight, skinfolds for fat and muscle protein reserves, and hemoglobin levels) were compared within and between the bean and the cereal groups pre- and post-intervention separately for children six to nine years and ten to 15 years. Findings – Older children (ten to 15 years) fared worse overall compared to those who were younger (six to nine years) children in anthropometric and protein status indicators both at baseline and post-intervention. Among children six to nine years, the mid arm circumference and blood hemoglobin levels improved significantly in both the bean and cereal groups (p < 0.01 and p < 0.05, respectively). Although the BMI for age z-score and the triceps skinfold decreased significantly in the bean group, the post-intervention subscapular skinfold (fat stores) was significantly higher for the bean group compared to the cereal group (p < 0.05). Among children ten to 15 years, both the bean and the cereal groups also showed improvement in mid arm circumference (p < 0.001), but only those in the bean group showed improvement in hemoglobin (p < 0.01) post-intervention. Originality/value – Similar significant nutritional status findings and trends were found for both food interventions and age within group pre- vs post-comparisons, except hemoglobin in the older children. Post-intervention hemoglobin levels for the type food supplement was higher for the “bean” vs the “cereal” food in the younger age group. The fact that all children, but especially those who were older were in poor nutritional status supports the need for nutrition intervention in conjunction with ARV treatment in children with HIV/AIDS, perhaps using a scaled up future approach to enhance desired outcomes.

Pulmonary tuberculosis (TB) is a global health problem and has become the leading cause of death. Tuberculosis eradication is inhibited due to the tendency of patients to not complete the TB treatment. The purpose of this study was to determine the relationship between knowledge, nutritional status, oral medication adherence, and family support as risk factors for pulmonary tuberculosis treatment failure. The design of this study is that of case-control, and this study involved samples of pulmonary TB patients who were declared not cured after treatment (14 people), and control samples of pulmonary TB patients who were declared cured (28 people). The data were obtained through measurements of nutritional status and interviews, while treatment failure was based on data from the TB 01 form. Data was analysed using univariable and bivariable analyses, and the magnitude of risk factors was based on the odds ratio (OR) and 95% confidence interval (CI). The results showed that the factors associated with failure of pulmonary TB treatment are knowledge (p = 0.022; OR = 6.6; 95% CI = 1.48 - 29.36), nutritional status (p = 0.005; OR = 9.16; 95% CI = 2.11 - 39.85), and medication adherence (p = 0.003; OR =11.0; 95% CI = 2.37 - 54.14), whereas the unrelated factor is family support (p = 0.47). It is recommended to provide counselling, nutritional guidance, medication assistance, and family support to patients during the treatment period for pulmonary tuberculosis.                                                                                                                        Keywords: treatment failure, knowledge, nutritional status, compliance
